1.Diagnosis and treatment of colonoscopic perforation

Colonoscopy has become the most commonly used method to the diagnosis and treatment of colorectal diseases at present.Colonoscopic perforation is considered the most serious complication with high mortality, especially in the patients with certain risk factors and iatrogenic factors, despite of its low incidence.According to the clinical features in the period of perioperative colonoscopy, early detection of colonoscopic perforation is a prerequisite for successful treatment and the key to lower mortality.So far in the treatment,some methods can be selected, including conservative treatment, open surgery, laparoscopic surgery and endoscopy folders, etc.Clinical judgement and analysis of disease should be specific and accurate, but not to be generalized.Doctors should adopt reasonable and effective treatment options according to clinical performance, the size of perforation,intestinal readiness, time of diagnosis, colon primary lesion and so on.
2.Effect of Taurine on proliferation of neural stem cell after cerebral ischemia in rats

Objective:To study the influence of Taurine on proliferation of neural stem cells after focal cerebral ischemia in rats and to explore the mechanism of taurine in treating ischemic brain injury.Methods:The model of middle cerebral artery occlusion was made by using opening ranium hot coagulation in 80 rats,who were divided randomly into normal group,sham operate group,model group and taurine group.Taurine was administrated once daily by i.p.in taurine group,and immunohistochemistry method was used to observe the number of Brdu positive cells on the 3rd,7th,14th and 21st day after ischemia.Results:There were Brdu positive cells in the cortex,hippocampal dentate gyrus(DG) and striatum at different time points after cerebral ischemia,and the number of Brdu positive cells in various brain regions increased significantly on the 7th day as(18.65 ?4.38),(25.63?3.58),(19.46 ?2.98) and having statistical significance(P
3.Clinical analysis of 16 cases with complete rectal prolapse undergoing Gant-Miwa and anal encircling procedure with Xiaozhiling injection

Objective To evaluate the efficacy of Gant-Miwa and anal encircling procedure with Xiaozhiling injection for complete rectal prolapse.Methods Clinical data of 16 patients with complete rectal prolapse undergoing Gant-Miwa and anal encircling procedure with Xiaozhiling injection between Feb.2011 and Nov.2014 were retrospectively analyzed.Results All patients were operated successufully and theirs prolapse symptons disappeared.Urinary retention was found in 3 cases,2 cases were observed a replase who were infected in the area of the anal operation,1 cases was found delayed healing of perianal incision,and 2 cases were happened constipation duing to the rectal stenosis.Complications wasn't happened such as bleeding,perianal abscess,anal fistula,and so on.Thirteen cases were followed-up of average 2.2 years and 2 cases were observed postoperative recurrance.Conclusions Gant-Miwa and anal encircling procedure with Xiaozhiling injection for the treatment of complete rectal prolapse is a procedure with minimally trauma,fewer complications and low recurrence rate,which is worth promoting after innovation and improvement.
4.Value of the procedure of percutaneous catheter drainage guided by ultrasound in treatment on deep perianal abscess

Objective To discuss the best method to treat deep perianal abscess and to investigate the clinical efficacy and value of the procedure of percutaneous catheter drainage guided by ultrasound in treatment.Ninteen patients with deep abscess underwent the procedure from Jan.2012 to May.2013.Methods Clinical data of 19 patients were analyzed retrospectively,including 16 male cases and 3 female cases.Results The average age is 39.8.All patients were cured without complications.The average length of stay is 7.8 days,a mean follow-up of 6.6 months after discharge,no patient with recurrence or fistula.Conclusion No specific performance of colorectal carcinoid,surgical resection is the most effective method of the treatment of colorectal carcinoid,the procedure should be individual treatment.However,the long term prognosis need to be explored in the future.
5.Effect of emblic leafflower fruit on total antioxidation and levels of malondialdehyde as well as endothelin in plasma in rabbits with atherosclerosis

