1.Research progress of neoadjuvant therapy for borderline resectable pancreatic cancer

Pancreatic cancer is known as one of the most fatal malignant diseases in digestive system.Approximately 15%-20%of patients are deemed resectable at the time of diagnosis.Neoadjuvant therapy to the borderline resectable pancreatic cancer ( BRPC) has been challenged to achieve down-staging of cancer, to avoid unnecessary major operation if the pancreatic cancer progres-ses and distant metastasis develops during preoperative treatment.However, there are some controversial issues influencing the clinical decision of surgeon in managing BRPC.This manuscript reviews the current controversial issues in managing BRPC in order to enhance proper understanding for the current status and potential role of neoadjuvant therapy in managing BRPC.
2.Epidemological Study on Hihg Risk Factos for Large Intestine Cancer in Loudi City

Objective To explore high risk factors of large instestine cancer in loudi city.Methods 132 cases of large intestine cancer of in-patients were reviewed from jan 1990 to Dec 2001,we used paired patients with non-alimentaly system disease as comparative studing group.Results By one way analysis (ANOVA method) of condition logistic regression showed that familial tumor,ulcerative colitis,intestinal polyp,schistosome infection,hormone substitution therapy etc diseases history,social-ecnomical situation,consumed fibre (fruits,vegetables),consumtions of friying meat treated with high temperature,salted fish and salted meat,frying food in oil,smoking≥35 years,physical training were all related to large intestine pathogenesis.Five risk factors were scrrened by condition logistics multiple factors,they were familial tumor histor,intake frying food in oil,smoking ≥35-age,number of calcium intake.Conclusion Large intestine pathogenesis in loudicyte were related to familiar tumor history,intake frying fook in oil,salted fish and salted meat consumption,smoking ≥35 years and number of calcium intake.
3.Effects of recombinant human epidermal growth factor on wound healing after endoscopic sinus surgery

OBJECTIVE To investigate the effects of recombinant human epidermal growth factor (rhEGF) on wound healing of chronic sinusitis patients after functional endoscopic sinus surgery. METHODS A total of 110 patients with chronic sinusitis underwent functional endoscopic sinus surgery with both sides of their nose. The right side of their nasal cavity (control group) treated by nasal irrigation and steroid nasal spray, the left side of their nasal cavity (study group) treated by nasal irrigation and the nasal spray of steroid hormones and rhEGF. Surgical effect was evaluated under nasal endoscope at 2, 4, 12 and 24 weeks after surgery, while subjective symptom was evaluated depending on visual analogue scale (VAS). Both the score of endoscopic examination and VAS, also the rates of epithelization of the two groups were compared. RESULTS The score of endoscopic examination of the study group at 2 weeks after surgery was better than that of the control group, which had statistical significance, while the score of VAS had no statistical difference. The score of endoscopic examination and VAS, the rate of epithelization in study group at 4 and 12 weeks after surgery were better than those in control group, and the difference was statistically significant, but they were no statistical difference at 24 weeks after surgery. CONCLUSION The using of rhEGF nasal spray on chronic sinusitis patients with functional endoscopic sinus surgery can shorten the time of epithelization, improve the situation of nasal mucosa and patients' subjective symptoms, it is worthy of clinical application.

5.Clinical features of familial idiopathic basal ganglia calcification caused by a novel mutation in the SLC20A2 gene.

OBJECTIVETo describe clinical and genetic feature in a Chinese family with familial idiopathic basal ganglia calcification 3 (IBGC-3) caused by a novel mutation in the SLC20A2 gene.
METHODSClinical data was collected from a family with familial IBGC-3. All of the family members underwent cerebral CT. Potential mutation of the SLC20A2 gene were screened in the proband, 5 symptomatic patients, 5 asymptomatic family members, and 100 healthy Chinese controls. Exon 8 of the SLC20A2 gene was cloned into plasmid and sequenced.
RESULTSThere were 6 symptomatic patients (3 males and 3 females) in an autosomal dominant pedigree. The patients manifested as juvenile-onset paroxysmal kinesigenic dyskinesia, in addition to pyramidal signs in proband. 5 patients alive had calcification in bilateral basal ganglia and subcortical areas. One asymptomatic member also had calcification in the brain; and 2 cases of asymptomatic young members had bilateral globus pallidus calcification. A novel c.1086delC mutation in SLC20A2 gene has been identified in proband and 7 family members with intracranial calcification. The deletion mutation was not found in 2 family members without intracranial calcification and healthy controls members. There is no clear relationship between clinical symptoms and the severity of calcification in cerebral CT.
CONCLUSIONFamilial idiopathic basal ganglia calcification caused by the SLC20A2 gene mutation can manifest as juvenile onset paroxysmal kinesigenic dyskinesia. Further study should be done to validate the unrelated relationships between the severity of calcification in IBGC 3 cranial CT and clinical symptoms.

6.Clinical Effect of Day Surgery in Patients with Lung Caner by Optimize Operating Process.

BACKGROUND:
The types and number of day surgery are increasing, what is the result of day surgery of selected patients with lung cancer? To explore the operation process and clinical effect of day surgery in patients with lung cancer by fusing the concept of enhanced recovery after surgery (ERAS) and minimally invasive surgical techniques.
METHODS:
A prospective study was planned with the approval of our institutional review board. 153 lung cancer patients who underwent anatomic resection in a single medical group between June 2019 and Nov 2019 were randomized. 20 patients were applied day surgery and 28 patients by inpatient surgery and the average length of stay, average hospital cost , complications and adverse reactions were analysed.
RESULTS:
The average hospital day in DSG group (1 d) was significantly shorter than in ISG group (7.7±2.8) d (P=0.000). The average hospital cost in DSG group (38,297.3±3,408.7)¥ was significantly lower than in ISG group (47,831.1±7,376.1)¥ (P=0.000). There was no significant difference in the incidence of postoperative complications between the daytime surgery group (5.0%) and the inpatient surgery group (3.6%) (P=0.812). The postoperation adverse reactions in DSG (10.0%) and ISG (17.9%) is no difference (P=0.72).
CONCLUSIONS
Our study showed that the same clinical effect achieved between DSG and ISG, and recover quickly lung cancer patients after day surgery.
