Objective To analyze the cost-effectiveness of selective intraoperative laparoscopic cholangiography (LIOC) in the county level hospitals.Methods The medical costs and cost-effectiveness of 60 patients undergoing LIOC (group A) during laparoscopic cholecystectomy (LC) in 2014 and 52 patients undergoing LC (group B) only in 2013 at 3 county level hospitals were analyzed.Results The direct medical care costs of group A were less than that of group B significantly [(9 072 ± 1 282) Yuan vs.(10 941 ± 4 064) Yuan,t =-3.38,P =0.001],there were no significant different in indirect medical care costs between group A and group B[(2 039 ± 1 020) vs.(2 100 ± 1 172) Yuan,t =0.24,P =0.810].The cure rate was 100％ in two groups;however,the cost per patient in group A was less than that in group B [(11 111 ± 2 348)vs.(13 041 ± 5 118)Yuan].The direct medical care cost of the patients who needed additional treatment in group A was significantly less than that in group B [(10 828 ± 1 101)vs.(16 533 ±1 733)Yuan,t =-11.45,P =0.000].There were no significant different in direct operational costs between group A (LIOC + LC) groups and group B (LC) [(8 378 ± 326) vs.(8 225-± 363) Yuan,t =1.96,P =0.054].Conclusions Selective LIOC is a cost-effective procedure and should be recommended in county level hospitals in which magnetic resonance cholangiopancreatography is not available.

Objective To explore protective effect of Anisodamine against injure of cochlea hair cells induced by ionizing radiation. Methods 50 healthy guinea pigs were randomly divided into 3groups: radiation group( n = 20) , anisodamine protective group( n = 20) and control group( n = 10). 20ears were observed in each group. Tempora of guinea pigs in anisodamine protective group and radiation group were exposed to fractional doses of 6 MeV electron beam produced by linear accelerators (2 Gy/d)with a total dose of 60 Gy. Animals were injected intramuscularly with anisodamine (20 mg/kg) in protection group and equal saline solution in irradiated group, respectively, 30 min pre-irradiation.Morphological and functional changes in the guinea pig cochlea were observed. Results After radiation,the average ABR threshold value of the radiation group (52. 27 ± 2.42 dB peSPL) was significantly higher than that of the anisodamine protection group (37.65 ± 1.92 dB peSPL, t = 2.01, P＜ 0.05 ), and outer fair cells of cochlea reduced greatly in quantity by observing the stretched preparations of cochlea basilar membrane ( F=135. 362, P＜ 0.05 ). Under transmission electron microscope, outer fair cells of cochlea in the radiation group had obscure boundaries and swelled to lose their original shapes. Meanwhile, cell implement and the nucleus were obviously obnormal after radiation. In the anisodamine protection group,the boundaries of cochlea outer fair cells were very clear and the cells swelled a little. Cell implement and nuclear envelope were complete. Cochlea outer fair cells in the control group were found to line up in order with no collapse and reduction in number under scanning electron microscope, but those in the radiation group collapsed obviously, reduced in number and aligned in a state of chaos, while the outer fair cells in the anisodamine protection group regularly posed on the whole and the collapse was occasionally found.Conclusions The 60 Gy radiation at fractioned dose on the tempuses of the guinea pigs could cause damage to the cochlea hair cells. Anisodamine might have a protective effect against radiation damage.

Objective To analyze the clinicopathologic features,effect of surgical treatment and prognosis of elderly patients with gastric cancer.Methods The records of 84 elderly patients(aged 80 years or above) with gastric cancer who underwent operation in our hospital from 2000-2008 were analyzed retrospectively.Results Ten patients had early gastric cancer(1a,8 cases,1b,2cases),stage II 12 cases,stage III 45 cases(53.6%) and stage IV 17 cases.Among them,81% patients had one or more co-morbidities.Cardiovascular disease was present in 43 cases(51.2%).Resection rate was 85.7%(72/84).Radical resection rate was 60.7%(51/84).Limited nodal dissection(

Objective To investigate the predictive values of P-selectin (P-sel),thrombus precursor protein (TpP) and D-dimer (D-D) in the early diagnosis of portal vein thrombosis (PVT) in patients submitted to splenectomy (or to devascularization).Methods The clinical data of 48 patients with portal hypertension (the PVT group,n=26; and the non-PVT group,n=22) who received operation in our hospital from 2009 to 2011 were retrospectively analyzed.Detecting the P-sel,TpP and D-D levels in the two groups of patients were done on preoperative day 1 and postoperative day 1,3,5,7,14.The SPSS software was used for statistic analysis.Results There were no significant differences on the preoperative day 1 levels of P-sel,TpP and D-D in the two groups (P＞0.05).The postoperative day1 levels of the three indicators in the PVT group were significantly higher than the non-PVT group (P＜0.05).Receiver operating characteristic (ROC) curves showed the area under curve (AUC) of P-sel was largest (0.893),followed by D-D,and TpP.The combined detection of the 3 indicators was highest,with the AUC up to 0.977.Conclusions Combined detection of P-sel,D-D and TpP levels were useful in the early diagnosis of PVT after splenectomy in patients with portal hypertension.

We developed a microfluidic device to integrate sample introduction, bacteria culturing and results reading. The identification of multiple bacteria was achieved by combining the spatial resolution of the arrayed bacteria culture chambers and the color resolution benefited from the bacteria specific chromogenic media. A set of 4 common pathogenic bacteria responsible for urinary tract infection were used as a model to test the microfluidic assay. Our results showed that the bacteria identification assay can be completed in 15 h, with a limit of detection (LOD) of bacteria density down to 10 cfu / mL. Clinical sample testing using the microchip approach showed a coincidence rate of 96. 3% as compared with the conventional method. The developed microfluidic approach is simple and rapid, thus hold the potential to serve as a powerful tool for detection of multiple bacteria.