BACKGROUND: Emblic leafflower fruit contains rich vitamin C, organic acid and mineral substance. It is indicated in modern research that emblic leaafflower fruit acts on preventing from cancer and anti-aging too. In recent years, more and more concerns have been paid to the researches on functional factor of emblic leafflower fruit and the development of the relevant functional food. It has been verified that emblic leafflower fruit acts on reducing lipid and anti-oxidation in vitro, but there are still lacks of enough basic researches to support it.OBJECTIVE: To probe into mechanism of emblid leafflower fruit on anti-oxidation and protection of vascular endothelial function in rabbits with hyperlipemia.DESIGN: Randomized controlled experimental study based on the experimental animals.SETTING: Atherosclerosis(As) experimental room in a university hospital.MATERIALS: Totally 24 normal NewZealand male rabbits, mass weighted (2.2±0.5) kg.METHODS: The experiment was performed in As Experimental Room of Beijing Anzhen Hospital from September 2001 to May 2002. Totally 24 New Zealand male rabbits were randomized into the control, the power of emblid leafflower fruit group(powder group)(4 g/kg day) and hypercholesterolemia model group (model group), 8 rabbits in each group. In both power and model groups, the rabbits were fed with hypercholesterol food. Oxidase method was applied to assay the content of serum lipid, chemical method to assay the plasma total anti-oxidation and the content of malondialdehyde (MDA), radio-immune method to assay the content of plasma endothelin-1 (ET-1), digoxin labeled probe and tissue hybridism in situ to assay the expression of endothelin mRNA of aortic tunica intima, and image analyzer to assay the area of aortic intimal atherosclerosis plaque and the ratio between the area of tunica intima and the area of tunica media.MAIN OUTCOME MEASURES: The primary outcomes included blood lipid level, serum MDA concentration and the comparison of artery plaque areas. The secondary outcomes included the changes in plasma ET-1.RESULTS: By the comparison between the powder group and model group in triglyceride(TG) and low density lipoprotein cholesterol(LDL-C) were significantly reduced [(11.70 ± 1.73), (14.32 ±2.22) mmol/L, P<0.05; (0.740 ± 0.107), (1.450 ± 0.220) mmol/L, P <0.01 successively]and thehigh density lipoprotein cholesterol(HDL-C) was increased MDA was significantly decreased [ (3.88 ± 0.51 ), (6. 29 ± 1.43 ) mmol / L,P < 0.01 ] and the total anti-oxidation significantly increased [ (10. 771der photo-microscope that the plaque area and the ratio between intima area and media area were remarkably decreased [(39.46±6.53), (50.69± 12.36)mmol/L, P < 0.05; (0. 62 ±0. 32), (1.38 ±0.38) mmol/L, Pgranules of aortic intima ET-1 gene were rem~kably decreased compared with the model group.CONCLUSION: Emblic leafflower fruit prevents from the formation of experimental atheromatous plaque in rabbits probably by regulating rabbit lipid metabolism, improving anti-oxidation to reduce lipid peroxidation and protecting endothelial function to inhibit the expression of artery intima ET-1 gene.
6.Effects of hydrogen gas on lipopolysaccharide-induced apoptosis in human umbilical vein endothelial cells in vitro

Objective To investigate the effect of hydrogen gas on lipopolysaccharide (LPS)-induced apoptosis in human umbilical vein endothelial cells (HUVECs) in vitro.Methods HUVEC-12 cells were seeded in 96-well plates with a density of 1 × 104/ml (200 μl/hole) or in 6-well plates (2 ml/hole) with a density of 1 × 106/ml and randomly divided into 4 groups (n =30 each):control group (group C),hydrogen gas (H2) group,LPS group and LPS + H2 group.The cells were cultured in the plain culture medium in groups C and LPS or in hydrogen-saturated culture medium in groups H2 and LPS + H2.In addition,LPS 1 μg/ml was added simultaneously in groups LPS and LPS + H2 and the equal volume of normal saline was added instead in groups C and H2.The cell viability and apoptosis were measured by MTT assay and flow cytometry respectively after 24 h incubation.The concentration of high-mobility group box 1 (HMGB1) in the supernatant was determined by ELISA.Results Compared with groups C and H2,the cell viability was significantly decreased,and the apoptotic rate and concentration of HMGB1 in the supernatant were significantly increased in groups LPS and LPS + H2 (P < 0.05).Compared with group LPS,the cell viability was significantly increased,and the apoptotic rate and concentration of HMGB1 in the supernatant were significantly decreased in group LPS + H2 (P < 0.05).Conclusion Hhydrogen gas can effectively reduce LPS-induced apoptosis in HUVECs through inhibiting the release of HMGB1.
7.Effects of electro-acupuncture at Zusanli and Feishu on endotoxin shock-induced acute lung injury in rabbits

