1.The effect of rosuvastatin on micro-inflammatory and serum lipids in hemodialysis patients

Objective To investigate the effect of Rosuvastatin on micro-inflammatory and serum lipids in hemodialysis patients. Method A total of 100 hemodialysis patients enrolled in our hospital from April 2014 to April 2016 was divided into control and treatment group. The control group received regular hemodialysis. The treat-ment group received regular hemodialysis combined with rosuvastatin. The general information ,micro inflammatory index ,serum lipids and clinical symptoms were compared between the two groups. There was no significant dif-ference between the two groups in terms of age,sexual,time of hemodialysis,primary disease,ultrafiltration vol-ume,MQSGA score,BUN,Scr,TC and TG. There was no significant difference between these two groups in terms of CRP ,IL-6 ,TNF-α,TC ,LDL ,TG before the treatment. The treatment group was significantly lower than that of control group ,in terms of CRP ,IL-6 ,TNF-α,TG ,LDL ,TC and BUA ,after treatment. In the 3-month treatment,50 patients in control and treatment group appears weakness:4 vs. 0(8%vs. 0%,P=0.041);fatigue symptom:7 vs. 1(14%vs. 2%,P=0.027);muscle spasm:8 vs. 2(16%vs. 4%,P=0.045);hypotension:5 vs. 0(4% vs. 2%,P = 0.021). Treatment group was remarkably lower than control group(P < 0.05). Conclusion Rosuvastatin can effectively alleviate the micro-inflammatory state of remission ,reduce serum lipid and uric acid in hemodialysis patients ,and improve the complications in hemodialysis.
2.Application of Team Situational Simulation Education and Teaching Mode in Clinical Pharmacy Teaching

OBJECTIVE:To explore th e applicatio n of team situatio nal simulation education and teaching mode in clinical pharmacy teaching. METHODS :A total of 60 clinical pharmacy interns were selected as the research objects ,and course disease was type 2 diabetes mellitus. Thirty interns were randomly selected as control group ,using traditional teaching mode ;other 30 interns were selected as trial group ,which carried out team situational simulation education and teaching mode. The teaching effects were evaluated by using the satisfaction of interns to the two modes ,the comprehensive score of graduation examination and the self-evaluation of learning effect. RESULTS :Compared with traditional teaching mode ,team situational simulation education and teaching mode was conducive to stimulate the learning interest of interns ,improve their interpersonal communication ability , cultivate teamwork spirit ,improve the awareness of humanistic care ,and cultivate the professional attitude of clinical pharmacists (P<0.05). Compared with control group ,the comprehensive score of trial group was dominantly increased (P<0.001),and the scores of professional quality ,humanistic care and communication skills in the trial group were significantly higher than control group(P<0.01). In terms of self-evaluation of learning effect ,except for the pathogenesis of type 2 diabetes and the commonly used treatment regimens ,the self-evaluation scores of the other items in trial group were significantly higher than control group (P<0.05 or P<0.01). CONCLUSIONS :Team situational simulation education and teaching mode is superior to traditional teaching mode for clinical pharmacy teaching.
3.Association of serum L-kynurenine with nutritional status and vascular stiffness in maintenance hemodialysis patients

Objective To study the association of serum L-kynurenine changes with nutritional status and vascular stiffness in maintenance hemodialysis (MHD) patients. Methods Twenty gender- and age-matched healthy volunteers (healthy group) and 40 MHD patients,including 20 cases with α-keto aicd(α-keto acid group)and 20 cases without α-keto aicd(non-α-keto acid group)were enrolled in the study.Serum L-kynurenine was measured by high performance liquid chromatography.C-reactive protein (CRP)and interleukin 6 (IL-6)were detected.Subjective global assessment(SGA)and malnutrition inflammation score(MIS)were applied to evaluate the nutritional status.Pulse wave velocity(PWV)was used to evaluate arterial stiffness for both groups of MHD patients. Results Serum L-kynurenine was significantly higher in MHD patients than that in healthy subjects[(3.20±1.12)μmol/L vs (1.74±0.27)μmol/L,P<0.01],while such difference was not found between α-keto aicd group and non-α-keto-aicd group [(3.20±0.88)μmol/L vs (3.29±1.34)μmol/L,P>0.05].IL-6 was significantly higher in MHD patients as compared to healthy subjects[(6.45±3.78)ng/L vs(1.38±1.59)ng/L,P<0.01],while such difference was found between α-keto aicd group and non-α-keto aicd group[(3.37±0.82)ng/L vs (9.62±2.48)ng/L,P<0.051.There was no difference of CRP concentration between two MHD groups.As compared to non-α-keto acid group,higher SGA score(26.00±1.75 vs 22.67±2.61,P=0.001),lower MIS score(5.82±2.27 vs 10.00±2.62,P=0.002),lower left side PWV[(21.11±8.21)m/s vs(24.57±5.45)m/s,P=0.244]and lower right side PWV[(19.27±3.22)m/s vs (24.19±5.41)m/s,P=0.015]were observed in α-keto aicd group.Pearson analysis showed positive correlation between serum L-kynurenine and IL-6(r=0.352,P=0.011)and negative correlation between L-kynurenine and pre-dialysis Scr(r=-0.412,P=0.019). Conclusions Inflammation is common in MHD patients.Tryptophan degeneration product L-kynurenine may indicate inflammation status.α-keto acid improves nutritional status,anemia and arterial stiffness maybe through the alleviation of inflammation in MHD patients.

