1.Clinical efficacy of puerarin injection combined with reduced glutathione in treatment of diabetic nephropathy

Objective To observe the clinical effects of puerarin injection combined with reduced glutathione in the treatment of stage Ⅲ of diabetic nephropathy.Methods One hundred inpatients with stage Ⅲ of diabetic nephropathy in the Chongqing Municipal Hospital of Traditional Chinese Medicine from January 2015 to October 2016 were selected and divided into the observation group and control group according to the random number table method,50 cases in each group.Both groups took the basic therapy of controlling blood sugar,blood lipids and blood pressure.On the basis of general treatment,the control group was added with reduced glutathione injection 1.8 g/d,while on the basis of the control treatment,the observation group was added with puerarin injection 300 mg/d.Both groups were treated for 4 weeks.Fasting plasma glucose (FPG),2 h postprandial plasma glucose (2 h PG),blood pressure,blood fat,serum creatinine(SCr),blood urea nitrogen (BUN),24 h urinary total protein,urine albumin,urine β2 micro-globulin (β2-MG),NAG,and other indicators before and after treatment were detected in the two groups.Results FPG,2 h PG,blood pressure and blood fat after treatment in the observation group and control group were improved,but the improvement in the observation group was better than that in the control group (P<0.05);the levels of SCr,BUN,24 h urine protein,urine albumin and β2 micro-globulin and urine NAG in the observation group and control group patients were reduced to some extent(P< 0.05),which were significantly improved compared with before treatment (P<0.05),but the improvement degree in the observation group were better than that in the control group (P<0.05).Conclusion Puerarin injection combined with reduced glutathione injection has obvious therapeutic effect in treating the stageⅢ of diabetic nephropathy,and has less adverse reactions,is safe and effective and can be promoted and applied in clinic.
2.Cool-tip Radiofrequency Ablation Therapy Instrument Based on Impedance Control Algorithm.

A new cool-tip radiofrequency (RF) ablation therapeutic instrument based on impedance control algorithm is introduced in this paper. The equipment is composed of hardware system and software system. The RF power output and real time data acquisition are completed by the hardware system, while the software is used mainly to finish the control of the ablation range, the core of which is impedance control algorithm, and it also used to complete the display of the real time data in the course of the experiment. The impedance algorithm has solved the problem of impedance increased rapidly during the RF ablation, which has also expanded the scope of ablation. The pig liver experiments showed that the impedance control algorithm had strong adaptability. It also obtained a result of ablation range up to 3.5-4.5 cm single needle. It has the high clinical practical value of one-time inactivation of 3-5 cm tumor.

3.Systemic delivery of C Ⅱ TA siRNA reduces disease severity in collagen-induced arthritis

Objective To examine the therapeutic effect of C Ⅱ TA inhibition in collagen-induced arthritis(CIA),using a delivery system tailored to target C ⅡTA gene by small interfering RNA (siRNA).Methods Mice with collagen-induced arthritis were injected intravenously with C Ⅱ TA siRNA.The clinical score was monitored for up to 4 weeks after treatment.The severity of inflammation of mouse joint was evaluated by histological examination.Real-time PCR was used to determine the cytokine mRNA expression.Cytokine production was measured by ELISA from serum.T cell proliferation was examined by MTT method.Results IFN-γ and IL-17 were elevated in CIA mice,but were iuhibited significantly by C Ⅱ TA siRNA either prevention or intervention of autoimmune arthritis.Collagen specific T cell proliferation was significantly suppressed.Increased level of IL-4 by T cells was observed in C Ⅱ TA siRNA treated group compared with that of control group.Conclusion Our findings indicate that systemic RNAi-mediated C Ⅱ TA gene silencing is effective in the treatment of CIA and regulateds the balance of Th1/Th2 differentiation.
4.Clinical Observation of Ultrasound Debridement Combined with Huangma Tincture in the Treatment of Dia-betic Foot Ulcer

OBJECTIVE:To observe the clinical efficacy and ADR of ultrasound debridement combined with Huangma tinc-ture in the treatment of diabetic foot ulcer. METHODS:90 diabetic foot ulcer patients were randomly divided into combination treatment group(ultrasound debridement combined with Huangma tincture wet compress),Huangma tincture group(Huangma tinc-ture wet compress) and control group (vaseline) with 30 cases in each group. 3 months after treatment,Ulcer healing rate,cure time,effective time,recovery time,cure rate and ADR were observed in each group. RESULTS:The ulcer healing rate of combi-nation treatment group was significantly higher than that of Huangma tincture group and control group,with statistical significance (P<0.05);there was statistical significance between Huangma tincture group and control group(P<0.05). Mean cure time,effec-tive time and recovery time of combination treatment group were all significantly lower than Huangma tincture group and control group,with statistical significance(P<0.05);there was statistical significance between Huangma tincture group and control group (P<0.05). Cure rate of combination treatment group was higher than that of Huangma tincture group and control group,with statis-tical significance (P<0.05);there was statistical significance between Huangma tincture group and control group (P<0.05). No ADR was found in combination treatment group and Huangma tincture group. CONCLUSIONS:The ultrasound debridement com-bined with Huangma tincture in the treatment of diabetic foot ulcer improve healing rate and shorten healing time significantly with-out obvious ADR. It has good clinical efficacy.
5.Clinical value of endoscopic submucosal dissection for early cancer and precancerous lesions in the pylorus (with video)

