1.A discussion about how to prevent and control adverse event of medical device based on metal materials science

Objective: To discuss the effect of metal materials science for how to prevent and control adverse events of medical devices, and provide forewarning measures for preventing this kind of events. Methods: To analyze the adverse events report of medical devices, and summarize 10 kinds of fault phenomenon and clinical manifestation of adverse events came from metal materials, such as respiration device and its accessories, color ultrasonic imaging device and ultrasound intervention, intracavity diagnosis and implants, and so on. To analyze the reason of the medical equipment accidents;and provide countermeasures for monitoring and management of adverse events of medical devices. Results: The advantages of preventing and controlling adverse events of medical equipment based on metal material science were:①monitor the metal material performance changes occur in force or strain and early alarming, ②identify implanted medical consumables degradation. Conclusion: The fatigue damages and defects of metal materials were the main reason of medical device adverse events. The hospital needs to establish the medical device adverse event reporting, evaluation model, evaluation and classification of time report supervision mechanism; establish and improve the monitoring and management mechanism, technical personnel and regulations; perfect the corresponding technical support system, and achieve monitoring and risk management for adverse events of clinical medical devices.
2.CT findings in adrenal ganglioneuroma

Objective To find the features and diagnostic values of CT for adrenal ganglioneuroma.Methods CT examinations were performed in 16 cases(7 with plain scan and 9 with both plain and enhanced scan).All cases were confirmed by pathological evidence.Results 2 cases showed solid -cystic mass,14 were solid mass,and 5 accompanied with calcification.The average CT value was from 20 to 40 HU.In 9 cases with enhanced scan,5 slightly enhanced on delayed phase and no enhancement was seen in the others.Conclusion Adrenal ganglioneuroma showed some characteristic CT appearance and CT scanning plays an important role in diagnosis of adrenal ganglioneuroma.
3.Medicine Storage Scale Model for Disaster Relief

Objective To study medicine storage scale model for disaster relief.Methods The medicine storage scale for disaster relief was dynamically simulated by using DAMANO programming language of System Dynamics (SD) and was calculated with discrete algorithm using FORTRAN programming language.Results The established model could be used for dynamic tracing of the inventory of the warehouse.Conclusion Medicine storage scale for disaster relief can be determined through the model established by dynamic simulation or discrete calculation.

Helicobacter pylori (H.pylori) infection rate is increasing these years.The incidence of related diseases subsequently rose.Hp infection can not only cause digestive diseases,but also lead to idiopathic thrombocytopenic purpura (ITP),iron deficiency anemia,and indigestion etc.which can affect the growth and nutritional health of children.H.pylori has become a global issue,both international and domestic colleagues have reached a consensus on treatment for adults.According to the new conscnsus,the aim of this article is to study the diagnosis and treatment options for children with Hp infection.
5.The effectiveness and safety of continuous and intermittent ice compression therapy after total knee arthroplasty:a prospective cohort study

Objective To compare the effectiveness and safety of continuous and intermittent ice compression therapy following total knee arthroplasty surgery. Methods Eighty patients were divided into two groups receiving continuous or intermittent 30 minutes every 2 two hours within 48 hours after the operation. The subjective pain (VAS score), additional pain-killer use, swelling of extremity, drainage, range of motion were observed and compared. Results Patients in the observation group had less pain than those in the control group (Z values in the first 3 postoperative day was-2.722,-3.359,-2.039, respectively, P<0.05). Less pain-killers were required in the experimental group (Z=-2.559, P<0.05). The postoperative swelling by the thigh circumferences in the first day in the observation group was (1.84 ± 1.11) cm, which was milder than the (3.30 ± 1.69) cm of the control group (t=4.565, P<0.01). So was it in the calf circumferences, (0.94 ± 0.89 ) cm vs. (1.46 ± 0.91) cm (t=2.627, P=0.01). Within the first 3 days after the operation, this mildness still existed in aspect of thigh circumferences, which was (3.09±1.39) cm in the observation group vs the (4.09 ± 1.71) cm in the control group, t=2.869, P < 0.01. Conclusions Continuous ice compression therapy has better effects than intermittent 48 hours after total knee replacement, with functional training and exercise not disturbed. It is considered safe and assured, and is therefore recommended.

7.Functional magnetic resonance imaging study in smoking addicts and its application in the field of the treatment and management of smoking cessation

Smoking and exposure to second hand smoke are the deadly killer of human health. Tobacco kills more than one million people each year in China. According to 2015 Chinese adult tobacco survey, the number of smokers in China increased by 15 million during five years, from 301 million in 2010 to 316 million in 2015. Smoking addiction is a chronic recurrent disease, and its pathogenesis is associated with the mesolimbic dopamine system. In recent years, domestic and overseas scholars studied the brain function of smoking addicts through resting state functional magnetic resonance imaging (fMRI), and found that the brain functional activity of smoking addicts has changed. fMRI can help us understand the mechanism of tobacco dependence, and provide the evidence of diagnosis, treatment and health management of tobacco dependence. In this paper, the present study situation and the application in the field of the treatment and management of smoking cessation of fMRI in smoking addicts are reviewed.
8.Clinical Observation of Saccharomyces boulardii Adjuvant with Aluminium Phosphate in the Treatment of Pediatric Acute Diarrhea

