1.On the Conception of Top-Quality Course Construction of Medical Ethics

In top-quality course construction of medical ethics,it is necessary to attach importance to practical operation as well as the consistency of theoretical logic.To well adapt to the current medical practice and medical technical advances,the idea of medical ethics should include virtue,harmony and value.Virtue is the prominent concept in top-quality course construction of medical ethics.Harmony is the core idea of medical ethics,and value is the innovative idea to solve the questions caused by life technologies.Therefore,in the top-quality course construction of medical ethics,coordination and unification of the above three ideas must be maintained in order to defend the demonstration significance of top-quality course of medical ethics.
2.Phacofragmentation combined with vitrectomy in the treatment of traumatic dislocation lens

AlM: To explore the clinical application effect of phacofragmentation combined with vitrectomy in the treatment of traumatic lens dislocation.
METHODS:Totally 16 cases (16 eyes) of traumatic lens dislocation treated with phacofragmentation combined with vitrectomy were retrospectively analyzed, including 6 cases of high intraocular pressure ( lOP) and 10 cases of vitreous hemorrhage, and 1 case of retinal detachment. All patients were given conventional flat line standard three channel vitreous operation incision to remove the anterior, middle part and peripheral vitreous around lens dislocation. The crystalline lens were drawn to the center cavity of vitreous body and treated by ultrasonic disintegrator. ln the operation, the retina was examined and 8 of them had no retinal damage and in the first stage underwent fixation of posterior chamber intraocular lenses.
RESULTS: All crystalline lens dislocated were completely grinded and suctioned. There was no retinal detachment occurred in 3mo followed up. 16 eyes had normal lOP (12-20mmHg) at 1wk after operation. The average visual acuity was improved and with 8 cases got improved of 0. 2 or more 1wk after operation.
CONCLUSlON: Our research shows that phacofragmentation combined with vitrectomy is a safe and effective method for the treatment of traumatic lens dislocation. The patients with nondestructive retina in the operation are feasible to do first stage operation of intraocular lens suture fixation, which contributes to the best visual acuity.
3.Surgical effect of traumatic lens dislocation with secondary glaucoma

AIM: To retrospectively evaluate the effect of lens extraction combined with vitrectomy to treat traumatic lens dislocation with secondary glaucoma.
METHODS: Thirty - one eyes ( 31 cases ) of lens dislocation caused by blunt trauma with secondary glaucoma were treated respectively with cataract extraction combined with anterior vitrectomy, trabeculectomy and intraocular lens implantation. The visual acuity and pressure were observed 1wk, 1 and 3mo after operative.
RESULTS: Thirty - one eyes were all complete the operation successfully, and 6 eyes were given combined trabeculectomy, 9 eyes were implanted anterior chamber intraocular lens implantation ( IOL ) and 15 eyes were given posterior chamber suture fixation. Sixteen eyes were implanted in one-stage operation, while 8 eyes were implanted in two-stage operation. All intraocular pressure ( IOP ) were controlled to the normal level after operation and 23 eyes had visual acuity of more than 0. 3. CONCLUSION: Lens extraction combined with vitrectomy is an effective method for treatment of lens dislocation with secondary glaucoma. In order to control the IOP and get well visual function, we should choose IOL implantation or trabeculectomy according to the patient's condition.
4.Interaction Between Anthracene Quinone Type Anti-tumor Antibiotics and DNA

OBJECTIVE To study the interaction of anthracene quinone type anti-tumor antibiotics and DNA as well as the experiment method.METHODS Spectrophotometry was developed including absorption spectrum,fluorescence spectrum,electrochemical method and so on.RESULTS The union of inlay and insertion observed in ultraviolet and visible spectrophotometry usually caused hypochromic effect and red shift.Under the certain concentrations of bovin serum albumin(BSA),the endogene fluorescence intensity of BSA orderly reduced with the increase in concentration of doxorubicin(adriamycin) hydrochloride(?em 344 and the peak shape were invariable);?ex and ?em at the biggest wave length of doxorubicin were 478 and 596 nm.The fluorescence intensity was maximal of the excitation and emission spectrum when pH was 3.0.CONCLUSIONS The interaction of doxorubicin and DNA is the strongest according to the experiment and is the most widely used at present in clinics.
5.Antibiotics Utilization among Non-surgical Patients in Respiratory Department of Our Hospital

