1.Clinical application and biocompatibility of the biliary tract stents

OBJECTIVE:To introduce the research progress of biliary tract prosthesis,to analyze the reason of complication following stent implantation,and to explore the biocompatibility of various biliary tracts and host. METHODS:We retrieved Medline database (1989/2009) and CNKI (1989/2009) using the computer. According to inclusion and exclusion criteria,the literatures were screened by the first author,and the data were collected,and the quality was evaluated. We described and arranged the treatment progress of biliary tract prosthesis implantation,complication of stent implantation and biocompatibility of biliary tract prosthesis to investigate treatment state,complication and biocompatibility of the biliary tract prosthesis. RESULTS:The literatures were summarized. The bilitary tract prosthesis contains plastic stent,mental stent,silica gel tract stent and degradable polylactic acid stent. The complications of stent implantation included transudation,hemorrhage,perforation and stenosis. Among various stents,the biocompatibility of plastic stent was poor,easy to be blocked,but the price was low,and easy to be taken out. The diameter of mental stent following distension could reach 7-10 mm,with long unobstructed time,but the price was higher compared with the plastic stent. The biocompatibility of silica gel tract stent was good. The silica gel tract had strong decay resistance and pliable texture,which could extend tumor excision range and elevate curative effects. Moreover,complications did not occur such as infection and stenosis of the biliary tract. Biocompatibility of nickel-titanium alloy biliary ducts was significantly better than the stent products,without complications. With the exception of price factor,the biocompatibility of degradable polylactic acid stent was the best,but further studies were needed to observe and study the formation of biliary sludge,stent displacement and blockage,stent degradation products. CONCLUSION:Biliary tract prosthesis implantation can be an important treatment method for stenosis of biliary tract. During treatment,enough preparation is needed prior to implantation according to various clinical symptoms to select suitable stent type. Indications are followed strictly to avoid the occurrence of complications. The biocompatibility of silica gel stent and Ni-Ti alloy stent is good,but the biocompatibility of other stents should be enhanced.
2.Research on Cognitive Mode and Cultivation of Medical Humanistic Spirit Based On Support Vector Machine

Under the biomedical mode, medical humanistic spirit is increasingly weakened. Medical humanis-tic education is the specific practice of humanistic education in the medical field, and based on medical science, a-chieves the humanistic spirit that doctor should possess through knowledge transfer, environmental influence and practice experience and self-cultivation. This paper obtained research data through questionnaire, constructed the cognitive classification index system of medical students' medical humanistic spirit, constructed cognitive classifica-tion model using support vector machine principle and put forward targeted education countermeasures for each kind of medical students.
3.Expressions of WWOX and CD133 in colorectal cancer and their clinical significance.

OBJECTIVETo investigate the expressions of WWOX and CD133 in colorectal cancer (CRC) and their relationship with the clinicopathologic characteristics of CRC.
METHODSThe expressions of WWOX and CD133 proteins were examined by immunohistochemistry in 174 specimens of CRC tissues and 80 normal colorectal mucosa tissues.
RESULTSThe positivity rates of WWOX and CD133 proteins were 41.4% and 53.4% in CRC tissues, respectively, significantly different from the rates in normal colorectal mucosa tissues (87.5% and 5.0%, respectively; P<0.05). WWOX and CD133 protein expressions were signi- ficantly correlated with the histological grades of the tumors, depth of invasion, lymph node metastasis, and Duke's stages (P<0.05). Spearman analysis showed a negative relationship between the WWOX expression and CD133 expression (P<0.05). Kaplan-Meier survival analysis showed that the overall survival time of CRC patients with a positive expression of WWOX was longer than that of patients with a negative expression of WWOX; the overall survival time of patients with a positive expression of CD133 was shorter than that of the negative patients (P<0.05). COX regression analysis identified positive expressions of WWOX and CD133 protein and Duke's stage as the independent prognostic factors of CRC.
CONCLUSIONAbnormal expressions of WWOX and CD133 might be involved in the initiation, development, invasion, and metastasis of CRC. A combined detection of WWOX and CD133 can help in predicting the progression and prognosis of CRC.

