1.Clinical Intervention of Septic Myocardial Injury with Xuebijing Injection

Objective To observe the clinical efficacy of Xuebijing injection ( XI) for the treatment of septic myocardial injury, and to study the possible therapeutic mechanism. Methods Thirty-nine qualified septic myocardial injury patients were randomly divided into XI group (N=24) and control group (N=15) . Both groups were treated with conventional therapy, including anti-inflammation treatment, transfusion for resuscitation, nutritional support, symptomatic and supportive treatment, and supportive treatment for keeping vital organ function. XI group was given intravenous drip of XI based on the conventional therapy. Before and after treatment, the peripheral blood concentrations of myocardial-specific isoenzyme of creatine kinase (CK-MB), cardiac troponin I (cTnI), brain natriuretic peptide ( BNP) and plasma calcitonin ( PCT) were detected, and the changes of electro-cardiogram ( ECG) were also observed. The mortality within 28 days was compared between XI group and the control group. Results ( 1) After treatment, cTnI, BNP and PCT concentrations in XI group were decreased significantly as compared with the control group, and there was statistical significance (P <0.05) . However, the decrease of CK-MB was not obvious in XI group ( P>0.05 compared with the control group) . ( 2) The difference of ECG was insignificant between the two groups after treatment ( P>0.05) . ( 3) The 28-day mortality was 8.33% in XI group, and was 13.33% in the control group, the difference being insignificant between the two groups (P>0.05) . (4) During the treatment, none case had adverse reaction. Conclusion XI can improve the myocardial injury, and relieve inflammation of septic myocardial injury patients.
2.Emergency Drug Supply and Pharmaceutical Support in Case of Emergent Events

OBJECTIVE:To provide reference for the hospitals about emergency drug supply and pharmaceutical support in case of emergent events.METHODS:The pertinent literature both at home and abroad was retrieved and summarized analytically.RESULTS:The definition and the classification of emergent events were introduced,and the emergency drug supply and pharmaceutical support both at home and abroad in case of emergent event were introduced.CONCLUSIONS:Drugs are essential thing in medical aid in case of emergent events,and medical institutions should establish a scientific and practical drug supply system,and the pharmacists should bring their professional talents into full play by participating in medical aid in case of emergent event to ensure safe,effective and economic drug use.
3.Countermeasures for Shortages of Certain Drugs in China

OBJECTIVE:To provide references for drug administrator in coping with the challenge of drug shortage and the promotion of rational drug use. METHODS: With American Society of Hospital Pharmacists Guidelines on Managing Drug Shortages as references,the feasible management strategies for drug shortage in China were put forward based on the reality of our country and our experience in the practice of drug shortage management. RESULTS & CONCLUSIONS: It is advisable to draw experiences from U.S. Food and Drug Administration to tackle the problem of the shortage of essential drugs through the co-efforts of Ministry of Public Health,State Food and Drug Administration and Stage Development and Reform Committee and develop contingency planning strategy in health care organization to deal with the shortages of certain drugs and reduce potential clinical safety hazard induced by drug shortages.
4.Uniform requirements for reporting Meta-analysis of randomized controlled trials: the QUOROM statement

The QUOROM statement means a set of quality standards of Meta analysis report. It comprises a checklist and a flowchart for reporting a Meta analysis. They are primarily used in writing, reviewing or evaluating reports of Meta analysis for simple two group, parallel RCTs. The statement is a continually evolving document. A few international medical journals have endorsed the QUOROM statement and require authors submitting manuscripts for publication to include all the information on the checklist and flowchart of QUOROM statement. Based on introducing the background, processes of forming QUOROM statement and reviewing the contents and requirements of QUOROM, we prepare this paper, hoping to improve the quality of Meta analysis report for randomized controlled clinical trials in our country. Some comments and suggestions are given at the same time.
5.Quality Standard for Tiaolitongbao I Capsules

Objective: To establish the quality standard for Tiaolitongbao I capsules.Methods: The components including Astragalus , Atractylodes, Finger citron and Radix aucklandiae were identified by TLC.Emodin, the effective component of Polygonum cuspidatum , was determined by HPLC.Results: The characteristic spots in TLC were clear without any interference.The linear range of emodin was 4.25-68.00 μg·ml-1 (r =0.999 9).The average recovery was 95.22% ,and RSD was 1.47% (n =6).Conclusion: The methods used for the identification and quantification are sensitive, simple and accurate, which can be used for the quality control of Tiaolitongbao I capsules.
6.Evaluation modes and effectiveness comparison of antimicrobial prescriptions of the hospital

To review the evaluation modes,implementation,workflow and effectiveness appraisal of antimicrobial prescriptions of PHMCH and to compare the effects of the evaluation data of such prescriptions during the period from January to June of 2012,and that from January to June of 2013 of the hospital.The results indicate that the 2013 period,compared with that in 2012,the variations such indexes as the proportion of rational prescriptions in outpatient and emergency departments,the proportion of outpatient antimicrobial prescriptions,AUD,the proportion of antimicrobials cost within total drug costs were 2.5%,-4.71%,-1.22 and-0.37% respectively.Most notable change among which is the proportion of outpatient antimicrobial prescriptions.Such results prove the prescription evaluation mode of the hospital as institutionalized,standardized and effective.
7.Attaching great importance to polypharmacy in elderly patients

