1.The modification research of bionic silk fibroin nerve guidance conduits by silk sericin

Objective To explore the modification of bionic silk fibroin nerve conduits (SF-NCs) by silk sencin.Methods The innovative SS/SF blended-NCs was fabricated by a vertical sequential cooling thermal induced phase separation (TIPS) processing with SF solution added sericin in proportion,its morphology was observed by Scanning electron microscopy (SEM),X-ray diffraction (XRD) and infrared spectroscopy (FTIR) were used to detect its internal molecular structure.MTT assay was used to quantitatively analyzed the PC12 cells viability co-cultured with the innovative SS/SF-NCs,SEM was used to observe the adhesion and morphology of PC12 cells seeded into the innovative SS/SF,PC12 cells were used to assess the NGF bioactivity released from the SS/SF.Results The SEM results showed that the new fabricated SS/SF-NCs had linearly oriented lamellar-like multiple-channel which distributed evenly,got great changes on the channel microstructure and their mechanical properties had been greatly improved,compared to SF-NCs.The XRD and FTIR results showed that the SS/SF-NCs had the similar internal molecular structure with natural silk.The spaces between parallel lamellar-like channels,porosities and compressive strengths of the SS/SF-NCs decreased with decreasing Sequential freezing temperature.MTT assay results showed that the viability of PC12 cell was better than the control group (P < 0.05).The SEM observation indicated that PC12 cells showed good adhesion and differentiation with neuritis outgrowth during the period of co-culture with the SS/SF-NCs.NGF release from the innovative SS/SF-NCs was prolonged over 4 weeks,and remained bioactive.Conclusion The new fabricated SS/SF-NCs modified though silk sericin,which was highly bionic the structure of peripheral nerve fasciculus,had excellent mechanical properties and could be used as another alternative of artificial nerve conduits.
2.Effects of carvedilol at different doses on cardiac myocyte apoptosis and apoptosis-associated gene in rats with pressure overload

AIM: To study the effects and mechanism of carvedilol (CAR) at different doses on cardiac myocyte apoptosis in rats with myocardial hypertrophy of CHF. METHODS: Using the animal model of CHF, induced by abdominal aortic constriction in male Wistar rats, hemodyanmics parameters, cardiac myocyte apoptosis, and the expression of Bcl-2 and P53 were investigated in the untreated experimental group (CHF group) at 1 week after operation and treated experimental groups in which rats were treated with CAR at a lower dose (LCAR group, 0.1 mg?kg -1?d -1) and at a higher dose (HCAR group, 10 mg?kg -1?d -1) for 7 weeks since 1 week after operation. The sham-operated rats were as controls (SH group, n=8). RESULTS: Either doses improved heart function and decreased apoptosis index in rats with CHF. The number of myocytes apoptosis and the level of Bcl-2 were significantly higher and P53 was markedly lower in HCAR group than those in LCAR and MCAR groups. CONCLUSION: CAR can effectively decrease myocardiocyte apoptosis, prevent and cure CHF. The effects are dose-dependent, associated with changes of the expression of apoptosis-associated gene.
3.Expression and significance of P21 and CDK6 protein in cervical squamous cell carcinoma

Objective To observe the expression of P21 and CDK6 in cervical squamous cell carcinoma,and to investigate the relationship between their expression and cervical squamous cell carcinoma.Methods The expression of CDK6 and P21 in 100 cases of cervical squamous cell carcinoma,20 cases of cervical CIN lesions and 20 cases of normal cervical tissues were detected by immunohistochemical ABC method,and the relationship between them and tumor differentiation,invasion depth,lymph node metastasis and clinical stage were analyzed.Results The expression rates of P21 and CDK6 in cervical squamous cell carcinoma tissues were significantly higher than those in CIN lesions and normal cervical tissues;The low expression of P21 was associated with the depth of tumor invasion,lymph node metastasis and clinical stage (P<0.05),and was not associated with the degree of tumor differentiation (P>0.05);The high expression of CDK6 was associated with tumor differentiation,invasion depth,lymph node metastasis and clinical stage (P<0.05).Conclusion The abnormal expression of P21 and CDK6 may play an important role in the occurrence and development of cervical squamous cell carcinoma,and the two may have a certain significance in the prognosis of cervical squamous cell carcinoma.
4. The efficacy of internet-based cognitive behavioral therapy for insomnia

