1.Analysis of the Situation of the Minimum Sales Package Unit

OBJECTIVE:To analyze the situation of the minimum sales package unit,and to provide reference for standardiz-ing package. METHODS:According to related regulations of New Edited Materia Medica(17th edition)and package inserts,373 kinds of oral drugs,57 kinds of oral drugs for lowering blood glucose,blood lipid and blood pressure,6 kinds of second kind psy-chotropic oral drugs were analyzed statistically in respect of minimum sales package unit and medication duration. RESULTS:There were 30 kinds of minimum sale package unit for 373 oral drugs,among which common capacity were 20,10,100 grain/tab-let/pill,accounting for 14.21%,14.21%,12.33% respectively,and 57.11% were used within 7 d. There were 13 kinds of mini-mum sale package unit for 57 oral drugs for lowering blood glucose,blood lipid and blood pressure,among which the common ca-pacity were 7,30,14,10 grain/tablet/pill,accounting for 33.34%,14.04%,14.04%,14.04% respectively,and 85.97% were used within 14 d,indicating sales package unit often based on weekly dosage. Among 6 kinds of type Ⅱ psychotropic oral drugs, minimum sales package unit capacity of 3 kinds were all 20 grain/tablet/pill,and those of other 3 kinds were 10,7,6 grain/tablet/pill;one of them were used more than 7 d. CONCLUSIONS:Minimum sales package unit is confirmed discretionarily. It is sug-gested that minimum sale package unit of oral drugs for lowering blood glucose,blood lipid and blood pressure should be packaged according to the dosage of 2 weeks to one nonth;the capacity of type Ⅱ psychotropic oral drugs is lower than the dosage of 7 days,and daily dose package is the best.
2.Inhibition of miR-802 on insulin secretion by targeting Hnf1B

To investigate the effect of miR-802 on insulin secretion by islet β cells and its mechanism, miR-802 was overexpressed or knocked down in primary islet cells and Min6 cells via transfecting miR-802 mimic and miR-802 inhibitor, respectively. The effect of miR-802 on insulin secretion was detected by ELISA. The target gene of miR-802 was confirmed by miRNA target gene database prediction, luciferase report and Western blot. The function recovery experiment was carried out to clarify the mechanism of miR-802 regulating β cell secretion of insulin. The results showed that overexpression of miR-802 in islet primary cells and Min6 cells inhibited insulin secretion. qPCR and Western blot showed that miR-802 inhibited insulin secretion by inhibiting the transcription and translation of the target gene, hepatocyte nuclear factor 1β(Hnf1B).
3.MRI features of solitary fibrous tumors in the spinal canal

Objective To investigate the MRI features of solitary fibrous tumors(SFT)in the spinal canal.Methods MRI images of 5 cases with pathologically proved STF in the spinal canal were analyzed retrospectively.Results Of 5 lesions,there were 2 in the cervical spine,3 in the thoracic spine;1 in the epidural space and 4 in the subdural extramedullary space.On MRI plain scan,3 lesions showed homogeneous iso-intense signal on T1WI and hypo-intense signal on T2WI,2 lesions showed heterogeneous signal,1 showed patchy hypo-intense signal on T1WI and T2WI at the upper edge of lesion,which had been confirmed as hemorrhage and the other lesion showed internal cystic variation.All of the 5 lesions enhanced on enhancement scan,with moderate enhancement in 2 lesions and significant enhancement in 3 lesions.Cystic and hemorrhagic area were not enhanced.The"dural tail sign"was showed in 3 cases.Conclusion The diagnosis of SFT should be considered when a lesion shows a localized solitary mass in the spinal canal with hypo-intense on T2WI and moderate to significant enhancement.
4.Identification of latent class of sleep quality among breast cancer patients during chemotherapy and differences on quality of life based on latent profile analysis

