1.Distribution and drug sensitivity of pathogens in 133 children with suppurative otitis media

Objective To guide proper clinical medication through analysing the main pathogen and the drug resistant situation of suppurative otitis media of the local children .Methods Making the bacterial culture and antibiotic susceptibility tests with the ear secretion collected randomly of 133 children with suppurative otitis media .Results Among the 133 cases ,110 pathogenic germs were separated totally ,as the detectable rate was 98 .23% ,and Among them ,56 streptococcus pneumoniae ,26 staphylococcus au‐reus (3 multi‐drug resistant to oxacillin) ,11 streptococcus pyogenes ,4 Haemophilus influenza and 13 others .Streptococcus pneu‐moniae was susceptible of ceftriaxoneo ,cefotaxime ,chloramphenicol ,levofloxacin ,quinupristin/dalfopristin and vancomycin ,while was resistant to clindamycin ,tetracycline ,sulfamethoxazole and erythromycin .11 .54% of the total detectable rate of MRSA in Staphylococcus aureus ,produce to β‐ lactamase detection rate was 84 .61% .Streptococcus pyogenes had a high resistance rate to erythrocin ,while a low rate to the other antimicrobial .Conclusion The main pathogen of the local children suppurative otitis media are Streptococcus pneumonia ,Staphylococcus aureus and Streptococcus pyogenes .The clinicians should use antibiotics reasonably according to the antimicrobial susceptibility results .
2.Clinical analysis of common bronchiectasis pathogenic bacteria and their drug resistance for children

Objective To analyze the distribution of common pathogenic bacteria and their drug resistance ,in children with bron‐chial pneumonia ,and provide reference for making antimicrobial treatment strategies .Methods 857 hospitalized pediatric patients with bronchopneumonia from January to December 2014 were enrolled in the study ,bacterial culture and drug susceptibility test were performed ,and the results were analyzed .Results In the 857 pdiatric patients ,the positive rate was 36 .87% (316/857) .A‐mong them ,175 cases were gram positive bacteria(20 .42% ) ,141 cases were gram negative bacteria(16 .45% ) ,140 cases were Streptococcus pneumoniae(16 .34% ) ,70 cases were Haemophilus influenzae(8 .17% ) .The susceptibility of Streptococcus pneu‐moniae to penicillin with enzyme inhibitors ,levofloxacin and vancomycin was the highest(higher than 90 .00% ) ,followed by the third generation cephalosporin(non meningitis) ,to which the sensitivity rate were above 80 .00% .The susceptibility of Haemophi‐lus influenzae to the third generation cephalosporins ,ampicillin/Sulbactam ,levofloxacin and imipenem were the highest(more than 90 .00% ) ,while the resistance to ampicillin and cotrimoxazole was 80 .00% .Conclusion Gram positive bacteria are the most im‐portant pathogenic bacteria ,and Streptococcus pneumoniae is the most common bacteria in bronchopneumonia .In clinical treatment , the results of drug sensitivity test should be paid attention to ,so as to adjust the treatment plan and achieve the ideal effect .
3.Fishbone diagram reduces the incidence of blood residual after sealing tube indwelling needle

Objective To investigate the influence of fishbone diagram in the incidence of blood residual after sealing tube indwelling needle in cardiovascular disease patients.Methods Two hundred and ninety-three patients(1,035 times)with sealing tube intravenous indwelling needle were assigned into control group and another 272 ones(1,276 times)with the needle into experiment group according to admission sequence. In the former group the method of conventionally sealing the tube for intravenous indwelling needle was applied and in the latter,fishbone diagram was applied to analyze the key factors for blood residuals and regulate pertinent measures for effective sealing of tube.The two groups were compared in terms of incidence of blood residuals.Result The incidence of blood residuals in the experiment group was significantly lower than that in the control group(P<0.05).Conclusion The fishbone diagram is effective in reducing the incidence of blood residuals after sealing the tube for intravenous indwelling needle by way of taking effective interventional measures after screening out the key influential factors for blood residuals.
4.Comparison of Effects of Radix Astragali and Triperygium Glucosides on Glomerular Mesangial Cells Proliferation and Interleukin-6 Secretion

