2.A case of oncocytic papilloma in nasal ethmoid sinus.

The incidence of oncocytic papilloma of nasal ethmoid sinus is very low. In order to improve the understanding of the disease for clinicians,we report one case of oncocytic papilloma in nasal ethmoid sinus. The case performs mainly congestion of the left nasal cavity, running pus nose and hyposmia. Nasal endoscopy found new creature in the meatus,and CT indicates soft tissue shadow in the left superior nasal passages,ethmoid sinus involved. Tumor is resected completely. There is no recurrence after five years follow-up.

3.Laryngeal neuroendocrine carcinoma: report of one case and review of the literature.

The clinical data of one case of laryngeal neuroendocrine carcinoma in our hospital were respectively analyzed. The patient was a 61-year-old male, and complained of progressive pain of right ear for 7-year, it was misdiagnosed as glossopharyngeal neuralgia and larynx hemangioma. This patient was achieved total laryngectomy and bilateral cervical lymph nodes dissection. As of 12 months after surgery, the patient remains under follow-up observation, and no findings indicating obvious recurrence or metastasis has been observed. This disease is a rare and highly malignant tumor that is lack of specific feature in clinical performances. Recognition at larynx is essential so that proper patient management can be initiated.

4. Identification of traditional She medicine Gegongniugen and its confusable species of genus Rubus using ITS2 barcode

Objective: To discriminate traditional She medicine Gegongniugen (the root of Rubus chingii Hu) from its confusable species of genus Rubus based on ITS2 barcode, and to guarantee the medicine quality and clinical effect of Gegongniugen. Methods: A total of 140 samples of R. corchorifolius L. f., R. hirsutus Thunb., R. parvifolius L., R. coreanus Miq., R. buergeri Miq., R.tsangorum Hand. -Mazz., R. trianthus Focke, R. jiangxiensis Z. X. Yu, W. T. Ji et H. Zheng were collected from Zhejiang, Guangxi, Jiangxi, and Anhui provinces. Genomic DNA was extracted from the collected samples, followed by ITS2 sequences amplification through PCR, bi-directional sequencing, assembly and annotation based on HMMer method. At the same time, 37 other sequences of corresponding species were downloaded from GenBank. Sequence characteristics such as lengths, GC contents, and variable sites were analyzed by Gene Tool software while genetic distance calculation and build-up of neighbor-joining (NJ) phylogenetic tree were carried out through Clustal X and MEGA 7.0. Results: The lengths of ITS2 sequences of Gegongniugen and its nine confusable species of genus Rubus ranged from 211 to 213 bp with GC content of 52.1%-58.0%; These ITS2 sequences had 14 mutation sites and one insertion/deletion site; The average intra-specific K2P genetic distance of Gegongniugen was 0.007 2, which was significantly lower than the respective inter-specific K2P genetic distance between Gegongniugen and its confusable species of genus Rubus. The genetic distantes between Gegongniugen and R. corchorifolius or Gegongniugen and R. jiangxiensis were minimum, while that between Gegongniugen and R. hirsutus was maxmum. It was obvious in the N-J phylogenetic tree that the ITS2 sequences of Gegongniugen formed into one separate clade, which could be successfully distinguished other nine species. Conclusion: ITS2 sequence can be used as an effective DNA marker to discriminate the original plants of Gegongniugen and its confusable species of genus Rubus, which provided an important molecular evidence for the identification of traditional She medicines and contributed to the information supervision of traditional She medicine market.
5.Chondrogenesis of bone marrow mesenchymal stem cells in co-culture system with chondrocyte

