1.Update on immunology associated with bacterial meningitis

Bacterial meningitis is one of the most common infectious diseases of the central nervous system in childhood,with high mortality and disability rate despite available treatment.Studies have shown that the host's immunity dysfunction after the infection plays a central role in neurological sequelae.The deposition and activation of complement component C3 on the bacterial surface is a key step in the complement cascade leading to elimination of the microbe,and the level of complement component C3 or C5 in cerebrospinal fluid may predict outcome of bacterial meningitis.After the microorganisms cross the blood-brain barrier,bacterial compounds are recognized by pattern recognition receptors,triggering the production of excessive cytokines,attracting leukocytes and generating oxidative stress,which contributes to neuron injury irreversibly.This review focuses on current research progress on immunology associated with pneumococcal meningitis.
2.Design and application of fall risk scale in hospitalized patients in neurology department

Objective To score the fall risk of hospitalized patients with neurologic disease using fall risk scale and observe the changes of fall risk score after this fall risk scale was used. Methods The fall risk scale in hospitalized patients in neurology department was designed. The fall risk of hospitalized patients in neurology department from January 2005 to December 2007 were assessed. Moreover, safety nursing mea-sures were brought into practice to prevent from falling down. The fall risk of hospitalized patients in neurolo-gy department from January 2002 to December 2004 were assessed too. Results Target patients enhanced the sense of preventing from falling down after the fall risk scale was used, and the incidence of falls in pa-tients was significantly decreased. Conclusions To assess the fall risk of hospitalized patients in neurology department and implement safety nursing measures among the target patients may be effective in preventing the patients from falling down.
3.Diagnostic value of auxiliary examination for childhood purulent meningitis complicated with subdural effusion

Objective To explore the clinical and peripheral blood characteristics of childhood purulent meningitis complicated with subdural effusion. Methods A retrospective cohort chart review was performed. A total of 162 consecutive cases of purulent meningitis were identified in Shenzhen Children′s Hospital from January 2009 to De-cember 2013,all of them were divided into 2 groups according to whether complicating with subdural effusion, and the clinical characteristics of 2 groups were analyzed. The clinical characteristics were analyzed. The statistical data were determined by SPSS 17. 0 software. Results Forty-nine cases(30. 25%)of them were complicated with subdural effu-sion;and 37 cases (75. 5%)of them suffered from the disease in the first 10 days after the onset of the meningitis,and the incidence was 48. 84% (42 cases) in infancy. Fever,lethargy,seizures and bulging fontanelle were its common clinical signs,and there were no significant differences in the incidence between subdural effusion group and the control group(non-subdural effusion group),the white blood cell (WBC) count did not increase markedly and C-reactive pro-tein(CRP) was high in the subdural effusion group on the admission day(Z=1. 74,2. 10,all P<0. 05),but no differ-ence in platelet( PLT) . The PLT was high in the whole meningitis cohort,the ratio of cases whose PLT was higher than 300×109/L reached 93. 75%,but no difference between the 2 groups. There were many factors of WBC on admission, including age, cerebrospinal fluid culture results and the application of antibiotics before admission, but CRP was only affected by antibiotics. Conclusions Subdural effusion is common in infancy,and most occurred in the first 10 days af-ter onset of the meningitis. On admission day the CRP,was high, while WBC did not increase too much. If CRP rises for the second time it might predict the occurrence of subdural effusion in bacterial meningitis. The increased PLT may be considered as clinical feature of infectious diseases. The clinical implication of inflammatory pathophysiology in men-ingitis needs further investigation.
4.Effect of Partial Body Weight Support Treadmill Training Combined with Brain Function Therapy on Walking Ability of Stroke Patients

Objective To explore the effects of partial body weight support treadmill training (PBWSTT) combined with brain function therapy on walking ability of stroke patients. Methods 112 stroke patients with hemiplegia were randomly allocated into treatment group (n=57) and control group (n=55). The control group received routine rehabilitation training, while the treatment group received PBWSTT combined with brain function therapy in addition. They were assessed with lower limb's movement and balance function assessment of the Fugl-Meyer Assessment (FMA), Functional Ambulation Category (FAC), and Modified Barthel Index (MBI) before and after the treatment. Results The scores of FMA, FAC, and MBI improved more in treatment group than in control group after treatment (P<0.05). Conclusion Based on the conventional rehabilitation, PBWSTT combined with brain function therapy may improve the motor function of lower limb, balance function,walking ability, and ADL more significantly in stroke patients with hemiplegia.
5.Clinical analysis of six cases of prurigo pigmentosa associated with ketogenic diet and bariatric surgery

