Objective To explore the clinical and pathological characteristics of ovarian endometriosis and malignant cases, and to improve our recognition of this disease. Methods A total of 1 289 cases of ovarian endometriosis were divided into two groups: the benign group (1 275 cases)and the malignant group (14 cases). They were analyzed on: dysmenorrhea, infertility, features of ultrasound examination,site of tumour,pathological types and complications. Results Malignancy arising from ovarian endometriosis was 1.09%. As far as degree of dysmenorrhea, size of cyst >10 cm, CA125 >500 U/L, features of ultrasound were concerned, difference between the two groups was statistically significant. Conclusions In the present study, the malignant rate of ovarian endometriosis was about 1%. Malignancy should be considered seriously when the following situations occured: change in the degree of dysmenorrhea, size of cyst larger than 10 cm, CA125 higher than 500 U/L, mamilla observed by ultrasound examination.

The clinical and pathological data of eight cases of nasal solitary keratoacanthoma were analyzed and relevant literatures were reviewed. All of the cases were verified via pathological examination after local lesion excision, along with reconstruction of tissue defects in external nose. The patients were all primary healed. With a follow up from 8 months up to 7 years, only one case relapsed one year after first surgery, and given another operation, no recurrence happened thereafter. The other seven cases did not recur so far. The nasal solitary keratoacanthoma is a squamous tumor with potential malignancy, which manifests similarly with squamous cell carcinoma, and the correct diagnosis requires comprehensive judgment from both the clinical and histopathological characteristics. The disease shows favorable prognosis and low recurrence rate.

To study the underlying mechanism of vasovasotomy infertility, we set up the rabbit model and observed the levels of testosterone, IL-1 activity and TNFα, which represent the functions of monocytes and macrophages on the animal model of vasovasostomy, and probed into the in vitro effect of IL-1β on the production of testosterone by Leydig cells. Rabbit model was set up and divided into vasovasostomy fertility group (VFG) and vasovasostomy infertility group (VIG), and set up peer group of Vasectomy group (VG) and sham operation group (SOG) as controls. The results were as follows: The sperm density and total number is highest in SOG, with that in VFG markedly lower than that in SOG, but significantly higher than that in VIG. 　　1. VFG showed significant higher level of testosterone (Ts) than SOG, VG and VIG; Ts of VIG was significantly lower than SOG or VFG. In the vasovasostomy groups, sperm density is positively correlated to the level of testosterone. 　　2. The level of serum cortisol in VIG significantly higher than that in VFG, SOG, and VG. The level of serum cortisol negatively correlated to serum testosterone. 3. Serum estradiol(E2) was similar among all the groups (P＞0.05). E2/T ratio is significantly different among the groups, with that in VIG highest and that in VFG lowest. 4. Serum IL-1 activity in VIG was significantly higher than that in SOG and VFG(P＜0.05). 　　5. Serum TNFα and serum IL-1 activity share similar change, with TNFα in VG significantly higher than that in SOG(P＜0.01), VIG was the highest among the four groups(P＜0.01). Correlation test showed that serum IL-1 activity and TNFα level was negatively correlated to testosterone level; positively correlated to cortisol level and E2/T ratio. 6. Immuno-histo-chemistry of CD68 and TNFα in rabbits with vasovasostomy showed that in the testis tissue of SOG, interstitial tissue was normal with no filtration of inflammation cells including CD68 cells. In VFG and VIG, the tubules with resumed sperm production showed negative staining, and atrophic tubules and their surroundings were expression positive of CD68, with that markedly deeper in VIG than that in VFG. TNFα positive expressed granules was found only in atrophic convolted seminiferous tubules and their surroundings, with that deeper in VIG than that in VFG. 7. Culture Leydig cells were put in the following holes:(1) Control; (2) hCG; (3) IL-1; (4)IL-1 β+hCG; (5) IL-1 β+IL-1Ra; and (6) hCG+IL-1β+IL-1Ra. Determined the levels of testosterone by immuno-radiation technique. (1) IL-1 β showed no obvious effect on the Leydig cultured in vitro, but depressed the induction of Leydig cells by hCG to synthesize testosterone; (2) IL-1Ra blocked the depression of IL-1β Leydig cells' bio-synthesis of testosterone induced by hCG. In summary, one of the obvious changes in animals with vasovasotomy is low density of sperms, which is related to the lowering of testosterone. Low level of testosterone is related to the activation of monocytes and macrophages to produce IL-1 and TNFα, which negatively regulate the production of testosterone.

Objective To characterize the CT and MRI features of Osgood-Schlatter disease (OSD),and explore the feasibility of staging OSD by using CT and MRI.Methods The imaging data of the 27 cases with OSD were retrospectively analyzed,and fea-tures of MRI and CT were characterized.Results All 27 cases with OSD were featured with different extent of edema within the pa-tellar tendon.The portion already exhibited edema in the secondary ossification center in 5 cases,which showed high signal intensity on the T2 WI.There were 1 5 cases which exhibited ossified or tears in the secondary ossification center,and 5 of them were featured with opened shell-like separation on sagittal radiograph.Seven cases had isolated solitary ossicles located at the lower part of the pa-tellar ligament,which is also thickening.Among the 7 cases,one showed hypertrophy in the tibial tuberosity.Three cases examined with CT scan,exhibited ossified nodules located at the lower part of the patellar ligament.Conclusion OSD has characteristic CT and MRI features,and MRI scan is an important imaging method to show edema and morphological changes of patellar tendon in OSD,which could help to identify the stage of OSD and predict the prognosis.

