1.Utrasonic technology-assited microsurgery resection of ventral or ventrolateral spinal schwannomas

Objective To explore the application of utrasonic technology-assited microsurgery resection of ventral or ventrolateral spinal schwannomas.Methods Forty patients with ventral or ventrolateral spinal schwannomas were treated by CUSA-assited microsurgery.Results The tumors were totally resected suc-cessfully acompanied by minimal blood loss and adjacent structures could avoid damaged.Preoperative and postoperative magnetic resonance imaging demonstrated complete resection in all cases.Neurological func-tions had good recovery.With follow-up of 3 months to 3 years,there was no instability of the spine or any recurrence.Conclusion utrasonic technology-assited microsurgery resection of ventral or ventrolateral spi-nal schwannomas by posterior approach appears a safe,efficient mini-invasive surgical approach,without mechanical or heat injury.
2.MYC suppresses ovarian cancer cell adhesion and invasion ability through downregulation of human integrinβ1

Objective To explore the influence of MYC expression level on ovarian cancer cells adhesion and invasion ability,and the involvement of the integrinβ1 in the adhesion and invasion control. Methods Comparative study was done to analyze the relationship between the mRNA level of MYC and ITGB1 among sixty cell lines from the NCI-60 cells bank; Western blot was performed to demonstrate the correlation of MYC and ITGB1 protein level in ovarian cancer cell lines;Immunohistochemistry was adopted to compare the expression level of MYC and ITGB1 in paired primary and metastatic ovarian cancer tissue. Furthermore ,we employed in vitro adhesion test to detect the alteration of ovarian cancer cell adhesion ability to ECM molecular like fibronectin ,collagen I and laminin after fluctuation of MYC and ITGB1 expression; Transwell assay was applied to check the invasion ability variation with the disturbance of MYC and ITGB1 level. Results The mRNA levels of MYC and ITGB1 were dramatically inverse-correlated in NCI-60 cells bank(P<0.01);The protein levels of MYC and ITGB1 in ovarian cancer cell lines were also negative correlated; The primary ovarian cancer tissue showed lower expression of ITGB1 and higher expression of MYC,while the matched metastasis displayed the opposite outcome;Ovarian cancer cells reduced adhesive and invasive capacity in case of ITGB1 being down-regulated ,however, the adhesive and metastatic ability improved tremendously with elevation of ITGB1;Promoted adhesion and invasion ability and rise of ITGB1 expression were observed after downregulation of MYC,meanwhile,and inhibition of ITGB1 reversed the increasement of adhesion and invasion ability. Conclusions MYC,inversely-correlated with ITGB1 in ovarian cancer cells,inhibited ovarian cancer cells adhesion and invasion. Inhibition of MYC could lead to enhanced adhesion and invasion ability of ovarian cancer cells.
3.Comparison of three methods on bilateral proper digital nerve injuries for repairing severed fingers