Objective:To analyze clinical characteristics of ketosis-associated prurigo pigmentosa after ketogenic diet and bariatric surgery.Methods:Clinical data were collected from patients with ketosis-associated prurigo pigmentosa, who were diagnosed and treated in Department of Dermatology, Peking University People′s Hospital from September 2018 to September 2020. The clinical characteristics, sequelae and therapeutic effect of dietary modification were analyzed and summarized.Results:A total of 6 patients with ketosis-associated prurigo pigmentosa were collected, including 5 females who developed prurigo pigmentosa after ketogenic diet, and 1 male who developed prurigo pigmentosa after bariatric surgery. The skin lesions mainly involved the chest, back, waist and abdomen, and rarely involved the eyelids, axillae, elbows and mons pubis. Common skin lesions included urticaria-like erythema, papules and pigmentation arranged in a reticular distribution, and rare skin lesions included mung bean- to soybean-sized blisters, whose walls were liable to break. Among 5 patients undergoing routine urine analysis, 4 were positive (from + to ++++) for ketone bodies in the urine, and 3 were positive for urinary protein (+) . Pathological examination in 2 patients showed epidermal spongiosis, scattered necrotic keratinocytes, basal cell liquefaction, lymphocyte infiltration in the superficial dermis, and erythrocyte extravasation. The 6 patients were advised to eat staple foods. After dietary modification, 5 patients were nearly cured within 1 week; 1 patient, who continued ketogenic diet for weight loss, still received marked improvement after the treatment with minocycline at a dose of 100 mg/d in spite of restriction of carbohydrate intake. The levels of urinary ketone bodies and urinary protein in the 6 patients all returned to normal within 1 week after treatment.Conclusions:Ketosis plays an important role in the occurrence of prurigo pigmentosa. Dietary modification alone or adjuvant medical treatment such as minocycline is effective for the treatment of ketosis-related prurigo pigmentosa.
6.Comparative study on the relationship between depression and personality and social support of urban and rural empty-nest elderly

Objective To investigate the relationship between depression and personality and social support of the urban and rural empty-nest, clear the influence factors of depression in the empty-nest elderly. Methods 398 cases of urban and rural empty-nesters selected from the Chifeng district, were valuated with the general information questionnaire, the Geriatric Depression Scale (GDS), Eysenck personality Questionnaire- Revised Short Scalr for Chinese (EPQ-RSC) and the Social Support Scale (SSRS), and to investigate the demographic data, depression, personality and social support. Results Rural cases had a higher level (14.41±3.09) points of depression than (12.90±2.64) points of urban cases, and the difference was statistically significant (t=5.254, P<0.01);rural empty nesters′subjective support (18.35 ± 2.14) points, objective support (6.37 ± 1.40) points, support utilization degree (6.01 ± 1.17) points and social support score (30.37 ± 4.24) points, were lower than (19.83 ± 2.27) points, (7.34 ± 1.13) points, (6.95 ± 1.12) points, (34.12 ± 3.73) points of urban′s, and the difference was statistically significant (t=6.673-8.490, P<0.01); the extraversion score of urban empty nesters was (7.58 ± 2.51) points which was higher than (6.37 ± 2.02) points of rural empty nesters, and the difference was statistically significant (t=5.273, P<0.01);the neuroticism score (10.38 ± 2.06) points and psychoticism score (9.42 ± 2.17) points of urban empty-nest were lower than (12.41 ± 2.25) points, (10.94 ± 2.10) points of empty-nest′s, and the difference was statistically significant (t=9.401, 7.107, P<0.01). Neuroticism and psychoticism was shown to be positively associated with depression while extraversion and social support to be negatively associated with depression (P<0.01). Regression analysis showed that marital status, income, children visit frequency and the education level were the main influencing factors of depression in the urban and rural empty-nesters. Conclusions There are differences between rural and urban empty-nest elderly in the aspect of general data, depression status, personality and social support, and should intervention them effectively according to their different characteristics.
7.Mucosal healing quality of hydrotalcite combined with esomeprazole in gastric ulcer