[Objective] To compare the effects of Radix Astragali (RA) and triperygium glucosides (TG) on glomerular mesangial cells (GMC) proliferation and interleukin-6 (IL-6) secretion. [Methods] GMC were cultured with in-vitro cell culture technology. Serum containing RA and serum containing TG were prepared with serum pharmacological methods. GMC proliferation was detected by methylthiazolyltetrazolium (MTT) assay and IL-6 level by enzyme-linked immunosorbent assay (ELISA) to evaluate the effect of low-, moderate- and high-dosages of RA-containing serum and TG-containing serum under the condition of GMC over-proliferation. [ Results] Different dosages of RA-containing serum and TG-containing serum inhibited GMC proliferation and IL-6 secretion in a dose-effect manner, and the inhibitory effect of high- and moderate-dosage TG and high-dosage RA was stronger. [Conclusion] Under the condition of in-vitro GMC over-proliferation, RA has the similar inhibitory effects on GMC proliferation and IL-6 secretion with TG. The combined treatment of RA and TG can reduce the dose of TG and probably has an action in increasing effect and reducing toxicity.

ObjectiveTo investigate the error monitoring and attention network in patients with obsessive-compulsive disorder (OCD).Methods 33 OCD patients and 31 healthy subjects were tested with the error monitoring task and attention network test.ResultsThere wcrc significant differences between the patients with OCD and heath controls on Full Correct(FC),Full Error(FE),the numbers of uncertain,the numbers of dropout in the error monitoring task ( FC ( 121.03 ± 4.41 ) / ( 135.61 ± 3.28 ),FE ( 19.30 ± 3.69 ) / ( 9.45 ± 2.33 ),the numbets of uncertain( 8.27 ± 2.16) / ( 2.87 ± 0.70),the numbers of dropout ( 13.70 ± 2.60 ) / (4.26 ± 1.24 ) ) (P <0.05 ).In the attention network test,the comparison between the patients with OCD and healthy subjects on the RT of the alerting network was significant differences (P < 0.05 ).There was a significant correlation between attention network and error monitoring.ConclusionThe impairment of the error monitoring as well as attention network in patients with OCD may be involved in the dysfunction of anterior cingulate cortex.
7.Ligaturing pericardial blood vessel combined splenectomy for treating children with cavernous transformation of portal vein

Objective To retrospectively analyze the therapeutic effect of ligaturing pericardial blood vessel combined splenectomy for treating children with gastrointestinal bleeding and hypersplenism secondary to cavernous transformation of portal vein (CTPV).Methods Retrospectively analyzed 30 cases of children with CTPV admitted by Beijing Children's Hospital in General Surgery Department from Jan.2005 to Dec.2012,and evaluated the clinical efficacy of ligaturing pericardial blood vessel combined splenectomy for treating children with gastrointestinal bleeding and hypersplenism secondary to CTPV.Results All of 30 cases admitted operation without serious complication.All children admitted follow-up from 6 to 60 months.Seven cases of 22 patients with gastrointestinal bleeding relapsed bleeding post operation and were cured by conservative treatment.Four cases of 7 relapsing bleeding children had NSAID.All cases of 8 children with simple hypersplenism suggested normal results of blood routine post operation,2 children occurred bloody stools who were cured by conservative treatment.None of patients had serious infection correlating with splenectomy.The size of spleen of patients with gastrointestinal bleeding is larger than patients with simple hypersplenism.Conclusions (1) Ligaturing pericardial blood vessel combined splenectomy is a good choice for children with gastrointestinal bleeding and hypersplenism secondary to CTPV.(2) NSAID may increase the recurrence rate of gastrointestinal bleeding post operation.(3) There may be a negative correlation between the size of spleen and the risk of gastrointestinal bleeding for children with CTPV.
8.Effect of recombinant rat CC16 protein on LPS-induced expression of TNF-α, IL-6 and IL-8 in rat tracheal epithelial cells

