1.Indirect ultrasonic signs of the callosal partial agenesis and hypoplasia in the fetus

Objective To investigate antenatal sonographic findings of the fetal isolated callosal hypoplasia and partial agenesis. Methods A retrospective study was performed on the cases of hypoplasia and partial agenesis of the corpus callosum suspected at antenatal sonographic basic examination from 2006 to 2014, all the cases were confirmed by pathology or magnetic resonance imaging(MRI). For the surviving infants, clinical follow-up had been performed to assess the developmental outcome. Results Thirteen fetuses suspected with callosal underdevelopment were identified at a median gestational age of 31 (range, 18~39) weeks. Ten cases were confirmed by autopsy and MRI, including 9 with partial agenesis and 1 with hypoplasia. Among the 10 fetuses confirmed with isolated partial agenesis or with hypoplasia, incidence of the absent cavum septum pellucidum was 20%, the ‘Tear-drop’ lateral ventricles was 40%, the upward displacement of the third ventricle was 80%. Pregnancy was terminated electively in 8 of the cases with partial agenesis or with hypoplasia. Among the 2 surviving infants, apparent normal development was observed in only one case, but we lost the follow-up of this case at two-year-old. Six fetuses received the chromosome identification, almost all of them were normal. Conclusion The basic ultrasonic examination is feasible for the antenatal diagnosis of isolated callosal underdevelopment the. The indirect classical signs of callosal partial agenesis and hypoplasia are different with those of complete agenesis of the corpus callosum. The incidences of the‘Tear-drop’ lateral ventricles and the upward displacement of the third ventricle are higher than the absence of CSP. The chromosome of isolated callosal partial agenesis or hypoplasia is normal, however, the prognosis is uncertain.

Objective To evaluate the feasibility of getting retroperitoneal lymph node biopsy via technique of natural orifice transluminal endoscopic surgery(NOTES)in human being with current available devices.Methods We performed trans-gastric endoscopic biopsy of retroperitoneal lymph node with the aid of laparoscopy in a 50-year-old man,who presented with abdominal pain and enlarged retroperitoneal lymph nodes and signed a written informed consent before the procedure.After routine anesthesia and abdominal skin sterilization,a pneumoperitoneum was induced with a Veress needle placed in the umbilical area,followed by the introduction of a 5-mm trocar.Gastral cavity Was sterilized with antibiotics and povidone iodine.Under laparoscopie optical control,we made a styliform incision in the anterior wall of gastric corpus with a needle knife,and enlarged the incision by a dilatation balloon and then entered the peritoneal cavity with a sterile endoscope.We got two biopsies from the enlarged lymph node with a heat forceps assisted by laparoscopy.The specimen was taken out by retrieval basket through the stomach.The gastric incision Was closed with metal clips.Results The biopsy by means of NOTES was successfully performed without intra-or postoperative complications.The diagnosis was confirmed as lymphoma pathologically.The patient received chemotherapy and was discharged on the sixth postoperative day.There was no short or long-term complication.Conclusion Transgastric access for laparoscopy-assisted biopsy of retroperitoneal lymph node is feasible and safe in human being.
4.Human genetic resources management of the Investigator-Initiated Clinical Trials

Objective:To encourage the innovation of medical science and technology, regulate the Investigator Initiated Clinical Trials (IIT), this paper analyzed the problems faced by the management of human genetic resources in medical institution, and the corresponding management measures.Methods:Existing problems of human genetic resource management in IIT were discussed and summarized, taking into consideration of regulatory requirements and daily practices in the hospital.Results:Problems identified include imperfect supervision mechanism, incomplete policy interpretation, insufficient understanding of human genetic resources management policies by researchers, and how to support innovation with human gentic resources and so on.Conclusions:Medical institutions should establish and improve the supervision system and strengthen the whole process management of IIT. Accurately interpret regulatory requirements to improve application efficiency. Improve researchers′ recognition of human genetic resources to avoid omission and misrepresentation. Build a biobank to encourage legal and compliant use of human genetic resources to carry out high-quality IIT.

