1.Osteoperiosteal decortications combined with other fixation methods for nonunion:bone graft and implant management

BACKGROUND:Osteoperiosteal decortication can treat the bone nonunion effectively, promote healing of fracture, but its action principle and key operation are not very clear, and there is less research addressing the postoperative treatment. OBJECTIVE:To review research progress of osteoperiosteal decortication for treating nonunion. METHODS: China National Knowledge Infrastructure, Wanfang database and PubMed database were used to search the related articles. The retrieval key words were “osteoperiosteal decortication, stripping, bone nonunion, bone disunion, bone delayed union”. Osteoperiosteal decortications combined with other fixation methods to repair the healing of bone nonunion were compared and analyzed. Bone implantation and implant management were summarized during repair. RESULTS AND CONCLUTION:105 literatures were retrieved by computer. After primarily screening by reading titles and abstract, irrelevant and repeated literatures were excluded. Finaly, 45 articles were summarized. Action principle, indications and operating points of osteoperiosteal decortications for bone nonunion were summarized. Osteoperiosteal decortications combined with other different fixation methods for the healing of bone nonunion were compared so as to better serve the clinic. Simultaneously, the paper also explores the combination with bone implant and management of implant during repair. Osteoperiosteal decortication with its simplicity and effectiveness has been respected in the field of orthopedics. However, for special patients, such as patients combined with destructive bone disease, osteoporosis, nonunion of hand and foot short bones, the treatment with osteoperiosteal decortications needs further researches.
2.Familial testicular tumor (report of 2 cases)

Objective To study the biological and clinical features of familial testicular tumor. Methods 2 cases of familial testicular tumor were reviewed of which one was between father son and the other uncle nephew. Results Of the case between father son,the son was testicular teratoid cancer and father seminoma.Of the other case,both were testicular seminoma. Conclusions Familial testicular tumor presents specific clinical and biological features which help its diagnosis.
3.The application of hip joint traction device in arthroplasty for serious developmental dysplasia of the hip

Objective To evaluate the application value of hip joint traction device in treating serious developmental dysplasia of the hip. Methods From April 2006 to October 2008, 6 cases Crowe Ⅳ hip dysplasia and 1 case dysplasia of acetabulum in coxa vara underwent arthroplasty. There were 2 males and 5 females with an average age of 30 years (range, 12-45). All patients were admitted to the hospital for the distraction, which was 1.0-2.0 cm on the first day and 1 mm/d thereafter, until slowly pulling femur head to the real acetabulum. Changing to the skin distraction with 3 kg, arthroplasty was performed after pin sites healing. Results Seven femur heads of 5 cases and two greater trochanters of 1 case were pulled into the real acetabulum, average distraction length was 4.45 cm, and average time was 43 d, no nerve injury and infection of pin sites. Soft tissue release and femur shortening were not required in arthroplasty. One case accelerated pulling speed to 1.25 mm/d when left femur head was distracted to 6.0 cm, so that the partial left deep peroneal nerve injury happened after 12 h. Ending distraction, bilateral THA was undergone by stages and femur shortening 3 cm in operation, nerve injury restored after half year. All patients were followed up for average 2.5 years (range, 1.5-3.0). In all patients, two limb lengths were equal, middle gluteal muscle myodynamia improved about 1-2 grades. The mean Harris hip score improved from 56.2 to 92.2 one year after operation. Conclusion The hip joint traction device could pulled femoral head to the real acetabulum rotating center, as a result, femur shortening and soft tissue release were not required while performing arthroplasty, and through which wound and operative time were decreased, middle gluteal muscle function was improved. However, we should strictly pulled the femur head at 1 mm/d to avoid nerve injury.
4.The application of teaching method of grouping and numbering the experimental animals on the teaching of basic operative surgery

Teaching method of grouping and numbering the experimental animals refers to management of laboratory animals by marking them with numbers. In an experimental group, the same marked experimental animal is performed surgery operation by each group member. As a whole, each group member gets the same score according to the evaluation of operation performance of that group and the score is recorded as usual performance score. This method is simple and easy to implement. It is the optimizing of traditional basic operative surgery teaching methods which is conducted not only to improve medical students' sense of responsibility and animal protection awareness, to make full use of teaching resources and reduce waste, to cultivate medical students' team cooperation awareness, but also to deepen medical students' knowledge about surgical complications and improve the quality of teaching.
5.Ultrastructure changes after blocking hepatic perfusion using microbubble-enhanced non-focused ultrasound

Objective To investigate the effect of microbubble-enhanced non-focused ultrasound on liver uhrastructure during the treatment of hepatic trauma.Methods The model of hepatic trauma was established in 18 healthy New Zealand rabbits.The subjects were divided into trauma group,ultrasound group and ultrasound-microbubble group according to the random number table,with 6 rabbits each.Thicker region of the left hepatic lobe was treated by custom-made non-focused ultrasound for 5 min.Peak intensity (PI) was used to evaluate the blood reperfusion of the target region after treatment.Liver specimens were performed transmission electron microscope examination immediately and 24 h after treatment to analyze ultrastructure changes.Results PI ratio in ultrasound-microbubble group (15.1 ± 2.6) was significantly lower than that in trauma group (23.1 ± 1.1) and ultrasound group (23.4 ± 1.3) (P < 0.05).But the difference between trauma and ultrasound groups was insignificant (P > 0.05).Compared with trauma and ultrasound groups immediately after treatment,hepatic cells in ultrasoundmicrobubble group had obvious edema,sinusoids thinned,organelles arranged in disorder,mitochondrial edema was present,endothelia cells of interlobular hepatic artery,interlobular vein and bile canaliculi in the portal area damaged,and microvilli of bile canaliculi disappeared.Hepatic cells showed morphology of apoptosis 24 h after treatment.Conclusion Microbubble-enhanced non-focused ultrasound can be used to make rapid hemostasis by decreasing the blood perfusion,but it causes certain damage to the ultrastructure of hepatic cells and may induce apoptosis in the radiation zone and neighboring cells.
6.Hospital ship and medical exchanges overseas:an experience

