Neuroleptic malignant syndrome (NMS) is a fatal complication most often caused by an adverse reaction to neuroleptic or antipsychotic drugs, and is mostly brought out in the initial stage of medication. Late onset NMS after long term intake of neuroleptics is uncommon, and the neurochemical mechanism is undiscovered. We report a patient of late onset NMS after a long term intake of haloperidol who was successfully treated with dantrolene and bromocriptine.

Fever (body temperature above 38℃) is relatively common during the first few days after general anesthesia. Postoperative fever is usually caused by the inflammation induced by surgery and resolves spontaneously; however, it can be a manifestation of a serious complication such as malignant hyperthermia. We report a case of postoperative hyperthermia (body temperature > 40℃) that was refractory to conventional anti-pyretic measures and finally resolved with dantrolene administration.

Masseter muscle rigidity may herald a fulminant malignant hyperthermia reaction or may be an isolated abnormal anesthetic response. It is very hard to differentiate, Significant masseter muscle rigidity is a muscle spasm of at least 30 seconds duration that interferes with mouth opening despite an appropriate succinylcholine dose in a patient who has no temporo-mandibular joint dysfuntion. They thought mild to moderate foims of spasm are tolerable for anesthesia. We suffered incomplete relaxation of masseter muscle after succinylcholine that followed by fulminant malignant hyperthermic reaction. At that time, a little resistance was felt during mouth opening put intubation was done . without difficulty. After this experience, because Caffeine-Halothane contracture test is performed only in limited institutes and dantrolene is not available in our hospital, we decide to hold the anesthetic practice in case of any degree of masseter muscle rigidity.

We experienced a case of malignant hyperthermia (MH) in 6-year-old boy during anesthesia induction for strabismus surgery. It has been generally reported that sevoflurane can induce the delayed onset of MH in the absence of succinylcholine. Our case of MH was elicited after about 2-3 min of sevoflurane administration with N2O, O2 and rocuronium. However, we successfully treated the patient by early recognition of his condition and administering symptomatic treatment and dantrolene.

Mortality and morbidity of malignant hyperthermia has decreased markedly by the avoidance of succinylcholine, and the earlier detection and introduction of dantrolene. We report a fourteen-year-old boy who developed malignant hyperthermia during general anesthesia. He showed the earlier clinical signs, such as elevation of end-tidal CO2, tachycardia, and hypertension. After prompt administration of dantrolene, operation was continued with profopol and midazolam. Rhabdomyolysis and myoglobinuria followed, and were managed by hydration and alkalinization of urine. Azotemia did not occur, and he was discharged without any sequelae on the 10th postoperative day.

Malignant hyperthermia is of particular interest to ophthalmologist because of higher incidence in children and its' potential for occurrence in strabismus and blepharoptosis patients. We analyzed the serologic data including serum CPK and the effectiveness of dantrolene sodium in strabismus children for screening carriers of malignant hyperthermia. The results were as follows; 1. Comprehensive studies including family history, laboratory test, underlying muscular disorder, muscle biopsy for susceptible individuals can help to make the diagnosis accurately. 2. No significant difference on serum CPK, LDH, GOT levels between the strabismus patients and control groups. 3. Dantrolene Sodium is effective agent for preventing malignant hyperthermia. 4. General anesthesia or stress associated with surgery and sex were the factors influencing serum CPK levels(i.e., values were significantly higher in postoperative periods and males).
