1.Application of double C program in the perioperative period of coronary artery bypass

Objective To observe the clinical efficacy of the double C program consisting of glucose monitoring systems (CGMS) and insulin pump (CSⅡ) in perioperative period of patients undergoing coronary artery bypass patients with type2 diabetes mellitus (T2DM).Methods Divided participants who underwent coronary artery bypass surgery with T2DM into double C treatment group (double C group,n =20) and multiple subcutaneous insulin group (MSⅡ group,n =30).Before and after surgery,blood sugar control in blood glucose time,blood sugar fluctuations,the amount of insulin,the incidence of low blood sugar,incision infection,and total hospitalization days were observed.Results Comparing the double C treatment group with the MSⅡ group,blood sugar fluctuations ((5.76 ± 1.42) mmol/L vs.(7.91 ± 1.68) mmol/L,P < 0.05),the amount of insulin ((38.82 ± 16.97) U/d vs.(49.00 ± 15.32) U/d,P =0.032),the blood sugar compliance time ((3.52 ± 1.13) d vs.(6.00 ± 4.27) d,P =0.002),hypoglycemia (4 cases vs.1 case,P =0.025),the incision infection rates in both the cases(P =0.948),the total admission days((23.68 ± 13.67) d vs.(30.12 ± 2.94) d,P =0.084).Conclusion The double C program is comprehensively effective.for the perioperative glycemic control of patients with T2DM undergoing coronary artery bypass.
2.Preparation of medical polypropylene anti-bacterial mesh coated with triclosan and detection of its nature

Objective To explore the production of medical polypropylene anti-bacterial mesh coated with triclosan,and study the properties of its controlled-release drugs,as well as the efficiency of the mesh coated with triclosan.Methods A medical polypropylene anti-bacterial mesh coated with triclosan was produced by the method of solvent evaporation,with triclosan as antibacterial agent and ethylene-vinyl alcohol copolymer as carrier.The meshes were divided into four groups: A,B,C and D.The meshes in A,B and C(experimental) groups were coated with triclosan,and D group was control group.The ultrastructure of meshes of the 4 groups was observed under electron microscope.The slip diffusion method was used to observe the diameter of bacteriostasis-rings in different time periods.At the same time,we observed the maximum tensile strength of the mesh in transverse and longitudinal axis when they were pulled off before and after the experiment.Results The surfaces of B group were uniform and smooth,and the coating could continuously release drugs during 7 days in sodium chloride.The diameter of bacteriostasis-rings in B group was significantly larger than that in control group(P
3.A follow-up study of left ventricular function in ischemic mitral regurgitation patients after percutaneous ;coronary intervention

Objective To analyze the relation between ischemic mitral regurgitation (IMR)and the changes of left ventricular systolic synchrony and volume parameters before and after percutaneous coronary intervention (PCI)in patients with subacute myocardial infarction and to evaluate the impact of PCI on IMR by measuring such changes with three-dimensional echocardiography volume quantitation technology. Methods Eighty-seven patients were enrolled and echo data were acquired before and 6 months after PCI. Patients were classified into alleviation group (group Ⅰ)and non-alleviation group (group Ⅱ)according to the change of mitral regurgitation volume (MRvol).The systolic synchrony indexes (Tmsv 1 6-SD%,Tmsv 1 6-Dif%)and left ventricle volume parameters (LVEDVI and LVESVI)before and after PCI for patients of two groups were measured and contrasted.Results Systolic synchrony indexes of all patients of two groups were improved after PCI.Left ventricular volume parameters were decreased after PCI in group Ⅰ,and the remodeling was reduced.Correlation analysis of the parameters variations showed that,there was significant positive correlation between variations of left ventricular volume and MRvol in all patients and no significant correlation between variation of systolic synchrony index and MRvol.Conclusions PCI can improve left ventricular systolic dyssynchrony for submyocardial infarct patients.Left ventricular volume index that reflects the remodeling process is closely related to IMR.

