1.Content Determination of Vitamin B1 and Vitamin B2 in Trivitamin and Calcium Gluconate,Calcium Hydro-gen Phosphate Chewable Tablets by HPLC

OBJECTIVE:To establish a method for the content determination of vitamin B1 and vitamin B2 in Trivitamin and calci-um gluconate,calcium hydrogen phosphate chewable tablets. METHODS:HPLC was conducted. The column was Inertsil C18 with the mobile phase of 1%triethylamine contained 0.02 mol/L of sodium heptane solution(pH was adjusted to 5.5 by phosphate)-methanol (74∶26,V/V)at the flow rate of 1.0 ml/min,the detection wavelength was 260 nm,temperature was 30℃and the volume was 20μl. RESULTS:There was a good linear relationship between the quality concentration of vitamin B1 and peak area in the range of 0.22-1.10μg/ml(r=0.999 8)and vitamin B2 was in the range of 0.22-1.09μg/ml(r=0.999 7). The RSDs of precision,reproducibility and stabil-ity tests were all less than 1%;the average recovery was respectively 99.66%(RSD=0.96%,n=9)and 100.31%(RSD=0.92%,n=9). CONCLUSIONS:The method is simple,accurate,sensitive and reliable and suitable for the content determination of vitamin B1 and vitamin B2 in Trivitamin and calcium gluconate,calcium hydrogen phosphate chewable tablets.
2.Simultaneous Determination of Two Components in Compound Artemisinin Analgin Tablets

Objective:To establish an HPLC method for the determination of two components in compound artemisinin analgin tab-lets. Methods:A Waters Symmetry C18 column (250 mm × 4. 6 mm,5 μm) was used with methanol-0. 02 mol·L-1 KH2 PO4 solution (25 ∶75, adjusting pH to 3. 5 with phosphoric acid ) as the mobile phase. The flow rate was 1. 0 ml·min-1 and the detection wave-length was 250 nm. The column temperature was 30℃ and the injection volume was 20 μl. Results:There was a good linear relation-ship when the content of analgin and puerarin was within the range of 50.11-601.36 μg·ml-1(r=0.999 9) and 2.64-31.74 μg· ml-1(r=0. 999 8), respectively. The average recovery was 99. 1%(RSD=0. 7%,n=9)and 99. 9%(RSD=0. 6%,n=9),respec-tively. Conclusion:The method is simple and accurate with good reproducibility,which can be used to control the quality of the prepa-ration.
3.Determination of Content and Related Substances of Cefetamet Pivoxil Hydrochloride Tablets by HPLC

Objective:To establish an HPLC method to determine the content and related substances of cefetamet pivoxil hydro-chloride tablets. Methods:A Hypersil ODS2 C18 (250 mm × 4. 6 mm,5 μm) column was used. The mobile phase was 0. 005 mol· L-1 tetrabutylammonium hydroxide solution-acetonitrile (64: 36, adjusting pH to 4. 5 with phosphoric acid). The flow rate was 1. 0 ml·min-1 . The detection wavelength was 232 nm. The column temperature was 30℃ and the injection volume was 10 μl. Results:Cefetamet was completely separated from the related substances. The equation of linear regression was Y=2.29 ×104X-4.30 ×104(r=0. 999 9) and the linear range of cefetamet was 51. 01-510. 07 μg·ml-1 . The average recovery was 99. 60% and RSD was 0. 84%(n=9). Conclusion:The method is simple, accurate and suitable. It can be used for the determination of content and related sub-stances of cefetamet pivoxil hydrochloride tablets.
4.Value of body adiposity index in the prediction of metabolic disease

Objective To discuss the value of body adiposity index( BAI) in the prediction of the risk of dysglycemia,dyslipidemia and hyperuricemia.Methods Five thousand and thirty residents as the participants from Jinshan New Area and nearby of Jinshan,Shanghai were enrolled,including 2004 males and 3026 females. The receiver operating characteristic curve( ROC) was drew to predict dysglycemia,dyslipidemia and hyperurice?mia by body mass index( BMI) ,waist circumference( WC) ,waist?hip ratio( WHR) and BAI in different gender groups.Medcale soft was used to compare the area under the curve.Results According to the ROC analysis,in males the area under curve in the prediction of dysglycemia of BMI,WC,WHR and BAI was 0.553,0.556,0. 538,0.540(P<0.05),and 0.513,0.523,0.523,0.535 in females(P<0.05).The area under curve in the predic?tion of dyslipidemia of BMI,WC,WHR and BAI was 0.641,0.626,0.563,0.588(P<0.05) in males and 0.617, 0.613,0.597,0.587(P<0.05) in females.The area under curve in the prediction of hyperuricemia of BMI,WC, WHR and BAI was 0.685,0.665,0.609,0.577(P<0.05) in males and 0.730,0.708,0.656,0.649(P<0.05) in females,and BMI, WC of AUC were higher than BAI both in males and females(P<0.05).Conclusion BAI could be a predictor for the risk of metabolic disease,but less than BMI and WC.
5.Safty analysis of cisapride combined with polyethylene glycol electrolytic solution in preoperative bowel prepara-tion for patients with colorectal cancer

