1.Efficacy analysis of accelerated partial breast irradiation versus whole breast irradiation with simultaneous integrated boost after breast-conserving surgery for early-stage breast cancer

Objective To evaluate the efficacy of accelerated partial breast irradiation ( APBI ) and whole breast irradiation ( WBI ) with simultaneous integrated boost ( SIB ) from the perspective of economics, and provide a reference for postoperative adjuvant therapy mode selection for early-stage breast cancer after breast-conserving surgery. Methods A total of 355 early-stage breast cancer patients who underwent APBI or WBI-SIB after breast-conserving surgery were evaluated on efficacy and cost-effectiveness, of which 177 patients received APBI, and 178 patients received WBI-SIB. Survival analysis was done according to treatment received. NCI-CTC 3.0 was used to score the toxicities. Breast aesthetic outcome were evaluated with Harris standards. Results Median follow-up was 42 months ( 5.8 -92.7 months) . The 3-year locoregional recurrence free survival( LRFS) rates in APBI group and WBI-SIB group were 98.2% and 97.6%, distant metastasis free survival( DMFS) were 94.3% and 93.7%, disease-free survival ( DFS) were 93.1% and 91.6%, and overall survival 95.5% and 94.3%, respectively, without statistically significant differences(P>0.05). Compared with WBI-SIB group, the acute reaction rates in APBI group decreased from 5. 6% to 3.4%(χ2 =6.044, P <0. 05), and late reactions from 5.6% to 2.3% (χ2 =6.149, P<0. 05), while the cosmetic outcome improved from 88.8% to 93.8%(χ2 =5.22, P<0. 05). Moreover, the processing average time was shortened by 26.5 d (χ2 =40.76, P<0. 05). Conclusions After breast-conserving surgery, the efficacy of APBI showed no difference from WBI-SIB with respect to 3-year local control, disease-free survival, and overall survival, but displayed a significantly better toxicity profile and cost-effectiveness ratio for early breast cancer patients. It can be used as a good radiotherapy model after breast-conserving surgery in early-stage breast cancer.
2.The prevalence and influencing factors of abuse and negligence against elderly in rural areas of Anhui province

Objective To investigate the prevalence and influencing factors related to abuse and negligence against the elderly in the rural areas. Methods 975 elderly over 60 years from 41counties in Anhui province were included. All participants completed an anonymous questionnaire including items as: educational background, marital condition, income, child-discipline, rude action to parents, daily activities, physical functions, having chronic illness, abuse and negligence against the elderly, etc. Results In the last year, rates of common physical abuse, serious physical abuse,emotional abuse, financial exploitation, negligence, overall abuse and negligence against the elderly were 4.5%, 1.5%, 26.9%, 4.9%, 7.2%, 29.9% respectively. Among the 281 victims, 80.4% reported that they were suffered more than 3 times of abuse and neglect episodes, and 34.9% reported that they were suffered more than 2 forms of abuse and negligence. The primary sadism was carried out by the daughter-in-law or son-in-law (43.2%) of the elderly. Low activity on daily life and having chronic illness were the risk factors causing common physical abuse while better education was the protective factor to it, Low ability in managing daily activity of living was the risk factor causing serions physical abuse. Less active on daily life and having rude action to parents were the risk factors to emotional abuse, but being strict with their children was the protective factor to emotional abuse. Less active on daily life, often beating their children and having rude action to parents were the risk factors related to financial exploitation. Less active on daily life, having rude action to parents and having bad physical functions were the risk factors causing negligence. Less active on daily life and having rude manner to parents were the risk factors of overall elderly abuse and negligence, but being strict with their children was protective factor to the abuse and negligence against the elderly. Conclusion High prevalence on abuse and negligence against the elderly was seen in the rural areas of China. Different forms on elderly abuse and negligence were affected by different factors that called for more attention to be paid to those elderly with lower ability in managing their daily life.
3.Analysis of iodine nutritional status of key population in Puhui Town, Korla Xinjiang

Objective To investigate iodine nutritional status of key population in Puhui Town,Korla Xinjiang.Methods In 2018,totally 29 samples of factory water,peripheral water and self-prepared well water from centralized water supply in Puhui Town were collected,and urine samples,salt samples and blood samples were collected from 200 children aged 8 to 10 years old and 100 women of childbearing age (including pregnant women)in Puhui Town.Arsenic-cerium catalytic spectrophotometric method was used for determination of iodine in water and urine;the salt iodine was determined using direct titration and the thyroid was examined by ultrasound.The assessment was conducted according to the standard of "Delineation of Water-source Areas and Endemic Areas of High Iodine Goiter" (GB/T 19380-2016).Results In this survey,the median of water iodine in Puhui town (n =29)was 76.1 μg/L,and the median of salt iodine (n =175) was 27.76 mg/kg;the rate of iodized salt was 98.29% (172/175);the consumption rate of qualified iodized salt was 96.00% (168/175);the medians of urinary iodine of children aged 8 to 10 years old (n =208),women of childbearing age (n =73),and pregnant women (n =25) were 292.47,248.23,and 246.36 μg/L,respectively.The thyroid enlargement rate was 1.9% (4/208) in children aged 8-10 years old,no swelling of women of childbearing age and pregnant women.Conclusions Iodine nutrition of pregnant women is in an appropriate state;both the children aged 8 to 10 years old and the women of childbearing aged are in excess of the appropriate level;goiter rate is lower in children.According to the national standards,Puhui Town does not belong to waterborne high iodine area.
4.Drinking tea type of endemic fluorosis in Xinjiang in 2014-2016

