1.Research on clinical application of navigation bronchoscopy system

Objective:To analyze the clinical application of navigation bronchoscopy system, and improve the accuracy during routine use.Methods:Through discussing ENB, System composition, clinical application and environment, the paper presents the clinical application standard and working environment standard of the navigation bronchoscopy system.Results:Electromagnetic navigation bronchoscopy(ENB) combined with the advantages of electromagnetic technology, virtual bronchoscopy and 3D CT reconstruction, which can perform the accurate localization of peripheral pulmonary lesion and mediastinal lymph node to acquire tissue samples for pathological diagnosis. ENB-guided localization can be used in interventional therapy(local injection or radioactive seed implantation).Conclusion:Navigation bronchoscopy system shows the merit of minimally invasive technique and medical visualization. It is a great progress in interventional pulmonology history.
2.Investigation of status quo of self-perceived burden of young and middle-aged patients with first-attack of ischemic stroke and analysis of its influencing factors

Objective To know the self-perceived burden (SPB) of young and middle-aged patients with first-attack of ischemic stroke and to analyze the influential factors of SPB as well as its relevance to the quality of life.Methods The patients' general information questionnaire,SPB Self-evaluation Scale,QOL-BREF were adopted to investigate young and middle-aged patients with first-attack of ischemic stroke.Results 100 patients were included,66 were male,34 were female.The Minimum age was 19.4 years old.The maximum age was 59 years old.The mean age was (50.80 ± 7.98) years old.The average SPB score was (24.21 ± 7.39) points,which was in the medium level.31 cases (31%) didn't have obvious SPB; 65 cases (65%) suffered mild-to-moderate SPB; 4 cases (4%) claimed they had severe SPB.There was negative correlation between the SPB and their quality of life.The multi-factor analysis showed that gender,the patients' understanding of disease,and caregivers' health situation were the influential factors of SPB.Conclusions Since self-perceived burden is common in young and middle-aged patients with ischemic stroke,clinical nurses should assess patients' self-perceived burden so as to provide targeted nursing intervention and health education,thereby to ease the burden on patients' self feeling,eventually to improve the level of their mental and physical health.
3.Canonical correlation between health literacy and self-management ability of patients with hypertension in community

Objective:To understand the status and association of health literacy and self-management ability of hypertensive patients in the community and to provide reference for further intervention research.Methods:From June 2019 to October 2019, a random number table method was used to randomly select multiple streets or towns/townships, and then 401 residents in the community or village aged above 18 years of age diagnosed as hypertension were selected as the subjects of this study. The general data questionnaire, high blood pressure-health literacy scale into Chinese(C-HBP-HLS) and hypertension patients self-management behavior rating scale(HPSMBRS)were used to conduct one-to-one field survey. SPSS 24.0 was used to analyze the scores of health literacy and self-management behavior of patients with hypertension. SAS 9.4 was used to analyze the canonical correlation between health literacy and self-management behavior of patients with hypertension.Results:The score of health literacy and self-management ability were (38.94±17.56) points and (129.45±16.53) points, respectively.The results of canonical correlation analysis showed that the canonical correlation coefficient between health literacy and self-management behavior reached 0.57, which was mainly reflected in the great correlation between " drug label" and " diet management" .Conclusion:Attention should be paid to the positive effect of health literacy on self-management ability, and further intervention research should take the " drug label" and " written health literacy" dimensions of health literacy as entry points to effectively improve the self-management ability of hypertension patients in the community.
4.Therapies of simvastatin on AECOPD combined with PAH through affecting VEGF and the pulmonary function

Objective To assess the effect of simvastatin on secretion of VEGF in blood serum and sputum of elderly chronic ob-structive pulmonary disease(COPD)combined with PAH patients and the pulmonary function.Methods Seventy cases of elderly COPD patients combined with PAH patients were randomized in two groups,namely the normal treatment group and combined sim-vastatin group (35 cases in each group),then compare the changes of the VEGF in blood serum,sputum and lung function.Results VEGF in blood serum and sputum showed significant changes after therapies of simvastatin (P <0.05).After 4 weeks′therapy, the FEV1% pre and FEV1/FVC increased significantly (P <0.05).Conclusion Normal treatment combine with simvastatin could decrease VEGF in blood serum and sputum of elderly COPD combined with PAH patients,meanwhile improve pulmonary function.
5.Establishment and evaluation of adjuvant-induced arthritis animal model by Mtb in Sprague-Dawley rats

Aim To establish and evaluate the simple and effective animal model of rheumatoid arthritis in SD rats induced by heat-killed mycobacterium tuberculosis H37Ra(Mtb).Methods SD rats were immunized s.c.at the base of the tail with Mtb(1 mg/rat) in mineral oil,and then body weight,blood routine,clinical signs of arthritis were observed regularly.And also hind paw volume in SD rats with AA was tested.Meanwhile,T-lymphocyte subset was determined by flow cytometry in peripheral blood,the level of TNF-?in serum and IL-1 and IL-6 and vascular endothe-lial growth factor(VEGF) in joint tissue were tested by ELISA.The ankle pathological change and radiographic evidence of the hind paws of SD rats were observed to evaluate the AA model effects and availability in SD rats.Results When compared with normal control rats,the total number of white blood cells,platelets and monocytes in peripheral blood and ratio of CD4/CD8 was markedly raised after immunization with Mtb.But body weight of model rats was significantly low from day 14 to 28 after injection of Mtb.The inflammatory arthritic lesions such as edema and erythema in the paws appeared after 9~10 d.The peak of inflammation was 14~16 d.The level of TNF-?in serum and IL-1,IL-6 and VEGF in joint tissue was significantly higher in SD rats with Mtb than that in normal control rats at d 28.The histopathological examination revealed cellular infiltration,synovial and cartilage hyperplasia in ankle joints of SD rats.Conclusions The model induced by Mtb in SD rats is more similar in clinical characterization of rheumatoid arthritis in human being.It is a more efficient and simpler manipulative procedure for screening of new anti-arthritis drugs and experimental studying of anti-rheumatoid arthritis.
6.Analysis of Nosocomial Infection Pathogens from Lower Respiratory Tract in Senile Patients in Different Season