5.Changes in blood hemorheology and early morning blood pressure in patients with obstructive sleep apnea-hypoventilation syndrome

Objective To investigate changes in morning blood pressure surge (MBPS) and blood hemorheology in patients with obstructive sleep apnea-hypoventilation syndrome (OSAHS) of various severity. Methods Total 118 patients with OSHAS diagnosed at the Department of Respiratory Diseases, People's Hospital of Guizhou Province were divided into three groups, i.e. , 31 mild, 40 moderate and 47 severe cases, based on their apnea-hypoventilation index (AHI), and 25 healthy persons were selected as controls. Blood hemorheology, oxygen saturation, morning blood pressure surge of the patients, as well as percentage of the time with oxygen saturation more than 90 percent of the total monitoring time (TS90%) were recorded. Results Early morning blood pressure surge was found in 21 cases (43%) with severe, eight (20%) with moderate and one (3%) with mild OSAHS. All indicators of blood hemarheology and TS90% were significantly higher in the patients with OSAHS than those in controls (P<0.05 ), those in moderate and severe OSAHS were significantly higher than those in controls (P<0.05 ) and those in severe OSAHS was significantly higher than those in moderate and mild OSAHS (P<0.05). All indicators of blood hemorheology were significantly higher in those with early morning blood pressure surge than those without it (P<0.05). Conclusions Blood viscosity, percentage of early morning blood pressure surge and TS90% in the patients with OSAHS are increased with severity of apnea-hypoventilation, and changes in blood hemorheology correlated with early morning blood pressure surge.
6.Immunosuppressive effect of naked DNA vaccine targeting tissue factor by intrasplenic inoculation on colorectal cancer

AIM: To investigate the expression of objective gene and the immunosuppressive effect of naked DNA vaccine pγ1.Ig H.SPTT targeting tissue factor by intrasplenic inoculation on colorectal cancer ( CRC) .METH-ODS:A special naked DNA vaccine which carried the SPTT peptides and immunoglobulin H chain gene ( named pγ1.Ig H.SPTT DNA plasmid ) was constructed by molecular biological techniques .After a single injection of this plasmid into the spleen, the concentrations of transgene product SPTT-Ig H in the peripheral blood at different time points were detected by ELISA, and the plasmid transfection efficiency and characteristics were analysis by PCR and Southern blotting .The immu-nologic effect of the plasmid on the CRC was observed in the mice .RESULTS:The strongest expression of SPTT-Ig H was observed during the 4th week after a single injection of the plasmid , which began to decline at the 12th week and disap-peared at the 16th week.The concentration of SPTT-Ig H in the peripheral blood at the 2nd week after transfection of plas-mid was 7.2 μg/L, then increased gradually , and reached a peak of 13.11μg/L at the 8th week.The plasmid-transcrip-tional gene was only expressed in the spleen , and was not detected in the lymph nodes , bone marrow, liver, kidney, and other organisms.Transfection of pγ1.Ig H.SPTT into the spleen had inhibitory effects on colorectal cancer as compared with control group .CONCLUSION:Naked DNA vaccine pγ1.Ig H.SPTT stimulates the immune response for protecting the body against colorectal cancer , which is a safe and effective method for CRC immunotherapy .
7.Study on hyperosmotic regulation-involved reciprocal pathways between medullary visceral zone and hypothalamic paraventricular nucleus or supraoptic nucleus

Objective To test the possibility of reciprocal pathways between medullary visceral zone (MVZ) and hypothalamic paraventricular nucleus (PVN) or supraoptic nucleus (SON) following hyperosmotic stimulation. Methods Hyperosmotic pressure animal model was established by administering3% sodium chloride as drinking water to rats. The distributions and expressions ofHRP retrogradely labeled neurons, Fos, tyrosine hydroxylase (TH) or vasopressin (VP) positive neuron and lial fibrillary acidic protein (GFAP) positive astrocytes (AST) in MVZ, SON and PVN were observed by quadruple labeling methods of WGA-HRP retrograde tracing combined with anti-Fos, TH (or VP) and GFAP immunohistochemical technique. Results Fos positive neurons within the MVZ, PVN and SON increased markedly. There were also a large number of GFAP positive structures in the brain and their distribution pattern was fundamentally similar or analogous to Fos positive neurons in the above-mentioned areas. The augmented GFAP reactivities took on hypertrophic cell bodies, thicker and longer processes.Quadruplicate immunohistochemical staining showed that a neuron could be closely surrounded by many AST and they formed neuron-astrocytic complex (N-ASC). Conclusion The neurons and AST might be very active following hyperosmotic pressure and N-ASC as a functional unit might serve to modulate the osmotic pressure. There was reciprocal osmoregulation pathways between the MVZ and SON or PVN in the brain.

