4.Effects of Shexiang injection on expression of CD54 of mononuclear cells and soluble vascular cell adhesion molecule-1 in patients with acute cerebral infarction.

OBJECTIVETo investigate the clinical efficacy of Shexiang Injection (SI) on inflammatory reaction in patients with acute cerebral infarction (ACI).
METHODSForty-two patients with ACI were randomly divided into two groups, 21 in each group. The control group treated with conventional therapy and the SI group treated with conventional therapy plus SI. Besides, 21 healthy people were arranged in the normal group for control. Expression of CD54 of mononuclear cell (MC-CD54) and serum level of soluble vascular cell adhesion molecule-1 (sVCAM-1) were determined and the clinical efficacy was observed dynamically before treatment and on the 7th, 14th day of the course.
RESULTSLevels of MC-CD54 expression and sVCAM-1 in the ACI patients increased obviously (P < 0.01), reached the peak on the 7th day, and declined obviously on the 14th day in both groups, however, the lowering in the SI group was more significant than that in the control group (P < 0.01); and positive correlation was shown between these two indexes (P < 0.01). After treatment, score of neural defect was improved more significantly (P < 0.01), and the markedly effective and curative rate was higher in the SI group than those in the control group (P < 0.05), respectively.
CONCLUSIONSI could inhibit the subsequent inflammatory reaction, and thus improve the clinical efficacy of conventional therapy in treating patients with ACI.

6.Protective effect of Angelica sinensis polysaccharides on subacute renal damages induced by D-galactose in mice and its mechanism.

To explore the protective effect of Angelica sinensis polysaccharides(ASP) on subacute renal damages induced by D-galactose in mice and its mechanism. Male C57BL/6J mice were randomly divided into 3 groups, with 10 mice in each group. The D-galactose model group was subcutaneously injected with D-galactose (120 mg x kg(-1)), qd x 42; the ASP + D-galactose model group was intraperitoneally injected with ASP since the 8th day of the replication of the D-galactose model, qd x 35; and the normal control group was subcutaneously injected with saline at the same dose and time. On the 2nd day of after the injection, the peripheral blood was collected to measure the content of BUN, Crea, UA, Cys-C; paraffin sections were made to observe the renal histomorphology by HE staining; senescence-associated β-g-alactosidase (SA-β-Gal) stain was used to observe the relative optical density (ROD) in renal tissues; transmission electron microscopy was assayed to observe the renal ultrastructure; the renal tissue homogenate was prepared to measure the content of SOD, GSH-PX, MDA; the content of AGEs and 8-OH-dG were measured by ELISA. According to the result, compared with the D-galactose model group, the ASP + D-galactose model group showed obviously decreases in the content of BUN, Crea, UA, Cysc, AGES, 8-OH-dG, the number of hardening renal corpuscle, renal capsular space and renal tubular lumen, ROD of SA-β-Gal staining positive kidney cells, mesangial cells, basement membrane thickness, podocyte secondary processes fusion and MDA and increases in the number of normal renal corpuscle, ribosome and rough endoplasmic reticulum in podocytes, the activity of SOD and GSH-PX. In Conclusion, A. sinensis polysaccharides can antagonize kidney subacute damages induced by D-galactose in mice. Its protective mechanism may be correlated with the inhibition of the oxidative stress injury.

8.Cochlear implantation in patients with secretory otitis media

Objective To evaluate the indications and surgery time of cochlear implantation in patients with otitis media with effusion (OME). Methods Cochlear implantation was performed in 26 patients with bilateral profound sensorineural hearing loss and OME. Non-Surgical Treatment was conducted for OME before the following implantation. According to the standards of the indications and surgery time, surgery was carried out. During the process of surgery operation, related lesions of OME were completely removed and proper drainage was then constructed. After the surgery, the patients were regularly followed up, and the effective of cochlear implantation was evaluated. Results Unilateral ear of 26 patients was successfully operated. Among them, 5 cases were left ears and the remaining was right ears. All implant devices worked normally after 1 month. Had been followed up from 13 months to 4. 3 years, 19 patients with OME were healed and the remaining was turning better. No evidence showed phenomenon of the eardrum perforation and acute attack of OME. Conclusions Once the indications of cochlear implantation in patients with OME are guaranteed, the surgery should be conducted as early as possible. The complications of cochlear implantation in patients with OME can be effectively avoided through proper treatment in different stages.
9.Successful treatment of high risk/refractory leukemia by tumor-ablative individualized conditioning allogeneic hematopoietic stem cell transplantation.