Objective To observe the effect of octreotide on human pancreatic cancer cells (PC-3) apoptosis after PC-3 transfected with somatostatin receptor type 2 (SST 2) gene. Methods SST 2 was transfected into PC-3 by liposome,the result of transfection was detected by immunohistochemistry. Objective To develop a phenylketonuria (PKU) screening method based on a compact disk (CD) type microfluidic chip capable of generating reciprocating flow within the microchannels that facilitate rapid DNA hybridization. Methods This microfluidic device consists of a two-layer structure: a polydimethylsiloxane (PDMS) top layer containing 12 DNA hybridization microchannels, and a bottom glass layer with immobilized hydrogel conjugated DNA arrays. The DNA arrays included R243Q, V245V and the blank control probes. When the CD device was spun, the PCR products were driven into the hybridization channel by centrifugal force. When the rotation of the CD device was stopped, capillary force pulled the PCR products solution to flow back to the channel. After the on-chip hybridization, the hybridization signals were captured on a fluorescence microscope. The specificity, detection limitation and reproducibility of this device were evaluated. Thirty DNA samples from pregnant women with suspected PKU were detected by this device.Then the results were compared with DNA sequencing results. Results With the compact disk type microfluidic chip, the hybridization time could be reduced to 15 min, sample consume could be as low as 1. 5 μl and the detection limitation was 0. 7 ng/μl. With the chip based method, samples of PKU patients and healthy controls were detected and the results were consistent with DNA sequencing results. Five different batches of chips and five micro-channels of each chip were selected to test one PKU patients with V245V mutation. All the results were positive, indicating good reproducibility. Four cases of V245V mutation and 1 case of R243Q mutation were found in 30 suspected PKU carried pregnant women. Conclusion The compact disk microfluidic device has advantages of simple, rapid and highly sensitive, thus is well suited to PKU screening.

In this study, the time-dependent changes on dynamic computed tomograph (CT) of radiation-induced liver injury in gastric cancer patients was examined. The CT images of 52 gastric cancer patients who had received chemoradiotherapies were reviewed on the PACS system. Dynamic CT scan was performed in all the subjects. Our results showed that 18 patients were found to have radiation-induced liver injury. The CT findings of radiation-induced liver injury in gastric cancer patients tend to show up one month after radiation treatment. The damaged area was of low density on all three phases, and then it was enhanced on portal vein phase or delay phase. The focal radiation reaction of liver without basic disease vanished 9-11 months later after treatment. We are led to conclude that dynamic CT is of help in the diagnosis of CRT-induced liver injury, and it may be the method of choice for following up the whole course of the CRT-induced liver injury, i.e., form hepatic damage to healing. The classification of CT findings we recommend can avoid the influence of technological factors, and thereby serve as a better guide for treatment of CRT-induced liver injury.

Objective:To investigate the expression of triggering receptor expressed on myeloid cells(TREMs)in periventricular leukomalacia(PVL) of the neonatal rat model.Methods:Thirty-two 3-day-old neonatal rats were double-blinded randomly divided into Sham group and Model group.The PVL rat model was established by ligating right carotid artery and oxygen deprivation.Hematoxylin-eosin (HE) was adopted to compare pathological changes of brain tissue between the two groups, and immunofluorescence was adopted to detect the expression of myline basic protein (MBP) in the right hemisphere of the two groups.Western blot was performed to detect the expression of TREM1 and TREM2 in the right hemisphere of the two groups.Results:The results of HE staining showed that the brain tissues of Model group were significantly damaged compared with that of Sham group, and the mean fluorescence intensity of MBP in Model group(26.629±2.317) was significantly lower than that in Sham group(33.579±2.824), with statistically significant differences( t=9.124, P<0.05). The expression of TREM1 in Model group(0.789 ±0.120) was higher than that in Sham group(0.567±0.093), with statistically significant differences( t=-3.891, P<0.05). The expression of TREM2 in Model group(0.544±0.133) was lower than that in Sham group(0.791±0.118), with statistically significant differences( t=3.667, P<0.05). Conclusion:The expressions of TREM1 and TREM2 in the neonatal rat model of PVL change abnormally, suggesting that TREMs may be involved in the pathological process of preterm white matter injury.

OBJECTIVE: To study the expressions of Cyclin D1 and p16 proteins in hypopharyngeal squamous cell carcinoma and their clinical significance. METHOD: Immunohistochemical technology (P-V) was applied to detect the expression of Cyclin D1 and p16 in 36 cases of hypopharyngeal squamous cell carcinoma and 10 cases of normal epithelium. RESULT: (1) The expression of cyclin D1 in the tumorous cell was significantly higher than that in normal epithelium (P < 0.05). The expression of p16 in the tumorous cell was significantly lower than that in normal epithelium (P < 0.05); (2) The positive rate of Cyclin D1 was significantly correlated with clinical stage and cervical lymph node metastases (P < 0.05). The positive rate of p16 was correlated with cervical lymph node metastases (P < 0.05). The positive rates of Cyclin D1 and p16 were not correlated with the gender, age, tumor size, primary lesion and cell differentiation (P > 0.05); (3) There was correlation between the expression of Cyclin D1 and the expression of p16 (r(s) = -0.420, P < 0.05). CONCLUSION The over-expression of Cyclin D1 and the under-expression of p16 may play a significant role in the occurrence incidence and development of hypopharyngeal squamous cell carcinoma, and may be important indicators for cervical lymph node metastases.