Objective To strengthen human genetic resource management in clinical trials.Methods This article analyzes the common problems in the process of reviewing human genetic resources application by the drug clinical trial institute in our hospital,and proposes solutions for solving the problems.Results Common questions are also the key points for future review of human genetic resources applications,including the collection of sample and consistency with clinical trial plan,ethical review and informed consent,intellectual property rights etc.Conclusions Strictly reviewing applications of human genetic resources,as well as strengthen the management of human genetic resources in clinical trials,are not only make traceability of human genetic resources traceable,but also have important significance for the authenticity and scientific validity of trial results.
6.Effects of different antigen stimulation methods on the proliferation of HIV specific CD8 T cells

Objective To study the factors that influence the ex vivo proliferation of human immunodeficiency virus (HIV) specific CD8 T cells.Methods Three methods to use HIV-specific antigen peptides stimulating the proliferation of virus specific CD8 T cells from HIV patients were compared,and the effect of adding exogenous IL-2 into co-culture on proliferation was evaluated.Results In comparison to directly adding MHC I matched HIV peptides into peripheral blood mononuclear cells (PBMCs) by presence during co-culture or incubating for 2 hours,the more efficient proliferation of HIV-specific CD8 T cells was stimulated by loading HIV peptides on the mixed antigen presenting cells (APCs) that harvested by removing CD8 T cells from PBMCs.Adding IL-2 into co-culture could largely enhance the number of proliferated HIV-specific CD8 T cells but also induced non-specific proliferation of CD8 T cells.Conclusions Stimulation of HIV-specific CD8 T cells by loading HIV-specific antigen peptides on mixed APCs and in absence of IL-2 can specifically induce the efficient proliferation of HIV-specific CD8 T cells by providing both antigen and co-stimulation signals.
7.Analysis on correlation between serum levels of TAT,t-PAIC,TM and the progress of intracranial atherosclerotic stenosis

Aim To investigate the serum levels of thrombin-antithrombin complex(TAT),tissue type plas-minogen activator-inhibitor complex(t-PAIC)and thrombomodulin(TM)in patients with intracranial atherosclerotic steno-sis(ICAS),and their correlations with the degree of stenosis.Methods A total of 196 ICAS patients(ICAS group)who underwent treatment in Cangzhou People's Hospital from January 2021 to February 2023 were enrolled as research sub-jects.Based on the degree of vascular stenosis,they were separated into three groups:mild group(n=78),moderate group(n.=64),and severe group(n=54).A group of 196 healthy outpatient with similar clinical basic data to ICAS patients was selected as controls.The serum levels of TAT,t-PAIC,and TM in each group were compared;Spearman method was applied to analyze the correlation between serum levels of TAT,t-PAIC,TM and stenosis severity in ICAS pa-tients;Multivariate Logistic regression was applied to analyze the influencing factors of severe stenosis in ICAS patients;ROC curve was applied to analyze the predictive value of serum TAT,t-PAIC,TM and total cholesterol(TC)levels for se-vere stenosis in ICAS patients.Results Compared with the control group,the serum levels of TAT,t-PAIC,and TM were significantly increased in the ICAS group(P<0.05);the levels of serum TAT,t-PAIC,TM,and TC in the mild,moderate,and severe groups increased accordingly(P<0.05).Spearman analysis showed that the serum levels of TAT,t-PAIC,and TM in ICAS patients were positively correlated with the degree of stenosis(r=0.574,0.695,0.628;all P<0.05).Multivariate Logistic regression analysis showed that TAT,t-PAIC,TM,and TC were independent risk factors for severe stenosis in ICAS patients(P<0.05).The ROC curve showed that the AUC of severe stenosis in ICAS patients predicted by combination of TAT,t-PAIC,TM,and TC was 0.927,with a sensitivity of 83.33%and a specificity of 86.62%,which was superior to the independent prediction of TAT,t-PAIC,TM and TC(Zcombined detection-TAT=4.617,Zcombined deteetion-t-PAIC=4.024,Zcombined detection-TM=4.004,Zcombined detection-TC=7.078,all P=0.000).Conclusion The ser-um levels of TAT,t-PAIC,and TM in the ICAS group were significantly increased,and were positively correlated with the severity of stenosis.The combination of the three and TC has a high predictive value for the occurrence of severe stenosis in ICAS patients.

With the continuous development of gene therapy, more new technologies have been found and gradually applied in the basic research and clinical treatment of various diseases. In the field of AIDS treatment, gene therapy shows obvious advantages over traditional antiretroviral therapy. In recent years, short hairpin RNA (shRNA), clustered regulatory interspaced short palindrome repeat (CRISPR), transcription activator-like effector nucleases (TALEN), and zinc finger nucleases (ZFN) have been applied in AIDS treatment and have fruitful results. This paper reviews the major breakthroughs in this field since the emergence of gene therapy and the latest research achievements and their progress, with a view to providing a useful reference on a wider range of gene therapy application.