OBJECTIVETo explore the efficacy of tumor-ablative individualized allogeneic hematopoietic stem cell transplantation for the treatment of patients with high risk/refractory leukemia.
METHODSFivety-seven patients with high risk/refractory leukemia were enrolled. Tumor-ablative individualized conditioning regimens included HDAra-C + Bu/Cy, Ara-C + Bu/Fludarabine, G-CSF primed HDAra-C + Bu/Cy, and FLAG followed by reduced-intensified BuCy. Overall survival (OS), disease free survival (DFS), graft versus host disease, infection and relapse post grafting were analyzed.
RESULTSFifty-six patients attained durable engraftment. The median follow-up duration was 17.5 (2 - 34) months. The 18 months probabilities of OS and DFS were (74.7 ± 6.1)% and (62.4 ± 6.7)%, respectively. In addition, the 18 months probabilities of OS and DFS in patients who attained complete remission (CR) before transplantation were (74.2 ± 7.1)% and (58.8 ± 8.1)%, respectively, while in those not attained CR were (77.0 ± 11.8)% and (72.7 ± 11.7)%, respectively. Twenty nine patients developed acute GVHD (aGVHD) (grade I in 18, grade II in 4, grade III in 2 and grade IV in 5). The probabilities of aGVHD was (50.9 ± 6.6)% by Kaplan-Meier curve analysis. The probabilities of grades 2-4 and grades 3-4 aGVHD were (19.3 ± 5.2)% and (12.3 ± 4.3)% respectively. Extensive chronic GVHD (cGVHD) was observed in 36 patients. The probabilities of cGVHD was (64.3 ± 6.4)% by Kaplan-Meier curve analysis. Cytomegaloviremia (CMV) was observed in 39 (68.42%) patients, hemorrhagic cystitis in 13 (22.8%) patients, fungous infection in 16 (28.07%) patients and bacterial infection in 38 (66.67%) patients. Relapse occurred in 14 patients (hematologic relapse in 11 and extramedullary relapse in 3), probabilities of relapse being (24.6 ± 5.7)%. The 17.5-month probability of relapse in patients who attained CR before transplantation was (28.1 ± 7.7)%, while in those not attained CR was (15.6 ± 10.2)%. Fifteen patients died (6 from hematological relapse, 5 from infection of bacterial and fungous, 4 from cGVHD) after 100 days.
CONCLUSIONTumor-ablative individualized allogeneic hematopoietic stem cell transplantation is a promising and safe choice for treatment of high risk/refractory leukemia, even with high leukemia burden.

10.The relationship between adiponectin gene polymorphisms and the risk of the chronic periodontitis and diabetes mellitus

Objective To study the relationship between adiponectin gene polymorphisms and the susceptibility of the chronic periodontitis and diabetes mellitus.Methods Two hundreds patients were selected and divided into four groups,according to their physical and periodontal status.The four group were as follows,the healthy group as control,the one with chronic periodontitis,the one with type Ⅱ diabetes mellitus,and the one suffered from the both.Then,DNA was extracted from buccal mucosa swabs,and the genotype of single nucleotide polymorphism (SNP) 276 as well as SNP 45 in adiponectin gene can be obtained using polymerase chain reaction (PCR) together with restrictive fragment length polymorphism analysis.The relationship between the adiponectin gene polymorphism and chronic periodontitis and diabetes mellitus was analyzed statistically.Results There was significant difference between the normal healthy group (GG:20,GT:26,TT:4) and the diabetes mellitus group (GG:16,GT:20,TT:14) at SNP 276 (P =0.034).But there is no statistically difference between the normal healthy group (GG:26,TG:21,TT:3) and the diabetes mellitus group (GG:27,TG:22,TT:1) at SNP 45 (P =0.856).Moreover,the distribution of SNP 276 shows no difference between the normal healthy group aud the group with chronic periodontitis (GG:14,GT:30,TT:6) (P =0.418).No significant difference was found from the adiponectin haplotype analysis of SNP 45 between the normal healthy group and the group with chronic periodontitis (GG:30,TG:18,TT:2) (P =0.699).According to Logistic regression,the OR of sex is 2.938 in the chronic periodontitis group with the mellitus diabetes(P =0.011).Conclusions These results suggest that SNP 276 in the ADIPOQ gene is associated with diabetes mellitus type 2,but not with chronic periodontitis.There was no association between the adiponectin gene 45 polymorphism and diabetes mellitus type 2.Besides,neither of SNP 276 and 45 has somethiug to do with chronic periodontitis.