OBJECTIVE To investigate the distribution and drug resistanc of nosocomial infection pathogens from lower respiratory tract in senile patients in different season.METHODS The sputum samples from lower respiratory tract infection in senile patients in two years,were collected to identify pathogens and drug sensitivity test j udged according to NCCLS standard.RESULTS The gram-negative bacilli accounted for 85.7%.The gram- posutive bacilli accounted for 14.3%.The predominant pathogens were Pseudomonas aeruginosa(20.3%),Kleb- siella pneumoniae(14.5%),Acinetobacter lwof fi(9.8%),coagulase-negative staphylococci(9.0%).The distribution and antibiotic-resistanc of main pathogens had different characteristics in different season. CONCLUSIONS Understanding the characteristics of local pathogen spectruma and the antibiotic-resistanc of main pathogens in different season were signincant on prevention and therapy of the lower respiratory tract infection in senile patients.
7.Nosocomial Infection Pathogens from Lower Respiratory Tract in Senile Deceased Patients

OBJECTIVE To investigate the distribution and drug resistance of nosocomial infection pathogens from lower respiratory tract in senile patients. METHODS The sputum and lower respiratory tract secretion in the senile patients with lower respiratory tract infection were collected nearly five years,and identified.The drug sensitivity test,the results of examination were judged according to NCCLS standard. RESULTS The Gram-negative bacilli accounted for 79.5%.The Gram-positive bacteria accounted for 20.5%.The predominant pathogens were Pseudomonas aeruginosa(19.6%),coagulase-negative Staphylococci(CNS)(18.7%),and acinetobacter lwoffi(11.6%).The resistant bacteria were markedly increasing. CONCLUSIONS The Gram-negative bacilli are the major pathogens in the senile deceased patients.The incidence of CNS infection is markedly increasing.The isolating rate of meticillin-resistant CNS is 100.0%.
9.Study on APC,p53 and K-ras genes mutation of tissues and feces in the patients with colon tumor

Objective To investigate the feasibility and clinical significance of fecal DNA tests for colon tumor diagnosis and screening,we inspected the mutations of APC,p53 and K-ras genes in the tissue and feces of patients with colorectal tumor.Methods We collected 46 patients with colorectal cancer(CRC),60 patients with colorectal adenomas( CRA) and 30 cases of normal person in Huizhou First People Hospital in Guangdong Province from Nov.2011 to Aug.2012.Then all the tumor tissues and feces of these people were detected the mu-tation rate about the APC,p53 and K-ras genes using polymerase chain reaction single strand conformation poly-morphism analysis method(PCR-SSCP).Results The mutation rates of APC,p53,K-ras genes of feces in CRC group,CRA group and normal group were 58.7%(27/46),65.2%(30/46)and 60.9%(28/46),20.0%(12/60),25.0%(15/60)and 23.3%(14/60),3.33%(1/30),0%(0/30)and 0%(0/30),respectively.How-ever the mutation rate in tissues were 63.0%(29/46),69.6%(32/46)and 63.0%(29/46),25.0%(15/60), 26.7%(16/60)and 26.7%(16/60),0%(0/30),0%(0/30) and 0%(0/30).Corresponding mutations could be found in feces and tumor tissues.Consistency checking for mutations rate in feces and tumor tissues of CRC group and CRA group showed that the Kappa value were 0.818(P<0.001),0.901(P<0.001),0.862(P<0.001)and 0.857(P<0.001),0.870(P<0.001),0.822(P<0.001).It means an excellent consistency.Con-clusion Fecal DNA testing is expected to become an effective noninvasive colon tumor early diagnosis and screening method.
10.The curative effect and safety research of bisoprolol in the treament of the elderly COPD patients with chronic heart failure

Objective To assess the curative effect and security of bisoprolol in the elderly chronic obstructive pulmonary disease (COPD) patients combined with chronic heart failure (CHF) .Methods 61 cases of elderly COPD patients combined with CHF in our hospital were selected .on the basis of conventional treatment ,the patients were applicated bisoprolol 2 .5 mg ,1 time each day , the NT-proBNP ,LVEF% ,LEVDD and FEV1% were respectively observed before treatment ,4 weeks after treatment ,8 weeks af-ter treatment ,6 months after treatment .Results 4 weeks after treatment ,NT-proBNP ,LVEF% ,LEVDD showed no significant changes .8 weeks after treatment ,NT-proBNP and LEVDD decreased(P<0 .05) ,LVEF% showed no significant changes .6 months after treatment ,NT-proBNP and LEVDD decreased significantly ,meanwhile LVEF% increased (P< 0 .05) .before treatment ,4 weeks after treatment ,8 weeks after treatment ,6 months after treatment ,FEV1% were 47 .33 ± 5 .57 ,47 .08 ± 5 .50 ,48 .30 ± 6 .31 , 49 .64 ± 6 .24 ,All groups showed no changes in the pulmonary function(P>0 .05) .NT-proBNP of all groups were negatively corre-lation with the FEV1% (r= -0 .708 ,-0 .702 ,-0 .833 ,-0 .778) .Conclusion Small doses of bisoprolol is safe and effective in the treament of the elderly COPD patients with chronic heart failure .
