1.A Logistic regression analysis of short-term residual symptoms after resolution of benign paroxysmal positional vertigo

Objective To investigate risk factors for short-term residual symptoms after resolution of benign paroxysmal positional vertigo (BPPV) through logistic regression analysis. Methods A total of 110 patients with BPPV in our hospital from July 2020 to January 2023 were enrolled. The patients were grouped according to whether they were cured or had residual symptoms at 8 weeks after repositioning maneuvers. A logistic regression analysis was used to analyze the risk factors for short-term residual symptoms after treatment. Results Forty-nine of the 110 patients had short-term residual symptoms. The univariable logistic regression analyses showed that age, the duration of vertigo before treatment, recurrence, medical history (hypertension, diabetes, and ischemic cerebrovascular disease), anxiety, depression, sleep quality, and vestibular evoked myogenic potential were risk factors for short-term residual symptoms in patients with BPPV. The multivariable logistic regression analysis revealed that age (OR=0.942,95%CI 0.913-0.972,P<0.001), the duration of vertigo before treatment(OR=0.333,95%CI 1.015~1.019,P=0.002),recurrence(OR=0.777,95%CI 0.726-0.832,P<0.001), a history of hypertension(OR=0.682,95%CI 0.624-0.745,P<0.001), a history of diabetes(OR=0.854,95%CI 0.791-0.922,P<0.001),a history of ischemic cerebrovascular disease(OR=0.876,95%CI 0.806-0.953,P=0.002), anxiety(OR=1.158,95%CI 1.046-1.283,P=0.005),depression(OR=1.178,95%CI 1.033-1.344,P=0.014),sleep quality(OR=1.164,95%CI 1.009-1.343,P=0.037), and vestibular evoked myogenic potential(OR=1.196,95%CI 1.068-1.340,P=0.002) were independent risk factors for short-term residual symptoms in patients with BPPV. Conclusion Patients with BPPV are more likely to have short-term residual symptoms if they have a history of hypertension, diabetes, or cerebrovascular diseases, advanced age, a long duration of vertigo before treatment, and the presence of emotional disorders (anxiety, depression, sleep deficiency, vestibular evoked myogenic potential abnormalities).
2.Total arterial revascularization for patients with coronary artery disease and left ventricular dysfunction: A retrospective cohort study

Objective To compare the superiority of total arterial revascularization in patients with coronary artery disease (CAD) complicated with left ventricular dysfunction. Methods This retrospective study included the patients who were diagnosed with CAD and the left ventricular ejection fraction (LVEF) of ≤40% and underwent coronary artery bypass grafting (CABG) at our hospital from January 2016 to July 2019. The patients were divided into two groups according to the different types of bypass vessels: a total arterial revascularization group (TAR group) and a conventional group (a CON group). The clinical data were compared between the two groups to explore the incidence of important complications and evaluate the safety of total arterial revascularization and its protective effect on cardiac function. Results Finally 75 patients were enrolled including 52 males and 23 females with a mean age of (61.58±7.93) years. There were 35 patients in the TAR group and 40 patients in the CON group. The operation time and the drainage volume at 24 hours after operation in the TAR group were longer or more than those in the CON group (P<0.001), but there was no statistical difference in hospital stay, postoperative complications (such as respiratory failure, mediastinal infection, renal failure), intra-aortic balloon pump or extracorporeal membrane oxygenation use rate (P>0.05). After 2 years of follow-up, compared with the CON group, the cardiac function of the TAR group was significantly improved, the LVEF was higher, the left ventricular end diastolic diameter was reduced, and the graft stenosis rate was lower (all P<0.05). Conclusion Total arterial revascularization is a safe and feasible surgical method, which is helpful to improve the cardiac function and improve the quality of life.
3.Sero-epidemiological study and infection rate evaluation of pertussis in Henan Province

