1.Sclerosing Encapsulating Peritonitis in Chronic Ambulatory Peritoneal Dialysis; Preoperative Catheter Drainage: A Case Report.

Sclerosing encapsulating peritonitis is a well recognized, but uncommon, complication of chronic ambulatoryperitoneal dialysis, I report a case of sclerosing encapsulating peritonitis in which percutaneous catheterdrainage was performed preoperatively. Ultrasonography(US) and computed tomography(CT) showed a largemulti-septated cystic mass which occupied nearly all the peritoneal cavity. Percutaneous drainage with two 8.5French catheters was preoperatively performed under fluoroscopy and about 2100ml of bloody fluid was drained for20 days. On follow-up CT, the size of the cyst had significantly decreased and an operation was performed. It isconsidered that percutaneous catheter drainage is useful in the preoperative decompression of sclerosing encapsulating peritonitis.

2.MR Findings of Mayer-Rokitansky-Kuster-Hauser Syndrome: Two Cases Report.

Magnetic resonance imaging (MRI) was used to study Mayer-Rokitansky-Kuster-Hauser (MRKH) syndrome in two 21-year-old women. In one case MRI revealed that fluid filled an area of the uterus measuring about 4 x 4cm, but that the ovaeies were normal ; in the other case, a rudimentary uterus and normal ovaries were seen. MRI is useful both in the planning of surgical management and in evaluating the details of pelvic anatomy and uterine morphologic characteristics.

3.Decompressive Effect of Indirect Decompression in Thoracolumbar Burst Fracture.

This study was performed to establish a radiological indication and contraindication of indirect decompression in the thoracolumbar burst fracture as well as to clarify an acceptable degree of the canal constriction to enhance neurologic recovery and to prevent the spinal stenosis. The canal diameter ratio (CDR) of the constricted level was determined using pre-and post-operative CAT images of 31 thoracolumbar burst fractures, decompressed indirectly. The acceptable CDR was decided by the lower limit of the 95% confidence interval of the post-operative CDR in cases without neurologic deficit associated with the canal constriction. A radiological indication and contraindication of indirect decompression was establish based on the calculation of the discriminant equation and linear regression equation respectively. The results were as followings. 1. The acceptable CDR was 46% at the cauda equina level and 37% at the conus medullaris level. 2. According to the result of calculation of the regression equation, the radiological contraindication of indirect decompression was the pre-operative CDR of 13% or less at the cauda equina level and 27% or less at the conus medullaris level. In conclusion, the radiological indication of indirect decompression was the pre-operative CDR of 34% at the cauda eqbina level and 42% at the conus medullaris.

4.Anti-metastatic Effect of Natural Product-motivated Synthetic PPAR-γ Ligands

Colorectal cancer is one of the most common cancers globally, ranking second for the number of cancer-related deaths. Metastasis has been reported as the main cause of death in patients with colorectal cancer. Peroxisome proliferator-activated receptor gamma (PPAR-γ) is a transcription factor that functions as a tumor suppressor by inhibiting cellular proliferation, migration, and invasion. In our previous efforts to generate natural product-motivated PPAR-γ ligands, the compounds 1 and 2 were obtained. These compounds activated PPAR-γ and inhibited the migration and invasion of HCT116 colorectal cancer cells, and they were also found to inhibit the epithelial-to-mesenchymal transition, which is a key process in cancer metastasis. Compounds 1 and 2 upregulated expression of the epithelial marker (E-cadherin), and downregulated expression of the mesenchymal marker (N-cadherin) and transcriptional factor (Snail). Therefore, the PPAR-γ agonists 1 and 2 could serve as a valuable model for the study on anti-metastatic leads for the treatment of colorectal cancer.
5.Relationship of Smartphone Addiction to Physical Symptoms and Psychological Well-being among Nursing Students: Mediating Effect of Internet Ethics.

