Equality should be followed in allocating all health resources.By scaling the severity of inpatients with high blood pressure and investigating the relationship between severity and the amount of payment at discharge,the author discovered that patients' demands are out of line with their sonsumption of health resources,and thus equality should be paid more attention in healthcare policy making in our country.
2.Development of X-ray image guided radiofrequency catheter ablation system

Objective To investigave the X-ray guided radiofrequency ablation system. Methods The ablation catheter was guided using the X-ray image so the location of electrode on top of the catheter can be fixed on. Alternating current was carried out at the focus to make the ablation. The multi-channel electrophysiology recorder was combined here to record the electrophysiology status before, during and after the surgery to ensure safety of the patient. Results One case was reviewed, which was treated with the X-ray guided radiofrequency ablation system. After the ablation, the A waves and the V wave were separated and the ablation surgery was successful. Conclusion Some simple and fast arrhythmia can be treated effectively by the above system.
3.Epidemiological analysis of hemorrhagic fever with renal syndrome in Zhangzhou, China, 2006-2015

In this study,we analyzed the data of hemorrhagic fever with renal syndrome (HFRS) and host animals monitoring in Zhangzhou City,Fujian Province,China,2006-2015,in order to find out the epidemic situation and risk factors,and provide a scientific basis for the prevention and control strategy.A total of 171 HFRS cases,including 1 death,were reported in Zhangzhou in this decade.The incidence rate of 0.352/105 showed an upward trend (xtrend2 =58.60,P<0.01).And 70 villages and towns in 11 counties have affected,accounting for 56.00% of the counties in Zhangzhou City.The cases were mainly reported in some towns of Huaan,Nanjing and Zhaoan countries.The cases mainly occurred in countryside,which reported 142 cases.The cases in farmers accounted for the highest proportion (79.58 %),and pig farming staff accounting for 23.01% in farmer cases.Most cases were reported in municipal hospitals (86.55 %).The common clinical manifestations of those cases were fever,nausea,vomiting,backache,headache,pain,oliguria or anuria,eyelid edema and so on,with 72.67% of thrombocytopenia and of 83.09 % proteinuria positive.Compared with the proportion (80.49 %) of mice in patients' house from 2006 to 2010,that (45.83%) from 2011 to 2015 decreased obviously;however,the proportion of mice or mice droppings in workplaces were rising from 60.98% to 73.33%.Mean rodent density was 6.40% and total infection rate in rats was 7.42%.The main kind of rats with virus infection was Rattus norvegicus,carrying Seoul virus Ⅱ.Results of this study indicate that Zhangzhou is one of the foci of HFRS rodent,and the epidemic is rising in recent years,the overall in the highly distributed,while some counties showed a rising trend year by year.Thus,monitoring and control efforts in prone areas should be increased,and to carry out clinics training of HFRS in primary health care institutions.
4.The relationship between leptin receptor gene polymorphism and type 2 diabetes complicated with non-alcoholic fatty liver diseases

Objective To investigate the relationship between human leptin receptor(LEPR)gene G3057A polymorphism and type 2 diabetes complicated with non-alcoholic fatty liver disease(NAFLD).Methods 216 cases of newly diagnosed type 2 diabetes(104 cases complicated with NAFLD)and 108 cases of normal glucose tolerances(NGT)were recruited.Hemi-nested PCR-RFLP and PCR direct sequence analysis were conducted to detect the polymorphisms of LEPR G3057A polymorphism.The plasma leptin and insulin levels were measured by ELISA kit.Plasma lipid and glucose metabolic parameters were measured routinely.Liver ultrasound scanning Was carried out among all subjects.Results Type 2 diabetic patients complicated with NAFLD had higher plasma alanine aminotransferase(ALT),triglycerides(TG),low density lipaprotein cholesterol(LDL-C),leptin levels and lower plasma insulin levels than those cages without NAFLD and NGT.The variant frequency at nucleotide 3057 G to A transversion Was 76.0% in type 2 diabetic patients complicated with NAFLD,which Was also significant higher than those cases without NAFLD(62.1%)or NGT cases(53.2%)(x2=14.63,P<0.01).Conclusions The polymorphism of LEPR gene 3057 probably contributes to the onset of NAFLD in type 2 diabetes by regulating lipid metabolism and affecting insulin sensitivity.
5.The death way and its mechanisms of pancreatic cancer PANC1 cells induced by pharmacologic ascorbic acid concentrations

