1.Vertigo:an etiological analysis of 200 patients in department of neurology of polyclinic

Objective To discuss the causes of diseases presenting vertigo as a chief complaint in patients in department of neurology of polyclinic and to compare the clinical features of these disease in order to draw the attention of the physicians to benign paroxysmal positional vertigo(BPPV)and posterior circulation ischenmia(PCI).Methods Prospective clinical study was carried out.Followed up outpatients (135)and inpatients(65)with a chief complaint of vertigo were followed up in the department of neurology, the second affiliated hospital of Harbin Medical University.Thorough history, especially the timing and duration of vertigo, provoking and aggravating factors, symptoms associated and risk factors for cerebrovascular were obtained.Physical examinations with special attention to neurologic systems and Dix- Hallpike maneuver were performed.Based on the history, the physical examination, Dix-Hallpike maneuver and special techniques, the diagnoses of 200 patients with vertigo was made.Two weeks after each patient' s first consultation, a follow-up was made.Results The main diagnoses for these 200 patients were BPPV (56.0%), PCI(21.5%), neurosis(13.0%), M?ni?re' s disease(2.5%), multiple sclerosis (1.0%), probably cervical disease(1.0%), acoustic neuroma(1.0%), labyrinthine apoplexy(1.0%), brainstem hemorrhage(0.5%), vestibular neuronitis(0.5%), cerebellum cyst(0.5%), unknown causes (1.5%).Such clinical features of the diseases with vertigo, as age, vertigo duration, and provoking factors, associated symptoms, risk factors, the physical examinations, the treatment and so on were summed up.Conclusions Presentations of vertigo can be clinically diagnosed in most cases, with a larger number of patients suffering from BPPV.Combination of these prominent clinical features may guide the general physicians to the likely causes of vertigo.
2.Effects of Wuling Powder on Insulin Resistance of Mice Induced by High Lipid Diet

Objective To investigate the effects of Wuling Powder on insulin resistance of C57BL/6J mice induced by high lipid diet, and discuss the mechanism. Methods C57BL/6J mice were randomly divided into six groups:normal group, model group, rosiglitazone group, Wuling Powder in low, middle, and high dose groups, 10 mice per group. Besides the normal group, other five groups were fed with high fat and sugar diet, with a purpose to establish insulin resistance model. Normal group and model group were given pure water. Rosiglitazone group received a gavage with rosiglitazone of 0.75 mg/kg. Wuling Power low, middle, and high groups received gavage with Wuling Power of 1.23, 3.69, 11.07 g/kg, respectively, the does volume was 0.2 mL/10 g, once a day. The weight and abdominal girth were detected every week. At the end of the sixth week, mice were given 12-hour fasting, and their eyeball were taken for blood. The body weight, length, and fat in abdomen and both kidneys were detected. Paraffin section was made with HE staining. FPG and FINS of each group were detected. ISI and IRI were calculated, and TC and TG were detected. Results Compared with the model group, Wuling Powder can significantly reduce the body weight and abdominal girth of mice (P<0.01, P<0.001), improve liver fatty degeneration, lower the FPG, FINS, TCH, TG, IRI, and increase the ISI in mice (P<0.05, P<0.01). Conclusions Wuling Powder has the effect of preventing insulin resistance of C57BL/6J mice induced by high lipid diet.
3.Research on ECG de-noising method based on ensemble empirical mode decomposition and wavelet transform using improved threshold function.

A de-noising method for electrocardiogram (ECG) based on ensemble empirical mode decomposition (EEMD) and wavelet threshold de-noising theory is proposed in our school. We decomposed noised ECG signals with the proposed method using the EEMD and calculated a series of intrinsic mode functions (IMFs). Then we selected IMFs and reconstructed them to realize the de-noising for ECG. The processed ECG signals were filtered again with wavelet transform using improved threshold function. In the experiments, MIT-BIH ECG database was used for evaluating the performance of the proposed method, contrasting with de-noising method based on EEMD and wavelet transform with improved threshold function alone in parameters of signal to noise ratio (SNR) and mean square error (MSE). The results showed that the ECG waveforms de-noised with the proposed method were smooth and the amplitudes of ECG features did not attenuate. In conclusion, the method discussed in this paper can realize the ECG denoising and meanwhile keep the characteristics of original ECG signal.