ObjectiveTo explore the effect of osteoarthritis (OA) chondrocytes and normal chondrocytes on the chondrogenesis of bone marrow mesenchymal stem cells(BMSCs) in self designed co-culture system. MethodsRabbit BMSCs and chondrocytes were isolated and expanded in vitro. OA chondrocytes were harvested from the rabbit of established osteoarthritis model. We made a BMSCs-low melting agarose constructs, then put it onto the self-made 6 well plates lattice assembly for co-culture with chondrocytes. The groups were divided into Normal P0-BMSCs, Normal P3-BMSCs, OA PO-BMSCs, OA P3-BMSCs and BMSCs (control) group. At 3, 7, 14 day culture, cultured cells in all groups were collected for real-time PCR, glycosaminoglycan (GAG) content, cytoactive detection, and histological observation. ResultsType Ⅱ collagen gene expression was up-regulated in group Normal PO-BMSCs, which showed 5.1-, 7.2-, 11.2-fold increase over that of control group at 3, 7, 14 days, respectively. Type Ⅰ, Ⅱ collagen and aggrecan gene expressions were not obviously up-regulated. In group OA P3-BMSCs, type Ⅰ collagen gene expression level lower than the control group in 3, 7, 14 day. In normal PO-BMSCs, GAG content showed 2.59-fold increase over that of the control group. GAG content of group OA PO-BMSCs and control group showed no significant differences.Others groups showed significant differences in comparison with the control group(P<0.05). Alcian stain showed positive in all groups. The normal PO-BMSCs group showed the darkest blue-stained. Conclusion Rabbit normal PO chondrocytes and rabbit OA P0 chondrocytes significantly enhanced chondrogenic differentiation of rabbit BMSCs. The secreted morphogens of the rabbit normal P3 chondrocytes and rabbit OA P3 chondrocytes do not have a significant effect on chondrogenic differentiation of rabbit BMSCs.
6.Surgical effect of traumatic lens dislocation with secondary glaucoma

AIM: To retrospectively evaluate the effect of lens extraction combined with vitrectomy to treat traumatic lens dislocation with secondary glaucoma.
METHODS: Thirty - one eyes ( 31 cases ) of lens dislocation caused by blunt trauma with secondary glaucoma were treated respectively with cataract extraction combined with anterior vitrectomy, trabeculectomy and intraocular lens implantation. The visual acuity and pressure were observed 1wk, 1 and 3mo after operative.
RESULTS: Thirty - one eyes were all complete the operation successfully, and 6 eyes were given combined trabeculectomy, 9 eyes were implanted anterior chamber intraocular lens implantation ( IOL ) and 15 eyes were given posterior chamber suture fixation. Sixteen eyes were implanted in one-stage operation, while 8 eyes were implanted in two-stage operation. All intraocular pressure ( IOP ) were controlled to the normal level after operation and 23 eyes had visual acuity of more than 0. 3. CONCLUSION: Lens extraction combined with vitrectomy is an effective method for treatment of lens dislocation with secondary glaucoma. In order to control the IOP and get well visual function, we should choose IOL implantation or trabeculectomy according to the patient's condition.
7.Weight analysis of influencing factors of human angle Kappa

Background Angle Kappa is the angle between the pupillary axis and visual axis,and it is a major consideration in corneal refractive surgery and strabismic surgery.Researches showed that age and refractive status affect angle Kappa value,and additionally,the measuring results of angle Kappa are associated with instruments.Objective This study was to investigate and weight the influences of axial length,anterior chamber depth and corneal curvature to angle Kappa.Methods A cross-sectional study was designed.Fifty health volunteers were included from Dec 2009 to Aug 2010,with the age of 18-38 years and average diopter of-1.0 D and best corrected visual acuity of 1.0.The horizontal and vertical angle Kappa was binocular measured with a testing machine designed by laboratory of School of Optometry & Ophthalmology Wenzhou Medical University.IOLMaster was employed to measure the ocular axial length,anterior chamber depth and corneal curvature radius.A multiple linear regression model was established to analyze the influences of axial length,anterior chamber depth and corneal curvature radius to angle Kappa.Results An efficacious regression equation was established as Y =15.554-0.587X1+0.724X2+ 0.232X3 with the independent variables as axial length(X1),anterior chamber depth (X2),corneal curvature (X3) and dependent variable as horizontal angle Kappa (Y) (R =0.788,R2 =0.621,P =0.000).Horizontal angle Kappa showed a negative correlation with axial length (b1 =-0.587,β1 =-1.002,P =0.000) and a positive correlation with anterior chamber depth (b2 =0.724,β32 =0.296,P =0.030).No significant correlation was found between corneal curvature radius and horizontal angle Kappa (b3 =-0.232,β3 =-0.068,P=0.338).A new regression equation was Y =14.235-0.622X1 + 0.824X2 after removed the corneal curvature variable (R =0.786,R2=0.618,P =0.000),with a negative correlation between horizontal angle Kappa and axial length (b1 =-0.622,β1 =-1.062,P<0.05) or positive correlation between horizontal angle Kappa and the anterior chamber depth (b2 =0.824,β2 =0.337,P<0.05).In addition,a multiple linear regression equation among 3 independent variables and dependent variable of vertical angle Kappa was Y =0.492-0.020X1 +0.038X2 +0.089X3 (R =0.436,R2 =0.191,P =0.000).However,no significant correlations were seen between vertical angle Kappa and axial length,anterior chamber depth or corneal curvature radius (all at P > 0.05).Conclusions Ocular axial length and anterior chamber depth have an obvious influence to horizontal but not vertical angle Kappa.Axial length appears to have more influence to horizontal angle Kappa than anterior chamber depth.
8.Influence of Sevoflurane or Propofol Anesthesia Methods for The Myocardial Injury Markers of Patients with Heart Valve Replacement Surgery