OBJECTIVETo investigate the effects of interferon -alpha1b (IFN-alpha1b) on hepatic intercellular adhesion molecule-1 (ICAM-1) expression and serum HBV DNA in patients with chronic hepatitis B.
METHODSBefore and 6 months after IFN-alpha1b treatment, liver biopsy was performed in patients with chronic hepatitis B to detect the expression of ICAM-1 in the liver tissues using immunohistochemistry. Serum HBV load was detected with real-time fluorescence polymerase chain reaction.
RESULTCAM-1 expression in the liver tissue was significantly down-regulated after IFN treatment in patients with severe and moderate chronic hepatitis B (P<0.05). No significant variation was noted in the expression of ICAM-1 in the livers of patients with mild chronic hepatitis B after the treatment (P>0.05). In the patients weakly positive for ICAM-1 expression (+), serum HBV DNA varied scarcely after the treatment (P>0.05), while in the patients with strong ICAM-1 positivity (++, +++, or ++++), significant variation of serum HBV DNA occurred after the treatment (P<0.05 or P<0.01).
CONCLUSIONThe therapeutic effect of IFN-alpha1b is associated with the expression of ICAM-1 in the hepatocytes, and its expression might enhance the effects of IFN on HBV DNA in patients with chronic hepatitis B.

10.Clinical observation of the changes of serum helper T-lymphocytes (TH) cytokines in patients with HIV-infection and opportunistic infection.

OBJECTIVETo observe the changes of serum helper T-Lymphocyte (Th) cytokines at each stage in patients with human immunodeficiency virus (HIV) infection and with opportunistic infection.
METHODSSeventeen normal subjects were studied as controls. Among the 85 patients with HIV-infection studied, 31 had opportunistic infection. The study was divided into stage A, B, and C according to the standards set forth by The US Centers for Disease Control and Prevention (CDC). 17, 29, and 39 subjects were respectively at stage A, B, and C. The levels of the CD4+ T cells and the CD8+ T cells were measured by flow cytometry (FCM), while the levels of interleukin-2 (IL-2), interferon-gamma (IFN-gamma), interleukin-6 (IL-6), and interleukin-10 (IL-10) were measured using enzyme-linked immunosorbent assay (ELISA). All data were analyzed with statistic software SPSS11.0.
RESULTSThe level of the CD4+ T cells was (361.85 +/- 230.61) 10(6)/L in the experimental group, lower than that of the control group (772.41 +/- 161.56) 10(6)/L (t = 6.992, P < 0. 01). The level of IL-2 was (61.82 +/- 63.59) pg/ml, lower than that of the control group (111.25 +/- 66.14) pg/ml (t = 2.907, P < 0.01). In the experimental group, the level of the CD8+ T cells was 713.36 +/- 317.59 10(6)/L, higher than that of the control group (583.24 +/- 96.28) 10(6)/L (t = 3.127, P < 0.01), the level of IL-10 was (1362.70 +/- 869.49) pg/ml, higher than that of the control group (818.54 +/- 276.22) pg/ml (t = 4.704, P < 0.01), and the level of IL-6 was (1883.14 +/- 1058.61) pg/m, higher than that of the control group [(1208.52 +/-745.36) pg/ml] (t = 2.502, P < 0.05). Along with the progression of the disease, the level of IL-2 in the experimental group was decreasing gradually, reaching (51.72 +/- 62.28) pg/ml at stage C and (69.02 +/- 62.77) pg/ml at stage B, both of which were lower than those of the control group. The levels of IL-6 and IL-10 rose gradually and were (2040.27 +/- 1078.95) pg/ml and (1472.10 +/-982.03 ) pg/ml respectively at stage C, higher than those of the control group. At stage B, the level of IL-10 was (1347.35 +/- 780.95) pg/ml, higher than that of the control group (818.54 +/- 276.22) pg/ml. The level of IL-6 was (2236.24 +/- 1052.42) pg/ml in patients with opportunistic infection, higher than that in those without opportunistic infection (1680.43 +/- 1017.05) pg/ml (t = 2. 395, P < 0. 05).
CONCLUSIONDynamical measure of the levels of the serum IL-2, IL-6 and IL-10 in patients with HIV infection is a must. Therefore, the progression of AIDS can be controlled by increasing the level of IL-2, decreasing the levels of IL-6 and IL-10, and adjusting the balance of TH1/TH2 cells.