Four compounds were isolated from the lateral roots of Aconitum carmichaeli by Al2O3,silica gel and preparative HPLC.Their structures were identified as:(13R,15S,19S)-13,15,19-triol-hetisan (1),fuziline (2),neoline (3) and beiwutine (4),by methods of NMR,MS spectroscopy as well as single crystal X-ray crystallography.Among them,compound 1 was considered as a new diterpenoid alkaloid.

Objective:
To observe the effect of poloxamer 188 (P188) on megakaryocyte cultivation and induction from cord blood mononuclear cells in order to obtain more megakaryocyte progenitor cells (MPC).
Methods:
The cord blood mononuclear cells were isolated and inoculated in cell culture bag or cell culture flask respectively. The WIGGENS shaker and cell culture bags were used to mimick WAVE Bioreactor for three-dimensional (3D) cell culture, and the P188 was added to induction medium, The cells were detected for morphology, surface marker, viability, and number on day 14.

【Objective】 To investigate the diagnostic value of neutrophil/lymphocyte ratio (NLR), monocyte/lymphocyte (MLR), and lymphocyte (LYM) count in children with infectious mononucleosis (IM). 【Methods】 Clinical and laboratory data of 134 IM children hospitalized between September 2017 and March 2020 were collected. A total of 60 healthy children during the same period were selected as the control group. The subjects’ NLR, MLR, and LYM were calculated according to the results of peripheral blood. The values of NLR, MLR, and LYM in diagnosing and predicting IM were analyzed with ROC. 【Results】 The MLR and LYM values of IM children were significantly higher than those of the control group (P<0.01), while the NLR values of the two groups were not statistically significant (P>0.05). ROC results showed that the area under the ROC curve (AUC) of NLR was 0.397, AUC of MLR was 0.648, and LYM's AUC was 0.680. The ROC curve AUC of MLR+LYM was 0.878, the optimal threshold was 0.59, the sensitivity was 82.09%, and the specificity was 85.00%. 【Conclusion】 MLR and LYM are independent biomarkers for the prediction of IM in children.
7.Rapid Identification and Subtyping of Enterobacter cloacae Clinical Isolates Using Peptide Mass Fingerprinting.

OBJECTIVETo establish a domestic database of Enterobacteria cloacae (E. cloacae), and improve the identification efficiency using peptide mass fingerprinting.
METHODSPeptide mass fingerprinting was used for the identification and subtyping of E. cloacae. Eighty-seven strains, identified based on hsp60 genotyping, were used to construct and evaluate a new reference database.
RESULTSCompared with the original reference database, the identification efficiency and accuracy of the new reference database was greatly improved at the species level. The first super reference database for E. cloacae identification was also constructed and evaluated. Based on the super reference database and the main spectra projection dendrogram, E. cloacae strains were divided into two clades.
CONCLUSIONPeptide mass fingerprinting is a powerful method to identify and subtype E. cloacae, and the use of this method will allow us to obtain more information to understand the heterogeneous organism E. cloacae.

Nonalcoholic fatty liver disease (NAFLD) is characterized by hepatic steatosis and insulin resistance and there are currently no approved drugs for its treatment. Hyperactivation of mTOR complex 1 (mTORC1) and subsequent impairment of the transcription factor EB (TFEB)-mediated autophagy-lysosomal pathway (ALP) are implicated in the development of NAFLD. Accordingly, agents that augment hepatic TFEB transcriptional activity may have therapeutic potential against NAFLD. The objective of this study was to investigate the effects of nuciferine, a major active component from lotus leaf, on NAFLD and its underlying mechanism of action. Here we show that nuciferine activated ALP and alleviated steatosis, insulin resistance in the livers of NAFLD mice and palmitic acid-challenged hepatocytes in a TFEB-dependent manner. Mechanistic investigation revealed that nuciferine interacts with the Ragulator subunit hepatitis B X-interacting protein and impairs the interaction of the Ragulator complex with Rag GTPases, thereby suppressing lysosomal localization and activity of mTORC1, which activates TFEB-mediated ALP and further ameliorates hepatic steatosis and insulin resistance. Our present results indicate that nuciferine may be a potential agent for treating NAFLD and that regulation of the mTORC1-TFEB-ALP axis could represent a novel pharmacological strategy to combat NAFLD.

During coronavirus disease 2019 epidemic, the treatment of severe trauma has been impacted. The Consensus on emergency surgery and infection prevention and control for severe trauma patients with 2019 novel corona virus pneumonia was published online on February 12, 2020, providing a strong guidance for the emergency treatment of severe trauma and the self-protection of medical staffs in the early stage of the epidemic. With the Joint Prevention and Control Mechanism of the State Council renaming "novel coronavirus pneumonia" to "novel coronavirus infection" and the infection being managed with measures against class B infectious diseases since January 8, 2023, the consensus published in 2020 is no longer applicable to the emergency treatment of severe trauma in the new stage of epidemic prevention and control. In this context, led by the Chinese Traumatology Association, Chinese Trauma Surgeon Association, Trauma Medicine Branch of Chinese International Exchange and Promotive Association for Medical and Health Care, and Editorial Board of Chinese Journal of Traumatology, the Chinese expert consensus on emergency surgery for severe trauma and infection prevention during coronavirus disease 2019 epidemic ( version 2023) is formulated to ensure the effectiveness and safety in the treatment of severe trauma in the new stage. Based on the policy of the Joint Prevention and Control Mechanism of the State Council and by using evidence-based medical evidence as well as Delphi expert consultation and voting, 16 recommendations are put forward from the four aspects of the related definitions, infection prevention, preoperative assessment and preparation, emergency operation and postoperative management, hoping to provide a reference for severe trauma care in the new stage of the epidemic prevention and control.