ObjectiveTo investigate the biological effects and its mechanisms of ascorbic acid on pancreatic cancer PANC1 cells. Methods PANC1 cells were treated by ascorbic acid of different concentrations (0 ~40 mmol/L) for 24,48,72 hours.The proliferation of PANC1 cells was analyzed by MTT method; cell cycle and apoptosis were assessed by flow cytometry (FCM); inverted microscopy and transmission electron microscopy were used to observe cell morphology. The membrane potential of mitochondria were mearured by with JC-1 staining and FCM.Meanwhile,the changes of cell morphology and mitochondrial membrane potential induced by ascorbic acid after pretreatment with hydrogen peroxidescavenging enzyme (catalase) and red blood cells were also detected. Results Ascorbic acid in pharmacologic concentrations selectively inhibited the proliferation of PANC1 cells in a dose and time dependent manner.PANC1 cells were arrested in G2/M phase after treatment with 5 mmol/L ascorbic acid [ (32.55 ± 7.14)% vs (22.00 ±1.27)%,t =5.808,P<0.05],but there was no changes on apoptosis rate [ (1.98 ± 1.80)% vs (1.09 ±0.16)% ].Inverted microscope and transmission electron microscopy showed that oncosislike cell death of PANC1 cells was induced after treatment with ≥5 mmol/L ascorbic acid.Mitochondrial membrane potential of PANC1 cells was significantly lower than that of the control group in a dose dependent manner.The descent of mitochondrial membrane potential was significantly inhibited by pretreatment with catalase and red blood cells,and the degree of cell oncosis was attenuated.ConclusionsAscorbic acid significantly inhibited the proliferation of pancreatic cancer PANC1 cells in vitro.Ascorbic acid induced PANC1 cell oncosis,but not apoptosis.The possible mechanisms of inducing oncosis may be related to the descent of mitochondrial membrane potential.
6.A conceptual framework for synopsis of information of health examination reports and its style sheet

Objective To develop a conceptual framework for the synopsis of information of health examination reports (HERs) and its style sheet so as to provide a reference for information collection and management in health examination institutions.Methods A conceptual framework for the synopsis of information of HERs and its core data elements were developed based on HERs items from 11 health examination institutions,and a style sheet for the synopsis of information of HERs was then designed.Results The conceptual framework included 4 dimensions (e.g.identification information,health-related information,health exam summary,health evaluation and guidance),16 sub-dimensions,52 core data elements and 25 value domains.The structure and content of the designed sheet were consistent with the dimensions,subdimensions and core data elements of the conceptual framework.Conclusions The developed synopsis of information of HERs could be a feasible solution for structured and standardized representation of clients' health examination information.The selection and filling of some items should be validated and refined in the future.
7.Conceptual framework and core data elements of health lifestyle questionnaire

Objective To construct a conceptual framework of a lifestyle questionnaire and define its dimensions,sub-dimensions and core data elements.Methods After review of lifestyle questionnaires of large-scale studies,domestic and international health information standards,health risk factors defined by the World Health Organization and paper-based health records,a conceptual framework was developed by using top-down and bottom-up strategies.Core data elements were chosen according to their importance,utility and experts' advice.Results Our conceptual framework included 9 dimensions (cigarette smoking,alcohol consumption,diet,water drinking,employment,physical exercises,daily living habits,sleep and toxic exposure) and 29 sub-dimensions.For instance,cigarette smoking included status,intensity,duration of smoking and kinds of tobacco.Furthermore,24 core data elements were extracted,and coded values were defined for parts of these data elements.Conclusions Our conceptual framework may serve as a reference for the development of lifestyle questionnaire,and the standardized data elements we extracted could be used in information exchang between institutions.However,standardization of the data elements should be further refined.
8.Influence of humanistic nursing intervention on psychological state and life quality in patients with chronic glomerulonephritis

