2.Establishment of family health assessment index system in Shanghai urban communities

Objective To establish a standardized and feasible family health assessment index system based on the family practice system in Shanghai urban communities.Methods On the basis of literature review and special topic discussion, a preliminary index system framework was developed and an expert consultation questionnaire was compiled.Twenty one experts of general practice, medical specialties, health administration and public health management were invited and two rounds of improved Delphi expert consultation were conducted.The results of statistical analysis were summarized and the evaluation index system was established.Results The evaluation index system included 5 first-level indicators, 17 second-level indicators and 63 third-level indicators.The weight coefficients of first-level indicators in basic information, family structure, family function, family resources and health status of the family members were 0.199 6, 0.197 5, 0.201 7, 0.185 0, 0.216 2, respectively.In the two rounds of consultation, the positive coefficient of experts was both 100%, the degree of expert authority was 0.867 and 0.880, and the coordination coefficient was 0.294 and 0.415, respectively (P<0.05).The operational coordination coefficients were 0.320 and 0.402 (P<0.05), respectively.Conclusion The family health assessment index system constructed by the Delphi expert consultation method can provide reference for family doctors to carry out family health assessment in urban communities.

Myoblast line C_2C_ 12 cells were co-cultured with various concentrations of glucagon-like peptide-1 (GLP-1). Being induced by GLP-1, myoblast line C_2C_ 12 cells have the potential to differentiate into cells which are able to secrete insulin-like substance.
4.Effect of Electromyographic Biofeedback with Electric Stimulation Training on Dorsiflexion of Foot of Children with Spastic Diplegia

Objective To investigate the effect of electromyographic biofeedback with electric stimulation (Est-EMGBFT) training on dorsiflexion of foot of cerebral palsy children with spastic diplegia type.Methods 66 cerebral palsy children with spastic diplegia type were randomly divided into the treatment group and control group with 33 cases in each group. The treatment group was treated with rehabilitation training plus Est-EMGBFT, while the control group with rehabilitation training only. The manual muscle test (MMT) of tibial muscle and the ankle joint initiative range of motion (I-ROM) were performed before and after 4 courses of treatment, and the treatment group was assessed by electromyographic (EMG) apparatus additionally.Results After treatment, the muscular force of children in the treatment group was higher than those in the control group significantly ( P<0.01) and the level of integrated electromyography (iEMG) and root mean square (RMS) assessed by sEMG increased significantly ( P<0.05), the I-ROM of the treatment group was higher than the control group significantly ( P<0.05).Conclusion Est-EMGBFT training can provide a satisfactory rehabilitation effect for the dorsiflexion of foot of cerebral palsy children with spastic diplegia type.
5.Predictive value of ventilation and perfusion scintigraphy for the postoperative pulmonary function in patients with lung cancer

Objective To evaluate the value of ventilation and perfusion scintigraphy for predicting the postoperative pulmonary function in patients with lung cancer. Methods Twenty-six patients with lung cancer, male 21 and female 5, aged from 44 to 86 ys, were recruited into the study. Before surgery, 21 patients underwent 99Tcm-DTPA aerosol ventilation and 99Tcm-macroaggregated albumin ( MAA ) perfusion scintigraphic imaging. The other five patients were studied with perfusion imaging only. Pulmonary function was measured as forced expiratory volume in the first second ( FEV1 ) at about one week before surgery for all patients, and at two months after surgery. The predicted postoperative FEV1 ( ppoFEVt ) was calculated by Neuhaus' formula based on the ventilation or perfusion function obtained from scintigraphy studies, and compared with the measured post surgery FEV1. Eighteen patients underwent the surgical resection successfully. The t-test and correlation analysis were used. Results The ppoFEV1 values of ventilation and perfusion were (1.347±0.539) Land (1.410±0.543) L, respectively (n=21, t =0.062, P>0. 05). Both the ppoFEV1 values of ventilation and perfusion showed no significant difference with the respective post-surgeryFEV1(n=13, (1.545 ±0.588) Lvs (1.45 ±0.521) L, t=0.092, P>0.05; n=18, (1.697±0. 546) L vs ( 1.457±0.488) L, t =0. 017, P >0.05, respectively). Both the ventilation ppoFEV1 (n =13) and perfusion ppoFEV1 (n = 13, n = 18) correlated well with the post-surgery FEV1 respectively (r =0. 950, 0. 937, 0. 922, all P < 0. 01 ). Conclusion Ventilation and perfusion imaging can predict the postoperative pulmonary function for patients with lung cancer, especially for those with borderline pulmonary function, thus useful for selection of suitable candidates for surgical resection.