Objective To evaluate the effectiveness of internet-based cognitive behavioral therapy (ICBT) for the treatment of insomnia by comparison of sleep parameters, degrees of anxiety and depression of the ICBT, with traditional face-to-face cognitive behavioral therapy (CBT) and pharmacotherapy for insomnia. Methods Seventy-nine cases meeting proposed DSM-5 criteria for insomnia disorder were randomly assigned to ICBT (n=27), CBT (n=26), and pharmacotherapy (n=26) group, and treated accordingly for 8 consecutive weeks. The sleep parameters, the levels of anxiety and depression in the 3 groups were compared and analyzed before, 4 weeks after and the termination of treatment. Results Comparing to that of pre-treatment, the sleep parameters were significantly improved, anxiety and depression levels obviously decreased after treatment for 4 and 8 consecutive weeks, the differences were statistically significant (P<0.05). After treatment for 4 consecutive weeks, the sleep latency, total asleep time and wake time after sleep were significantly different (P<0.05) when compared with pharmacotherapy group with ICBT and CBT groups. After the treatment, the sleep latency, anxiety and depression levels were lower in ICBT and CBT groups than those in pharmacotherapy group, and the difference was statistically significant (P<0.05). In addition, no significant difference (P>0.05) was found in sleep parameters and anxiety level between ICBT group and CBT group. Conclusion ICBT may display a slower effect on improving speed in falling asleep than the pharmacotherapy does, but the efficacy of ICBT is better than that of pharmacotherapy after treatment, and there is no significant difference compared to traditional face-to-face CBT.
5.Therapeutic effect of phacoemulsification for complicated cataract seco ndary to uveitis

Objective To evaluate the therapeutic effect of phacoemulsification for complicated cataract secondary to uveitis. Metho ds Twenty-one eyes of 19 patients with complicated cataract secondary to uveitis were emulsified in situ frature technique after blunt separation of iris posterior synechiae, excision of organized membrane with scissors and st retching pupil. Results The visual acuities with correction on e week after operation was≥ 0.5 in 16 eyes (76.2%), and ≥0.2 in 18 eyes (85.7% ). The major complications were iris damage and corneal edema. No rupture of pos terior capsule and corneal decompensation were found. After operation, a round p upil was obtained in 19 eyes (90.5%) and an irregular pupil in 2 eyes (9.5%).Conclusion This operation is suitable for complicated cataract se condary to uveitis, and worth to popularize due to the minimization of iris dam age and postoperative inflammation in the anterior chamber, good visual acuity r ehabilitation earlier.
6.Changes in sleep and psychological characteristics of patients with insomnia under exposure to noise pollution

Objective Exposure to nocturnal traffic noise at the same levels can produce different effects on sleep and psychological health in different populations. This study compared the electroencephalographic characteristics during sleep of people with insomnia and those with normal sleep quality under the same levels of nocturnal traffic noise exposure to elucidate the different effects of noise pollution on different populations,and to provide individualized precise treatment plans for people with insomnia who are sensitive to noise pollution. Methods We selected people who lived near highways,railways,and aircraft routes for more than six months where noise levels were relatively fixed(50-60 decibels,10-15 seconds per time) during the nighttime sleep period (22:00-6:00). We included 26 people with normal sleep quality and 30 people with insomnia based on the Pittsburgh Sleep Quality Index. The Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) were administered. Polysomnography (PSG) was used to monitor the spindle density index during sleep. The sleep quality and psychological status of the two groups were compared. Results There were no significant differences in demographic characteristics between the two groups under the same nocturnal noise exposure conditions. The incidence of anxiety and depression was significantly lower in the people with normal sleep quality than in those with insomnia(P<0.01). Among the 26 people with normal sleep quality,5(19.23%) had mild anxiety,and no one had moderate to severe anxiety or depression. Among the 30 people with insomnia,6(20.00%) had mild anxiety,15 (50.00%) had moderate anxiety,and 4(13.33%) had severe anxiety;3(10.00%) had mild depression,9(30.00%) had moderate depression,and 3(10%) had severe depression. PSG showed that the sleep spindle index of the normal sleep group was significantly higher than that of the insomnia group(25.43 vs 16.60,P<0.05),with no significant differences in the average duration of sleep spindles during sleep between the two groups (P>0.05). Conclusion Under the same levels of nocturnal traffic noise exposure,people with insomnia have a higher incidence of anxiety and depression than people with normal sleep quality. Patients with insomnia with a reduced spindle density index have worse sleep quality under exposure to noise pollution.
7.Efficacy of Brief Behavioral Therapy for Insomnia (BBT-I) in patients suffering Paradoxical insomnia

To explore the effect of BBT-I on subjective insomnia and its influencing factors. Methods 60 primary insomnia patients were collected in outpatient department,and divided into two groups:Subjective insomnia group (SI group) and non-subjective insomnia group (NSI group) by sleep data collected by standard polysomnography monitoring and Pittsburgh Sleep Quality Index (PSQI),sleep diaries during the intervention were collected,and the Pre Sleep Arousal Scale(PSAS),Brief Version of Dysfunctional Beliefs and Attitudes About Sleep (DBAS) and the Sleep Hygiene Practice Scale(SHPS) were collected to evaluate the influencing factors related to sleep attitudes,beliefs and behaviors of patients. All patients received BBT-I for 4 weeks,and collected the sleep diaries and PSQI 1 week before and 1 week after treatment. Results Compared with NSI group,SI group had higher arousal index(P<0.05);compared with NSI group,SI group had lower scores in factor 3 indicated as drug belief and factor 4 representing anticipation of sleep in DBAS,that is,there were more irrational beliefs in SI (P<0.05).Intragroup comparison showed that there were statistically significant differences insleep latency、sleep efficiency,daytime function and PSQI score in SI group before and after treatment (P<0.05),and there were statistically significant differences in SE difference before and after intervention between SI group and NSI group (P<0.05).In the subjective insomnia group,daytime functional difference before and after intervention was negatively correlated with the first factor in DBAS(r=0.270,P=0.037). daytime functional difference before and after intervention was negatively correlated with PSAS(r=0.268,P=0.039);The difference of sleep latency before and after intervention was negatively correlated with physiological stimulation of PSAS(r =0.288,P=0.042). Conclusion Compared with nonsubjective patients,subjective insomnia patients have higher arousals and more irrational beliefs and attitudes.BBT-I may improve sleep latency/、sleep efficiency,daytime function and sleep sensation in subjective insomnia patients.
8.Preliminary application and efficacy analysis of transumbilical single-incision plus one port robotic total mesorectal excision.