Objective:To identify the classification characteristics and quality of life (QOL) of breast cancer (BC) patients during chemotherapy, so as to provide basis for improving the sleep and QOL of this group.Methods:A cross-sectional investigation was completed among 421 BC patients in 5 tertiary hospitals in Shanghai, Wuhan, Tangshan and Nanning in 1-12 months of 2016 using validated instruments including self-made general information questionnaire, Pittsburgh Sleep Quality Index (PSQI) and Functional Assessment of Cancer Therapy-Breast (FACT-B).Results:Four latent class of patients were identified through latent profile analysis (LPA), named by badly worse sleep quality(SQ) (C1, n=23), medium-SQ with difficulty to fall asleep (C2, n=127), medium-SQ with worse sleeping process (C3, n=30), none sleep disorders (C4, n=241). Total points of SQ among C1-C4 had significant difference ( χ2 value was 309.28, P<0.05). Age, BMI, job status, whether had surgery and course of chemotherapy between classes had statistically significant differences ( χ2 values were 9.57-25.28, all P<0.05). It had significant difference between C2 and C3, C2 and C4, C3 and C1, C3 and C4 on QOL ( χ2 values were 5.96-52.73, all P<0.05). Conclusion:SQ of BC patients during chemotherapy has heterogeneity among population. Different features of SQ of BC patients have different performance on QOL. Health professionals should keep an eye on patients with features of older age, high BMI, in job status, already received surgery and during early-stage chemotherapy, provide personal nursing intervention to improve SQ and QOL.
5.Fear memory and heart rate and blood pressure during conditioned fear in rats with hippocampal lesion or fimbria-fornix transection

Objective To investigate the effects of dorsal hippocampal lesions (DH) or fimbria-fornix transection (FF) on the learning and memory of conditioned fear and the heart rate and blood pressure in rats.Methods Nineteen male adult Wistar rats were used in this experiment.They were implanted telemetry sensors in their abdominal aortas.Two week later,six of the rats were subjected to permanent NMDA-induced neurotoxic lesions to the dorsal hippocampus (DH) and seven for the fimbria-fornix transection (FF)through stereotactic brain surgery,the left six were treated with saline as the control (Sham).All rats were subjected to a conditioned fear experiment.Meanwhile,changes in heart rate and blood pressure were measured.Results There was no significant difference in heart rate and blood pressure among the rats with the hippocampal operation or fimbria-fornix transection.In the acquisition of conditioned fear,there were significant difference in freezing time among the three group in both inter-trial-interval (ITI) and conditioned stimulus (CS) process (all P<0.05).The freezing time of the FF group showed significantly lower than that of the Sham group (P<0.05).There was no significant difference in heart rate and blood pressure among the three group(P>0.05).In the test of conditioned contextual fear memory,the freezing time percentage in the FF group ((0.31±0.16) %) significantly lower than that in the Sham group ((2.78± 1.23) %) (P<0.05)at the first 3 min of the test.There was a significant difference in heart rate among the three group.The heart rate of FF group ((436.42± 10.16) times/min) was significantly lower than that of the Sham group ((472.48±4.43) times/min,P<0.01) and the DH group ((469.94 ±7.36)times/min,P<0.01).In the test of conditioned tone fear memory.The freezing time percentage in FF group ((18.78±6.29) %) was significantly lower than that in the Sham ((51.77±9.33)%,P<0.01) and DH group ((59.19±8.13)%,P<0.01),but the freezing time percentage between the later two groups had no difference (P=0.52).The synchronous telemetry measurement showed there was no significant difference both in the heart rate and the blood pressure among the groups (all P>0.05) during the conditioned tone test.Conclusion The dorsal hippocampal lesions and fimbria-fornix transection in rats can significantly reduce the learning and memory ability in conditioned fear and scene fear in rats,and the effect of fimbria-fornix transection is more obvious.The decrease in,fear memory is not synchronously reflected in heart rate and blood pressure in rats.
6.Differential expression profile in Neuro-2a cells infected by rabies virus