Based on the ongoing clinical experiences at our outpatient pharmacy and geriatrics multidisciplinary inpatient and outpatient services,we believe that polypharmacy among elderly patients is serious and pervasive in China.This observation is supported by the findings from literature review.To help address this serious issue in senior care,we recommend to gain better understanding of age-related physiological changes of the elderly as well as those in pharmacokinetics and pharmacodynamics,and to conduct prescription review and medication reconciliation.The multi-and interdisciplinary approach employed by modern geriatrics care model,involving doctors,pharmacists and nurses,should further help address this issue and promote rational use of pharmaceuticals in the elderly patients.
8.Investigation of surfactant protein-C gene on respiratory distress syndrome in the Han nationality new-borns of the Inner Mongolia

Objective To investigate the relationship between single nucleotide polymorphisms of surfactant protein C(SP-C)gene and respiratory distress syndrome(RDS)in the Han nationality newborns in Inner Mongolia and whether there is a mutation occurs on SP-C gene exon 4 and 5.Methods One hundred newborns with RDS(case group)and 100 newborns without RDS(control group)were selected.PCR gene analysis was used to establish the genotype and allele frequencies of exon 4 (T138N)and 5 (S186N)on SP-C.Results In the Han nationality newborns of Inner Mongolia region,there was no mutation on SP-C gene exon 4 and 5.Exon 4(T138N)on SP-C could be checked out three genotypes:namely AA,AC and CC.The genetic polymorphisms of exon 4 on SP-C were not statistically different between the case group and the control group(χ2 ﹦0.744,P ﹦0.689).Besides,exon 5(S186N)on SP-C could also be checked out three genotypes:namely AA,AG and GG.The genetic polymorphisms of exon 5 on SP-C were also not statistically different between the case group and the control group(χ2 ﹦0.770,P ﹦0.681 ).Conclusion There is no mutation on SP-C gene exon 4 and 5.The genetic polymorphism of exon 4 and 5 on SP-C displays no signifi-cant correlation with RDS of the Han nationality newborns in Inner Mongolia.
9.Retrospective Analysis of ADR Induced by Shuxuening Injection in Beijing Area during 2003-2013

OBJECTIVE:To investigate the occurrence of ADR induced by Shuxuening injection in Beijing area,and to pro-vide reference for rational drug use in clinic. METHODS:ADR induced by Shuxuening injection in Beijing area during 2003-2013 was classified statistically in respects of population,organs/systems involved in ADR,clinical manifestations,occurrence time, drug combination and prognosis. RESULTS:Of 1648 ADR cases,the incidence of ADR in the elderly patients was higher than young patients,and that of female patients (60.92%) was higher than that of male patients (39.14%). Multiple organs/systems were involved in Shuxuening injection-induced ADR,and top 3 were nervous system (422 cases,21.33%),skin and its appen-dants (411 cases,20.78%) and others (305 cases,15.42%). Clinical manifestations were skin rash,itching,dizziness and ner-vousness,palpitation,arrhythmia,etc. ADR mostly occurred within 5-30 min after medication. In ADR reports,there were a vari-ety of drugs combined with Shuxuening injection. Shuxuening injection combined with Alprostadil injection was most common(61 cases,accounting for 23.46% of drug combination). Among 1648 ADR cases,there were 622 cured cases and 1039 cases im-proved,4 cases were not improved,3 cases had sequelae and 1 case died. CONCLUSIONS:Clinical application of Shuxuening in-jection should strictly follow the indications of drug package inserts;differentiation of symptoms and signs is based on specific dos-age,solvent,route of administration,etc. It is necessary to strengthen the prescription check,and strengthen the observation after medication especially for the elderly and other special groups so as to reduce and avoid ADR.
10.Comparison of the Quality of Domperidone Tablets Produced by Three Different Factories

OBJECTIVE:To compare the quality of domperidone tablets produced by 3 different factories METHODS:The quality of different products was investigated according to the standards of Ministry of Public Health and manufacture factories,including dissolubility,uniformity of dosage units,related substances and seals of blister packaging RESULTS:In 3 batches of products manufactured by Y,W and L,the contents of domperidone were 98 23%~99 74%,96 92%~98 31% and 97 42%~98 72%,and content uniformity A+1 8S was 2 11~3 38,3 97~5 25 and 5 49~7 47 respectively,which were in keeping with the Chinese pharmacopoeia;impurity R061668 was not found,and contents of impurity R052211 were 0 004%~0 024%,0 029%~0 072% and 0 003%~0 056%;the total impurity substances were 0 030%~0 095%,0 102%~0 128% and 0 232%~0 489%;the dissolubity(45min) was 95 09%~95 77%,91 25%~96 43% and 95 48%~96 58% respectively In experimental condition,no leakage was found from blister package of Y and W products,however,the leak rate of L product reached more than 38% CONCLUSION:According to this survey,the quality of product Y is the best,W is better