OBJECTIVE To find out the actual situations of antibiotics utilization in Respiratory Department of our hospital and provide feedback of improper applications to the clinic,so as to promote proper utilization of antibiotics.METHODS To take samples from clinical cases of 141 samples in Oct to Nov 2006 and fill the forms of the basic information and investigation items of General Hospital of Chinese People′s Armed Police Forces,the cases were finally sorted and summarized with Excel.RESULTS The antibiotics utilization ratio was 96.5%,their combination usage accounted for 70.2%,the etiology detection rate was 61.7%,the improper usage accounted for 34.8%.CONCLUSIONS The situation of antibiotics utilization in our hospital has changed a lot.Hospitals should define regulations and reinforce the management according to The Principle Guidelines of Antibiotics Utilization and actual situations.
6.Nerve protection mechanism of acupuncture to the secondary injury in the rat of intercerebral hemorrhage

Objective Expwre to the mechanism of the function of nerve protection to the secondary injury in the rat of intercerebral hemorrhage by acupuncture.Methods 160 healthy Wistar male rats were selected.The rats were assigned into 3 group randomly:model group,pinprick set,piracetam group.containing 50 rats in each.Each group were again divided into five time phase point of6 hours,1 day,2days,3days,7days.containing 10 rats in each.All rats were made to be the intercerebral hemorrhage model.Another 10 rats were assigned as the control.The pinprick group were acupunctured on the "Baihui" penetrating "Qubin".Results In the pinprick group,the expression 0fNGF of ratg brain dssue surrounding the hematoma was increased gradually and to the peak in 7days with the increasing of different time phase point.In the same time phase point,compare with the model group,the pinprick groupb expression of NGF was higher obviously.Both were relatively showed the significant difference(P<0.01)and had statistical sigllificanle.Conclusion The therapy of acupuncture Can promote the expression of NGF of the rat'sbrain tissue surrounding the hematoma.
7.Detection of antigen-specific CD14+ monocytes in blood of tuberculosis patients by using CD4+ T-cell receptor tetramer-based flow cytometric analysis and cell climbing slice assay

Objective To evaluate the utility of CD4+ TCR tetramers‐based flow cytometric analysis and cell climbing slice assay in detecting antigen‐specific CD14+ monocytes in the blood of tuberculosis (TB) patients .Methods CD4+ TCR tetramers were used to detect tetramer‐positive CD14+ monocytes in the peripheral blood (PBL ) samples of inpatients with advanced pulmonary TB (PTB) by flow cytometric analysis .The PBL samples obtained from non‐TB patients and umbilical cords were used as controls .These tetramers were also used to examine tetramer‐bound CD14+ monocytes and Mycobacterium tuberculosis (MTB) antigen‐specific and tetramer‐bound cells by cell climbing slice in situ staining .Results The median percentage of tetramer‐bound CD14+ monocytes in PBL samples from PTB patients ,non‐TB patients and umbilical cords were 1 .32% , 0 .50% and 0 .26% respectively by using CD4+ Vα21‐J39/Vβ29‐D1‐J2 tetramer , while the medians were 1 .05% , 0 .49% and 0 .19% respectively by using CD4+ Vα21‐J39/Vβ29‐D2‐J2 tetramer . The percentage of tetramer‐bound CD14+ monocytes in PTB patients group was significantly higher than the other two control groups .In cell climbing slice in situ staining ,tetramer‐bound CD14+ monocytes ,and MTB antigen‐specific and tetramer‐bound cells were positive in PTB tissue compared with negative in control tissues . Conclusions CD4+ TCR tetramers‐based flow cytometric analysis and cell climbing slice assay could be used to sensitively detect M TB antigen‐specific CD14+ monocytes in the blood of TB patients ,and more accurately evaluate the changing profile and clinical significance of these cells in TB patients .

9.Antitumor activity of recombinant antimicrobial peptide penaeidin-2 against kidney cancer cells.