Objective To compare the clinical efficacy of the nerve bow (digital nerve and cutaneous antebrachii later-als) with end-to-side neuroanastomosis and traditional end-to-end neuroanastomosis for repairing bilateral proper digital nerve inju-ries while replanting injured fingers. Methods A total of 57 patients with bilateral proper digital nerve injuries from March 2009 to September 2012 were retrospectively analyzed. The patients were divided into three groups according to different treatments:19 patients underwent nerve graft bow end-to-side neuroanastomosis. During operation, a cutaneous antebrachii laterals nerve was freed and obtained from the homolateral forearm, which were sutured with bilateral distal digital nerve end to end, then nerve bow was formed. The bilateral proximal ends of digital nerve were sutured end-to-side bow, respectively. 22 patients underwent digital nerve bow end-to-side neuroanastomosis. During operation, bilateral distal ends and proximal ends were sutured respectively;con-sequently, the distal and proximal nerve bows were formed. A cutaneous antebrachii laterals nerve was obtained from the homolat-eral forearm, then divided equally to 2 parts which were used to bridge the 2 nerve digital nerve bow end-to-side neuroanastomosis bows. 16 patients underwent nerve graft with end-to-end neuroanastomosis. The sensation of finger plup, two point discrimination and motion of joints were evaluated. Results All patients achieved primary healing of wound after operation, with 57 fingers re-covered uneventfully. In nerve graft bow end-to-side neuroanastomosis group, 18 patients were followed up for 3-15 months;the average result of sensation measurement was S3+;the average result of two point discrimination was 5.1±0.8 mm. In digital nerve bow end-to-side neuroanastomosis group, 19 patients were followed up for 4-15 months;the average result of sensation measure-ment was S3; the average result of two point discrimination was 6.3 ± 0.9 mm. In nerve graft with end-to-end neuroanastomosis group, 12 patients were followed up for 3-14 months;the average result of sensation measurement was S2, the average result of two point discrimination was 7.2±1.4 mm. According to total active motion scales, there had no difference in results of motion of joints in the 3 groups. Conclusion The nerve bow end-to-side neuroanastomosis is valuable method for repairing bilateral proper digi-tal nerve injuries at the same time, which can restore sensation of fingers.
4.Application of navigation-assited microsurgery resection of posterior fossa solid hemangioblastoma

objective To explore the application of navigation-assited microsurgery resection of posterior fossa solid hemangioblastoma.Methods The data of 16 patients with posterior fossa solid hemangioblastoma treated by navigation-assited microsurgery were analyzed retrospectively,compared with 19 patiems treated by microsurgery who suffered the same disease in the similar location(control group).Results The tumors were totally resected successfully acompanied by minimal blood loss and shortening of the operation time,compared with the control group.Manipulation and removal of these tumors were actually easier,in addition,adjacent structures can avoid damage.Conclusions Navigation-assited microsurgery is useful technique to surgical resection of fossa solid hemangioblastoma by significantly reducing blood loss at the time of surgery and operation procedure time,avoiding damage to the adjacent structures,and reducing the surgery complications and mortality.

Objective Survey the classification on diseases of patients in hospital. Discuss personnel arrange-ment of nursing staff. Methods Survey and star sickbed number,CD rate/month,nurse number accounted on nursing level and sickbed-nurse ratio in 2007, discuss personnel arrangement of nursing staff. Results It is different that the nurse number accounted by two means, Z=2.234,P=0.025. The correlation about CD rate and nurse number in theories: r=0.782,p=0.004, nurse number in theories= CD ratex0.51-17.11, F=16.543,p=0.003.Conclusion CD rate should be reasonable personnel arrangement of nursing staff.
6.Analysis of using Danhong injection to treatment coronary heart disease patients medicines based on real world HIS database.

OBJECTIVETo analyze the rules of medicinal groups that doctors treat angina pectoris using Danhong injection.
METHODTwo thousands and seventy cases from hospital information system of 10 hospitals in Beijing 3a grade hospital were collected. Then use frequencies to find the most common drug and use association rules with apriori algorithm to analyze doctor's prescription compatibility rules curing coronary heart disease.
RESULTThe compatibility among aspirin, oral nitrate, oral dose of clopidogrel and oral dose of Xintong was the most frequently used medicinal groups with Danhong injection in real world HIS database.
CONCLUSIONDanhong injection was used at treatment of coronary heart disease is often combination with similar pharmacological effects chemical drugs. Doctors should balance the benefits and the risks.

OBJECTIVETo know how Danhong injection is used in clinical practice and to provide a reference for guiding clinical use of Danhong Injection.
METHODExtract Danhong injection's post-marketing re-evaluation data from the Hospital Information System of ten three grade III-A General Hospitals in Beijing, use basic statistical analysis methods to analyze Danhong injection's indications, usage and dosage, days of treatment etc. in clinical practice.
RESULTIn patients using Danhong injection, there were more than 60 percent patients were prescribed based on main-diagnosis, first-visit and other diagnosis, which were also coincided with Danhong injection's instruction. In clinical practice, 95.5 percent of Danhong injection's administration routes conformed to the instruction and more than 90 percent of Danhong injection's dosage were within the limits prescribed by the instruction. Danhong injection was used less than 20 days successively. Danhong injection was used in combination with drugs with the action of removing blood stasis, antianginal drug, antiplatelet drug, drugs for cerebrovascular disease and so on.
CONCLUSIONDanhong injection has been used according to instruction in practice almostly.