Objective To explore the effect of cisapride combined with polyethylene glycol electrolytic solution on preoperative bowel preparation for patients receiving colorectal cancer operation. Methods One hundred and fifty-two patients received colorectal cancer opera-tion in our hospital from October 2010 to October 2014 were selected. Those who were treated with cisapride combined with polyethylene gly-col electrolytic solution for bowel preparation before operation were assigned to oberservation group(n=83),those who were treated with poly-ethylene glycol electrolytic solution for bowel preparation before operation were assigned to control group(n=69). The side effect after medi-cation, electrolyte concentration and incidence of incision infection between groups after operation were collected and compared. Results The incidence of nausea and vomiting was 7. 23% in observation group,which was significantly lower than 18. 84% in control group (χ2 =4. 64,P=0. 03). The difference of Na+,K+ and Cl- concentration between groups were not significant (t=1. 83,P=0. 07;t=0. 42,P=0. 68;t=1. 86,P=0. 07). The body temperature,leukocyte count and incidence of incision infection after operation in observation group were significantly lower than those in control group (t=2. 68,P<0. 01;t=3. 29,P<0. 01;χ2 =4. 90,P=0. 03). Conclusion Cisapride com-bined with polyethylene glycol electrolytic solution before operation can significantly decrease the incidence of gastrointestinal side effect after medicaiton and incision infection after operation and it is beneficial to recovery for patients with colorectal cancer operation.
6.Comparison of two kinds of noninvasive ventilation mode in premature infants with respiratory failure

Objective To investigate the difference of two models of respiratory failure in preterm infants with nasal continuous positive airway pressure(nCPAP)and nasal biphasic positive airway pressure (nBiPAP).Methods Comparative analysis of 81cases of premature infants with respiratory failure requi-ring respiratory support was needed.They were applied nCPAP and nBiPAP ventilation mode treatment.The use of breathing machine and total oxygen inhalation time,the number of treatment failures,the PEEP param-eters of the ventilator,the total number of recent complications and secondary infections were compared.Re-sults In the nBiPAP treatment group,the average time of noninvasive ventilation was(96.34±31.5)h,the total time of oxygen inhalation was(10.11 ±9.39)d,3cases of treatment failure,the incidence of severe complications was 5.56%(2/36),PEEP (4.08±0.73)cmH2O (1cmH2O =0.098kPa ).In the nCPAP group,the average time of noninvasive ventilation was(114.14±41.69)h,the total time of oxygen inhalation was(17.53±15.96)d,15cases were failed,the incidence of severe complications was 15.56%(7/45), PEEP(5.11±1.01)cmH2O.There were significant differences between the two groups(P﹤0.05).But there were no significant differences in infection control and patent ductus arteriosus (P ﹥0.05).Conclusion nBiPAP as one of the first choice treatment for premature infants with respiratory failure significantly better than nCPAP,as soon as the proper use can avoid invasive ventilation.
7.Quality Analysis of 105 Batches of Domestic Cefetamet Pivoxil Hydrochloride Oral Solid Preparation

OBJECTIVE:To provide reference for improving the homogeneity and stability of domestic cefetamet pivoxil hydro-chloride oral solid preparation and its control standrads. METHODS:105 batches of cefetamet pivoxil hydrochloride preparations (tablets,granules,dry suspensions and dispersible tablets)were tested by statutory inspection test in respects of property,identifi-cation,weight difference or load difference,moisture,microbiological limits,related substances,dissolution degree and content, etc.,and results were analyzed. Exploratory research was conducted for its impurity sources,dissolution consistency evaluation, the correlation between the remaining validity period with relative substance and content,etc. RESULTS:Statutory tests showed, 103 batches were qualified in the 105 batches of samples(98.1%). The unqualified items were property and related substances,the other items met relevant regulations. Besides,the determination method for related substances in dispersed tablets was quite differ-ent with other preparations. Results of exploratory research showed the related substances in preparations originated from raw materi-als,degradation reaction in manufacturing or storage. Compared with domestic reference tablets and granules,f2 of dissolution of other products were mostly less than 50;there was no correlation in related substances,content with the remaining validity period. CONCLUSIONS:The 105 batches of domestic oral solid preparation of cefetamet pivoxil hydrochloride are basically qualified;cur-rent standard is basically feasible for tablets,granules and dry suspensions,while the standard for dispersible tablets needed to be improved immediately.
8.Diagnosis and treatment of solitary pterygoid benign lesions.

