T-cell acute lymphoblastic leukemia (T-ALL) is a hematopoietic malignancy derived from T precursor cells. Morbidity of children accounted for 15%of ALL. Studies have found that PI3K-Akt-mTOR signaling pathway is associated with the occurrence of T-ALL, cell proliferation, cell apoptosis and drug resistance. The PI3K-Akt-mTOR signaling pathway inhibitors, which can target to inhibit T-ALL cell proliferation in the treatment of T-ALL children, plays an effective cytotoxic role. By shortening the duration of treatment, it can significantly improve the quality of life of children with T-ALL and reducing disease recurrence. It is a more effective way for traditional chemotherapy resistant T-ALL children patients.

Objective To explore the potential mechanisms of therapeutic effect of propolis in the treatment of diabetic wound. Methods Human fibroblasts(Fb) and monocytic THP-1 cells were routinely cultured at 37℃ in an environment of 95% air and 5% CO2 in RPMI medium containing 11mmol/l glucose, 15mmol/l HEPES and 5% fetal calf serum. To a portion of culturing THP-1 monocytes phorbol myristate actetate (PMA; 2?10 -7mol/L) was added. After 24 hours when approximately 75% of the monocytes became macrophage which attached to the plate. Propolis was added to a culture system of human fibroblasts(Fb), THP-1 monocyte and THP-1 monocyte-derived macrophage(Mac) with different concentration of propolis(10, 50, 100, 200?g/ml) and different glucose(5mmol/L; 25mmol/L), For each experiment the cells were cultured for 48 hours in the presence of Propolis. Then the media were collected and the protein expression of matrix metalloproternase-9 (MMP-9) were determined by gelatin zymography using a MDPF- labeled gelatin as substrate. The media were loaded onto an 8% SDS PAGE gel containing MDPF labeled gelatin (1%) after electrophoresis, the gels were placed on a long wave ultra violet light box and photographed. The area degraded by the MMPs was observed as dark bands on a light background. The MMPs were quantified by measurement of the band area using the Phoretix_1D_Advanced Digital Science Electrophoresis Documentation and Analysis System. Objective To analyze the risk factors of acute kidney injury (AKI) in the patients undergoing gastrointestinal surgical procedures.Methods We retrospectively analyzed all gastrointestinal surgical patients nearly 2 years who were in the first affiliated hospital of Kunming Medical University and the first people's hospital of Yunnan province.Results The incidence of AKI was 2.1%.Univariate analysis and multivariate analysis indicated that: age>65 years, emergency operation and serious infection were significantly related with the incidence of AKI.Conclusion AKI is a common complication after gastrointestinal surgical procedures.This study showed that age>65 years, emergency operation, serious infection were risk factors most likely associated with postoperative AKI development.

Objective To study the diagnostic utility of hemoglobin electrophoresis in neonatal cord blood screening for thalassemia. Methods Between January 2012 and December 2013, 14032 core blood samples which were from different 21 Women and Children Hospitals in Guangdong were performed for the neonatal screening with hemoglobin electrophoresis. The positive samples of hemoglobin electrophoresis were recalled for genetic testing. Results Out of 1445 (11.07%) positive samples of hemoglobin electrophoresis , 1075 (54.08%) cases were suspected for α-thalassemia, 478 (3.41%) cases were suspected for β-thalassemia, 127 (0.91%) cases were suspected for abnormal hemoglobin. With the genetic testing, 967 cases were diagnosed as α-thalassemia, 404 cases were diagnosed asβ-thalassemia. The coincidence rate ofα-thalassemia andβ-thalassemia were 89.95%and 82.96%, respectively. Besides, 124 cases were diagnosed as abnormal hemoglobin, including 38 cases of Hb E, 28 cases of Hb Q, 21 cases of Hb D, 19 cases of Hb New York, 13 cases of Hb J, and 5 cases of Hb J. Conclusion Hemoglobin electrophoresis was definitely helpful in the neonatal cord blood screening for thalassemia and abnormal hemoglobin.