Objective To study and explore the influence of humanized nursing on psychological state and life quality in patients with chronic glomerulonephritis. Methods From January 2013 to January 2014 in our hospital 60 patients with chronic glomerulonephritis according to the different nursing methods were divided into observation group (n=30) and control group (n=30). The patients in the control group treated by conventional methods of care. The observation group with humanistic nursing, the efficacy of the two groups, and two groups of the patient's quality of life score, SAS score, SDS score were compared. Results The total effective rate in the observation group was 90.00% (27/30), was sig-nificantly higher than that in the control group 70.00%(21/30), the difference was statistically significant (P<0.05). The SAS score, SDS score of intervention group and the control group after the intervention were significantly lower than be-fore intervention, and the intervention group was significantly lower than the control group, the difference was statisti-cally significant(P<0.05), Energy, pain, emotional reactions, sleep, social isolation and somatic activity score of the in-tervention group and the control group after the intervention were significantly reduced compared with those before in-tervention, and the intervention group was significantly lower than the control group, the difference was statistically sig-nificant (P<0.05). Conclusion The humanized nursing intervention can improve the effect and quality of life in chronic glomerulonephritis, anxiety, depression, so as to enable them to actively cooperate with treatment and nursing.
9.The Development of Clinical Document Standards for Semantic Interoperability in China.

OBJECTIVES: This study is aimed at developing a set of data groups (DGs) to be employed as reusable building blocks for the construction of the eight most common clinical documents used in China's general hospitals in order to achieve their structural and semantic standardization. METHODS: The Diagnostics knowledge framework, the related approaches taken from the Health Level Seven (HL7), the Integrating the Healthcare Enterprise (IHE), and the Healthcare Information Technology Standards Panel (HITSP) and 1,487 original clinical records were considered together to form the DG architecture and data sets. The internal structure, content, and semantics of each DG were then defined by mapping each DG data set to a corresponding Clinical Document Architecture data element and matching each DG data set to the metadata in the Chinese National Health Data Dictionary. By using the DGs as reusable building blocks, standardized structures and semantics regarding the clinical documents for semantic interoperability were able to be constructed. RESULTS: Altogether, 5 header DGs, 48 section DGs, and 17 entry DGs were developed. Several issues regarding the DGs, including their internal structure, identifiers, data set names, definitions, length and format, data types, and value sets, were further defined. Standardized structures and semantics regarding the eight clinical documents were structured by the DGs. CONCLUSIONS: This approach of constructing clinical document standards using DGs is a feasible standard-driven solution useful in preparing documents possessing semantic interoperability among the disparate information systems in China. These standards need to be validated and refined through further study.

10.Investigation of Data Representation Issues in Computerizing Clinical Practice Guidelines in China.

OBJECTIVES: From the point of view of clinical data representation, this study attempted to identify obstacles in translating clinical narrative guidelines into computer interpretable format and integrating the guidelines with data in Electronic Health Records in China. METHODS: Based on SAGE and K4CARE formulism, a Chinese clinical practice guideline for hypertension was modeled in Protege by building an ontology that had three components: flowchart, node, and vMR. Meanwhile, data items imperative in Electronic Health Records for patients with hypertension were reviewed and compared with those from the ontology so as to identify conflicts and gaps between. RESULTS: A set of flowcharts was built. A flowchart comprises three kinds of node: State, Decision, and Act, each has a set of attributes, including data input/output that exports data items, which then were specified following ClinicalStatement of HL7 vMR. A total of 140 data items were extracted from the ontology. In modeling the guideline, some narratives were found too inexplicit to formulate, and encoding data was quite difficult. Additionally, it was found in the healthcare records that there were 8 data items left out, and 10 data items defined differently compared to the extracted data items. CONCLUSIONS: The obstacles in modeling a clinical guideline and integrating with data in Electronic Health Records include narrative ambiguity of the guideline, gaps and inconsistencies in representing some data items between the guideline and the patient' records, and unavailability of a unified medical coding system. Therefore, collaborations among various participants in developing guidelines and Electronic Health Record specifications is needed in China.