Objective:To analyze the epidemiological distribution characteristics, influencing factors, and infection rates of pertussis in the population of Henan Province.Methods:From 2022 to 2023, a cross-sectional survey was conducted to investigate the permanent population in Henan Province. Enzyme-linked immunosorbent assay (ELISA) was used to detect anti-pertussis toxin IgG (PT-IgG), analyze the antibody positivity rate (≥20 IU/ml) and median concentration (MC), and estimate the pertussis infection rate based on PT IgG ≥40 IU/ml. The rank sum test was used to compare antibody levels among groups, and the χ2 test was used to compare antibody positive rates and infection rates among groups. Results:A total of 4 810 research subjects were included in this study. The overall positive rate of PT-IgG was 12.10% and MC was 3.04 (0.35, 10.36) IU/ml. There were significant differences both in positive rates and antibody levels of PT-IgG among different regions or age groups (region positive rate: χ2=134.06, P<0.001, MC: H=337.74, P<0.001; age group positive rate: χ2=45.27, P<0.001, MC: H=134.49, P<0.001). Both the positive rate of PT-IgG (25.26%) and MC (8.01 IU/ml) were the highest within one year after completing a full course of vaccination. There were significant differences in positive rates and antibody levels among people receiving different types of pertussis vaccines (positive rate: χ2=12.38, P=0.006, MC: H=17.93, P<0.001). The antibody positivity rate (35.71%) and MC (8.88 IU/ml) of the people who received cell-free pertussis inactivated poliomyelitis influenza type b (combined) vaccine throughout the course were higher than those who received other types of vaccines. The natural infection rate of pertussis was evaluated for individuals aged≥3 years who had no history of pertussis vaccine immunization within the year prior to sampling. With a high vaccination rate, the estimated infection rate of pertussis in the population was 5 757.22/100 000. The infection rates in the 3-year-old (1 940.16/100 000) and 4-year-old (1 765.68/100 000) populations were at a low level among the entire population, reaching their peak at the age of 6 (12 656.71/100 000). Subsequently, although the infection rate continued to decline, it remained at a high level and peaked again at the age of 40-49 years (8 740.39/100 000). There was a statistically significant difference in the estimated infection rate of pertussis among different age groups ( χ2=53.21, P<0.001). Conclusion:The PT-IgG level of pertussis in the population of Henan Province is generally at a low level. The estimated infection rate of pertussis is much higher than the reported incidence rate. A booster dose of pertussis vaccine is recommended at 6 years old.

5.A survey of tetanus antibody levels in 7 519 healthy people in Henan Province

Objective:To analyze tetanus antibody levels in healthy people in Henan Province and to provide reference data for the adjustment of tetanus vaccine immunization strategy.Methods:A total of 7 519 healthy people were selected using stratified sampling in Henan Province from 2020 to 2021. Serum samples were collected and tested for anti-tetanus toxoid IgG antibody (anti-TT) by ELISA. The positive rate (≥0.01 IU/ml), protection rate (≥0.1 IU/ml) and mean concentration (MC) of anti-TT were analyzed.Results:The overall positive rate, protection rate and MC of anti-TT among the 7 519 subjects were 70.33% (5 288/7 519), 37.07% (2 787/7 519) and 0.035 IU/ml, respectively. Both basic and booster tetanus vaccines were effective. The positive rate, protective rate and MC of anti-TT decreased from 99.63% (272/273), 82.05% (224/273) and 0.215 IU/ml in 6-year-olds to 64.22% (262/408), 21.57% (88/408) and 0.023 IU/ml in 16- to 19-year-olds, and further decreased to 40.97% (1 302/3 178), 5.48% (174/3 178) and 0.007 IU/ml in people ≥20 years old.Conclusions:The level of tetanus antibody decreased with age in healthy people in Henan Province. It was suggested that the sixth dose of tetanus vaccine should to be given at 16-20 years old.
6.Prevalence and risk factors of permanent tooth caries among junior high school students in Haikou