Objective To compare the influence of whole sevoflurane inhaling and target-controlled infusion of propofol for the myocardial protective effect on patients with heart valve replacement surgery. Methods 30 adult patients who went through heart valve replacement surgery with cardiopulmonary by pass were selected, including ASA staging II-III and cardiac function classification (NYHA) II-III. All patients were randomly divided equally into sevoflurane group (Group S) and propofol group (Group P) . Patients were monitored before anesthetic induction. Group S got 1%sevoflurane (fresh gas flow 6 L/min) with concentration of the vaporizer increased from 1%to 3%with 1 minute interval during anesthetic induction. Group P got target-controlled infusion of propofol during anesthetic induction,the initial target plasma concentration was set at 0.8μg/mL,and the concentration increased 0.5 μg/mL every minute until intubation. All the patients got fentanyl 5 μg/kg and rocuronium 0.6 mg/kg, and intubation was conducted when BIS decreased lower than 60 and mean arterial pressure (MAP) <20%basic MAP. During anesthesia maintaince,patients got 0.5-2 MAC sevoflurane inhaling or target-controlled infusion of propofol 2-4μg/mL with discontinuous intravenous fentanyl and rocuronium, and maintained BIS 40-60, MAP<±20%basic MAP, central venous pressure 5-15 cm H2O. Outcome variables included demographic characteristics of patients. The following parameters were also recorded, including cardiac troponin I (cTnI), creatine kinase (CK), creatine kinase isoenzyme (CK-MB) and lactate (LAC) in before anesthetic induction (T0), aortic inbation (T1),30 minutes after aorta opening (T2), 6 hours after aorta opening (T3) and 24 hours after aorta opening (T4) . Results There was no statistical significance in demographic characteristics during peri-operation between the two groups ( > 0.05) . The pre-opertaive cTnI, CK, CK-MB and LAC were within the normal range, but increased siginicantly on T2, T3 and T4, and was more significant on T3 ( < 0.01) between two groups, and the intra-group comparison showed no difference on other time points. Conclusion When myocardial injury markers used as myocardial protection outcome variables, whole sevoflurane inhaling could not reduce the release of cTnI compared to propofol TIVA in heart valve replacement surgery.
9.Thirteen cases with tuberculous otitis media and mastoiditis.

OBJECTIVE:
To analyze the clinical features and treatment of tuberculous otitis media and mastoiditis.
METHOD:
Thirteen patients with tuberculous otitis media and mastoiditis were retrospectively analyzed and the related literatures were reviewed. One case was treated by surgery only, and anti-tuberculosis treatment was given to another one patient, and surgical removal of disease lesions in combination with anti-tuberculosis treatment were given to 12 patients.
RESULT:
One patient received surgery only was found to recur after follow-up for seven months. The patient did not recur after seven months' follow-up after antituberculosis therapy,and other patients did not recur.
CONCLUSION
Although the regular anti-tuberculosis chemotherapy treatment was the main treatment for tuberculous otitis and mastoiditis, surgical treatment was helpful to achieved more rapid healing of the ear.

10.Paraglottic space primary paraganglioma: a case report and review.

OBJECTIVE:
To investigate the clinical feature, diagnose and therapeutic methods of paraglottic space primary paraganglioma.
METHOD:
One case of paraglottic space primary paraganglioma was reported and the relevant literatures were reviewed.
RESULT:
One case showed a hoarse voice, who was cured after the surgery of neck incision. NSE and CgA were positively expressed.
CONCLUSION
Paraganglioma of the paraglottic space is very rare. The diagnosis of paraglottic space primary paraganglioma bases on histopathology and immunohistochemistry. The immunohistochemistry and clinical character must be comprehensively analyzed to increase the diagnosis accuracy.
