1.Influence of budesonide combined with montelukast on lung function and transforming growth factor-β1 of children with cough variant asthma

Objective To investigate the clinical efficacy of budesonide combined with montelukast on treatment of children with cough variant asthma,and its influence on lung function and transforming growth factor-βl.Methods One hundred and seventeen children with cough variant asthma inXi'an Central Hospital from August 2013 to June 2016 were divided into control group and observation group.Fifty-nine children in control group were treated with aerosol budesonide,while 58 children in observation group were treated with montelukast based on control group.The lung function index including VC,FEV1,FEV1/FVC,and PEF pred%,TGF-β1,inflammatory factors including IgE,IL-8,TNF-α,and clinical symptom score in both groups were detected and analyzed before and after treatment.Results After two months of treatment,clinical symptom score in each group obviously decreased,and that in observation group was significantly lower than that of control group (P < 0.05).The FEV1,FEV1/FVC,and PEF pred% of two groups showed remarkable increase,and those in observation group were much higher than those of control group (P < 0.05),while no conspicuous changes were detected in VC.Additionally,TGF-β1 of each group increased markedly,and that in observation group was much higher,while IgE,IL-8,and TNF-αin two groups showed obvious decrease,and those in observation group were significantly lower than those of control group (P < 0.05).Conclusion Budesonide combined with montelukast had a great ability on treatment of children with cough variant asthma,and could significantly improve the lung function and levels of TGF-β1,and inhibit the levels of inflammatory factors.
2.Long-term Follow-up of Patients with Varicose Veins of the Lower Extremities Treated by Electrocoagulation

Objective To summarize the therapeutic efficacy of electrocoagulation for varicose veins of the lower extremities.Methods Totally 168 cases(238 limbs)of varicose veins of the lower extremities were treated with electrocoagulation in our department from April 2000 to April 2007.Through a small ankle incision,electrocoagulation probe was inserted ascendingly into the groin via the great saphenous vein.After high ligation of the great saphenous vein,the varicose veins and great saphenous trunk were electrocoagulated.Results The patients were discharged from hospital in one to two days after the operation.None of them had incisional infection.The ulcers at the ankle were healed in four weeks.Postoperative complications in this series,included Ⅰ degree burn injury in one limb,Ⅱ degree burn injury in one limb,superficial phlebitic pain in two limbs(two cases);and numbness around the ankle in 14 limbs(10 cases).Totally 148 cases with 208 limbs were followed up for 2 to 9 years with a mean of 6.6 years.During the period,the rate of recurrent varicosity was 3.4%(7/208);and healing rates of ache,dermatitis and eczema,ankle pigment,swelling around the calf and ankle were 95.3%(162/170),85.7%(12/14),41.7%(15/36)and 36.4%(16/44)respectively.Conclusion Electrocoagulation is a minimally invasive and effective method for the treatment of varicose veins of the lower extremities,with quick recovery and low rate of recurrence.

The harm of SARS(severe acute respiratory syndrome) in 2003 is remarkable.Through analyzing the influencing factors of the infectious diseases in late 30 years,we find that the biological factors and social circumstance closely related the infectious diseases.The author suggests that we should build a general hygiene concept which aimed to improve social environment,and make effective public hygiene system the measure to prevent and control the infectious diseases.
4.Sex differences in the length ratios of metapodials in Macaca mulatta from the Taihang mountains

Objective Sex difference of the length ratios of metacarpals and metatarsals in Macaca mulatta from the Taihang mountains was studied in our laboratory. Methods The lengths of 27 metacarpals (10 males, 17 females) and 30 metatarsals(12 males, 18 females) were measured from the skeletons of 30 adult Macaca mulatta. Length ratios were constructed for all possible pairings of the five bones in each individual hand and foot. One-Way ANOVA adopting SPSS13.0 for windows was used to study the sex differences of length ratios of metacarpals and metatarsals. Results For Macaca mulatta, several of these lengths ratios exhibited substantial differences between the sexes. The metacarpal(Mc) length ratios showing the largest sex differences were 2Mc∶5Mc and 4Mc∶5Mc in both hands (P<0.01), and the metatarsal(Mt) length ratios showing the largest sex difference was 1Mt:3Mt in both feet (P<0.05). Conclusion The sex differences of metacarpals and metatarsals remained when specimens of similar size were compared. It showed that body size was not the basis for these sex differences. Various facts suggested that the sex difference of length ratios in primate metapodials was associated with sex hormones exposure, possibly during prenatal development.
5.The value of CT score in predicting surgical methods and difficulty of hepatic cystic echinococcosis