BACKGROUND:Fragile bone in senile osteoporosis patients easily weakened pedicle screw fixation capacity. Therefore, bone cement leakage and screw removal cannot be solved during pedicle screw repair in osteoporotic patients with degenerative lumbar spine. OBJECTIVE:To investigate the biomechanical properties of novel pourable pedicle screws and bone cement application effect in osteoporotic patients with degenerative lumbar spine. METHODS:Six lumbar specimens (T11-L5) at the mean age of (72.9±4.2) years were selected, total y 42 vertebrae. The average bone density was 0.696 g/cm2. Any side of al vertebrae was inserted with pourable pedicle screws. 2 mL of bone cement was perfused under the X-ray fluoroscopy with cement push rod and fil ing cylinder. The same number of conventional screws was inserted into the opposite side of the samples. Three-point bending test was performed in two kinds of screws. The maximum vertebral axial pul out force test and the maximum spin torque test were conducted in vertebra to observe the destruction of the vertebral body and implanted effect. RESULTS AND CONCLUSION:(1) The ultimate strength and yield load of novel pourable cement pedicle screws were significantly more than conventional screws (P<0.05). Ultimate displacement and yield displacement were significantly less in novel pourable cement pedicle screws than in conventional screws (P<0.05). (2) The maximum vertebral axial pul out force and the maximum spin torque were significantly higher in novel pourable pedicle screws than in conventional screws (P<0.05). (3) In summary, novel pourable cement pedicle screw is simple to operate, can effectively control bone cement penetration, and improve screw stability in osteoporotic vertebrae. Moreover, after treatment, the removal of screws is convenient, cannot evidently destroy vertebral body or screw channel, and promote early recovery.

Objective To study the maternal blood glucose management of gestational diabetes mellitus during the labor and relationship of maternal blood glucose in labor and neonatal blood sugar. Methods A prospective study on maternal blood glucose monitoring and control during labor was carried in 40 pregnant women with abnormal glucose metabolism. We adjusted maternal blood glucose by low-dosage constant insulin drop during labor. Results Maternal blood glucose in labor arranged between 3.8 ～ 11.2 mmol/L,and low-dosage constant insulin drop was used in 17 cases (42.5%). The neonatal blood glucose was (4.0?1.5)mmol/L following delivery and (3.9?1.0)mmol/L at 24-hour postpartum. There were 2 cases with neonatal hypoglycemia. The neonatal blood glucose of the rest 23 cases was (4.2?1.5)mmol/L,and (3.9?1.0)mmol/L at 24-hour postpartum,and 1 case with neonatal hypoglycemia. There was no the difference of the blood glucose level of newborns between mothers with abnormal glucose metabolism and the normal pregnant women. Conclusion It is necessary to monitor and control the blood glucose level by low dose constant insulin during labor in pregnant women with abnormal glucose metabolism.

OBJECTIVETo determine the regulatory effects of the circadian clock gene Per1 on cell cycle-related genes and its influence on the proliferation, apoptosis, cycle, and tumorigenicity in vivo of human oral squamous cell carcinoma SCC15 cells.

METHODSThree groups of the short hairpin RNA (shRNA) of lentivirus recombinant plasmids were designed against the RNA of Per1 and then transfected to the SCC15 cells. The optimum interference group was screened through Western blot and quantitative real-time PCR (qRT-PCR) and assigned as the experimental group. The transfected lentivirus plasmid without an interference effect on any gene was set as the control group (Control-shRNA). Untreated SCC15 cells were set as the blank group. The mRNA expressions of cell cycle-related genes, namely, Per1, p53, Cyclin D1, Cyclin E, Cyclin A2, Cyclin B1, CDK1, CDK2, CDK4, CDK6, p16, p21, Wee1, cdc25, E2F, and Rbl1 in each group were detected through qRT-PCR. The cell proliferation, apoptosis, and cell cycle distribution in each group were evaluated through flow cytometry. The cells of the experimental group and the blank group were subcutaneously inoculated in nude mice to observe tumorigenesis.