Objective To discuss the effect of syphilis treatment during pregnancy on serological test results in infants born to syphilis mothers.Methods Data of pregnant women with syphilis and their infants in Zhejiang Province between January 2013 and December 2014 were retrospectively analyzed and the effect of syphilis treatment during pregnancy on non-treponemal antigen serological test results in infants were compared.x2 test was used for positive rate of serological tests,incidence of congenital syphilis,and neonatal mortality in infants with syphilis,and rank sum test was used to compare the titers of rapid plasma reagin (RPR) test and tolulized red unheated serum teat (TRUST).Results A total of 3 953 infants were born to 3 901 mothers with syphilis.Of which 3 262 were tested,including 2 226 by RPR and 1 036 by TRUST.The rate of infants with positive RPR was significant lower in treated mothers (45.02％)compared with untreated mothers (55.04％;x2 =12.88,P＜0.01).The significant differences of titers of RPR or TRUST were observed among mothers with or without treatment (RPR:Z=-2.45,P=0.01;TRUST:Z=-2.65,P=0.01),different course of treatment (RPR:Z=-2.33,P=0.02;TRUST:Z=-2.60,P=0.01).Fifty-seven infants were diagnosed with congenital syphilis.The rate of congenital syphilis was 3.50％(27/772) born to untreated mothers,which was dramatically higher than treated mothers (0.96 ％,30/3 129,x2 =27.72,P＜ 0.01).Forty-two infants died.Neonatal mortality was also higher in the untreated group (2.59 ％,20/772) than the treated group (0.70％,22/3 129,x2 =20.72,P＜ 0.01).Conclusion Adequate treatment for women with syphilis during pregnancy could decrease serological tests of RPR and TRUST among new infants and improve the infants their outcomes.

Objective: To explore the predictive value of fractional lfow reserve (FFR) level for long-term prognosis in patients after coronary drug-eluting stent (DES) implantation, and to analyze the relevant factors affecting the level of post-operative FFR. Methods: A total of 135 patients who received DES implantation in our hospital from 2012-01 to 2013-07 with coronary intermediate lesion at (50-80) % were studied. The relevant factors for MACE occurrence were studied by multivariate logistic regression analysis, the post-stent FFR level for predicting the long term prognosis after DES implantation was ifnally analyzed by ROC curve. Results: All patients ifnished 1 year follow-up study including 104 male and 31 female with the mean age of (63 ± 9) years. The post-stent FFR level was lower in MACE group than that in Non-MACE group, (0.82 ± 0.07) vs (0.87 ± 0.06),P=0.004. Multivariate logistic regression analysis presented that the higher level of post-stent FFR was the protective factor for MACE occurrence (OR=0.212,P=0.039); the post-stent FFR level had certain predictive value for MACE occurrence at 1 year after DES implantation (AUC=0.706,P=0.006); Kaplan-Meier survival study showed that the patients with post-stent FFR<0.875 had the less MACE occurrence than those with FFR≥0.875,P=0.012. Multivariate logistic regression analysis also indicated that post-stent FFR≥0.875 was positively related to right coronary target vessel, higher pre-operative FFR level and larger stent diameter.Conclusion: Post-stent FFR level had certain predictive value for MACE occurrence in patients at 1 year after DES implantation, the patients with post-stent FFR≥0.875 had the lower MACE occurrence rate than those with FFR<0.875.

OBJECTIVETo explore the relationship of continuous positive airway pressure and pulmonary function in patients with obstructive sleep apnea hypopnea syndrome (OSAHS), and formulate the prediction equation for the effective therapeutic pressure.

METHODSIn a retrospective cross-sectional study of 48 patients with established OSAHS, all the patients were carefully examined for their medical history, and overnight sleep monitoring was carried out to measure the sleep apnea-hypopnea index, mean oxygen saturation, minimum oxygen saturation, and oxygen drop index. The data of manual pressure titration (effective pressure) and pulmonary function tests (tidal volume, one second forced expiratory volume, central airway resistance, and peripheral airway resistance were collected for multiple linear regression analysis.

