Objectives To investigate the correlation between dietary fatty acids intake of pregnant women and neonatal anthropometry at birth. Methods Women in early pregnancy were recruited with appropriate value of weight gain in pregnancy. Instant photography was used to assess the dietary situation in both the second trimester and the third trimester to calculate the intakes of food, energy, macronutrients, and fatty acids. The body weight, height and BMI at birth were evaluated with Z scores. The correlation between dietary fatty acids of pregnant women and neonatal anthropometry at birth were analyzed. Results There were 516 pregnant women recruited in this study. The average intakes of polyunsaturated fatty acids (PUFA) and the proportion of total fatty acids in the two trimesters were 15 . 09 g/d, 23 . 93% and 17 . 18 g/d, 24 . 86%. In the second trimester the intakes of n-6 and n-3 PUFA were 14 . 23 g/d and 3 . 45 g/d, and in the third trimester, n-6 and n-3 PUFA were 16 . 08 g/d and 3 . 81 g/d, the average intakes in the third trimester were signiifcantly higher than those in the second trimester (P?0.05).n-6/n-3PUFA ratio was 4.11 and 4.28?in the second trimester and the third trimester, respectively, without signiifcantdifference between the two trimesters (P?>?0 . 05 ). The intake of DHA ( 64 . 43 mg/d) in the second trimester was lower than that in the third trimester 75 . 12 mg/d, (P?0 . 05 ). The percentage of linoleic acid (LA) and α-linolenic acid (ALA) contribution to energy were 5 . 95%, 1 . 42% and 6 . 20%, 1 . 45% in the second and the third trimesters, respectively. There was no signiifcant difference between the two trimesters (P?>?0 . 05 ).The dietary intakes of n-3 PUFA, n-6 PUFA and n-6/n-3 ratio in the second trimester were positively correlated with neonatal BMI r=0 . 142~0 . 189 , P?0.05). But in the third trimester, only n-3PUFA and DHA were positively correlated with birth weight ( r=0.206, 0.193, P?0 . 05 ); there was no correlation between n-6/n-3 ratio and neonatal BMI ( r=-0 . 018 , P?>?0 . 05 ). Conclusions The dietary ratio of n-6/n-3 PUFA in the second trimester was positively correlated with neonatal BMI, suggesting that moderately increasing the intake of n-3 PUFA may play a positive role in reducing childhood obesity.

3.A long-term follow-up study on effect of chronic tonsillitis with mesangial proliferative glomerulonephritis after tonsillectomy
Chinese Journal of Modern Nursing 2015;(12):1372-1375
Objective To observe the curative effect of chronic tonsillitis with primary mesangial proliferative glomerulonephritis after tonsillectomy by follow-up. Methods We retrospect to analyze 67 patients with definite diagnosis of mesangial proliferative glomerulonephritis and complete data in hospital during January 2009 to December 2011. According to whether they allowed to have tonsillectomy, we divided them into experimental group ( 49 cases, accepted tonsillectomy ) and control group ( 18 cases, do not accepted tonsillectomy) . All patients in two groups underwent routine care of nephritis during follow-up. We compared and analyzed the general information, precursory symptom, the process of disease, clinical symptom, and 24 h urine protein amount and 1 h urine red cell count before treatment and 0. 5 month, 1 month, 6 month, 1 year and 3 years after treatment between two groups. The software of SPSS15. 0 had been used for statistical analysis. Results The amount of 24 h urine protein and 1 h urine red cell had no statistical significance before treatment, 0. 5 month after treatment between two groups (P>0. 05);24 h urine protein amounts were (0. 89 ± 0. 38), (0. 59 ± 0. 23), (0. 41 ± 0. 15), (0. 38 ± 0. 17) g at 1 month, 6 months, 1 year, 3 years after treatment lower than those of the control group (t=5. 711,8. 563,10. 282,10. 488,respectively; P<0. 001); the amounts of urine red cell were statistically significant at 1 month, 6 months, 1 year, 3 years after treatment between two groups (P<0. 001). The total curative effect in the experiment group was higher than that of the control group (Z=2. 316,P=0. 024);the further improvement rate of experimental group was better than that of the control group after 1 year′s treatment, but it represented an opposite trend between two group after 3 years′ treatment (Z=5. 832,P=0. 002). Conclusions The three-year follow-up results confirms that tonsillectomy could delay the process of glomerulonephritis and might benefit long-term prognosis and the existence of kidney, but we should consider the contraindications and adapted symptom, and utilized tonsillectomy at the early stage of disease with stable general condition of health.