RESULTSThree groups of Per1-shRNA lentivirus plasmids were constructed successfully. Among the groups, the Per1-shRNA- I group exhibited the highest interference effect, as indicated by qRT-PCR and Western blot analysis. As such, this group was set as the experimental group. The mRNA expression levels of CyclinD1, CyclinE, CyclinB1, CDK1, and Wee1 gene in the Per1-shRNA-I group were significantly higher than those in the Control-shRNA group and the SCC15 group (P < 0.05). By contrast, the mRNA expression levels of p53, Cyclin A2, p16, p21, and cdc25 in the Per1-shRNA-I group were significantly lower than those in the Control-shRNA group and the SCC15 group (P < 0.05). The mRNA expression levels of each gene between the Control-sLRNA group and the SCC15 group did not significantly differ (P > 0.05). The mRNA expression levels of CDK2, CDK4, CDK6, E2F, and Rb1 did not significantly differed in the three groups (P > 0.05). The proliferation index of the Perl-shRNA-I group was significantly higher than those of the Control-shRNA group and the SCC15 group (P < 0.05). The apoptosis index of the Per1-shRNA-I group was significantly lower than those of the Control-shRNA group and the SCC15 group (P < 0.05). The number of S-phase cells in the Per1-shRNA-I group was significantly lower than those of S-phase cells in the Control-shRNA group and the SCC15 group (P < 0.05). The number of G2/M-phase cells in the Per1-shRNA-I group was significantly higher than those of G2/M-phase cells in the Control-shRNA group and the SCC15 group (P < 0.05). Conversely, the proliferation index, apoptotic index, and cell cycle distribution of the cells in the Control-shRNA group did not significantly differ from those of the SCC15 group (P > 0.05). The tumorigenic ability in vivo was significantly enhanced in the Per1-shRNA-I group (P < 0.05).

CONCLUSIONPer1 is an important tumor suppressor gene. Per1 can regulate a large number of downstream cell cycle-related genes. The alteration of its expression can affect cell cycle progression, proliferation, apoptosis imbalance, and tumorigenic ability in vivo. Further studies on Per1 may elucidate cancer development and provide novel effective molecular targets for cancer treatment.

OBJECTIVE: To analyze the clinical features and treatment of tuberculous otitis media and mastoiditis. METHOD: Thirteen patients with tuberculous otitis media and mastoiditis were retrospectively analyzed and the related literatures were reviewed. One case was treated by surgery only, and anti-tuberculosis treatment was given to another one patient, and surgical removal of disease lesions in combination with anti-tuberculosis treatment were given to 12 patients. RESULT: One patient received surgery only was found to recur after follow-up for seven months. The patient did not recur after seven months' follow-up after antituberculosis therapy,and other patients did not recur. CONCLUSION Although the regular anti-tuberculosis chemotherapy treatment was the main treatment for tuberculous otitis and mastoiditis, surgical treatment was helpful to achieved more rapid healing of the ear.

AIM:Changes of rat renal aquaporin (AQP1) and AQP2 mRNA expression after partially ligating left renal vein were observed to explore molecular mechanism of renal tubular reabsorption decrease resulted by increasing renal vein pressure.METHODS:Adult male rats were randomly divided into left renal vein partial ligation group(ie. VCG, varicocele group ) and sham operation group (SOG), left renal mRNA expression was analyed by Northen blot two months later.RESULTS:Intensity of left renal AQP1 mRNA expression in VCG was weaker than that in SOG, there was not any difference in left renal AQP2 mRNA expression between two groups, no hybridization signal was found in testes.CONCLUSION:Decrease of renal tubular reabsorption resulted by partially ligating renal vein might be related to decrease of AQP1 mRNA expression; “hormone escape" might occur in collecting tubules.

Purpose To investigate the clinical application of iodine concentration using dual-energy contrast enhanced CT in distinguishing malignant from benign thyroid nodules. Materials and Methods Patients with a total of 90 pathology proven thyroid nodules (60 malignant, 30 benign) underwent dual-energy contrast enhanced CT scanning. The iodine concentration and CT value were measured in arterial phase and venous phase, then the normalized iodine concentration (NICnod) and the normalized CT value were calculated. All results were compared in each groups, while morphology and capsule of these nodules on iodine image were analyzed. Results The morphology and capsule of nodules on iodine image were significantly different between benign and malignant nodules (Z=- 4.55, P<0.05). On iodine image, the sensitivity of partial capsule diagnosing malignant nodules was 78.33% with specificity of 66.67%. The NICnod and normalized CT value for normal group, benign group and malignant group showed significant differences in arterial phase and venous phase (F=36.87-69.89, P<0.05); the NICnod and normalized CT value between benign group and malignant group showed statistic difference in arterial phase (Z=- 3.48- -2.33, P<0.05), and those among normal group, benign group and malignant group also showed statistic differences (Z= -7.01- - 4.87, P<0.05). NICnod had significant correlation with normalized CT value in each phases (r=0.89, 0.74 and 0.75, P<0.05). When using NICnod and normalized CT value of 0.76 and 0.79 to differentiate malignant thyroid nodules, the area under ROC curve values were 0.91 and 0.92. NIC and normalized CT value showed a good consistency with iodine map in venous phase (Kappa=0.762 and 0.768), Combining morphology and capsule of nodules on iodine image, the sensitivity was 90.01% and the specificity was 93.60%. Conclusion Iodine concentration with dual-energy contrast enhanced CT scanning can differentiate malignant from benign thyroid nodules; combining morphology and capsule of nodules improves the accuracy of diagnosis.