OBJECTIVE:
To investigate the clinical and pathological features of solitary pterygoid benign lesions, as well as the treatment and outcome of endoscopic surgery.
METHOD:
We retrospectively analyzed clinical data of 4 patients with pterygoid benign lesions in our department. High resolution CT and enhanced MRI were performed before the operations, then endoscopic surgeries were carried out under the circumstance of general anesthesia. After a follow-up in 12 months to 48 months, nasal endoscopy and MRI examination were performed. Therefore we are able to understand the situation of operations and postoperative recurrences, and to inquire about the changes of symptoms and the relief of symptoms before and after surgery.
RESULT:
From the Pathological diagnosis, it showed 2 cases of cystic lesions, 1 case of spindle cell lipoma, 1 case of inflammatory lesion. Postoperative follow up showed scar formation, smooth surface, no recurrence, and no new symptom. Nasal obstruction is relieved after the surgery, and no changes in the sympotoms of headache were observed.
CONCLUSION
The combination of high resolution CT with enhanced MRI isimportant for ascertaining the location, extent and nature of the pterygoid lesions. Endoscopic surgery is a minimally invasive, safe, and effective method for the treatment of solitary pterygoid benign lesions.

9.Effects of hypertonic-hyperoncotic solution on cardiac function and extravascular lung water in children after open-heart surgery

Objectives To evaluate the effects of hypertonic-hyperoncotic solution (HHS) on cardiac function and extravascular lung water in children after open-heart surgery for congenital cardiac disease. Methods 50 children with congenital cardiac disease were randomly assigned to 2 groups. The HHS group received HHS (7.5% sodium chloride with 6% hydroxyethyl-stareh 200 kDa). The ISS group received isotonic saline solution (ISS 0.9% sodium chloride). Cardiac index (CI), extravascular lung water index (ELWI), stroke volume index (SVI), mean arterial pressure (MAP), and systemic vascular resistance index (SVRI) were measured. Immediately after sur-gery, patients were loaded either with HHS or with ISS (4 ml/kg). Sodium concentration, osmolality, thrombocyte count(TC), fibrinogen, and arterial blood gases were detected before operation, immediately after loading, 15 minutes, 1,4, 12, and 24 hours after the end of vol-ume loading. Hemodynamic parameters were recorded at the same time. The total amount of dobutamine required was documented. Results In HHS group, MAP, SVI and CI increased, and SVRI decreased significantly after the administration of HHS, compared with ISS group and before administration(P<0.01 or 0.05). Both CVP and HR were unchanged in both groups. In HHS group, ELWI decreased signifi-cantly, compared with before volume administration. But ELWI increased directly and remained elevated for 60 minutes after the administra-tion of ISS. Sodium concentration increased immediately after infusion of HHS. The postoperative need for infused dobutamine in the patients in HHS group was decreased, compared with ISS group (P<0.05). All patients left the hospital in a clinically sufficient state. Condu-sions A single infusion of HHS after cardiac surgery is safe. After cardiopuimonary bypass surgery, the administration of HHS increased CI by elevating SVI in combination with a decreased SVRI. ELWI significantly decreased, which suggest that HHS effectively counteracts, the capillary leakage.
10.Diagnostic value of exfoliative cytology for chronic oral ulcers

Purpose To evaluate the effectiveness of exfoliative cytology in chronic oral ulcers diagnosis.Methods To examined 107 cases of chronic oral ulcers which were difficult to determine the nature of the ulcer in exfoliaticve cytology,and compared postoperative histopathological results or clinical results,and made the final diagnosis with cytology.Results The qualitative diagnostic accuracy of cytology was 95.3%.The sensitivity and specificity for benign and malignant lesions was 94.6% and 100%,respectively.False positive rate was 0,and false negative rate was 5.4%,and the coincidence rate of cytological examination with the final pathology was 67.0%.Conclusion Exfoliative cytology has important reference value in chronic oral ulcer diagnosis.It is characterized by simple,rapid procedure and less trauma.Doctors can develop next treatment plan based on the results of exfoliative cytology.