Rabies is a zoonotic disease that poses a global public health threat.Rabies virus(RABV)is neurotropic and can cause severe neurological disorders and behavioral abnormalities in host,with a fatality rate nearly 100%.In order to identify the key genes for RABV affecting neuronal cell function,we established and analyzed the mRNA expression profile of Neuro-2a(N2a)cell in-fected with challenge virus standard(CVS)-11 by RNA sequencing(RNA-seq).Biological func-tions of differentially expressed genes(DEGs)were determined by GO and KEGG enrichment a-nalysis.The results showed that there were 415 differentially expressed genes in N2a infected with CVS-11 strain,of which 89 were up-regulated and 326 were down-regulated.These genes were re-lated to a variety of biological processes,such as axon guidance pathway,cholesterol metabolism pathway,nitrogen metabolism pathway,and glycosphingolipid biosynthesis pathway,many of them have been shown to be closely associated with RABV infection.A total of 12 DEGs related to axon conduction,antigen processing and presentation pathways were selected and detected by real-time PCR,and their expression trends were consistent with the RNA-seq results.The genomic tran-scriptomic data on N2a cell under RABV infection will provide new clues for probing the mecha-nisms of RABV infection and transmission in the nervous system in the future.
7.Investigation on Optimization of Standardized Training Mode for Resident Pharmacists in Beijing

OBJECTIVE:To provide reference for improving the pharmaceutical care in China. METHODs :By comparing the development history ,training contents and status quo of the standardized training system of resident pharmacists in Beijing and clinical pharmacists in China ,combining with policy guidance of the current development trend of clinical pharmacy ,the regins were discussed to optimize the mode of standardization training for inpatient pharmacists in Beijing. RESULTS & CONCLUSIONS:Standardized training for resident pharmacists in Beijing was a compulsory post-graduate education for pharmacists in Beijing hospitals. Post training of clinical pharmacists mainly focused on post-graduate education ,and the main training content was the practical ability of pharmacists in clinical pharmacy ,could be divided to general clinical pharmacists and specialized clinical pharmacists. The standardized training for resident pharmacists in Beijing consisted of two stages ,the first stage was general skills training ,the second stage was specialized skills training. The training content and rotation department of clinical pharmacy in the first stage were similar to post training of general clinical pharmacists in China ,and the training assessment required was more workload for trainees ,but it was not the key point of completion assessment for standardized training of resident pharmacists. During 2015-2017,154 trainees completed the training in Beijing and participated in the second stage completion examination of the standardized training of the resident pharmacists. At the same time ,43 trainees(27.92%)participated in the training of clinical pharmacists. 228 trainees participated in the training of clinical pharmacists in Beijing during the same period , and showed an increasing trend ;most of them had passed the Beijing standardized training for resident pharmacists. It is suggested that the standardized training of resident pharmacists in Beijing can absorb the advantages of post training of clinical pharmacists. In the first stage of training ,the post training of general clinical pharmacists should be carried out at the same time. The post training of specialized clinical pharmacists can be included in the two-stage training of inpatient pharmacists.
8.Status on the use of anti-hypertensive drugs in the southwest areas of China