Objective To explore the mucosal healing quality of hydrotalcite combined with esomeprazole in gastric ulcer.Methods Forty-two patients visiting from June 2014 to December 2015 and diagnosed with gastric ulcer were selected and divided into combination therapy group and single therapy group with 21 patients in each group.The patients of combination therapy group received esomeprazole combined with hydrotalcite,and the patients of single therapy group received esomeprazole alone.The total therapeutic course was eight weeks.At the same period,21 health check-up participants were enrolled as normal control group.The healing of gastric ulcer was observed under white light endoscopy.The morphological changes of gastric pits and microvessel of mucosal at peripheral mucosa around ulcer and normal gastric mucosal were observed under narrow band imaging magnifying endoscopy.The gastric mucosa tissues of the two groups before and after treatment,and normal gastric mucosa of healthy control group were taken.The amount of deposition and composition of collagen fibers,the expression level of factor Ⅷ,the level of transforming growth factor (TGF)-beta1 and the content of hydroxyproline were analyzed by Masson,immunofluorescent and immunohistochemistry staining as well as enzyme linked immunosorbent assay.Chi square test,one-way analysis of variance (ANOVA),least significant difference (LSD) method,Dunnett's T3 and Kruskal-Wallis test and other method were used for comparison.Results After treatment,18 patients of combination therapy group (21 patients) had regular microvessel nets (85.7%),which was significantly more than those of single therapy group (12 cases,57.1%) and healthy control group (10 cases,47.6 %),and the differences were statistically significant (x2 =4.200,P=0.040).In the comparison of maturity of regenerative mucosa between combination therapy group and single therapy group after treatment,the ration between collagen type Ⅰ and Ⅱ,deposition of collagen fibers,the number of factor Ⅷ positive cells,the level of TGF-beta1 and the content of hydroxyproline were 36.05 and 23.14;269 375.63.± 171 608.63 and 137 693.14±98 330.93;34.91±8.40 and 28.24±6.93;104 498.71±40 487.96 and 70 757.11±19 323.95;(1 897.80±879.35) and (1 230.57±536.05) μg/L,respectively;while in healthy control group,the above parameters were 36.81,245 696.90 ± 224 687.00,23.10 ± 8.40,94 048.04 ±41 306.55 and 1 681.20 ± 423.61 μg/L,and the differences were statitically significant among these three groups (H=7.375,F=3.465,11.680,5.190,5.160;all P<0.05).Those parameters of combination therapy group were significantly higher than those of single therapy group (H=2.416,LSD method;all P<0.05).Conclusion Hydrotalcite combined with esomeprazole in the treatment of gastric ulcer could significantly improve microvessel morphology,maturity degree of regenerative mucosal structure and function,and the mucosal healing quality was also superior to single esomeprazole group.
8.CT imaging characteristics and comparative analysis of parotid pleomorphic adeonoma and adenolymphoma

Objective To investigate CT image features of pleomorphic adenoma and adenolymphoma,and to improve the diagno-sis accuracy.Methods CT data of 1 3 cases of pleomorphic adenomas and 9 cases of adenolymphomas confirmed by pathology were retrospectively analyzed and compared.Results All 1 3 cases of pleomorphic adeonoma were solitary lesions,in which 1 1 cases were located in the non posterior inferior quadrant,and 1 1 cases showed persistent or delayed enhancement,with “slowly forward and slowly quit”enhancement performance.In all of the 9 cases of adenolymphoma,3 cases were multiple lesions.In total of 1 3 lesions, 9 lesions were located in the posterior inferior quadrant,and 9 lesions were significantly enhanced in the arterial phase and enhance-ment was decreased in the delayed phase,which showed “quickly forward and quickly quit”.Conclusion CT image features of pleo-morphic adenoma and adenolymphoma are different.Most of lesions can be diagnosed correctly before surgery by using image fea-tures with clinical information.

Objective To observe the effect of glutamine on gut barrier function and antioxidative capacity in rats with radiation damage. Methods A total of 30 healthy Wistar male rats were randomly divided into three groups: control group, GLN enriched group(+GLN) and GLN free group( GLN). Rats in control and GLN group were fed with a basic diet, while 3% GLN was added into the diet of the +GLN group. On the 15th day, rats in GLN and +GLN groups received 9.0 Gy 60 Co ? irradiation in total body. The rate of the gut bacterial translocation and the level of endotoxin, GSH and SOD in the serum were determined at the end of the experiment. Results The rate of bacterial translocation and serum endoxin level were significantly lower in +GLN than in GLN, and GSH and SOD levels in serum were significantly higher in +GLN than in GLN. Conclusion Glutamine supplement is helpful to protect the gut barrier function and improve the antioxidation capacity in rats with radiation injury.
