Objective:To explore the protective effect of curcumin on type 2 diabetic nephropathy rats and the possible mechanism.Methods:30 Wistar rats were randomly divided into control group, the DN group and curcumin group,10 cases in each group, rats in DN and curcumin group were fed with high fat, after that they were used the streptozotocin injection to set up DN model. In addition, rats in curcumin group were given 200 mg/kg curcumin once a day, other two group were given sodium carboxymethylcellulose half-and-half.After 12 weeks, the 24 h urine were collected at the last delivery, the blood glucose(BG),total cholesterol(TC),triglyceride(TG),blood urea nitrogen(BUN),serum creatinine(Scr),microalbu minuria(mAlb) were tested, the urinary albumin excretion rate(UAER) was calculated.Furthermore, rats were sacrificed, kidney mass weighed, and the kidney index(KI) was calculated.Renal cortical was fixed with polymers cresol for hematoxylin-eosin(HE) staining, renal cortical was also made renal cortex homogenate and the SOD activity,MDA were detected, the expression of nuclear factor related factor 2(Nrf2) and heme oxygenase 1(HO-1) protein in the kidney tissues was detected with Western blot detection. Results:BG,TC,TG,KI,BUN,Scr,UAER,MDA of DN group and curcumin group were significantly higher than control group, and BM,KM,SOD was significantly lower than the control group, the amplitude raising or reducing in curcumin group were less than DN group, the difference had statistical significance(P<0.05);HE staining results showed that, compared with DN group, renal tubule degeneration in curcumin group was obviously reduced, inflammatory cells infiltration was decreased significantly;Western blot results showed that expression of Nrf2 and HO-1 protein in control group were significantly lower than DN group and curcumin group, and curcumin group was significantly higher than the DN group, the differences were statistically significant(P<0.05).Conclusion:Curcumin may play a role of kidney protection through the activation of Nrf2/ARE signaling pathway to reduce oxidative stress in diabetic nephropathy rats.

Objective To investigate the value of multimodality MRI in diagnosis of breast lesions with ring enhancement (internal enhancement pattern) on dynamic contrast enhanced (DCE) MRI.Methods Data of 67 patients with breast lesions confirmed by surgical pathology who underwent DCE MRI and DWI examinations were analyzed retrospectively.Finally,67 patients with breast lesions manifested as ring enhancement on DCE MRI were included in the study.According to the pathological findings, all patients were divided into benign group and malignant group. The overall morphology, morphology of "enhanced ring",time-signal intensity curve (TIC) types on DCE MRI,the distribution location of limited diffusion on DWI between the two groups were compared. Also the differences in semi-quantitative parameters on DCE-MRI and multi-point ADC values of the "enhanced ring" between the two groups were compared.Using pathological results as the dependent variables, indexes with statistical differences as the independent variables, the Logistic regression model was established to construct the newly combined parameters and to evaluate the diagnostic efficacy of the stepwise combined parameter. Results There were significant differences of different wall shapes concerning "enhanced ring" (x2 =14.19,P=0.001),wall nodules (x2 =12.48,P=0.001) and distribution locations of limited diffusion on DWI (x2 =19.60,P=0.003) between the two groups. There were also significant differences of the semi-quantitative parameters including early enhancement ratio (EER;t=2.35,P=0.02),maximum enhancement time (Tmax;t=-4.66,P=0.007),ADC ringi (t=1.35,P=0.001) and ADC ring wall(t=-2.88,P=0.005)between the two groups. The maximum Youden index of a newly-constructed parameter combination(morphological indexes of "enhanced ring"+distribution locations of limited diffusion on DWI+Tmax+ADCring inner) was 0.83 for combined diagnosis. The area under ROC curve was 0.87 (95%CI [0.82,0.91]),and the diagnostic sensitivity and specificity was 91.96% and 91.31%,respectively.Conclusion Multimo dality MR can provide high diagnostic efficacy for breast lesions with ring enhancement

Objective:To observe the epidemiological characteristics and transmission chain of COVID-19 in Harbin, and to provide epidemiological evidence for improving the COVID-19 preventive measures and optimizing prevention and control strategies. Methods:The epidemic situation of COVID-19 in Harbin in January 2021 was analyzed by using the Infectious Disease Report Information Management System and the Public Health Emergency Management Information System of the China Disease Prevention and Control Information System, the epidemic situation information publicly released by the Heilongjiang Provincial Health Commission, and the epidemiological report of Heilongjiang Province Certer for Disease Control and Prevention and Harbin Center for Disease Control and Prevention. The main transmission chains were sorted out through combination of epidemiological field investigation, serological testing, gene sequencing, big data and other means. Results:From January 12 to February 4, 2021, 295 cases of COVID-19 infection (including confirmed cases and asymptomatic infections) were reported in Harbin, which affected 6 districts of Harbin and were concentrated in 41 of the 274 townships in the city. The sex ratio of male to female was 1.00∶1.12 (139∶156); the age ranged from 1 to 86 years old, and the median age was 45 years old. The proportion of confirmed cases and asymptomatic infection was 1.00 ∶ 1.02 (146 ∶ 149), and there was a significant difference in the distribution of different ages between them ( P = 0.042). The cases were mainly found through the health screening of the centralized isolation personnel (178 cases, 60.3%). Other detection methods included active screening (87 cases, 29.5%), screening of the home isolation personnel (26 cases, 8.8%), and medical treatment in medical institutions (4 cases, 1.4%). The main transmission chain of the outbreak was the case associated with a food processing enterprise, with a total of 259 cases, accounting for 87.8% of the total cases. The gene sequencing results showed that the case sequence was homologous with that of Wangkui County, Suihua City, Heilongjiang Province. Conclusions:A food processing enterprise is involved in the main transmission chain, which indicates that the epidemic prevention and control measures needs to be further optimized. Specifically, the supervision and management of food processing enterprises, cold chain storage companies and other enterprises should be strengthened. High attention should be paid to the hidden dangers of COVID-19 in large and medium sized enterprises with hermetic space in Harbin.
