1.Feasibility of Applying Extrusion-puffing Technique to the Preparation of Qingyin Teabag

Objective To study the feasibility of applying extrusion-puffing technique to the preparation of Qingyin Teabag.Methods Extrusion-puffing techique and water decoction were used to prepare the Qingyin Teabag,and thecorresponding products were compared by pharmacodynamic tests .Meanwhile,a dissolution test was carried out for the monarch drug Radix Glycyrrhizae in the teabag before and after pressurizing-expansion,thus to explore the influence of extrusion-puffing technique on the dissolution speed of ammonium glycyrrhetate from Radix Glycyrrhizae.Results With ammonium glycyrrhetate as the index,T50 and Td of Radix Glycyrrhizae before extrusion puffing extraction were 20.25 min and 56.20 min,while were 7.975 min and 18.975 min respectively after extrusion puffing extraction,the difference being very significant.Pharmacodynamic tests showed effects of the products processed by extrusion-puffing technique were slightly superior to those by water decoction.Conclusion It is feasible to adopt extrusion-puffing technique to the preparation of Qingyin Teabag.
2.Study on the Preparation Technology of Tangzu Granula

Objective To optimize the preparation process of Tangzu Granula. Methods With the paste-forming rate and total content of paeoniflorin as the indexes, the optimal water-extraction condition was screened by orthogonal design. With hydroscopicity, fluidity and formability as the indexes, the appropriate ratio of the excipient and the formula were selected. Results The optimum water-extraction condition was as follows: 10-fold water, decoction for 2 times and 1.5 hour for each time. The optimal ratio of the excipient and the formula for preparation of Tangzu granules were two proportions of the extract and three proportions of the excipient which consisted of lactose and dextrin(2 : 1, w/ w). Conclusion The technology is effec-tive, scientific, reasonable, and practical.
3.Comparative Study on Dissolution of the Total Flavones in Pollen Typhae Before and After Wall Breaking

Objective Dissolution test was applied to study the dissolution rates of total flav ones from Pollen Typhae before and after wall breaking.Methods The content of total flavones was determined by UV -spectrophotometry an d Basket -Stirring Method.Results Using total flavones as the index,T 50 was 29.57min and T d was 41.28min before wall break-ing,and 41.27min and 54.26min respectively after wall breaking,the dif ference being very significant.Conclusion The dissolution rate of the total fla vones from Pollen Typhae is faster after wall -breaking treatment than th at before treat-ment.
5.Combination of the contrast-enhanced ultrasound and color Doppler's grade to diagnose breast masses

Objective Comparing the enhancement of contrast-enhanced ultrasound(CEUS) with the intensity of the blood signals of breast masses, and producing the parameter of peak intensity (PI), to determine whether they can reflect the differentiation of the benign breast masses from the malignant ones.Methods Fifty patients with the breast masses (25 benign,25 malignant) were implemented the contrastenhanced ultrasound inspection.The blood signals of the masses could be got before performing the CEUS,then the CEUS was performed.The enhancement of the masses was divided into 4 grades according to the enhancement of breast which was around the mass (no enhancement, low enhancement, equal enhancement,and high enhancement as well).The PIs of all masses and high enhanced massed were calculated by software in machine,then them were compared according to "the groups which had been classified by their maximal diameters.Results Forty-one of 50 cases showed an obvious enhancement using CEUS compared with the routine CDFI.Malignant masses were more obviously than that of benign ones ( P<0.05).In the 50 cases,the no enhanceed ones( n = 2) and equal enhanced ones( n = 5) were benign,and 1 case of the low enhanced masses( n = 9) was malignant.The high enhanced masses ( n = 34) were malignant or benigh.About the high enhanced masses, there were statistics meanings using the parameter of PI for the masses whose maximal diameters<2 cm( P<0.05),and no statistics meanings when their maximal diameters≥2 cm(P>0.05).Conclusions The CEUS of breast can improve the appreance of the tumor' s blood vessel obviously, especially for malignant masses.The PI of the breast benign masses are different from the malignant ones.Combination of them can help to discriminate benign masses from malignant ones.The parameter of PI is useless for differentially diagnosing the breast masses if their maximal diameters≥2 cm and the blood flow grade Ⅲ before CEUS.
6.Correlation between grey scale ultrasonographic feature and estrogen receptor, progesterone receptor and human epidermal growth factor receptor-2 expression in infiltrating ductal cancer of breast

Objective To evaluate the relationship between the grey scale ultrasonographic feature and estrogen receptor(ER),progesterone receptor(PR)or human epidermal growth factor receptor-2(HER-2)expression in infiltrating ductal cancer(IDC)of breast.Methods Seventy-three patients with IDC confirmed pathologically were involved in this study.The specimens wete examined immunohistochemistically to judge the expression of the related factors.ER,PR and HER-2 expression of the mastectomy specimens were determined with immunohistochemical staining technique.The correlation between the ultrasonographic features and ER,PR or HER-2 positive expression was analyzed. Results The ER positive rate in lesions with the spicule sign,echogenic halo or posterior shadowing were higher than in lesions without the spicule sign,echogenic halo or posterior shadowing(P<0.01).The HER-2 positive rate in lesion with the calcifications and lymphadenectasis in axillary was higher than that in lesions without calcifications,or lymphadenectasis in axillary(P<0.05).Conclusions A certain correlation exists between the ultrasonographic features and ER,HER-2 expression in patients with IDC.
7.Effects of phenylethanoid glycosides from seeds of Plantago asiatica on maturation of dendritic cells.

