1.The risk factors of critical hand,foot and mouth disease

Objective To explore the risk factors of severe hand,foot and mouth disease(HFMD) that progressed to critical illness among children.Methods The clinical data of 100 cases with severe and critical HFMD(82 cases were severe HFMD and 18 cases were critical)treated in the First Affiliated Hospital of Guangxi Medical University from January 2009 to September 2010 were analyzed retrospectively.We used univariate and multiple non-conditional Logistic regression analysis to compare the differences of the clinical features and laboratory examination between two groups,survey the risk factors of severe HFMD progressing. Results Most of the patients in both groups were under 5 years old,mainly under 3 years old which accoun-ted for 85.4% of severe HFMD cases and 88.9% of critical HFMD cases.The dominant sex was male in both groups,the sex ratio were 2.28∶1 and 8.00∶1 .The main pathogen was enteral virus 71 .Fever and rash were found in most of the severe and critical patients.The main neurological symptoms were myoclonus, tremors,limb asthenia,somnolence,vomiting and convulsion.Nervous system symptoms in critical cases were even worse to develop to coma and accompany with serious respiratory and circulatory manifestations.Univa-riate analysis showed that age ﹤2 years,tachycardia,tachypnea,elevated leukocyte count,platelet count and blood glucose level,persistent high fever,limb asthenia,pulmonary moist rales and changes on chest radio-graph were the risk factors that progressed to critical illness.The multiple non-conditional Logistic regression analysis showed that age ﹤2 years,tachycardia,limb asthenia and pulmonary moist rales were independent risk factors for severe HFMD cases progressing to critical illness.Conclusion The patients aged ﹤2-year-old,tachycardia,limb asthenia and pulmonary moist rales are closely related to severe HFMD cases progress-ing to fatal condition.

[Objective] To compare the effects of Radix Astragali (RA) and triperygium glucosides (TG) on glomerular mesangial cells (GMC) proliferation and interleukin-6 (IL-6) secretion. [Methods] GMC were cultured with in-vitro cell culture technology. Serum containing RA and serum containing TG were prepared with serum pharmacological methods. GMC proliferation was detected by methylthiazolyltetrazolium (MTT) assay and IL-6 level by enzyme-linked immunosorbent assay (ELISA) to evaluate the effect of low-, moderate- and high-dosages of RA-containing serum and TG-containing serum under the condition of GMC over-proliferation. [ Results] Different dosages of RA-containing serum and TG-containing serum inhibited GMC proliferation and IL-6 secretion in a dose-effect manner, and the inhibitory effect of high- and moderate-dosage TG and high-dosage RA was stronger. [Conclusion] Under the condition of in-vitro GMC over-proliferation, RA has the similar inhibitory effects on GMC proliferation and IL-6 secretion with TG. The combined treatment of RA and TG can reduce the dose of TG and probably has an action in increasing effect and reducing toxicity.
5.The comparison of the perioperative effect between the minimal invasive esophagectomy and open ;esophagectomy

Background and purpose:The traditional 3 incision surgery is an important means of esophageal cancer treatment, however, accompanied by more postoperative complications and higher mortality. Minimally invasive esophagectomy is a prospective technology with advantages, such as little trauma and quick recovery. This study retrospectively analyzed the perioperative effect of the esophagus cancer patients who accepted thoracoscopic-laparoscopic esophagectomy (TLE), open-laparoscopic esophagectomy (OLE) and open esophagectomy (OE) from Jan. 2013 to Jan. 2015. Methods:In this study, 72 patients received TLE, 76 patients received OLE and 115 patients received OE, respectively. One-way ANOVA, Kruskal Wallis test and Chi-square test were used to compare the differences of general clinical data, perioperative recover index, the number of lymphadenectomy and the postoperative complication among TLE, OLE and OE. Results:There were differences in the area of operative blood loss, duration of ICU stay and first standing time among the 3 groups. Pairwise comparison demonstrated that TLE group was signiifcantly better than OE group (P<0.012 5). The total number of lymphadenectomy among the 3 groups had no differences. However, the number of lymph node of the upper esophagus in the TLE groups was more than those in OLE group and OE group (P<0.001). The laryngeal recurrent nerve injury incidence in TLE group was signiifcantly higher than those in OLE group and OE group (P=0.012, 0.003). The total surgical complication had no differences among 3 groups. In areas of the cardiorespiratory system severe complication, 3 groups had statistical differences. Pairwise comparison showed TLE group was significantly less than OLE and OE group (P<0.0125). The first day and third day incidences of SIRS rate in TLE group were less than those in OLE group and OE group (P<0.0125). Conclusion:Comparing to OE group, with the same safety and feasibility, TLE had more advantages such as trauma, quick recovery, less complications. With the same effect of total lymphadenectomy to OE, TLE had the more advantage in upper mediastinal and recurrent laryngeal nerve lymph node cleaning.

The growth and development of plant is affected by environmental factors that include light,temperature and water. Many cis-elements that are response to these elicitors interact with transcription factors, and regulate the expression of target genes. It concludes by pointing out recent studies of cis-elements and transcription factors in the plant environmental response promoters, such as light, temperature and water inducible promoter. The molecular mechanisms of gene expression regulated by environmental factors was discussed. This is important to study the mechanisms that plant adapt the environment.