Objective:
To describe the prevalence of permanent tooth caries and associated factors among junior high school students in Haikou, and to provide reliable evidence for prevention and control of caries.
Methods:
A total of 3 573 students from 8 junior high school in Haikou City and towns were selected by the methods of clustered sampling survey. Questionnaire survey and oral health examinations were conducted to analyze the caries rate, mean decayed missing filled tooth (DMFT), filling rate, pit and fissure sealant rate. Logistic regression was used to analyze the influencing factors of caries in permanent tooth.
Results:
Among the surveyed junior high school students, the caries rate was 47.32%( n =1 691), the mean DMFT was 1.51. The caries rate and mean DMFT were higher in female students (49.59%) than in male students (44.95%), and higher in towns (50.77%) than in urban areas (44.04%), the difference were statistically significant ( χ 2=16.25, 7.72, P <0.05). The filling rate and pit and fissure sealant rate among junior high school students in Haikou were 17.13%, 6.27% respectively. The filling rate and pit and fissure sealant rate were higher in urban areas (18.97%, 7.17%) than towns (15.76%, 5.33%), the difference were statistically significant ( χ 2= 9.57, 5.13, P <0.05). The multivariate Logistic analysis showed that female student, town residence, daily consumption of sweets or sugary drinks (≥1 time), bedtime snack were risk factors for junior high school students suffering from permanent dental caries( OR =1.41, 1.45, 2.63, 2.09, 1.53), while using fluoride toothpaste daily, oral education in school were protective factors( OR =0.44, 0.34)( P <0.05).
Conclusion
The caries rate of permanent teeth among junior high school students in Haikou is at high level, but the filling rate of caries and pit and fissure sealant rate are lower. The prevention and treatment of dental caries should be carried out in high risk students, and oral health education in school is also needed to improve the oral health level of junior high school students.
7.Performance of commercialized kits for detecting IgG antibody to measles virus

Objective:To compare the performance of four ELISA kits for detecting measles virus IgG antibody and to provide support for sero-epidemiologic survey.Methods:A total of 176 specimens were tested by four ELISA kits, respectively. The result of 4 kits were analyzed by the McNemar’s test. The values such as Kappa value, CV value, sensitivity, specificity, positive predictive value and negative predictive values were calculated and compared.Results:There was no significant difference among the result from kits and references, the χ2 were 0.333, 2.000, 1.000 and 0 respectively. The Kappa value of kit C was the highest (0.976), the sensitivity of kit A, C and D were the highest (99.34%), the specificity of kit B and C were the highest (100%), the positive predictive value of kit B and C were the highest (100%) and the negative predictive value of kit C were the highest (96.00%), there was no significant difference among the indicators from 4 kits ( P >0.05). The CV value of kit D was the lowest (5.30%). The correlation coefficient between the result from kit C and D was 0.639 ( P<0.01). Conclusions:The result showed that the 4 kits were suitable for serum epidemiologic survey.
8.Effects of Adiponectin on the Expression of Myocardial AMPK in Myocardial Insulin Resistance Model Dogs during Cardio- pulmonary Bypass