4.Reflections on ethical review and management of medical artificial intelligence social experiment projects

The medical artificial intelligence(AI)social experiment aims to more comprehensively understand the comprehensive impact of medical AI technologies and products on society,with the characteristics of wide application scenarios,complex research types and methods,multi-dimensional evaluation indicators,cross-disciplinary,and diverse subjects.To ensure the quality of ethical review in medical AI social experiment,it is urgent for relevant parties such as government authorities,research initiators,medical and health institutions,universities,scientific research institutions,enterprises,and research teams to shoulder their respective responsibilities,establish a technology product access mechanism for medical AI social experiments,improve the ethical review ability of the ethical review committee,and construct a whole cycle regulatory system.
5.Examination of joint fluid TNF-α and TGF-β1 applied in knee os-teoarthritis

Objective To explore the changes and clinical significance of joint fluid TNF-α and TNF-β1 levels in patients with knee osteoarthritis (KOA). Methods A total of 90 patients with KOA hospitalized in our hospital were selected. They were assigned to group A with 30 patients at early stage, group B with 30 patients at medium stage,and group C with 30 patients at advanced stage according to the stage of X ray; 30 healthy volunteers were in the con-trol group. Levels of joint fluid TNF-αand TNF-β1 were tested in four groups of patients. Results Levels of joint fluid TNF-β1 at medium and advanced stages of KOA were significantly lower than those in the control group, and the dif-ferences were significant (P<0.01); Levels of joint fluid TNF-α at medium and advanced stages of KOA were signifi-cantly higher than those in the control group, and the differences were significant(P<0.01); the difference of levels of joint fluid TNF-α and TNF-β1 at early stage of KOA was not significant compared with those in the control group(P>0.05);levels of TNF-α/TNF-β1 at early,medium and advanced stage of KOA were significantly higher than those in the control group(P<0.01);TNF-αwas positively correlated with KOA stage(r=0.930,P<0.01);TNF-β1 was nega-tively correlated with KOA stage(r=-0.849,P<0.01);TNF-α/TNF-β1 was positively correlated with KOA stage(r=0.828,P<0.01). Conclusion TNF-α and TNF-β1 are involved in the formation and progression of OA, and levels of joint fluid TNF-α and TNF-β1 are able to reflect the severity of KOA lesions; joint fluid TNF-α/TNF-β1 is able to detect KOA early.
6.Reflections on the Establishment of Regional Medical Science and Technology Ethics Center

Medical science and technology innovation activities should meet the latest requirements of Opinions Issued to Strengthen Governance over Ethics in Science and Technology. Thus, attention should paid on the challenges facing medical science and technology ethics governance. This study discussed the necessity, responsibilities and operational guarantee elements of establishing an independent and professional regional medical science and technology ethics center. On the one hand, promote the innovation and development of medical science and technology and ensure the compliance with ethical norms, On the other hand, the government needs to carry out top-level design and build a science and technology ethics system with equal emphasis on supervision and technical services.
7.The practice of ethical collaborative review and mutual recognition model

With the development of more and more multi-center and cross-field cooperative medical research,the establishment of high-quality and efficient ethical collaborative review and mutual recognition systems is an inevitable demand for multi-institutional research,and an inevitable move to implement relevant national policies.Based on the work practice of ethical collaborative review and mutual recognition in Shenzhen,by analyzing the practical challenges of ethical collaborative review and mutual recognition in China,this paper proposed that to ensure the homogeneity and efficiency of review.Government departments need to take the lead,establish an ethical review alliance,and clarify responsibilities and rights.Based on actual needs,system first,and effective communication,ethical collaborative review and mutual recognition of results could be jointly promoted,aiming to provide a reference for our counterparts in China to promote ethical collaborative review and mutual recognition of cross-institutional research.