Objective To evaluate the effect of electro-acupuncture (EA) at Zusanli and Feishu on endotoxin shock-induced acute lung injury in rabbits.Methods Sixty healthy male New Zealand white rabbits aged 2 months weighing 1.5-2.0 kg were randomly divided into 6 groups (n =10 each):group sham operation (group S); group zinc protoporphyrin-Ⅸ (ZnPP-Ⅸ) (group Z); group lipopolysaccharide (LPS) (group L); group LPS + EA (group EL) ; group LPS + sham EA (group SEL) and group LPS + EA + ZnPP-Ⅸ (group ELZ).The animals were anesthetized with intraperitoneal 10% chloral hydrate 400 mg/kg and tracheostomized.The animals kept spontaneous breathing.Right internal carotid artery was cannulated for BP monitoring.Ear vein was cannulated for drug administration.LPS 5 mg/kg was injected iv in groups L,EL,SEL,ELZ.Endotoxin shock was confirmed by decrease in BP by 20 % of the baseline value and PaO2/FiO2 ≤ 300.ZnPP-Ⅸ (heme oxygenase (HO-1 ) inhibitor)10μmol/kg was injected intraperitoneal at 2 h after LPS injection in groups Z and ELZ.Bilateral 15 min EA stimulation of Zusanli and Feishu ( according to atlas of animal acu-points) was performed once a day for 5 days before LPS administration in groups EL and ELZ.The animals were sacrificed by blood-letting at 6 h after LPS administration.The lungs were removed for microscopic examination (0 =no injury,4 =most severe injury),detection of alveolar epithelial cell apoptosis (by TUNEL) and determination of HO-1 protein and mRNA expression.Results LPS significantly increased lung injury scores,alveolar epithelial cell apoptosis index (the number of apoptotic cells/total cells) and HO-1 protein and mRNA expression.EA significantly attenuated lung injury and alveolar epithelial cell apoptosis induced by LPS and further increased the expression of HO-1 protein and mRNA in group EL as compared with group L.The protective effects of EA was counteracted by ZnPP- Ⅸ in group ELZ.Conclusion EA at Zusanli and Feishu can attenuate endotoxin shock-induced lung injury by up-regulation of HO-1 expression and inhibiting alveolar epithelial cell apoptosis in the lung.
8.Effect of c-AMP-protein kinase A on up-regulation of heme oxygenase-1 expresion during lipopolysac-charide-induced acute lung injury in rats