Objective To understand the levels of water fluoride,tea fluorosis and the severity of drinking tea type of endemic fluorosis in Xinjiang from 2014 to 2016,and to provide a basis for making prevention and control measures.Methods In 2014-2016,a cross-sectional study was conducted to select six counties with serious fluorosis in Xinjiang as monitor counties,one diseased township was selected from five directions including east,south,west,north and center in each county,one village was selected in each township,and 10 families were selected to investigate the situation of brick tea drinking.Drinking water and brick tea samples were collected for detection of fluorine content,and epidemiological methods were used to analyze the prevalence of dental fluorosis and skeletal fluorosis.Results In 2014-2016,resident population per capita brick tea consumptions were 3.9-4.4 kg,per capita brick tea fluorine intakes were 4.6-6.1 mg,and the mean of tea fluorine was 433.1,385.2,432.7 mg/kg,respectively,the differences were not statistically significant (F =0.33,P > 0.05);the means of water fluorine were 0.06-1.26 mg/L,and they were 0.38,0.37,0.33 mg/L,respectively,the differences were not statistically significant (F =2.64,P > 0.05).The survey results showed that dental fluorosis of children aged 8-12 was 243 in three years,with a detection rate of 4.47% (243/5 442) and the range of 0.08-0.14,which was negative.The skeletal fluorosis of adults aged 36-45 was 25 in 2015,with a detection rate of 2.75% (25/910).Conclusions The fluorine content of some tea in drinking tea type of endemic fluorosis areas in Xinjiang is high,and the fluorine content of some water samples has exceed the standard of drinking water type disease area (1.2 mg/L).The prevalences of dental fluorosis and skeletal fluorosis are acceptable;it is recommended to maintain the previous prevention and control policies and to further monitor the tea market.
5.AMPK activator down-regulates the expression of tissue factor in fulminant hepatitis mice.

AMP activated protein kinase (AMPK) is a pivotal metabolic regulatory enzyme and novel target of controlling inflammation. Our previous studies had demonstrated that 5-amino-4-imidazolecarboxamide riboside (AICAR), an AMPK activator, attenuated lipopolysaccharide (LPS)/D-galactosamine (D-gal)-induced fulminant hepatitis via suppressing inflammatory response. Since inflammation usually activates the coagulation response and aggravates inflammation-induced tissue injury, the present study was to explore the effects of AICAR on inflammation-induced activation of coagulation. Male BALB/c mice received LPS/D-gal intraperitoneal injection were used as fulminant hepatitis model. Western blot was used to detect tissue factor (TF) and hypoxia-inducible factor 1α (HIF-1α) protein expressions in hepatic tissue, as well as nuclear factor kappa B (NF-κB) p65 translocation into the nucleus. Real-time quantitative PCR was used to analyze erythropoietin (EPO) mRNA expression level. Lactic acid (LA) level in hepatic tissue was detected by kit. The results showed that LPS/D-gal induced the enhanced expression of TF, elevation of NF-κB p65 nuclear translocation, up-regulation of HIF-1α and EPO expressions, and increased LA level. These above alterations could be suppressed by AICAR. These results suggest that AICAR may down-regulate LPS/D-gal-induced TF expression (coagulation activity), and relieve hepatic hypoxia and metabolic disorder via suppressing the activity of NF-κB, which may be a novel mechanism of the beneficial effect of AICAR on LPS/D-gal-induced fulminant hepatitis.

6.Analysis of influencing factors of secondary metabolites contents in cultivated Polygala tenuifolia.

This work was launched to explore the effect of habitat and growth year on the secondary metabolites contents of cultivated Polygala tenuifolia. The samples of cultivated P. tenuifolia were analyzed by ultra-high performance liquid chromatography(UPLC)-quadrupole time-of-flight mass spectrometry(Q-TOF MS), and the obtained data were analyzed using multiple statistical analysis and cluster analysis. The results showed that compared with growth year, habitat is a main influencing factor which affected the secondary metabolites contents of P. tenuifolia. The contents of sucrose esters and oligosacchride multi-esters are greatly dependent on the habitat (the sample-AG with high levels of components of tenuifoliside B and tenuifoliside C, and the sample-FY with high levels of 3,6'-disinapoyl sucrose, tenuifoliose S, tenuifoliose L, and tenuifoliose V). There is no obvious effect of habitat and growth year on xanthone. The contents of triterpene saponins are greatly dependent on the growth year, and the content of parts of triterpene saponins increased as time goes on.The result indicated that the effect of habitat and growth year on different types of secondary metabolites is not completely equivalent. This study will contribute to the breeding of P. tenuifolia and amendment of current commodity criteria.
7.Exploration on the relationship between acupuncture for mind-regulation and flow theory.

Based on the story of Chinese idiom, (a magical and skilled form of craftsmanship) as the breakthrough point, this paper discusses the both (cook) and the experienced acupuncture practitioner have the same high skills and explores the potential relationship between mind-regulation in treatment with acupuncture and flow theory. It is believed that the skills of ancient acupuncture practitioner in mind-regulation with acupuncture is not only a kind of "Tao" mode, but also a state of "flow". By the discussion on mind-regulation and flow theory, modern people may have more clear recognition on the mind regulation in treatment with acupuncture so as to better determine the therapeutic methods of acupuncture for mind-regulation.