OBJECTIVETo elucidate the immunomodulatory mechanism of phenylethanoid glycosides from the seeds of Plantago asiatica by testing its effects on the maturing of murine bone marrow derived dendritic cells (DCs).
METHODMonocytes generated from bone marrow of Balb/cj mouse were cultured for 6 days in complete RPMI 1640 medium containing 10% FBS, rmGM-CSF and rmIL-4.50 mg x L(-1) acteoside or isoacteoside was added to cells on day 6 of culture for 24 h. The surface molecules expression level of DCs and their phagocytose ability were analysis by flow cytometry.
RESULTBoth acteoside and isoacteoside could increase the expression of CD11c, CD86, MHC II and CD80 on DCs surface. The ability of unstimulated DCs to uptake FITC-dextran was higher than that of phenylethanoid glycosides or LPS treated DCs.
CONCLUSIONBoth acteoside and isoacteoside could induce maturation of murine dendritic cells.

8.Development and Application of Pharmacy Relevant Popular-science Video Courseware in Our Hospital

OBJECTIVE:To provide reference for the development and application of pharmacy popular-science video courseware. METHODS :Combined with the problems existing in the medication of patients with chronic diseases at the present stage and the key points of medication management ,the theme ,content and form of this popular-science were determined ;the video shooting and production were completed ,and publicity and promotion were carried out. Questionnaire survey was conducted , and the reception of pharmacy outpatient service in our hospital was counted ;the broadcasting effects of popular-science video were analyzed. RESULTS :The theme of popular-science video courseware was determined to be self-medication management of patients with chronic diseases at home assisted by pharmacists. Combined with the “five moments ”of medication safety proposed by WHO , the content included six units as knowing pharmacists ,understanding drugs ,taking drugs ,adding drugs ,checking drugs ,and discontinuing drugs. PPT video was used to explain the contents of each unit ,with an average duration of about 25 min. After the first unit was launched ,a total of 39 327 hits were received within two months. A total of 100 questionnaires were sent out ,97 questionnaires were collected (recovery rate of 97.0%). Among them ,80(82.5%)thought it helpful to watch the video. Within two months after the broadcast of unit 1 video,the reception volume of pharmacy outpatient department of our hospital (1 160 person times )increased by 104.2% compared with the two months in the same period last year (568 person times ),and increased by 18.7% compared with the two months before the broadcast (977 person times );99 patients came to the pharmacy outpatient department to consult because of watching the video ,and 55.6% of the registered patients consulted the demonstration of video courseware and repeatedly mentioned related problems. CONCLUSIONS :It is a beneficial exploration for pharmacists to develop popular-science video courseware with the theme of safe and rational drug use. It is the direction for future work to increase the scope of disease types ,target different groups of people or focus on different types of drugs and promot them in various forms.
9.Results of EGFR Mutations Detected in Pleural Effusion and Its Clinical Significance in 132 Patients with Advanced Non-small Cell Lung Cancer: A Retrospective Study in A Single Center.

BACKGROUND:
The lack of pathological quality control standard in detecting epidermal growth factor receptor (EGFR) gene mutation in malignant pleural effusion leads to confusion in the interpretation of detection results and the clinical use of EGFR-tyrosine kinase inhibitor (TKI). Therefore, it is very important to propose quality control standards and guide the detection of EGFR mutation in pleural effusion. The aim of this study is to retrospectively analyze the results of EGFR gene mutation in pleural effusion sediment section according to strict pathological quality control standards, and the therapeutic effect of EGFR-TKIs guided by this detection results.
METHODS:
From January 2012 to June 2018, the clinical data of patients with pleural effusion collected from Department of Pathology of Peking Union Medical College Hospital were analyzed retrospectively. Among them, 132 patients with relatively complete clinical data and with EGFR gene mutation detection of paraffin-embedded pleural effusion sediment section according to the established quality control standard were included. According to the results of EGFR gene mutation, it was divided into positive group and negative group, and the efficacy of EGFR-TKIs in different groups was compared.
RESULTS:
After the centrifugation of pleural effusion, the sediment was embedded in paraffin, sectioned, and observed under the microscope after HE staining. If the number of tumor cells ≥100, it met the pathological quality control standard, and it could be used for subsequent EGFR gene mutation detection. EGFR gene mutations were detected in 72 (54.5%) of 132 patients. EGFR-TKIs were used in 69 of 72 mutation positive patients. Of 60 EGFR mutation negative patients, only 15 used EGFR-TKIs. In EGFR mutation positive group, the disease control rate (DCR) was 95.8%, and the median progression-free survival (PFS) was 11 months. In EGFR mutation negative group, the DCR was 0%, and the median PFS was 1 month. The DCR and PFS were significantly different between the two groups (P<0.05).
CONCLUSIONS
According to the pathological quality control standards, the embedded section of pleural fluid sediment can be used to detect EGFR gene mutation, and the results can be used to guide the clinical use of EGFR-TKIs.