Objective:To explore the current situation of perceived stress and coping style based on SWOT analysis among nurses of Operating Room in the duration of second pregnancy and their influencing factors so as to give advice on reducing the stress of nurses of Operating Room in the duration of second pregnancy. Methods:From January to August 2019 , a total of 162 nurses of Operating Room in the duration of second pregnancy at all ClassⅢ Grade A hospitals in Zhengzhou were investigated with the Perceived Stress Scale (PSS) and the Simplified Coping Style Questionnaire (SCSQ) . SWOT analysis was used to assess the advantages, disadvantages, opportunity and threat of external environment, as well as the systematic summary analysis to formulate the effective coping strategy. Results:Among those nurses of Operating Room in the duration of second pregnancy, the total score of perceived stress was (27.43±7.74) ; the scores of perceived stress tension and stress out of control were (13.01±5.10) and (14.42±4.53) respectively. The total score of coping style was (32.30±6.98) ; the scores of positive and negative coping were (21.91±5.55) and (10.40±3.90) respectively. Perceived stress total score had a negative correlation with the coping style total score ( r=-0.295, P<0.01) . Hierarchical regression analysis showed that coping style could independently explain 31.6% of the total variation of perceived stress with a statistical difference ( F=4.130, P<0.01) . Conclusions:Nurses of Operating Room in the duration of second pregnancy have the stress of health risk and take positive coping style when facing stress. Managers should carry human-based management and health education, pay attention to mental health, seek family support and actively guide to reduce the stress of nurses of Operating Room in the duration of second pregnancy.

Objective To analyze the relationship between the brain functional alterations of patients with Cushing's disease (CD) and patients' mental symptom by applying the Evaluating Emotional Scales and task functional magnetic resonance imaging (Task fMRI).Methods Task fMRI was performed on 8 patients with diagnosed CD admitted in the Department of Endocrinology of Chinese PLA General Hospital from Nov. 2015 to Nov. 2016 and 21 healthy people with matched age, gender and education level as control. Meanwhile, Self-Rating Depression Scale (SDS), Self-Rating Anxiety Scale (SAS), Positive and Negative Affective Scale (PANAS) and Cushing Quality of Life Scale (Cushing QOL) were obtained to assess the brain functions. Results Significant depression and anxiety were observed in patients with CD, and their positive affective score was substantially lower while the negative affective score was relatively higher compared with that in the controls. Task fMRI revealed that, when watching the positive pictures, the activation degree of left cerebellum and right postcentral gyrus weakened in CD patients than in the controls, and the positive correlations existed between the activation degree of left cerebellum and the 16 o'clock adrenocorticotrophic hormone (ACTH) level, and between the activation degree of right postcentral gyrus and the urinary free cortisol (UFC) level in CD patients. In contrast, when watching the negative pictures, the activation degree of left cerebellum, bilateral parahippocampal gyrus and left inferior frontal gyrus was weakened in CD patients than in the controls, and the activation degree of left cerebellum was negatively correlated to the 0 o'clock cortisol level and SAS score, but is positively correlated to the UFC level. When watching the neutral pictures, the activation degree of left cerebellum and left parahippocampal gyrus was weakened in CD patients than in the controls. Conclusions CD patients may have impaired brain function with depression and anxiety mental symptoms. By Task fMRI, it can be found that the weakened activation degree of left inferior frontal gyrus, right postcentral gyrus, bilateral parahippocampal gyrus and left cerebellum may be related to CD patients' mental symptoms.