7.Expression of EMMPRIN, MMPs and TIMP2 in retinoblastoma and normal retinal tissue

AIM: To investigate the expression of EMMPRIN, MMP1, MMP9 and TIMP2 in retinoblastoma (RB) and normal retinal tissues and their clinicopathological significance and interrelationship.METHODS: Envision immunohistochemistry stainings of EMMPRIN, MMP1, MMP9 and TIMP2 were performed in 30 enucleated eyeballs with retinoblastoma and 15 specimens of normal retina tissue, which had been routinely imbedded with paraffin.RESULTS: Positive rate of EMMPRIN, MMP1, MMP9 expression was higher in RB tissue than in normal control (P<0.01), while TIMP2 expression was lower in RB than in normal retinal tissue (P<0.01). Samples from RB cases of clinical stage Ⅰ, differentiated type, and life span≥2 years had lower positive rate in expression of EMMPRIN, MMP1, MMP9 than those from RB cases of clinical stage Ⅲ, undifferentiated type, and life span<2 years (P<0.05 or P<0.01), while samples from RB cases of differentiated type, optic nerve unaffected, and life span≥2 years had markedly higher positive rate in expression of TIMP2 than those from RB cases of undifferentiated type, optic nerve involved and life span<2 years (P<0.05 or P<0.01). In RB tissues, EMMPRIN, MMP1, MMP9 expressions were highly consistent (P<0.05), whereas TIMP2 expression is highly inconsistent with EMMPRIN, MMP1, MMP9 expression levels (P<0.05).CONCLUSION: The expression level of EMMPRIN, MMP1, MMP9 and TIMP2 may be an important marker of RB progression, invasion and prognosis. There exist internally mutual regulation relations among them.
8.Protective effect of Jinhuang-1 on acute respiratory distress syndrome rats induced by oleic acid.

OBJECTIVETo observe the protective effect of Jinhuang-1 (JH-1) on oleic acid-induced acute respiratory distress syndrome (ARDS) in rats and provide a basis to clinical application.
METHODOleic acid was injected into the tail vein of rats and JH-1 were administered to rats. After four days, their blood were collected for the blood gas analysis. The superoxide dismutase (SOD), the malondialdehyde (MDA), nitic oxide (NO) content and the positive expression ratio of tumor necrosis factor-alpha (TNF-alpha) were determined. The pathological changes of microstructure of lung were observed.
RESULTThe results demonstrated that JH-1 could significantly increase PO2, reduce PCO2 of oleic acid-induced ARDS rats, increase the superoxide dismutase, decrease the malondialdehyde, nitic oxide content and the positive expression ratio of TNF-alpha, and improve the histological destruction on lung.
CONCLUSIONJH-1 plays a protective role for ARDS rats induced by oleic acid. The mechanism is probably related to attenuating lipid peroxidation and decreasing NO content and the expression of TNF-alpha in lung.

9.Reliability study on quantitative detection of extensor digitorum brevis strength with needle electromyography and nerve conduction.

OBJECTIVE:
To study the objectivity and reliability of needle electromyography and nerve conduction for detection of musculus extensor digitorum brevis strength, which may provide a basis for establishing a quantitative detection of muscle strength in forensic clinical study.
METHODS:
Forty-four healthy people were enrolled as the subjects, and during toe dorsiflexion, the following items including needle electromyography indexes, motor unit potential (MUP) amplitude, MUP count, recruitment reaction type, and nerve conduction detection indexes, compound muscle action potential (CMAP) amplitude, CMAP latent period and motor nerve conduction velocity (MNCV), were simultaneously detected under the cooperation and disguise condition.
RESULTS:
Under the cooperation condition, regardless of the same operator or different operators, there were good test-retest reliabilities in MUP amplitude, CMAP amplitude, CMAP latent period and MNCV, while there were normal test-retest reliabilities in MUP count and recruitment reaction type and the repeatability of the same operator was slightly better than the repeatability between different operators. Under the disguise condition, test-retest reliabilities of MUP amplitude, CMAP amplitude, CMAP latent period and MNCV were relatively high, while test-retest reliabilities of MUP count and recruitment reaction type were relatively low.
CONCLUSION
There are good test-retest reliabilities in MUP amplitude, CMAP amplitude, CMAP latent period and MNCV, which can be conducive to comparison between different operators and results at various times; MUP count and recruitment reaction type, which can be easily affected by subjectivity of operators and examinees, can be used to differentiate whether an examinee disguises or not. The indexes used to objectively judge muscle strength remain to be further investigated.

10.Effect of Jinhuang Fuzheng powder on cytokines of immunosuppressive mice with protein antibody micro-array.

OBJECTIVETo observe the effect of Jinhuang Fuzheng powder on cytokines of immunosuppressive mice by using protein antibody micro-array.
METHODThe immunosuppressive mice model was established by subcutaneously injecting cyclophosphamide. Rats were orally administered with low, middle and high dose of Huangjin Fuzheng powder for 10 days, and fasted for 12 hours after the final administration. 1 mL blood was drawn from caudal veins and isolated hearts of rats of each group. A quantitative test was conducted for cytokines with cytokine antibody array.
RESULTCompared with the control group, IFN-gamma and RANTES of the CTX group decreased significantly (P<0.05). After the administration, IFN-gamma of low, middle and high-dose groups, and RANTES of the high-dose group increased significantly (P<0.05). Compared with the control group, IL-5, IL-6, IL-9, IL-13 and MCP-1 of the CTX group increased remarkably (P<0.01, P<0.05). After the administration, IL-5, IL-6, IL-9, IL-13 and MCP-1 of low, middle and high-dose groups decreased to varying degrees. GM-CSF, IL-1alpha, IL-1beta, IL-2, IL-3, IL-4, IL-10, IL-12, IL-17, M-CSF, TNF-alpha, KC and VEGF of the 13 types of cytokines showed no significant change.
CONCLUSIONJinhuang Fuzheng powder shows effect on 20 types of cytokines of immunosuppressive mice to varying degrees, which may be related to the regulatory immunosuppression of Th1/Th2 subgroup in mice.