5.Preparation and Properties of Oxymatrine Pellets

OBJECTIVE:To prepare oxymatrine pellets and study the pellets' properties.METHODS:Oxymatrine pellets were prepared using an experimental low-temperature extrusion-spheronization granulator.L9(34)orthogonal design was used to obtain optimal formulation.The micromeritic properties and in vitro dissolution of the pellets at different dosage were determined.RESULTS:Oxymatrine pellets prepared by extrusion-spheronization were round and smooth and well-distributed.The optimal technical conditions were as follows:water∶MCC=0.90∶1;spheronization velocity=35 Hz;spheroniza-tion time=5 min;extrusion velocity=40 Hz.The in vitro dissolution was more than 75% within 30 minutes.CONCLUSION:The process of preparing pellets by extrusion-spheronization was simple and feasible and the quality of pellets was excellent.

9.Effects of Ginkgo biloba extract 761 on learning and memory in chronic alcoholic rats

Objective To investigate the effects of extracts of Ginkgo biloba leaves ( EGb761 ) on learning,memory and the hippocampal long-term potentiation (LTP) in chronic alcoholic rats.Methods 72 SD rats were randomly divided into control group,chronic alcoholism model group,low-dose EGb761 (100 mg/kg) and high-dose EGb761 (200 mg/kg) groups on average.55% alcohol was given ( 10 ml/kg) by gavages once every day for 6 weeks to create the model of chronic alcoholism.In low-dose and high-dose EGb761 groups,EGb761 was used for gastric infusion 30 minutes before model.The control group and the model group were infused with the same volume of distilled water.Morris water maze (MWM) was used to study the spatial learning and memory in rats.Electrophysiological methods were used to observe the changes of LTP through the comparison of field excitatory postsynaptic potential (fEPSP) slope in hippocampal CA3 region before and after high frequency stimulation (HFS).Results ( 1 ) The escape latency (EL) of model group ( (93.31 ± 14.21 ) s) was longer than that of the control group( (35.28 ±6.51 )s).The mean slope of fEPSP (relative to baseline) after HFS in model group was (96.91 ±7.65) %,in control group was ( 177.22 ± 10.12) % ; Compared with control group,the ability of learning and memory as well as LTP obviously descended in model group.(2) Compared with model group,EGb761(100,200 mg/kg) could shorten the EL.The slope of fEPSP after HFS in either low-dose (( 126.16 ± 5.84)% )or high-dose (141.31 ± 5.75)% EGb761 groups was enhanced significantly compared with the model group (96.91 ± 7.65 ) %,P =0.034,P =0.009 ).Conclusion EGb761 can significantly improve the alcohol-induced memory impairment by means of accelerating the recovery of the pathological synaptic plasticity.
10.Clinical efficacy of low-dose methimazole therapy in patients with subclinical hyperthyroidism complicated by paroxysmal atrial fibrillation

Twenty-seven patients with subclinical hyperthyroidism(SH)complicated by paroxysmal atrial fibrillation(PAF)were treated with methimazole plus bisoprolol.All patients were examined by Doppler echocardiogram and 24 h ambulatory electrocardiograms before and 3 months after treatment.Serum FT3,FT4,and TSH levels were measured with RIA.The results showed that low-dose methimazole therapy could improve the left ventricular diastolic function(P < 0.01)and help maintain sinus rhythm.The incidence of subclinical hypothyroidism was low.Low-dose methimazole was effective and safe in patients with SH complicated by PAF.