OBJECTIVE:To investigate the effect of Saccharomyces boulardii adjuvant with aluminium phosphate on symp-toms and signs disappearance time,immune function and inflammatory factors in children with acute diarrhea. METHODS:Totally 110 children with acute diarrhea were chosen from our hospital during Mar. 2013-Mar. 2016,and then randomly divided into con-trol group and observation group according to random number table,with 55 cases in each group. Control group was given Alumini-um phosphate gel 2.5 g,po,qd,on the basis of conventional treatment. Observation group was additionally given S. boulardii cap-sules 0.25 g,po,bid,on the basis of control group. Both groups were treated for 3 d. Clinical efficacies,symptoms and signs(di-arrhea,abdominal pain,vomiting,abnormal stool routine)disappearance time as well as stool frequency,stool character scores, immune function indexes(CD3+,CD4+,CD8+,CD4+/CD8+),inflammatory factors(IL-6,IL-10,TNF-α)before and after treat-ment,the occurrence of ADR were compared between 2 groups. RESULTS:The total response rate of observation group (90.91%)was significantly higher than that of control group(70.91%);the symptoms and signs disappearance time was signifi-cantly shorter than control group,with statistical significance(P<0.05). Before treatment,there was no statistical significance in stool frequency,stool character scores,immune function indexes or inflammatory factor levels between 2 groups(P>0.05). After treatment,stool frequency,stool character scores and inflammatory factor levels of 2 groups were significantly lower than before, and the observation group was significantly lower than the control group;the levels of CD3+,CD4+,CD4+/CD8+ were significantly higher than before treatment and control group,and the level of CD8 + was significantly lower than before treatment and control group,with statistical significance(P<0.05). There was no significant difference in immune function indexes of control group be-fore and after treatment(P>0.05). Total incidence of dry mouth and constipation was 5.45% in control group and 9.09% in obser-vation group,without statistical significance(P>0.05). CONCLUSIONS:S. boulardii adjuvant with aluminium phosphate in the treatment of pediatric acute diarrhea can efficiently improve symptoms and signs,shorten disease duration,improve immune func-tion and reduce inflammatory reaction with good safety.
9.Effects of Vitamin C and Hydrogen Dioxide on the Electric Current of Large-conductance Calcium-activated Potassium Channels in Isolated Outer Hair Cells of Old Guinea Pigs’ Cochlears

Objective To study the effects of hydrogen dioxide (oxygen free radical donator) and vitamin C (oxygen free radical scavenger) on the electric current of large conductance calcium -activated potassium channels (BKCa channels) in isolated outer hair cells in aging guinea pigs .Methods Acute enzyme was used to isolat outer hair cells of aging guinea pigs ,in which of BKCa channel's electric current was observed and recorded by whole-cell recording mode of patch -clamp .After recording the stable and normal electric current of BKCa channels ,added H2 O2 dilution (0 .2 mmol/L) 40 μl in the 2 ml chambers within freshly isolated outer hair cells so that the concen‐tration of H2 O2 in the balneum would be 4 μmol /L .The groups(n=5) received individually vitamin C solution (5 mg/ml) 0 ,10 ,20 ,40μl in the 2 ml chambers within freshly isolated outer hair cells so that the concentration of vi‐tamin C in the balneum would be 0 ,25 ,50 ,100 μg /ml ,observing and recording the effects of different concentration of vitamin C to electric current of BKCa channels .Results ①In the w hole-cell mode of patch -clamp ,the rapid activation and non-deactivation electric current with a string of large amplitude was recorded ,above -40~ -30 mV activation voltage .The electric current increased with the increasing membrane potential .The amplitude in‐creased continuously and performed characteristics of outward rectification .When the concentration of IbTX was 100 nmol /L ,the activity of the channel was completely blocked and confirmed BKCa channel's electric current .②Medication within three minutes ,when VT was +50 mV ,the BKCa channels'the maximum peak current densities of 4 μmol /L H2 O2 group rose from 22 .09 ± 0 .27 PA /PF to 43 .53 ± 1 .09 PA/PF ,amplification was 97 .06% .In H2 O2 4 μmol /L + vitamin C with different concentrations as 25 ,50 ,100 μg/ml groups ,the BKCa channels'elec‐tric current performed about concentration-dependent inhibition ,and electric current's amplitude and peak current density decreased with the increasing concentration of vitamin C ,the I-V curves were reduced .However ,this still could not be recovered to the normal levels .Conclusion The oxygen free radical /BKCa exists in the process .The vitamin C as oxygen free radical scavenger can reverse the process to a large extent .
10.Clinical features of 26 patients with H7N9 avian influenza and characteris-tics of viral gene

Objective To evaluate the clinical,epidemiological,and viral molecular biology features of 26 patients infected with H7N9 avian influenza A virus. Methods Clinical and epidemiological data of 26 patients with con-firmed avian influenza A (H7N9)infection in 2013 and 2014 were collected,virus isolated from human and poultry were identified and typed through sequencing.Results Of 26 patients,fever and cough were the most common symptoms,all patients had pneumonia;20 patients (76. 92% )developed acute respiratory distress syndrome (ARDS);25 patients (96.15% )had leucopenia or normal leukocytes at the initial diagnosis;treatment with antivi-ral drugs was initiated in 25 patients at a median of 10 days after the onset of illness;10 patients (38.46% )died. Gene sequencing indicated Gln226Leu and Gly186Val substitutions in human virus H7 gene and the PB2 Asp701Asn mutation. Conclusion Acute respiratory system damage is the main clinical manifestation of avian influenza (H7N9)virus infection in humans,live poultry exposure is an important risk factor for H7N9 infection in humans, adaptive mutation occurred at partial site of avian virus gene,which can be more easily be spread from birds to hu-man and cause serious diseases,it is necessary to strengthen the pathogen monitoring.