Objective:To understand the current status of anti-hypertensive drug use in patients with hypertension in the Southwest areas of China.Methods:Based on the Program of Screening and Intervention Subjects with High Risk Cardiovascular Diseases, this study presented information on adults aged 35-75 in Southwest China by convenient sampling method, from January 2016 to November 2018. Basic information and cardiovascular related data were collected. Data on hypertensive patients were recorded, including names, doses and frequency of anti-hypertensive drugs they used. Information on the use of anti-hypertensive drugs among different hypertension subgroups, potential related characteristics, types and combination patterns of drugs, etc., were analyzed. Results:A total of 394 957 subjects were included in the study, with 159 014 identified as being hypertensive [mean age (58.8±9.5) years, 40.2 % male]. 29.8 % of them ever received antihypertensive drugs. A total of 30 445 of the patients reported detailed information of the drugs they ever used and 22.5 % of them received therapy of combined drugs. Rates of using combination therapy were consistent among subgroups with different age, gender, blood pressure level and history of cardiovascular and cerebrovascular diseases. Results from the multivariate logistic regression analysis showed that patients with previous cardiovascular and cerebrovascular events, obesity or diabetes were more likely to have received combined therapy, while patients with less education or lower income were in the opposite. Calcium antagonists (58.6 %) were the main drugs being used in single drug therapy, while traditional fixed-dose combination drugs (31.4 %) were the most common ones in the drug-combination therapy, followed by angiotensin converting enzyme inhibitor/angiotensin receptor blocker combined with calcium antagonists (22.4 %). Angiotensin converting enzyme inhibitor/angiotensin receptor blocker combined with beta blocker was the main drug used in patients with coronary heart disease. Conclusions:Treatment programs using the antihypertensive drugs for hypertensive patients in Southwest China needs to be improved, since the irrational use of antihypertensive drugs still exists. However, we would encourage the use of combination therapy for hypertensive patients.
9.Correlation and consistency evaluation between electrochemiluminescence and direct chemiluminescence in detecting 12 tumor markers

Objective:To assess the correlation and consistency of electrochemiluminescence and direct chemiluminescence in detecting 12 tumor markers.Methods:A total of 2426 serum specimens were selected from the physical examination in Beijing Tiantan Hospital,Capital Medical University from March to August 2023.These specimens included 446 cases for alpha fetoprotein(AFP),284 cases for carcinoembryonic antigen(CEA),289 cases for carbohydrate antigen 72-4(CA72-4),87 cases for carbohydrate antigen 19-9(CA19-9),205 cases for carbohydrate antigen 125(CA-125),216 cases for carbohydrate antigen 15-3(CA 15-3),292 cases for total prostate specific antigen(TPSA),292 cases for free prostate specific antigen(FPSA),84 cases for serum cytokeratin 19 fragment(Cyfra21-1),84 cases for neuron specific enolase(NSE),84 cases for squamous cell carcinoma associated antigen(SCC)and 63 cases for pro-gastrin-releasing peptide(PROGrp).The electrochemiluminescence and direct chemiluminescence methods were respectively used to detect the above 12 indexes,and then,the correlation and consistency between the two detection methods were analyzed.Results:The results of Pearson and Spearman correlation analysis showed that there were significantly positive correlations between electrochemiluminescence and direct chemiluminescence methods in detecting 12 tumor indexes(AFP,CEA,CA72-4,CA19-9,CA125,CA15-3,TPSA,FPSA,Cyfra21-1,NSE,SCC and PROGrp)(r=0.971,0.934,0.945,0.975,0.900,0.948,0.994,0.984,0.982,0.828,0.879,0.922,P<0.05),respectively.The total coincidence rates between the two methods were respectively 98.21%,98.24%,98.27%,98.85%,97.07%,99.54%,99.66%,99.32%,92.86%,92.86%,95.24%and 96.83%.There were consistencies between electrochemiluminescence and direct chemiluminescence methods for 10 indexes excepted CA15-3 and NSE that could not calculate Kappa values due to data reasons(Kappa=0.848,0.728,0.930,0.794,0.485,0.887,0.664,0.540,0.477,0.652,P<0.05),respectively.Conclusion:In the detections of electrochemiluminescence and direct chemiluminescence methods for tumor indexes,there are favorable correlations and consistencies between them in detecting AFP,CA72-4,CA19-9 and TPSA,and there are favorable correlations between them in detecting CEA,CA125,FPSA,Cyfra21-1,SCC and PROGrp but the consistencies between them are average in detecting these indexes,and there are favorable correlations between them in detecting CA15-3 and NSE.Clinical detection should pay attention to there may be differences in the results between different detection methods when the detection is conducted in reference laboratory.