Objective:To evaluate the relationship between preoperative malnutrition and postoperative pulmonary complications (PPCs) in elderly patients undergoing thoracoscopic and laparoscopic radical esophagectomy. Methods:The elderly patients who underwent elective thoracoscopic and laparoscopic radical esophagectomy in the Affiliated Hospital of Jiangnan University were enrolled. The general clinical data and nutritional status, Assess Respiratory Risk in Surgical Patients in Catalonia (ARISCAT) score, tumor pathological stage and operation-related variables based on the Gobal Leader Initiative on Malnutrition criteria were recorded. The patients were divided into 2 groups according to whether PPCs occurred during hospitalization, and the differences between the variables were compared.Logistic regression analysis was used to identify the risk factors for PPCs in elderly patients undergoing thoracoscopic and laparoscopic radical esophagectomy. The accuracy of the ARISCAT score and ARISCAT score combined with malnutrition in predicting the occurrence of PPCs was evaluated by receiver operating characteristic curve. Results:A total of 256 elderly patients undergoing thoracoscopic and laparoscopic radical esophagectomy were included, and the incidence of PPCs was 23.8%.There were no significant differences between patients with and without PPCs in FEV 1/FVC, age, American Society of Anesthesiologists physical status, chronic obstructive pulmonary disease ratio, malnutrition ratio and ratio of patients with high ARISCAT score ( P<0.05). The results of logistic regression analysis showed that increasing age, chronic obstructive pulmonary disease, malnutrition and high ARISCAT score were independent risk factors for PPCs.The area under the receiver operating characteristic curve of ARISCAT score and malnutrition combined with ARISCAT score in predicting the occurrence of PPCs was 0.722 and 0.777, respectively, and the difference was statistically significant ( P<0.05). Conclusions:Preoperative malnutrition is an independent risk factor for the occurrence of PPCs in elderly patients undergoing thoracoscopic and laparoscopic radical esophagectomy, which is helpful in improving the accuracy of ARISCAT score in predicting the occurrence of PPCs.

Objective:To analyze the changes in plasma endothelin-1 (ET-1) concentrations in the patients with acute respiratory distress syndrome (ARDS).Methods:Fourteen patients with ARDS induced by trauma, 8 males and 6 females, aged 19-80 yr, were studied. The severity of ARDS was graded according to the Berlin definition of ARDS after admission to intensive care unit (ICU). Venous blood samples were obtained on 1st, 3rd and 5th days after admission to ICU, the plasma ET-1 concentrations were measured by radioimmunoassay, the pulmonary vascular permeability index (PVPI) was determined by PiCCO technique, and multiple organ dysfunction (MOD) score and lung injury score (LIS) were assessed. Spearman correlation of plasma ET-1 concentrations with MOD score, LIS and PVPI was analyzed. Results:MOD score, LIS, PVPI and plasma ET-1 concentrations were significantly decreased in mild ARDS patients ( n=5) as compared with moderate ARDS patients ( n=9, P<0.05). The plasma ET-1 concentration was positively correlated with MOD score, LIS and PVPI ( r=0.69, 0.76, 0.62, P<0.001). Conclusions:Plasma ET-1 concentrations can reflect the pulmonary vascular permeability and even the severity of the disease in the early stage of ARDS, so it is necessary to carry out dynamic monitoring in the patients.

Objective:To evaluate the role of aryl hydrocarbon receptor (AhR) in the down-regulation of Clara cell secretory protein (CCSP) expression during endotoxin-induced lung injury in rats.Methods:Twenty-four clean-grade healthy male Sprague-Dawley rats, aged 8 weeks, weighing 200-250 g, were divided into 4 groups ( n=6 each) using a random number table method: normal control group (group C), acute lung injury (ALI) group, ALI+ AhR antagonist group, and ALI+ vehicle group. Lipopolysaccharide(LPS) 1 mg/kg was intratracheally instilled to develop the model of lung injury, while the equal volume of normal saline was given instead in group C. At 2 h before LPS injection, AhR antagonist 6, 2′, 4′-trimethoxyflavone solution 5 mg/kg (diluted to 1 ml in dimethyl sulfoxide solution) was intraperitoneally injected in ALI+ AhR antagonist group, while dimethyl sulfoxide solution 1 ml was given in ALI+ vehicle group. The rats were sacrificed under anesthesia at 48 h after LPS administration. The left lung was lavaged and the broncho-alveolar lavage fluid (BALF) was collected for determination of the concentrations of CCSP by enzyme-linked immunosorbent assay, and the expression of CCSP in the bronchial epithelium in right lung tissues was determined by immunohistochemistry. Results:Compared with group C, the expression of CCSP in the bronchial epithelium was significantly down-regulated, and the concentrations of CCSP in BALF were decreased in the other three groups ( P<0.05 or 0.01). Compared with ALI group and ALI+ vehicle group, the histopathological injury was significantly reduced, the expression of CCSP in the bronchial epithelium was up-regulated, and the concentrations of CCSP in BALF were increased in ALI+ AhR antagonist group ( P<0.01). Conclusions:AhR partially mediates the down-regulation of CCSP expression during endotoxin-induced lung injury in rats.