Objective To observe the expression of Bcl-2,Bax and proliferating cell nuclear antigen(PCNA) in liver of rats with hepatic fibrosis and the effects of transforming growth factor (TGF)-?1 vaccine on them.Methods Thirty healthy male Sprague-Dawley rats were assigned into 3 groups,named healthy control group(n=10),hepatic fibrosis group(n=10) and TGF-?1 vaccine treated group(n=10).The animal model with hepatic fibrosis was established by injecting solution dimethylnitrosamine (DMN) into abdominal cavity with concentration as 0.5% and dose as 0.2 mL/ 100 g.In TGF-?1 vaccine treated group,every rat was not only injected with DMN but also 150?g TGF-?1 vaccine protein.On the 42nd day,all rats were sacrificed.Then the blood and the liver tis- sues were collected.The expression levels of Bcl-2,Bax and PCNA in liver tissues were detected by S -P immunohistochemistry and observed by routine pathological evaluation.Alanine aminotransferase (ALT),aspartate aminotransferase(AST) and albumin(Alb) were determined by auto biochemical analytical tool.Serum levels of hyaluronic acid (HA),laminin(LN) were detected by radioimmunoas- say (RIA).Results The expression of Bax,which promoted apoptosis,directly correlated with pathological grade in liver of rats,while the expression of Bcl-2 and Bcl-2/Bax,which protected a gainst apoptosis,inversely correlated with pathological grade in liver of rats.The expression levels of TGF-?1 and Bax in healthy control group were significantly lower than those of fibrosis group,how ever,the expression levels of Bcl-2 were comparable between these two groups.As compared with fi- brosis group,the expression of TGF-?1 was significantly lower while the expression of Bcl-2 was sig nificantly higher in TGF-?1 vaccine treated group.However,the expression of Bax was comparable between these two groups.The expression level of PCNA of fibrosis group was significantly higher than that of healthy control group but dramatically lower than that of TGF-?1 vaccine treated group (Both P
8.Immunal reconstitution after autologous purified CD+34 cells transplantation in patients with systemic lupus erythematosus

Objective To investigate the variation of immune index in patients with systemic lupus erythematosus (SLE) treated with autologous purified CD+34 cells transplantation and to clarify the relationship with pathogenesis and prognosis. Methods Flow cytometry (FCM) and enzyme linked immunosorbent assay(ELISA) were used to test lymphocyte subsets, C3, C4, CH50, autoantibodies and immunoglobulin for 18 cases of SLE before and after transplantation. Results The results showed that the ratio of all the T cell subsets reduced obviously in early postgraft and recovered gradually in 1 to 3 months after transplantation except CD45 RO+CD+4 cells. The levels of serum C3, C4, CH50 increased significantly after transplantation. No case relapsed within one year after transplantation, but 2 patients relapsed one year after transplantation. The levels of the indexes in the patients with relapse were significantly lower than those in the patients with persistent remission, including C4 in the entire course, CH50 in the 3rd and 12th month after transplantation and CD45 RA+ CD+8 cells in the 6th month after transplantation. However, the ratio of CD45 RO+ CD+4 cells in the first month after transplantation in the patients with relapse was higher than that in the patients with persistent remission. Conclusion Autologous purified CD+34 cells transplantation is effective for treating SLE. Survey of immune indexes before and after transplantation is important to investigate the pathogenesis of SLE. Moreover, these immune indexes can be used to predict therapeutic efficacy of SLE.
9.Computer assisted nutrition therapy for patients with type 2 diabetes.

OBJECTIVETo evaluate the efficacy of the" Diabetes Diet Advisor"(PC-DR vision 1.0)" for type 2 diabetes.
METHODSOne hundred and fifty type 2 diabetic patients were divided in the 2 groups. The study group included 88 type 2 diabetic patients managed by "Diabetes Diet Advisor(PC-DR vision 1.0)"; the control group included 62 cases with a fixed carbohydrate content. All patients were followed up for 8 weeks.
RESULTSAfter 8 weeks dietary therapy, the fasting blood glucose (FBG), cholesterol (TC), triglycerides (TG), HDL-cholesterol (HDL- C) and body mass index (BMI) in study group were all significantly less than those in the control group (P<0.05). In the study group,the significant decrease were achieved in all indicates and FBG and 2hPBG decreased persistently. Twenty-five patients (28.41%) in study group and 6 patients (9.68%) in control group reduced their dosage of hypoglycemic agents or stopped drug therapy (P=0.007).
CONCLUSIONThe software "Diabetes Diet Advisor (PC-DR vision 1.0)" is a useful tool of dietary management for patients with type 2 diabetes.

Viral hepatitis is clinical multiple strong infectious disease, to know characteristics of traditional Chinese medicine and western medicine clinical use in patients with viral hepatitis, the research object of this study is 41 180 cases of hospitalized patients with viral hepatitis in hospital information system from 17 grade A hospitals, using frequency statistics and association rules method to analyze the traditional Chinese medicine and western medicine clinical use information, the drug kinds analysis results: western medicine of reduced glutathione tablets use frequency is highest, 14 079 cases (34.61%), traditional Chinese medicine of diammonium glycyrrhizinateuse frequency is highest, 14 058 cases (34.56%); traditional Chinese medicine and western medicine drug combination in diammonium glycyrrhizinate combined with reduced glutathione tabletsuse frequency is highest, 8 607 cases (25.09%). The mechanism of drug classification results :both traditional Chinese medicine and western medicine are the sort of educed enzyme medicine that has the highest percentage of drug use, traditional Chinese medicine 10 983 cases (27.01%), western medicine, 9 595 cases (23.59%); traditional Chinese medicine and western medicine combination in a kind of medicine to clear heat and promote diuresis combined with educed enzyme drug use frequency is highest, 5 621 cases (13.82%). Through the analysis above, combine traditional Chinese and western medicine therapy for the treatment of viral hepatitis should be given priority. Traditional Chinese medicine to clear heat and promote diuresis combined with western medicine of educed enzyme drug is the most commonly appear in clinical two drug combination scheme, traditional Chinese medicine to clear heat and promote diuresis combined with western medicine of educed enzyme drug and nucleustide analogsis the most commonly appear in clinical three drug combination scheme.