AIM:To explore the effect of recombinamt rat CC16 protein (rCC16) on LPS-induced expression of tumor necrosis factor-alpha (TNF-α), interleukin-6 (IL-6) and IL-8 in the rat tracheal epithelial (RTE) cells. METHODS:The RTE cells were incubated with rCC16 at concentrations of 0.5, 1.0 and 2.0 mg/L in serum-free media for 2 h prior to LPS (0.1 mg/L) treatment for further 24 h.The cells were harvested for assessing the mRNA levels of TNF-α, IL-6 and IL-8 by RT-qPCR.The cell culture supernatants were collected for analyzing the protein levels of TNF -α, IL-6 and IL-8 by ELISA.In addition, the nuclear translocation of nuclear factor-κB (NF-κB) p65 was tested by Western blot.RESULTS:rCC16 inhibited LPS-induced IL-6 and IL-8 expression at both mRNA and protein levels in the RTE cells in a concentration-dependent (0~2 mg/L) manner, as demonstrated by RT-qPCR and ELISA.However, no concentra-tion-dependent manner between the dose of rCC 16 and TNF-αexpression was observed , and rCC16 inhibited LPS-induced TNF-αexpression at lower concentration (0.5 mg/L).rCC16 concentration-dependently inhibited the effects of LPS on the level of nuclear translocation of NF-κB p65.CONCLUSION:rCC16 suppresses LPS-mediated TNF-α, IL-6 and IL-8 pro-duction through inactivation of NF-κB activity in RTE cells .
9.The epidemiological impact of varicella vaccination in kindergartens, primary and secondary schools in Beijing.

OBJECTIVETo study the epidemiological impact of varicella vaccine vaccination on kindergartens and school children in Beijing.
METHODSAccording to "China Information System for Diseases Control and Prevention", the reported clinical diagnosis varicella cases were tracked in kindergartens, primary and secondary schools whose onset date were from 2008 to 2010. Epidemiological survey was conducted and epidemiological features were analyzed.
RESULTSA total of 21 474 varicella cases were investigated: 55.3% (11 883 cases) had been vaccinated by varicella vaccine. Of cases with definite immunization history, interval between vaccination date and onset date were from 30 days to 1 year accounted for 3.4% (286/8510), 1 to 3 years accounted for 18.2% (1551/8510), 3 to 5 years accounted for 28.6% (2431/8510), 5 to 10 years accounted for 34.3% (2916/8510) (left-closed right-open interval); The peak age of onset was 4 years old in cases without immunization history, which was 6 years old in cases with immunization history; The proportion of cases with immunization history (≥ 30 days) had increased from 42.4% (2862/6754) in 2008 to 56.3% (4327/7679) in 2010. The cases with no fever had a higher proportion (54.9%, 6413/11 679) of immunization history (≥ 30 days) than cases with fever (47.7%, 4533/9500) (P < 0.01); The cases with rashes less than 50 had a higher proportion (57.4%, 8045/14 020) of immunization history (≥ 30 days) than cases with rashes more than 50 (40.2%, 2902/7216) (P < 0.01).
CONCLUSIONVaricella vaccine delays the peak age of onset, alleviates the symptoms. The current immunization strategy can not block varicella spread in kindergartens, primary and secondary schools.

10.Role of microRNA-215 in nephropathy of type 2 diabetic db/db mice

Objective To investigate the renal expression changes of microRNA-215(miR-215) and its role in diabetic nephmpathy of type 2 diabetic db/db mice. Methods Fourweek-old diabetic db/db mice and norml control group non-diabetic db/m mice were selected.Real-time PCR was used to detect the relative level of miR-215 at the age of 8,12 and 16 weeks.Catenin beta interacting protein 1 (CTNNBIP1) mRNA and protein level were measured by realtime PCR,WesteRN blotting and immunohistochemisty.A lueiferase reporter assay was used to determine whether CTNNBIP1 was a direct target of miR-215. Results (1)With the growth of db/db mice,the major pathological characteristics of kidney included glomerular hypertrophy,segmental mesangial cells proliferation and mesangial matrix expansion.(2)Compared with the db/m mice,the db/db mice of 8,12 and 16 weeks showed obvious increase in body weight(BW),blood glucose (Glu) and 24 hour urinary albumin excretion (UAE) (P<0.05,respectively).(3)Compared with the db/m mice,special miR-215 was highly expressed in the kidney of db/db mice and was up-regulated significantly according to the development of DN (P<0.05).(4)The mRNA and protein expression of CTNNBIPl of kidney were consistently down-regulated in db/db mice than those in controls (P<0.05,respectively). (5)By luciferase reporter,miR-215 could negatively regulate CTNNBIP1 gene by targeting its 3'-UTR sequence (P<0.01). Conclusion High expression level of miR-215 plays a potential role in the initiation and progression of DN by down-regulating the expression of CTNNBIPl.