Objective To evaluate the value of MSCT scores on surgery method selection and its difficulty estimation for hepatic cystic echinococcosis. Methods Based on the CT features of hepatic cystic echinococcosis ( HCE), an integral system including 7 items wasestablished. Images of preoperative CT examination performed on 71 cases of hepatic cystic echnococcosis were retrospectively analyzed by the integral system and compared with actual surgical. Total 93 cystic lesions were divided into 3 groups based on CT scores: Group A suitable for total cystectom (0 to 5 score), Group B suitable for subtotal cystectom (6 to 10 score), and Group C suitable forendocystectomy ( ≥11 scores, or4 score in one item). Spearman rank correlation was used to analyze the relationship between WHO classification of cystic echinococcosis and actual surgery methods, the relationship between operation mode forecasted by CT scores and actual surgery methods, the relationship between vessel scores of cyst and actual surgery methods, and that between biliary score of cyst and actual surgery methods. Fisher exact test and Chi-square test were used to evaluate the incidence of residual cavity in patients with different biliary scores. Results Thirty nine cysts got 0 to 5 scores, 45 cysts got 6 to 10 scores and 9 cysts got more than 10 scores. Total cystectom was performed on 30 cysts, subtotal cystectomy on 40 cysts and performed endocystectomy on 23 cysts, actually. Actual operation methods were correlated with that predicated by CT scores ( r = 0. 741, P < 0. 01 ), and with vessels score and biliary score (r = 0. 587,0. 327 respectively,P < 0. 01 ). The higher the biliary score, the higher the incidence of postoperative biliary tract complications. Conclusions Preoperative CT scoring was an easy and objective way to accurately predict the surgery methods and its difficulty for hepatic cystic cnechnococosis.
6.Curative Efficacy of Transabdominal Ultrasound-guided Lauromacrogol Combined with Radiofrequency Ablation in Treatment of Fibroid and Its Effects on Serum E2 and LH FSH Levels

Objective:To study the curative efficacy of transabdominal ultrasound-guided Lauromacrogol combined with radiofrequency ablation in the treatment of fibroid and its effects on the serum estradiol (E2),luteinizing hormone (LH),follicle stimulating hormone (FSH) levels.Methods:90 patients of fibroid who were treated from June 2014 to June 2015 in our hospital were selected as the research objects.According to random number table,those patients were divided into the observation group (n=45) and the control group (n=45).Both groups were routinely examined before operation,the control group was treated with transabdominal ultrasound radiofrequency ablation,while the observation group was treated with transabdominal ultrasound-guided percutaneous radiofrequency ablation combined with radiofrequency ablation,Then visual analogue scale (VAS),menstrual volume,uterine fibroid volume,uterine volume,E2,LH,FSH levels,myoma symptoms and quality of life score,curative effect were compared between two groups.Results:After treatment,the total effective rate of observation group was significantly higher than that of the control group [95.55%(43/45)vs75.55%(34/45)] (P<0.05);the VAS score,menstrual volume were significantly lower than those of the control group[(1.05± 0.23)score vs.(3.62± 0.50)score,(192.35± 22.10)mL 1 vs.(236.54± 24.57)mL](P<0.05);The myoma volume,uterine volume were significantly lower than those of the control group[(32.13± 13.20)cm3 vs(40.81± 14.11)cm3,(101.93± 9.89)cm3 vs(118.36±11.20)cm3](p<0.05);the serum E2,LH,FSH levels were significantly lower than those before treatment,but there was no difference between the two groups (P >0.05);the fibroid symptoms score was significantly lower than that of the control group [(4.50 ± 1.02)score vs.(10.34± 2.21)score] (P<0.05),the quality of life score was significantly higher than that of the control group [(95.36± 14.29)score vs.(80.84 ±14.06)score] (P < 0.05).Conclusion:Transabdominal ultrasound-guided lauromacrogol combined with radiofrequency ablation could enhance the efficacy of fibroid,which might be related to the decrease of serum levels of E2,LH,and FSH.
7.Effects of propofol on the changes in myocardial Toll-like receptor 4 and TNF-αand NF-κb protein expressions and ultrastructure in ischemia-reperfusion inj ury in rats