PURPOSE: This study was done to identify the status of smartphone addiction among nursing students and to examine the mediating effects of Internet ethics on the relationship between smartphone addiction, physical symptoms and psychological well-being. METHODS: Data were collected between May 20 and June 23, 2014. Participants were a convenience sample of 214 nursing students of one university in Seoul. Structured questionnaires were used to measure degree of smartphone addiction, physical symptoms, psychological well-being and Internet ethics. Data was analyzed using Independent t-test, One-way ANOVA, Pearson correlation coefficient, multiple hierarchical regression. RESULTS: Of the nursing students, 14.5% had high level of risk of smartphone addiction and 16.8% had potential risk. Higher frequency of physical symptoms and lower level of Internet ethics were related to the higher level of smartphone addiction. Voluntary-control in Internet ethics had mediating effects on the relationship between smartphone addiction and physical symptoms. CONCLUSION: The results suggest that the level of smartphone addiction may lowered in nursing students by intensifying Internet ethics especially voluntary-control. Based on these findings, Internet ethics program for nursing students should be developed to facilitate the effective use of smartphones.

6.Development and Validation of Korea Sleep Scale A.

The purpose of study was to develop and validate Korean Sleep Scale A which can be easily administrated by nurse for a wide range of adult subjects. Dimensions of adult concept were derived by literature review. Previously developed sleep scales were also analyzed in order to develop scales items. The Korean sleep scale A was structured as a 15 item, 4 point like type scale. The Scale was tested an 298 female and male adult subjects after pilottest and content validation. The results showed reliability by internal consistency of Cronbach's alpha=75 Construct validity was tested by factor analysis and known group technique. Factor analysis showed 4 factor-sleep pattern, sleep evaluation, sleep result, and cause of sleep disturbance. Theses four factors explained 55.9% of total variance. Correlations between factors ranged from .15-.32. There was correlation among Korean Sleep Scale A score, Verran and Snyder-Halpern(VSH) Sleep Scale scale, and sleep satisfaction scale score, r= .71(p=.00), r=.51(p=.03), respectively. These study results partially support reliability, construct validity, and criterion related validity of Korean Sleep Scale A. Further validation study is required with other subjects and other settings.

7.Placement of an Implantable Central Venous Access Device.

PURPOSE: To evaluate the efficacy and safety of placement of a central venous catheter with infusion portinto the superior vena cava MATERIALS & METHODS: Central venous catheters with a infusion port were implanted in21 patients (M:F=4:17, age range:15-63, mean age:41) diagnosed as suffering from breast cancer(n=9),lymphoma(n=7), thymoma(n=2) rhabdomyosarcoma(n=2) and rectal cancer(n=1). The peripheral portion of the subclaveanvein was punctured under fluoroscopic guidance during injection of contrast media at the site of the ipsilateralperipheral vein (20 cases) and under ultrasonographic guidance (1 case). 9.6F central venous catheters placed inthe superior vena cava via the subclavian vein and the connected infusion ports were implanted in the subcutaneouspocket near the puncture site of the right anterosuperior chest wall. RESULTS: Catheter insertion in the superiorvena cava and port implantation in the subcutaneous pocket were successful in all patients. Mean procedure timewas 23 minutes and there were no early complications. Because the incision site had not healed, one patientunderwent resuturing 3 weeks after the procedure. In one case, thrombotic occlusion of the catheter occurred, butsuccessful recanalization, involving urokinase infusion, was performed. At the end of the chemotherapy schedule,at 180, 157 and 139 days after the procedure, three central venous catheters with a infusion port were removed inthe radiologic suite. Catheter days are 5 days-180 days(mean, 119) from now (1997. 7. 1). CONCLUSIONS: Radiologicplacement under fluoroscopic guidance of a central venous catheter with a infusion port is easy, safe and usefulfor patients requring long-term venous access.

8.Slipped Capital Femoral Epiphysis(SCFE).