Objective: To investigate the feasibility of transumbilical single-incision plus one port (SIPOP) robotic total mesorectal excision. Methods: Clinical data of a 70-year-old male patient with BMI 22.1 kg/m(2) who successfully underwent transumbilical single-incision plus 1 port robotic total mesorectal resection of upper rectal cancer at the General Surgery Department of Daping Hospital of Army Military Medical University on September 18, 2019 were retrospectively analyzed. Preoperative colonoscopy revealed that the distance of upper rectal cancer to anal edge was 14 cm, and the tumor size was 2.5 cm×1.5 cm×1 cm. Pathological result confirmed rectal moderately differentiated adenocarcinoma. The preoperative abdominal CT showed thickened bowel-wall of upper rectum and the blurred perirectal fat, suggesting tumor infiltration. Results: The operation was successful. There were no conversion to laparotomy or abdominal auxiliary incision, and the mesorectum of the specimen was intact. The operation time was 165 minutes, the blood loss was about 20 ml, and there were no complications such as injury to peripheral organs. Postoperative pathology showed ulcerative moderately differentiated adenocarcinoma of the upper rectum with TNM stage IVA (T4N2b). The postoperative recovery was smooth. Patient ambulated on the 1st day, the catheter was removed on the 7th day, and discharged from the hospital on the 8th day. Conclusion: The transumbilical SIPOP robotic total mesorectal excision is safe, effective and feasible.

9.Rabbit lung injury induced by explosive decompression.

OBJECTIVE: To study the mechanism of rabbit lung injury caused by explosive decompression. METHODS: A total of 42 rabbits and 10 rats were served as the experimental animals. A slow recompression-decompression test and an explosive decompression test were applied to the animals, respectively. And the effects of the given tests on the animals were discussed. RESULTS: The slow recompression-decompression did not cause an obvious lung injury, but the explosive decompression did cause lung injuries in different degrees. The greater the decompression range was, the shorter the decompression duration was, and the heavier the lung injuries were. CONCLUSIONS: Explosive decompression can cause a similar lung injury as shock wave does. The primary mechanical causes of the lung injury might be a tensile strain or stress in the alveolar wall and the pulmonary surface's impacts on the inside wall of the chest.
10.Analysis of risk factors for arrhythmia in patients after heart valve replacement

Objective To explore and analyze the risk factors for arrhythmia in patients after heart valve replacement. Methods A retrospective analysis of 213 patients undergoing cardiac valve replacement surgery under cardiopulmonary bypass in our hospital from August 2017 to August 2019 was performed, including 97 males and 116 females, with an average age of 53.4±10.5 year and cardiac function classification (NYHA) grade of Ⅱ-Ⅳ. According to the occurrence of postoperative arrhythmia, the patients were divided into a non-postoperative arrhythmia group and a postoperative arrhythmia group. The clinical data of the two groups were compared, and the influencing factors for arrhythmia after heart valve replacement were analyzed by logistic regression analysis. Results There were 96 (45%) patients with new arrhythmia after heart valve replacement surgery, and the most common type of arrhythmia was atrial fibrillation (45 patients, 18.44%). Preoperative arrhythmia rate, atrial fibrillation operation rate, postoperative minimum blood potassium value, blood magnesium value in the postoperative arrhythmia group were significantly lower than those in the non-postoperative arrhythmia group (P<0.05); hypoxemia incidence, hyperglycemia incidence, acidosis incidence, fever incidence probability were significantly higher than those in the non-postoperative arrhythmia group (P<0.05). The independent risk factors for postoperative arrhythmia were the lowest postoperative serum potassium value (OR=0.305, 95%CI 0.114-0.817), serum magnesium value (OR=0.021, 95%CI 0.002-0.218), and hypoxemia (OR=2.490, 95%CI 1.045-5.930). Conclusion Taking precautions before surgery, improving hypoxemia after surgery, maintaining electrolyte balance and acid-base balance, monitoring blood sugar, detecting arrhythmia as soon as possible and dealing with it in time can shorten the ICU stay time, reduce the occurrence of complications, and improve the prognosis of patients.