Objective To evaluate the role of c-AMP-protein kinase A (cAMP-PKA) on the up-regulation of heme oxygenase-1 (HO-1) expression during lipopolysaccharide (LPS)-induced acute lung injury (ALI) in rats.Methods Forty-eight healthy male Sprague-Dawley rats,weighing 180-220 g,aged 2.5-3.0 months,were randomly divided into 4 groups (n =12 each)∶ normal control group (group C),ALI group (group ALI),H89 +ALI group (group H + ALI) and H89 group (group H).In group C,normal saline (solvent for LPS) 0.5 ml was injected via the femoral vein and normal saline (solvent for H89) 0.5 ml was injected subcutaneously 2 h later.In group ALI,10 mg/kg LPS 0.5 ml was injected via the femoral vein and normal saline 0.5 ml was injected subcutaneously 2 h later.In group H +ALI,10 mg/kg LPS 0.5 ml was injected via the femoral vein and 5 mg/kg H89 0.5ml was injected subcutaneously 2 h later.In group H,normal saline 0.5 ml was injected via the femoral vein and 5 mg/kg H89 0.5 ml was injected subcutaneously 2 h later.The rats were then sacrificed at 6 h after iv injection of LPS and the lungs were removed for microscopic examination and lung water content.The pathological changes of the lung were scored.The expression of HO-1 and PKA (by Western blot) and HO-1 mRNA (by RT-PCR) was detected.Results Compared with group C,the pathological score and lung water content were significantly increased,and the expression of HO-1,PKA and HO-1 mRNA was up-regulated in groups ALI and H +ALI (P <0.05),and no significant change was found in the parameters mentioned above in group H (P > 0.05).The pathological score and lung water content were significantly higher,and the expression of HO-1,AP-1 and HO-1 mRNA was significantly lower in group H + ALI than in group ALI (P < 0.05).Conclusion Activation of signaling pathway c-AMP-PKA is involved in the up-regulation of HO-1 expression during LPS-induced ALI in rats.
9.Preparation and characterization of multi-functional targeting nano-carrier based on pH-redox tumor microenvironment

In order to construct a novel multifunctional targeting nano-carrier based on pH-redox characteristics of tumor microenvironment,ketone bonds and disulfide bonds were bonded to the oligomeric hyaluronic acid (oHA) being sensitive to pH and the reduction environment.The chemical structure of oligomeric hyaluronic acid-8-mercaptomenthone 1,2-glycerolketal (oHMST) was characterized by 1H NMR,IR and ESI-MS.Curcuminloaded micelles were prepared by dialysis.The single factor investigation was carried out on the dosage form.Some properties,including particle size zeta potential,the morphology of micelles,and pH-sensitivity were studied.The materials were synthesized successfully.The micelles were spheric with a diameter of about 100 nm.The Zeta potential of the micelles was-(21.97 ± 1.08) mV.The in vitro test showed that oHMST carriers have good pH-sensitivity and redox-sensitivity.
10.Preparation and evaluation of in situ film of compound iodine oral spray for promoting oral ulcer repair

In this study, a new in situ film of compound iodine oral spray was prepared by in situ gel technology, which was used to exert sustained-release effect for promoting the repair of oral ulcer wounds. Firstly, the formulation and process of the spray solution were optimized according to the spray state and film-forming time. The drug-liquid mixing ratio was evaluated by film-forming time and drug film adhesion. The drug content, stability, pH, and spraying effect of compound iodine oral spray prepared by the optimal formulation were investigated; and the physicochemical properties, including film formation time, solubility, hygroscopicity, moisture retention and in vitro release of drug film were evaluated. In addition, the biocompatibility of the film-forming materials and proliferation ability of drug film were investigated by cell experiment. Through the rabbit oral ulcer model, the in vivo film-forming and repair-promoting effects of compound iodine oral spray were evaluated. The results showed that the pH of liquid A and liquid B prepared were 6.21±0.02 and 6.42±0.03, respectively, which were in line with the normal pH range of oral mucosa; liquid A and liquid B had good stability and spray state; the iodine content in solution B was (1.96±0.01) mg/mL; the in situ membrane formation time in vitro and in the oral cavity were (118.3±3.6) s and (133.3±4.6) s, respectively; the 24-hour dissolution rate was (87.31±1.74)%, the moisture absorption rate was (124.17±7.13)%, and the moisture retention rate was (26.85±2.50)%; the iodine content in the oral spray was (47.42±0.39) mg/g, with good flexibility and adhesion, as well as some slow-release effect. In cell experiment, the film-forming materials showed good biocompatibility and growth promotion ability. The results of the rabbit oral ulcer experiment showed that the compound iodine oral spray could rapidly form a film in vivo and significantly promote the repair of oral ulcer. In conclusion, the compound iodine oral spray in situ film with a stable preparation process can effectively promote the repair of oral ulcer wounds, which provides a new idea for the research of novel oral mucosa formulations, with a good prospect of transfer.