Penaeidin-2 (Pen-2) is an important antimicrobial peptide derived from the Pacific white shrimp, Penaeus vannamei, and possesses both antibacterial and antifungal activities. Recent studies suggest that recombinant penaeidins show similar activities to the native Pen-2 protein. Previous researches have shown that some antimicrobial peptides (AMPs) exhibit cytotoxic activity against cancer cells. To date, there have been no studies on the antitumor effects of Pen-2. This study evaluated the potential of recombinant pen-2 (rPen-2) in the selective killing of kidney cancer cell lines ACHN and A498, and its action mechanism. MTT assays found the maximal growth inhibition of HK-2, ACHN and A498 cells treated with 100 μg/mL rPen-2 at 48 h was 13.2%, 62.4%, and 70.4%, respectively. DNA-specific fluorescent dye staining showed a high percentage of apoptosis on cancer cells. Flow cytometry revealed that the apoptosis rate of HK-2, ACHN and A498 cells was 15.2%, 55.2%, and 61.5% at 48 h respectively, suggesting that rPen-2 induced higher apoptosis rate in cancer cells than in HK-2 cells. Laser confocal scanning microscopy demonstrated that the plasma membrane was the key site where rPen-2 interacted with and destroyed tumor cells. Scanning electron microscopy showed the morphologic changes of the cell membranes of kidney cancer cells treated with rPen-2. These results suggest that rPen-2 is a novel potential therapeutic agent that may be useful in treating kidney cancers.
10.Effect of psychological factors on quality of life in patients with irritable bowel syndrome with diarrhea

Objective To investigate the effects of anxiety and depression on the quality of life (QOL) in patients with irritable bowel syndrome with diarrhea (IBS‐D) and the difference in gender .Methods IBS‐D patients met the Rome Ⅲ diagnostic and subtyping criteria were consecutively enrolled .The intestinal symptoms , psychological status , and QOL of patients were evaluated using IBS‐specific symptom questionnaires , the Hamilton anxiety scale (HAMA) ,the Hamilton depression scale (HAMD) ,and the Chinese Version of IBS‐QOL instrument .The data were analyzed by chi‐square test variance analysis ,t test or Spearman rank correlation analysis .Results A total of 155 IBS‐D patients were enrolled .Among them ,115 were complicated with anxiety and/or depression .The number of male and female with comorbid psychological disorders was 69 cases(71 .13% ) and 46 cases(79 .31% ) ,respectively ,and the difference was not statistically significant (χ2 = 1 .267 ,P= 0 .26) . Compared with 40 patients without anxiety or depression ,there was no statistically significant difference (F=1 .143 ,P> 0 .05) in the scores of intestinal symptoms in 59 patients with comorbid anxiety alone and 56 patients with anxiety and/or depression (20 .85 ± 5 .84 vs .21 .71 ± 7 .47 vs .22 .87 ± 6 .09) .The total IBS‐QOL score of the 155 patients was 71 .61 ± 19 .22 .There was negative correlation between IBS‐QOL score and HAMA , HAMD scores(r= -0 .262 and -0 .268 ,both P= 0 .001) .The total IBS‐QOL score of patients with comorbid anxiety and depression or depression were lower than that of patients without anxiety or depression (66 .05 ± 22 .88 vs 77 .22 ± 15 .35 , F = 4 .412 , P = 0 .005) .Except health worrying and sexual ,the scores in six dimensions(including dysphoria ,interference with activity ,body image ,food avoidance and social reaction) were all significantly decreased (F= 3 .309 ,3 .279 ,4 .177 ,3 .765 ,6 .041 and 3 .830 , P= 0 .013 ,0 .012 ,0 .013 , 0 .007 ,0 .001 and 0 .010) .In male patients ,the total score of IBS‐QOL and the scores of dysphoria ,body image and social reaction of patients with anxiety and depression or depression were lower than those of patients without anxiety and depression (t = 2 .143 ,2 .110 ,2 .279 ,3 .061 ; P = 0 .036 ,0 .039 ,0 .027 ,0 .003) .In female patients ,the score of food avoidance domain significantly decreased (t= 2 .812 ,P= 0 .008) .The scores of IBS‐QOL and six dimensions (including dysphoria ,interference with activity ,body image ,food avoidance and social reaction) of patients with history of severe mental disorders and being abused were even lower than those patients without such histories (t= 3 .241 ,3 .433 ,2 .499 ,2 .296 ,2 .514 ,3 .413 and 2 .601 ;P= 0 .001 ,0 .001 ,0 .014 , 0 .023 ,0 .013 ,0 .001 and 0 .010) .Conclusions The QOL of patients with IBS‐D was significantly affected by psychological factors with gender differences .The approaches to improve the psychological status of IBS patients were helpful to enhance the efficacy of comprehensive therapy .