Objective To investigate the effects of propofol on the changes in myocardial Toll-like receptor 4 (TLR-4)and TNF-αand NF-κb protein expressions in ischemia-reperfusion injury (I/R).Methods Thirty healthy male SD rats weighing 250-320 g were randomly divided into 3 groups (n=10 for each):Group A,sham operation;Group B,I/R;and Group C,propofol + I/R.In Groups B and C myocardial I/R was induced by occlusion of the left anterior descending artery (LAD)for 30 min,followed by 120 min reperfusion.In Group C propofol was given intravenously 1 0 min before myocardial ischemia,followed by continuous infusion of propofol at 5 mg/(kg·h)until the end of 120 min reperfusion.In Groups A and B normal saline instead of propofol was given. The myocardial tissues were taken at the end of 120 min;ultrastructural changes of myocardial cells were observed under X-ray electron microscope and the expressions of TLR-4 mRNA as well as TNF-αand NF-κb protein were determined.Results Ultrastructural observation under electron microscope showed significantly worsened damage in myocardial tissue structure and mitochondria in Groups B and C compared with Group A.The myocardial expressions of TLR-4 and TNF-αand NF-κb protein were significantly higher in Groups B and C than in control Group A.The myocardial expressions of TLR-4 and TNF-αand NF-κb protein were down-regulated in Group C compared with Group B.Conclusion Intravenous injection of propofol can protect against myocardial damage.Propofol can suppress the increase in myocardial TLR-4 and TNF-αand NF-κb protein expressions induced by I/R.

9.Indwelling transpulmonary artery thrombolytic therapy for acute pulmonary embolism

Objective To evaluate indwelling intrapulmonary artery catheter thromolysis for acute pulmonary embolism.Methods From June 2011 to June 2013 56 cases of acute pulmonary embolism were diagnosed by multi-row spiral CT and admitted at the Department of Vascular Surgery.The average age was (56 ± 11) years.Inferior vena cava filter was implanted and pulmonary angiography,catheter thrombectomy,indwelling pulmonary intraarterial thrombolytic therapy was performed.Postoperatively low molecular weight heparin and warfarin was given,dosage adjusted by prothrombin international normalized ratio at 2-3.Results Mean pulmonary arterial pressure (mPAP) decreased from (43 ± 7) mmHg to (22 ± 6) mmHg (P < 0.05),arterial partial pressure of oxygen (PO2) rose from (49 ± 8) mmHg to (83 ± 9) mmHg (P < 0.05).Clinical symptoms significantly relieved in 51 out of the 56 cases (91%).45 patients were followed up for an average of (15 ± 4) months,with one recurrence.There was no filter migration,vena cava thrombosis,chronic obstructive pulmonary disease and other complications.Conclusions Emergency pulmonary artery indwelling catheter thrombolysis is safe and effective therapy for acute pulmonary thromboembolism.
10.Meta-analysis of Fasudil Hydrochloride Injection in the Treatment of Aneurysm Postoperative Cerebral Vasospasm

Objective:To evaluate the curative effect and safety of fasudil hydrochloride injection in the prevention and treatment of aneurysm postoperative cerebral vasospasm by meta-analysis. Methods: The randomized controlled trials were retrieved from PubMed, EMBASE, Cochrane Library, VIP, Wangfang, CNKI and so on. Meta-analysis was conducted using RevMan 5. 0 software. Results:Totally 418 reference studies were screened, from which 11 ones were chosen including 786 patients in all. In the treatment of cerebral vasospasm (CVS), there was no significant difference between the groups (OR=1. 56, 95%CI:0. 95-2. 58, P>0. 05). While in the prevention of CVS, the incidence rate of CVS in fasudil group was significantly lower than that in nimodipine group ( OR=0. 43, 95%CI:0. 23-0. 81, P=0. 008). However, the incidence rate of ADR in fasudile group was higher than that in nimodipine group (OR=0. 43, 95%CI:0. 25-0. 75,P=0. 003). Conclusion:In the prevention of CVS, fasudil may be better than nimodipine, while the incidence of ADR is higher.