During the Peace Friendship-2015and Harmony Misson-2015, the medical personnel aboard the hospital ship performed extensive medical exchanges , medical service shows ,and skill training many times and visited some local medical institutions .The exchanges involved war wound , seawater-immersion wound, drill wound, common disease, infectious disease .This paper gave an account of characteristics of and specific methods used in oversea medical exchanges . The experience from these visits and afterthoughts were presented in order to contribute to medical exchanges of this kind in the future.
7.Bioequivalence of Ambroxol Hydrochloride Orally Disintegrating Tablets in Healthy Volunteers

OBJECTIVE:To study the time-concentration curves of single dose of ambroxol hydrochloride orally integrating tablets in healthy volunteers and to evaluate its bioequivalence.METHODS:A single oral dose of 90mg ambroxol hydrochloride orally dissolving tablets and 90 mg ambroxol hydrochloride tablets were given to 18 healthy volunteers by a randomized crossover design.The concentrations of ambroxol hydrochloride in plasma were determined by HPLC.RESULTS:The main pharmacokinetic parameters of the test and references preparations of ambroxol hydrochloride were as follows:Cmax were(169.58?39.43)ng?mL-1 and(170.28?43.26)ng?mL-1;tmax were(1.6?0.5)h and(2.2?0.6)h;t1/2 were(6.77?2.04)h and(6.50?1.27)h;AUC0~24 were(1 131.26?289.36)ng?h?mL-1 and(1 191.54?270.17)ng?h?mL-1;AUC0~∞ were(1 215.27?306.56)ng?h?mL-1 and(1 281.44?291.51)ng?h?mL-1,respectively.The relative bioavailability of the test preparation was(95.5?15.6)%.CONCLUSION:The result demonstrated that the two preparatioins were bioequivalent in absorption degree,suggesting the rapid release characteristics of the preparation.
8.Effect of the intestinal ischemia/reperfusion injuries on paracrine patterns of BM-MSCs

Objective To explore the effect of the intestinal ischemia/reperfusion injury on paracrine patterns of BM-MSCs in order to provide evidence for BMSCs repair.Methods BMSCs were isolated,cultured,characterized,and stained.The rat intestine ischemia/reperfusion model (IRI) was established.Intestinal epithelial mucosal extracts were obtained either from IRI rat or normal rat.Those two type extracts were added into the culture medium,and BM-MSCs were cultured with the two conditioned medium respectively.The culture medium was collected at 0 h and 24 h after culturing with intestinal epithelial mucosal extracts.The levels of TGF-ot,VEGF,FGF and IGF-1 in the medium were assayed by ELISA.Results The contents of all the growth factors detected in cultured medium with intestinal extracts from IRI rats were significantly higher than in regular rats (P < 0.05).The levels of TGF-α,VEGF in culture medium containing IRI epithelium extracts were significantly higher than that in the medium containing normal intestinal epithelium extracts (P < 0.05)Conclusion BM-MSCs can secret TGF-α,VEGF,FGF,and IGF-1 in the presence of intestinal extracts from IRI rat in the culture.These factors might stimulate the differentiation and proliferation of intestinal epithelial cells through paracrine mechanism.
9.Problem and strategy of laparoscopic clinical practice in children

Many pediatric surgeons have little training in laparoscopic application.The major problem in clinical practice of laparoscopic technician is without regularly training,unsuitable indication,and ignoring peroperation management.so that there are much more serious complications.For these reasons,it is a prerequisite that surgon and anesthetist must be trained before clinical practice,and the qualification of admission should be strictly controlled.
10.Expression of Foxo3a in lumbar dorsal root ganglia after rat sciatic nerve injury

Objective To investigate the expression of Foxo3a in the lumbar dorsal root ganglia (DRG) after sciatic nerve injury in rats. Methods Forty-two adult rats were randomly divided into control group and experimental group. Rats in experimental group were subjected to sciatic nerve clamp. Expression and distribution of Foxo3a, growth associated protein (CAP) and axon regeneration in DRC were detected by Western blot and immunofluorescent staining. Results (1) After sciatic nerve injury, Foxo3a protein levels began to reduce at day 1, reached the lowest level at day 2 and then gradually rose to nomal level at four weeks after sciatic nerve injury. Proliferating cell nuclear antigen (PCNA) and GAP-43 were up-regulated from day 2, and the level of PCNA reached peak at day 7 after sciatic nerve injury in DRC. (2) Down-regulation of Foxo3a was observed predominantly in neurons and glial cells. PCNA mostly expressed in glial cells and hardly in neurons. Conclusion Down-regulation of Foxo3a in lumbar DRG may correlate with axonal regeneration and the proliferation of glial cells after sciatic nerve injury.