Clinicopathological data of 15 patients with pyloric early cancer and precancerous lesions, who received endoscopic submucosal dissection (ESD) in Zhejiang Cancer Hospital from March 2011 to January 2020 were retrospectively analyzed. Postoperative pathology showed 7 cases of low-grade intraepithelial neoplasia, 3 cases of high-grade intraepithelial neoplasia, and 5 cases of early gastric cancer. R0 complete resection was achieved in all patients. The mean operation time was 55.2 min (35-78 min). One patient had delayed postoperative bleeding, and no other complications such as bleeding, perforation or abdominal pain occurred in other 14 patients. No recurrence, metastasis or pyloric stenosis was found during the follow-up of 31.3 months (1-106 months). ESD is safe and effective for early cancer and precancerous lesions in the pylorus.
6.Practice of applying the growth research index performance model to improve the discipline competitiveness of prefecture-level hospitals

Objective:Build a research index performance model with discipline growth evaluation, to evaluate the scientific research performance level of various disciplines in hospital more objectively, fairly and dynamically.Methods:Take the research projects, papers, patents and achievements as the main evaluation indicators and establish the hierarchical and classified scoring standards, to form the weight index evaluation model for the total score, per capita score and growth of the discipline research.Results:Using the growth research index to evaluate the scientific research performance of departments and individual researcher between January 2018 and December 2020. Excellent departments were selected for the top 10% of the scientific research index, those whose scientific research scores increased by more than 20% compared with the previous year were selected as the progressive departments, and the top 1% of individual scientific research scores were selected as the advanced researchers, which were commended and encouraged. For the departments ranked in the bottom 5% or whose scientific research index significantly declined compared with previous year, early warning, guidance and supervision were implemented. Since the implementation of the evaluation system, the research performance of disciplines has been significantly improved, and many achievements were made.Conclusions:This evaluation mode can stimulate the enthusiasm of the disciplines and scientific researchers for entrepreneurship and innovation to set up the standards and promote the continuous improvement of the research capacity of the whole hospital.
7.Blood-enriching effects and mechanism of albiflorin and paeoniflorin on immunosuppressional model mice with blood deficiency pattern

Objective To observe the blood-enriching effects and mechanism of albiflorin and paeoniflorin on immunosuppressional model mice with blood deficiency pattern induced by cyclophospha-mide.Methods The mouse model with blood deficiency pattern induced by intraperitoneal injection of cyclophosphamide was treated with intragastrical administration of albiflorin and paeoniflorin at high dose of 30 mg/kg,low dose of 15 mg/kg.Then the peripheral hemogram,thymus gland index and spleen index,granulocyte-macrophage colony stimulating factor (GM-CSF),interleukin-3 (IL-3),interleukin-6 (IL-6),tumor necrosis factor α(TNF-α)and granulocyte colony-stimulating factor were detected. Results High dose albiflorin and high dose paeoniflorin could increase WBC count(P <0.01 )and thymus gland index (P <0.01,P <0.05),serum contents of GM-CSF and G-CSF (P <0.001,P <0.01),also serum content of IL-3 (P <0.01,P <0.05),and decrease the serum content of IL-6(P <0.05)and TNF-α(P <0.001,P <0.01 ).Conclusion The blood-enriching effects against blood-deficiency via regulating the immunological function,suggests that albiflorin and paeoniflorin be the main effective components extracted from White Peony Root (Radix Paeoniae Alba,Baishao)with the effects of nourishing blood and liver.
8.Predictive value of labor progression angle,fetal head descent distance,and their change rate in the outcome of vaginal trial delivery of scarred uterus after cesarean section

