1.Cloning and sequence analyzing of the recombinant plasmid affecting gene DNMT1 translation by RNA interfering

Objective:To clone the recombinant plasmid affecting gene DNMT1 translation by RNA interfering and analyze the nucleic acid sequence of the recombinant for further searching new gene therapy method of tumor.Methods:Two DNA sequences containing small hairpin structure were designed and synthesized.The complement form was obtained by annealing and inserted into vector pTZU6+1,and the recombinant plasmid was transformed into Jm109 strain.Finally the plasmid identified by restriction enzyme was used for sequence analysis.Results:The recombinant was cloned and the aim sequence was obtained.Conclusion:Successful cloning of the recombinant helps to search new gene therapy method of tumor.
2.RNA interfering on the cell cycle of gastric cancer by the recombinantplasmid affecting DNMT1 gene expression

Objective:To clone the recombinant plasmid affecting gene DNMT1 expression by RNA interfering and transfect it into AGS in order to further search its effects on the cell cycle of gastric cancer AGS.Methods:Two DNA sequences containing short hairpin structure was designed and synthesized.The complement form was obtained by annealing and inserted into vector pTZU6+1.The recombinant plasmid was transfected into AGS.Finally the cell cycle was evaluated through flow cytometer.Results:The S cycle cells of the transfected AGS dropped dramatically.And the G2-M cycle cells increased significantly.Conclusion:Successful cloning and transfecting the recombinant make it possible to search new gene therapy for the tumors.
3.How to improve medical students' capability of writing case reports

How to improve medical students’ capability of writing case reports and avoid the occurrence of some medical conflictions turns out to be a problem that has to be solved. This article makes a tentative exploration in this respect.
4.How to improve interns' ability of doctor-patient communication

Objective With the increasing number of medical disputes,it is urgent to train the interns'skills. Methods Interns improved their doctor-patient communication skills by enhancing awareness on the doctor-patient communication,learning a variety of medical laws and regulations,improving clinical skills,training good medical ethics to master some communication skills. Results Through such training,medical disputes due to interns never happened during the internship period. Conclusion Our training program is practicable.
5.The research about the relationship between colorectal cancer tumor staging and detection of positive CEA and CA 19-9

Objective To approach the correlation between preoperative detection of serumal carcinoembryonic antigen (CEA ) and carbohydrate antigen 19‐9(CA19‐9) and primary tumor/regional lymph nodes/distant metastasis(TNM) staging ,clinicopatho‐logic staging and clinical features of patients with colorectal cancer (CRC) ,and to explore their clinical significance .Methods The dates of 231 patients with CRC treated in the First Affiliated Hospital of Chongqing Medical University from April 2011 to January 2013 were gathered .Then ,the relationship between detection of CEA and CA19‐9 and TNM staging ,clinicopathologic staging and clinical features of patients with CRC were analyzed . Results The positive rate of CEA in patients with CRC was 51 .5%(119/231) and the positive rate of CA19‐9 in patients with CRC was 46 .3% (107/231) .The positive rates of CEA and CA19‐9 were positively correlated with the TNM staging and clinicopathologic staging of CRC (P<0 .05) .Conclusion The positive rate of CEA and CA19‐9 were associated with the tumor progress and metastasis ,so the detection of CEA and CA19‐9 could be used as one of important serumal marker for clinicopathologic staging of CRC .Also ,detection of CEA and CA19‐9 could be used for prog‐nosis judgment and therapeutic schedule institution .
6.Application of standardized patient in diagnostic teaching

8.Endovenous laser treatment for venous ulcer of lower extremities

Objective To explore the value of endovenous laser treatment (EVLT) in treating the variceal ulcer of lower extremities (VULE).Methods Twenty cases of chronic venous insufficiency with VULE were treated with endovenous laser and received regular postoperative follow-up survey.Results No complication,such as burning of skin happened,all superficial varicose veins disappeared,and most of ulcers were healed within 2 -4 weeks after EVLT.Conclusion EVLT is one of the important methods which is minimally invasive and safe to treat VULE.
9.Application of modified loop ileostomy in laparoscopic low and super-low radical resection of the rectal cancer with preservation of anus

Objective To investigate the value of modified loop ileostomy in laparoscopic low and superlow radical resection of the rectal cancer with reservation of anus.Methods The clinical data of 108 patients who received laparoscopic low and super-low radical resection of rectal cancer with preservation of anus at the Subei Hospital of Jiangsu Province from January 2010 to June 2012 were retrospectively analyzed.Fifty-three patients who received preventive ileostomy were in the ileostomy group,and 55 patients who did not receive preventive ileostomy were in the non-ileostomy group.The operation,incidence of complication and follow-up of the 2 groups were analyzed.All data were analyzed using the t test,chi-square text or Fisher exact test.Results The operation time and blood loss were (185 ± 14)minutes and (31 ± 26)ml in the ileostomy group,and (154 ± 12)minutes and (28 ± 19)ml in the non-ileostomy group,with no significant difference between the 2 groups (t =2.34,1.82,P > 0.05).The time to first flatus,time to liquid diet intake,time to pelvic drainage tube removal,duration of hospital stay,hospital costs were (1.1 ± 0.3) days,(1.1 ± 0.2) days,(5.2 ± 1.0) days,(7.2 ± 1.4) days,(3.0 ± 0.2) ×104 yuan in the ileostomy group,and (4.2 ± 0.6) days,(4.3 ± 0.6) days,(8.4 ± 3.9) days,(12.9 ±4.4)days,(3.8 ±0.7) × 104 yuan in the non-ileostomy group,with significant differences between the 2 groups (t =7.25,28.12,15.34,34.01,7.83,P < 0.05).The incidences of postoperative complications and anastomotic fistula were 5.7% (3/53) and 0 in the ileostomy group,and 21.8% (12/55) and 12.7% (7/55) in the non-ileostomy group,with significant differences between the 2 groups (P < 0.05).Conclusion Modified loop ileostomy in laparoscopic low and super-low radical resection of rectal cancer can effectively reduce the incidence of anastomotic fistula,and helps the recovery of patients.
10.Pull-through Welch procedure for the lower rectal carcinoma: a report of 28 cases

Objective To find a novel operative modality with sphincter preservation in the treatment of middle and low rectal carcinoma. Methods Pull through lower resection was performed on 28 rectal cancer patients. The distance between the anal verge and the lower margin of the tumor was 6~8cm(20 patients) or 8~10cm(8 patients), including 8 patients in Dukes A stage, 16 Dukes B and 4 Dukes C. The resected line from tumor distal margin was 2cm, 3cm, and 4cm, respectively. Results There was no operative death, anastomotic fistula or anastomotic stenosis in these cases. Mean follow up period was 30 months. Local recurrence was found in two cases (7.1%) 18 months after the operation, and 26 cases were cancer free till the end of the follow up. Defecation was satisfactorily controlled 8~12 weeks after the operation. Conclusions Pull through Welch procedure could meet the criterion of the radical resection of rectal carcinoma,and keep the internal and external sphincter muscles intact in the superior lower anterior resection. The normal defecationcan can maintain after the operation due to the preservation of internal and external sphincter muscles.