Slipped capital femoral epiphysis(SCFE) is a disorder in which there is a gradual or acute disruption through the capital physeal plate. The physiolysis is through a widened zone of hypertrophy, which is weakened due to altered chondrocytic maturation and endochondral ossification. The cause or causes of SCFE remain uncertain. the association of obesity and adolescent age with growth rate are predisposing factors. The possibility that most patients with subclinical hormonal abnormality were proved. The goal of treatment of slipped capital femoral epiphysis is to reatore the function of the hip and delay the development of degenerative osteoarthrosis by prevention of additional displacement of the epiphysis. We report 10 patients(12hips) with SCFE who were treated by surgical means and followed along for more than one year, at Yeungnam University Hospital, from 1989 to 1996. There were six boys and four girls. The average age at operation was 11.8 years. Seven cases occurred in the left hip, one case in the right and 2 cases had bilateral involvement, five cases had a history of minor trauma on affected hip. Among hormonally studied six patients, panhypopituitarism patient was one case; decreased testosterone, two; decreased growth hormone, two; and decreased thyroid hormone, one. According to clinical stage, two cases were the acute type; five cases, acute on chronic type; and three cases, chronic type. On the radiological grades of slipping, mild slippage were nine hips; moderate, one; and severe, two. The eleven hips were treated by pin fixation in situ, and one, by cuneiform osteotomy. On the average follow-up of 2.6 years, ten hips were excellent or good functional results, two hips were failure.

9.Comparison of Intraocular Pressure Measurements of Tono-Pen and Perkins Tonometer in Children Under the General Anesthesia.

Tono-pne and Perkins tonometer have been widely accepted to measure the intraocular pressure(IOP) of children under the general anesthesia because of their portability. We evaluated the difference of IOP between two devices and the IOP variability in 39 children(77 eyes) under age 13 who had undergone surgery for strabismus or entropin under the general anesthesia. One observer measured the IOP three times alternately, and the order of measurement was randomized. Only 5% of Tono-pens coefficient of variance was considered valuable. The range of Tono-pen IOPs was 9~22mmHg, and the average was 15.41+/-3.23mmHg. The range of Perkins IOPs was 4~20 mmHg, with the average of 12.47+/-3.31mmHg. The difference between two methods on average was 2.94mmHg. Tono-pen IOPs were significantly greater than those recorded by Perkins tonometer, but both were within normal range. The IOP variability was not significantly different between two devices. Therefore both methods can be applied effectively in a clinical setting after sedation or general anesthesia of uncooperative children.

10.Expression and Clinical Significance of Transforming Growth Factorbeta1 (TGFbeta1) and TGFbeta ReceptorII in Pancreatic Cancer and in Chronic Pancreatitis.

PURPOSE: Transforming growth factorbeta (TGFbeta) is an extracellular ligand that binds to a heterodimeric receptor, initiating signals that regulate growth, differentiation, and apoptosis. Many cancers, including pancreatic cancer, harbor defects in TGFbeta signaling and are resistant to TGFbeta-mediated growth inhibition. Recently, it has been reported that enhanced expression of TGFbeta1 is associated with the progression of a pancreatic ductal cell carcinoma (PCA) and chronic pancreatitis (CP). We investigated the difference in the expressions of TGFbeta1 and the TGFbeta receptor II (TbetaRII) in PCA and CP and the clinical significance of TGFbeta1 and TbetaRII in PCA. METHODS: Surgically resected pancreatic specimen were obtained from 26 patients with a PCA and 12 with CP; 5 normal pancreatic tissue specimens were also obtained. The specimens were immunostained for TGFbeta1 and TbetaRII. RESULTS: Immunohistochemical analyses of TGFbeta1 and TbetaRII revealed positive immunostaining in 73.1% and 66.7% of the tumors, respectively. In the ductal epithelial cells of PCA and CP, there were no significant differences in the expressions of TGFbeta1 and TbetaRII. In contrast, their stromal expressions tended to be stronger in PCA than in CP; especially, the expression of TbetaRII was significantly higher in PCA (p=0.008). Also the presence of TGFbeta1 and TbetaRII in the cancer was significantly associated with node metastasis, advanced tumor stage (p<0.01), and short survival time (p<0.05). CONCLUSION: There might be some pathological difference in the stromal reactions of TGFbeta1 or TbetaRII between PCA and CP. The presence of TGFbeta1 or TbetaRII indicates a role in disease progression.