Objective To investigate the predictive value of labor progress angle(AOP),fetal head descent distance(HPD)and their change rates in the outcome of vaginal trial of cesarean scar uterus.Methods A total of 170 pregnant women who underwent vaginal trial production of scar ute-rus after cesarean section were selected as study subjects,and were divided into successful group and failed group based on the trial production outcomes.Advanced oxidation processes(AOP)and head-perineum distance(HPD)were measured by ultrasound during the active phase of the first stage of labor when the cervix dilated to 4 cm and at 1 hour after the cervix dilated to 4 cm,respectively.The AOP change rate and HPD change rate after 1 hour of progress were calculated.The receiver operating characteristic(ROC)curve was used to analyze the predictive efficacy of AOP,HPD and their change rates in the outcome of vaginal trial production of scar uterus after cesarean section.Delong test was used to compare the differences in area under curves(AUCs).Results Among 170 pregnant women with scarred uterus after cesarean section who were pregnant again,139 cases(success group)were succeed in transvaginal delivery,while 31 cases failed trial delivery,and transferred to cesarean section(failure group).The AOP of the successful group was significantly larger than that of the failed group when the cervix was opened to 4 cm,and the HPD was significantly shorter than that of the failure group(P<0.05).The AOP change rate and the change rate of HPD of the suc-cessful group were significantly higher than that of the failed group when the cervix dilated to 4 cm and at 1 hour(P<0.05).The AUC of AOP and HPD in predicting the outcome of vaginal trial de-livery of scar uterus after cesarean section were 0.846 and 0.812 respectively,and AUC predicted jointly by AOP and HPD showed no significant differences compared with AUC predicted separately(P>0.05).The AUC of the change rate of AOP and HPD in predicting the outcome of vaginal trial delivery of scarred uterus after cesarean section was 0.899 and 0.852 respectively,and the com-bined prediction of AOP change rate and HPD change rate had a higher AUC value than the AUC predicted separately.Its AUC value was higher than that of AOP combined with HPD(P<0.05).Conclusion The AOP,HPD and their change rates when the uterine orifice expands to 4 cm in the active phase of the first stage of labor have predictive value for the outcome of vaginal trial produc-tion of scarred uterus after cesarean section.
9.Analysis of the incidence of short-term illness in four counties of Hunan Province.

OBJECTIVE:
To determine the prevalence and incidence of illness of two-week duration, and the factors influencing these, among residents 15 years and older in four counties of Hunan Province.
METHODS:
Data were sampled from four counties of Hunan Province for the Fourth National Health Service Survey. Incidence and two-week prevalence of disease were used to assess the health service needs of residents. A non-conditional, stepwise logistic regression was employed to explore the influencing factors.
RESULTS:
The two-week prevalence and incidence were 11.5% and 3.9%, respectively, in four counties of Hunan. The three leading diseases of two-week prevalence were: respiratory diseases, digestive diseases, and musculoskeletal diseases. Non-conditional stepwise logistic regression showed that urban residents had 0.64 times the risk of two-week illness compared with the rural residents (P< 0.05); residents in the 45-59 year age group and the 60+ year age group had 1.69 and 2.62 times the risk of two-week illness compared with residents in the 15-44 year age group, respectively (P<0.05). The widowed had 1.91 times the risk of prevalence of two-week illness contrasted to singles (P<0.05); the students had 0.29 times the risk of two-week illness contrasted to the workers (P<0.05); urban residents had 0.63 times the risk of two-week illness compared with the rural (P<0.05); the widowed had 2.37 times the risk of incidence of two-week illness compared with singles (P<0.05).
CONCLUSION
The majority of health service needs of residents of four counties is generated by three diseases: respiratory diseases, digestive diseases, and musculoskeletal diseases. Relatively, rural residents, the elderly, employed persons and the widowed have higher health service needs than others and deserve specific attention.

10.Bibliometric analysis on research about low-level occupational benzene exposure

ObjectiveTo analyze the research status and trends in low-level occupational benzene exposure. Methods Articles on low-level occupational benzene exposure from Chinese and English journals from January 1st, 2000, to December 31th, 2022 were retrieved using the Web of Science and the China National Knowledge Infrastructure, and a bibliometric analysis was conducted. Results A total of 327 articles were included in the analysis, comprising 216 English articles and 111 Chinese articles. i) The number of articles published in English fluctuates greatly over the years, without a trend of continuous growth or decline. Authors from 359 research institutions in 45 countries and regions have published relevant English articles in 97 kinds of journals, involving 281 grants from 226 foundations. The top three countries in terms of articles amount were the United States, Italy, and China, with 81, 46, and 43 papers, respectively. The English articles mainly focused on mechanistic research at the genetic level, such as hematotoxicity, oxidative stress, and DNA damage. ii) The number of Chinese articles increased gradually after 2012, with the growth peak in 2017. Authors from 127 research institutions in 26 provinces, autonomous regions, and municipalities published Chinese articles in 51 kinds of journals, involving 154 grants from 78 foundations. Chinese articles tended to focus on benzene-induced hematotoxicity and occupational health damage. Conclusion Most studies on low-level occupational benzene exposure were conducted in China, the United States and Italy, focused on hematotoxicity. Monitoring international research topics and hotspots of the field has certain reference value for related research in China.