8.Exploring real world: analysis about Shenmai injection data extracted from hospital information system database in clinical practice.

OBJECTIVETo analyze the clinical application of Shenmai injection and to learn the rule of it in the real word so that we can provide inferences to doctors.
METHODCases using Shenmai injection in hospital information system from 10 grade A hospitals in Beijing were collected. Descriptive statistic methods were used to analyze the usage, dosage and medication time.
RESULTFor the clinical application of Shenmai injection, the average age of the patients using Shenmai injection was about 64.68, but most of them came from cardiovascular diseases department Shenmai injection was usually used to treat coronary disease in Western medicine and Xiongbi (Chinese medicine syndrome name) in traditional Chinese medicine. The course of treatment was from one day to seven days, which was in accord with the drug instruction as well as the dosage.
CONCLUSIONShenmai injection was usually used for the old people. In clinical practice, the use of Shenmai injection was basically in accord with the drug instruction by doctors. However, there are still some of the doctors who did not follow the instruction and should be careful of this.

9.Discussion on Methods for Tied Survival Times in Cox Model

Cox regression model is one of the most widely used methods in the survival analysis.One assumption of this model is that there is no tie in the failure times,that is,individual has different failure times.In practical applications,the existence of ties in time data is very common.In this paper,four common methods of dealing with ties in Cox model,including Exact method,discrete model method,Efron method and Breslow method,were compared with simulation.The results showed that Exact method and discrete model were the best,but they took the longest time.Efron method and Breslow method were faster but there was a greater deviation in parameter estimation.Moreover,the sample amount and ties degree also affect the results.In general,when there are a few ties,the difference between four methods was small;and in the case of large datasets or a large number of ties,the bias of three approximation methods increased except Exact method.However,there was no significant change on computational time.While the computational time of the Exact method increased rapidly.Therefore,if the estimation precision is not as important as the estimation time,Efron method and Breslow method will be good choices.Efron method is more preferably as it is more precise.And Breslow method tends to underestimate the true β.If there is no limit in time,Exact method and discrete model can be chosen to achieve more accurate results.
10.Effect of early systematic intestinal stimulation on patients with mechanical ventilation

Objective To explore the effect of early systematic intestinal stimulation on patients with mechanical ventilation .Methods Ninety-four patients with mechanical ventilation were chosen and divided into the control group and the observation group according to the random number table , each with 47 cases.The control group received the routine nursing , and the observation group received the early systematic intestinal stimulation.The score of APACHEⅡscale, the days of mechanical ventilation , the incidence rate of multiple organ dysfunction syndrome ( MODS ) , the days of hospitalization in ICU and the mortality were compared between two groups .Results No difference was found in the score of APACHEⅡ scale before the intervention between two groups (P>0.05).The score of APACHEⅡscale was (14.60 ±7.95) in the observation group after the intervention, and was lower than (18.38 ±8.24) in the control group and (19.57 ±2.64) before the intervention, and the differences were statistically significant (t=-2.268,-4.841, respectively;P<0.05). The scores of APACHEⅡ scale were respectively (19.47 ±2.28), (15.34 ±1.63) in the control group exclude the death cases before and after the intervention , and the difference was statistically significant ( t=-13.077,P<0.01).The days of mechanical ventilation was (7.66 ±6.89) d in the observation group, and was lower than (11.96 ±8.19) d in the control group, and the difference was statistically significant (t =-2.753,P<0.01).No differences were found in the incidence rate of MODS , the days of hospitalization in ICU and the mortality between two groups (P >0.05).Conclusions The preventive implement of early systematic intestinal stimulation on patients with mechanical ventilation can improve the condition of patients , and shorten the duration of mechanical ventilation .