OBJECTIVE: To investigate the effects of adiponectin (APN) on the expression of myocardial AMPK in myocardial insulin resistance (IR) model dogs during cardiopulmonary bypass (CPB). METHODS: Totally 24 dogs were randomly divided into control group, model group, APN group (36 μg/kg), AMPK inhibition group (APN 36 μg/kg+AMPK inhibitor compound C 0.5 mg/kg), with 6 dogs in each group. All dogs underwent CPB; except for control group without medicine, CPB myocardial IR model were established in other groups, and perfused with St.Thomas cardiac cardioplegia lipid no medicine or containing relevant drugs after main artery block. Coronary sinus blood and carotid artery blood samples were collected before bypass and after 15, 90 min reperfusion following 60 min myocardial ischemia. Left ventricular apical tissue was taken, and the uptake rate of myocardial glucose and insulin resistance index (IRI) were determined and calculated; the changes of myocardial injury indexes (cTnT concentration) and cardiac function indexes (LVSP, +dp/dtmax) were monitored. The level of p-AMPK was detected. RESULTS: There was no statistical significance in above indexes of dogs before bypass (P>0.05). Compared with control group, the rate of myocardial glucose uptake, the levels of LVSP, +dp/dtmax and p-AMPK in model group were decreased significantly after 15, 90 min reperfusion (P<0.05), and the concentrations of IRI and cTnT were increased significantly (P<0.05). Compared with model group, the rate of myocardial glucose uptake, LVSP, +dp/dtmax and p-AMPK were increased significantly in APN group and AMPK inhibitor group (P<0.05), while the concentrations of IRI and cTnT were decreased significantly (P<0.05); moreover, the effect of APN group was better than that of AMPK inhibitor group (P<0.05). CONCLUSIONS: APN can promote myocardial glucose uptake and metabolism, and contribute the recovery of cardiac function, the mechanism of which may be associated with increasing the activity of AMPK.
9.Genetic characteristics of mumps virus isolated from one outbreak in Henan province in 2016

Objective To analyze the genetic characteristics of mumps virus ( MuV) isolated from an outbreak in Henan province in 2016 and to provide reference data for the control of mumps in Henan prov-ince. Methods Swab specimens were collected from a mumps outbreak in Henan province in 2016. One virus strain was selected for whole genome sequencing after virus isolation and genotyping. MEGA7. 0 soft-ware was used to construct phylogenetic tree, to calculate P-distance and to analyze the characteristics of genes encoding small hydrophobic protein ( SH ) , fusion protein ( F ) and hemagglutinin neuraminidase ( HN) . Results Five swab specimens were collected in the outbreak and five MuV strains were successfully obtained. Phylogenetic analysis showed that all of them belonged to genotype F with a P-distance of 0. 047 (0. 046-0. 049 ) with the reference strain. The P-distance among the five MuV strains was 0. 001 ( 0-0. 003). Results of the whole genome sequencing of one MuV strain showed that the P-distance with the ref-erence virus was 0. 053 (0. 018-0. 072) and the smallest P-distance with genotype F was 0. 018. Compared with the vaccine strain (HQ416907. 1), the amino acid at 28-30 site of SH protein was IML. The 91, 195 and 383 amino acid sites in F protein were related to the pathogenicity of MuV, but no mutations were found in these sites. Some mutations were found in HN protein, such as P354Q, E356D and K464N, which were associated with the immunological characteristics of MuV. Conclusions MuV strains isolated from the out-break in Henan province in 2016 belonged to genotype F. The genetic characteristics of these MuV strains were similar to those prevalent in other provinces of China.
10. Analysis on the hemagglutinin genetic characteristic of 7 measles virus isolated in Henan Province in 2017

Objective:
Analyze the genetic characteristic of Hemagglutinin(H) gene of measles viruses isolated in Henan Province in 2017.
Methods:
Swab samples collected from 7 lab confirmed measles cases, and we got the measles virus by Vero/Slam inoculation. Fragment of H genes were amplified by reverse transcription polymerase chain reaction(RT-PCR), then the PCR products were sequenced and analyzed.
Results:
The age of the 7 measles confirmed cases were between 1 and 50 years old, and all of them were males. All the 7 measles viruses were identified as H1a genotype, and the average distance of the nucleotides and the amino acids was 0.005, respectively. Compared with the Shanghai-191/China-vaccine, there were some changes in isolated virus, such as 240th, 397th and 381st sites in the amino acid sequence.
Conclusion
The measles genotype which isolated in Henan Province in 2017 was H1a. There were some difference from Shanghai-191/China-vaccine in the nucleotides sequence of H gene, which suggested that it′s necessary to strengthen the monitor the variation of measles virus.
