Chinese Circulation Journal 2016;31(1):25-30
Objective: To explore the early diagnostic value of dynamic changes for high-sensitive troponin I (hs-cTnI) in patients with acute coronary syndrome (ACS).
Methods: A total of 186 ACS patients treated in our hospital from 2014-02 to 2015-04 were studied. The patients were divided into 2 groups:AMI group, n=169 and UA group, n=17, in addition, there was a Control group, n=13 healthy subjects. Blood levels of hs-cTnI were measured at admission and 2h, 4h after admission. Dynamic changes of hs-cTnI was calculated by slop coefifcient of hs-cTnI (Δhs-cTnI) at 3 time points and relative changes of hs-cTnI values. ROC curve was made by SPSS 16.0 statistical software and the best diagnostic cutoff point was determined by Youden index.
Results: In clinical practice, hs-cTnI=1 ng/ml was critical condition, hs-cTnI=0.15 ng/ml was recommended for AMI diagnosis. When hs-cTnI<1 ng/ml at admission, the AUC of AMI vs UA and AMI vs Control for 0 h hs-cTnI were 0.6746 and 0.8844;for relative changes of hs-cTnI were 0.9806 and 0.9631;forΔhs-cTnI were 0.9521 and 0.9778 respectively. Thus, the best diagnostic cutoff value of AMI for relative changes of hs-cTnI was 0.6705, sensitivity 70.59%, speciifcity 91.49%;forΔhs-cTnI was 0.0075, sensitivity 68.75%, speciifcity 97.78%. When hs-cTnI<0.15 ng/ml at admission, the AUC of AMI vs UA and AMI vs Control for 0 h hs-cTnI were 0.5677 and 0.7000;for relative changes of hs-cTnI were 0.9228 and 0.9975;forΔhs-cTnI were 0.9044 and 0.8906 respectively. Thus, the best diagnostic cutoff value of AMI for relative changes for hs-cTnI was 2.923, sensitivity 81.25%, speciifcity 94.12%;forΔhs-cTnI was 0.01125, sensitivity 76.47%, speciifcity 93.75%.
Conclusion: AMI could not be effectively diagnosed by hs-cTnI alone at admission, using combined relative changes of hs-cTnI andΔhs-cTnI was superior to 0 h hs-cTnI for AMI diagnosis.

To study the interaction of drugs of different properties, namely puerarin, borneol and catalpol in the process of in- clusion, in order to explore the inclusion regularity of multi-component and multi-property traditional Chinese medicine compound in- clusions. With HP-β-CD as the inclusion material, the freeze-drying method was used to prepare the inclusion. The inclusion between puerarin, borneol and catalpol was tested by measuring the inclusion concentration, DSC and X-ray diffraction. According to the find- ings, when insoluble drugs puerarin and borneol were included simultaneously, and puerarin was overdosed, puerarin included was almost equal to puerarin included, and borneol was not included. When puerarin was under-dosed, and HP-β-CD was overdosed, borne- ol was included, and the simultaneous inclusion was lower than the separate inclusion of borneol. When water-soluble drug catalpol was jointly included with puerarin or borneol, the simultaneous inclusion was almost the same with their separate inclusion, without charac- teristic peak of catalpol in DSC and X-ray diffraction patterns. There is a competition in the simultaneous inclusion between water-solu- ble drugs puerarin and borneol and a stronger competition in puerarin. The water-soluble drug catalpol could be included with HP-β-CD with no impact on the inclusion of puerarin or borneol.

OBJECTIVETo compare the difference of Euphorbia Pekinensis Radix before and after being processed with vinegar in the toxicity on rat small intestinal crypt epithelial cells IEC-6, and make a preliminary study on the mechanism of detoxication of Euphorbia Pekinensis Radix processed with vinegar.
METHODWith rat small intestinal crypt epithelial cells IEC-6 as the study object, the MTT method was adopted to detect the effect of Euphorbia Pekinensis Radix before and after being processed with vinegar on IEC-6 cell activity. The morphology of cells were observed by the inverted microscope. The down-regulated mitochondrial apoptosis pathway of enterocytes caused by the vinegar processing was analyzed by using the high content screening.
RESULTCompared with the negative control group, the proliferation inhibition experiment showed that Euphorbia Pekinensis Radix showed a relatively high intestinal cell toxicity (P < 0.01). The results of HCS analysis showed that Euphorbia Pekinensis Radix could significantly reduce the cell nucleus Hoechst fluorescence intensity and mitochondria membrane (P < 0.05, P < 0.01), and increase Annexin V-FITC and PI fluorescence intensity and membrane permeability (P < 0.01, P < 0.01, P < 0.01). After being processed with vinegar, compared with Euphorbia Pekinensis Radix groups with different doses, Euphorbia Pekinensis Radix processed with vinegar could significantly decrease the cell proliferation inhibition effect on enterocytes, increase the cell nuclear Hoechst fluorescence intensity and mitochondria membrane (P < 0.05, P < 0.05), and decrease Annexin V-FITC and PI fluorescence intensity and membrane permeability (P < 0.01, P < 0.01, P < 0.05), and showed a certain dose-effect relationship.
CONCLUSIONThe vinegar processing can further reduce the toxicity of Euphorbia Pekinensis Radix on enterocytes. Its possible mechanism can decrease the effect of Euphorbia Pekinensis Radix on the permeability of IEC-6 cell membrane, so as to provide a basis for further explanation of the detoxication mechanism of Euphorbia Pekinensis Radix processed with vinegar.
