Aneurysm of the internal carotid artery is a rare disease and is known to be associated with congenital arterial anomalies such as neurofibromatosis type I (NF-I). NF-I is an autosomal dominant neurocutaneous disorder characterized by a variety of manifestations that involve the central and peripheral nervous systems, skin, vascular system, and skeleton. In particular, the involvement of vascular abnormalities in NF-I is well known. Any vessel may be affected by this condition, although the renal artery is most frequently involved. The vascular abnormality can be occlusive or an aneurysmal degenerative change. Therefore, symptomatic presentations might assume an indolent pathophysiologic course such as hypertension, or manifest as a catastrophic event such as arterial rupture that could result in sudden death. We report a rare autopsy case of an aneurysmal rupture of the internal carotid artery in a woman with suspected NF-I, who collapsed in her home.

A rare case of hepatic metastasis with a choriocarcinomatous component from a composite adenocarcinoma and choriocarcinoma of the sigmoid colon in a 60-year-old man is reported. The hepatic metastasis displayed choriocarcinoma with extensive hemorrhagic necrosis. The tumor cells were poorly differentiated with scattered foci of bizzare syncytiotrophoblastic cells. Retrospective examination of the previous colonic carcinoma proved that the tumor was composed of two distinctive elements. One was a moderately well differentiated adenocarcinoma located in mucosa and submucosa. The other was a deep seated and undifferentiated carcinoma which was made up of hyperchromatic bizzare cells with syncytiotrophoblastic cells. There were transitional foci from adenocarcinoma to undifferentiated carcinoma with trophoblastic cells. Immunohistochemical staining showed beta-hCG expression in the undifferentiated cells of both the primary and the metastatic tumors. Implications for the possible origin and cause of tumor cell heterogeneity are briefly discussed.

This study aimed at examining speech defects of open bite patients, which were analized in terms of formant frequency for vowels and word pronunciation length for consonants. In addition, the upper and lower lip (perioral m.) activity was tested by the EMG. The tongue force was measured by the strain gauge, and the speech discrimination test was carried out. One experimental group and one control group were used for this study and they were respectively composed of six female openbite patients and six normal-occlusion females. Eight monophthongs, two fricatives and two affricatives were chosen for speech analysis. Speeches of the above-mentioned groups were recorded and then analized by the ILS/PC-I software. Four hundred most frequently used monosyllables were also chosen for discrimination score. Openbite patients showed the following characteristics: 1. Abnormality in case of /a/, /epsilon/, /e/, /i/ F2 and /e/, /a/ F1. 2. Significantly elongated length in their pronunciation of /h/ and /Ch/ and somewhat elongated length also in their pronunciation of /s/ and /c/. 3. Significant upper lip activity according to the EMG test during pronunciation of the bilabial consonants. 4. Relatively weak tongue force according to the strain gauge measurement. 5. According to the speech discrimination test, high rate of misarticulation in case of (a) initial /p/ /s'/ and /ts'/, (b) /a/, /epsilon/, /e/, /je/, /o/, /phi/, /jo/, /u/, /we/, and /i/ (c) final /eta/.

The study was undertaken to ascertain the speech defect of patients with malocclusion, especrally of poenbite patients, by means of the spectral analysis method. The expermental group was composed of ten female openbite patients and their mean age was 13.8 yrs. The control group was also composed of ten female girls ans then mean was 13.7 yrs. As for the speech materisl, eight Korean monphthrongs ans two Korean fricatives and two affricatives were used Speeches were recorded and then analyzed by a Kay 7800 drgital sonagraph. Formant frequency level or range was used as a phonemic parameter. The results were as follows. 1. Among Vowels /a/ F1,F3, and F1/F2 showed abnormahty /o/and/omega/F2,F2-F1 and F1/F2 showed abonrmahty. 2. Among Consonants /s/and/h/ The upper and lower borders of the frequency range showed abnormahty. /f/ The lower border of the ferquency range showed abonormahty. /C(h)/ The upper and lower borders of the frequency range and concentration point showed abonormahty.

A newly developed third generation enzyme immunoassay(Lucky HCD 3.0 EIA) for hepatitis C virus(HCV) antibodies was added with the envelope(E1E2)/NS4 fusion proteins and expanded NS5 proteins as well as the core/NS3 fusion proteins. Authors evaluated the HCD 3.0 EIA with the previously available second generation EIA(HCD 2.0) in 10,435 Red Cross blood donors. Among 10,435 donors who were screened for the presence of HCV antibodies by HCD 2.0 assay, 22(0.21%) sera were repeatedly reactive. All of these sera were tested for further testing. Only 13 of all tested sera were reactive by HCD 3.0 EIA, and nine sera were not reactive. Nine of 13 HCD 3.0 positive sera were reactive by recombinant immunoblot assay(Lucky-Confirm). Also seven of these 13 sera had detectable HCV genomic RNA by reverse transcriptase-polymerase chain reaction(RT-PCR). None of nine HCD 3.0 negative samples had detectable immunoblot assay and HCV genomic RNA. It is concluded that the new HCV EIA can decrease a significant false positivity of second generation EIA in a blood donor population. This new assay correlates well with detection of HCV-RNA by RT-PCR and identifies donors who are truly infected.

To evaluate the antiproteinuric effect and antihy-pertensive effect of angiotensin converting enzyme inhibitor according to the angiotensin converting enzyme(ACE) and endothelial nitric oxide synthase (ecNOS) genotypes, 72 proteinuric patients and 121 control subjects were examined. Patients who were on oral steroids or who had taken antihypertensive or antiproteinuric drug within four weeks before the base-line period were excluded from this study. Systolic blood pressure, diastolic blood pressure, 24 hour urine protein, and 24 hour urine creatinine level were measured before drug administration. The patients were given a four-week supply of enalapril 10mg if no cough were occurred. Systolic blood pressure, diastolic blood pressure, 24 hour urine protein, and 24 hour urine creatinine level were measured repeatedly after drug administration. In genotype distribution of the ACE gene poly- morphism, there was no significant difference between controls and proteinuric patients. The antiproteinuric effect was dominent in DD genotype. But, there were no significant association between antiproteinuric effect and ecNOS polymorphism in prote-inuric patients. We also failed to detect any signi- ficant association between the ACE and ecNOS ge-ne-polymorphisms and blood pressure lowering effect of angiotensin converting enzyme inhibitor. Our result suggested that antiproteinuric effect of enalapril in proteinuric patients was more prominent in DD genotype. But, a further study with larger number of patient population would be necessary.

Accessory fissures serve not only as natural barriers against infection but also help in localizing any focal pulmonary parenchymal diseases and in distinguishing pleural from parenchymal diseases. Knowledge of these fissures might be useful in differentiating unusual forms of atelectasis or consolidation occuring adjacent to the fissure. Left minor fissure (LMF) is a kind of unusual accessory fissures of the left lung, which separates adjacent segments of the upper lobe as clefts of various depths lined by two layers of visceral pleura. In this study, 4 cases of LMFs found in the left upper lobe during a routine dissection of 36 cadavers were observed. Of the 4 cases, 3 cases were true LMFs which located between the anterior segment of the upper lobe and superior segment of lingula, and 1 case was considered as left azygos fissure. Among the true LMFs, 2 LMFs coursed horizontally and 1 LMF coursed upward obliquely along the costal surface. The depth of LMFs was 0.5~1.2 cm and the length was 5~8 cm.