RESULTSThe effective therapeutic pressure was not correlated with the indices of the pulmonary functions in the patients, but showed correlations with the neck circumference, abdominal circumference, apnea-hypopnea index, mean oxygen saturation, least oxygen saturation, and oxygen desaturation index. Multiple linear regression (α=0.05 test level) identified only oxygen desaturation index (P=0.012) and mean oxygen saturation (P=0.036) as the dependent variables of the effective therapeutic pressure. Linear regression analysis showed that the effective therapeutic pressure had a linear relationship with the oxygen drop index and mean oxygen saturation, and was inversely correlated with the mean oxygen saturation.

CONCLUSIONSIn patients with OSAHS, the pulmonary function indices are not correlated with the effective therapeutic pressure. In the absence of manual pressure titration, the effective therapeutic pressure can be predicted using the prediction equation: effective therapy pressure = 24.262+0.044×oxygen desaturation index -0.19×average oxygen saturation.

OBJECTIVE: To investigate the effects of Xuesaitong for injection on microembolic signal (MES) and blood lipid of patients with middle cerebral artery (MCA) atherosclerotic stenosis complicated with MES positive transient ischemic attack (TIA). METHODS: A total of 98 patients with MCA atherosclerotic stenosis complicated with MES positive TIA in the Affiliated Hospital of Chengdu University of TCM during May 2015-May 2016 were divided into routine group and Xuesaitong group according to random numble table, with 49 cases in each group. Routine group received routine treatment. Xuesaitong group was additionally given Xuesaitong for injection (lyophilized) 400 mg intravenously, once a day, for consecutive 15 d, on the basis of routine treatment. The distribution of MES classification and the number of MES were observed in 2 groups before treatment, 15 d after treatment and 3rd and 6th month after treatment. The levels of TG, TC, LDL were observed before treatment and 15 d after treatment. The occurrence of ischemia event and ADR were recorded. RESULTS: Fifteen days after treatment, the distribution of MES classification in 2 groups was significantly better than before treatment; Xuesaitong group was significantly better than routine group, with statistical significance (P＜0. 05). At 3rd and 6th month after treatment, the distribution of MES classification in Xuesaitong group was significantly better than routine group; 6th month after treatment was significantly better than 3rd month after treatment, with statistical significance (P＜0. 05). Fifteen days and the 3rd, 6th month after treatment, the number of MES in 2 groups were significantly lower than before treatment and decreased as time; Xuesaitong group was significantly lower than routine group, with statistical significance (P＜0. 05). Fifteen days after treatment, TG and TC levels of 2 groups were significantly lower than before treatment; Xuesaitong group was significantly lower than routine group, with statistical significance (P＜0. 05); there was no statistical significance in the level of LDL before and after treatment (P＞0. 05). The incidence of ischemic events in routine group at 6th month after treatment was significantly higher than 3rd month after treatment, and the incidence of ischemic events in Xuesaitong group at 3rd and 6th month after treatment was significantly lower than routine group, with statistical significance (P＜0. 05). At 6th month after treatment, the incidence of ADR in 2 groups was significantly lower than 3rd month, with statistical significance (P＜0. 05). CONCLUSIONS: Based on routine treatment, Xuesaitong for injection can reduce the number of MES, reduce the incidence of ischemia events and further improve the level of blood lipid in patients with MCA atherosclerotic stenosis complicated with MES positive TIA without increasing the occurrence of ADR.

Neutrophils are the most abundant immune cells in the human,which can be caused by a variety of acquired or congenital factors.The clinical manifestation of children with neutropenia are ranging from different forms and parts,seriously affecting the quality of life.The novel sights in etiology,pathogenesis and treatment of children with acquired and congenital neutropenia are discussed.
