3.Urogenital Complication in Diabetes Mellitus.

The clinical records have been studied from 517 diabetes subjects which were admitted to Severance Hospital during the period from Jan. 1968 to Aug. 1971 with special reference to urogenital complications. The results were as follows: 1) Among the 517 cases of diabetes mellitus, most frequent age distribution showed more than 50 years of age with 66% (329 cases) and next 40 to 50 years with 22% (126 cases). The ratio between male and female was 2 : 1 (male 352 : female 165). 2) The incidence rate of urogenital complications among 517 cases of diabetes mellitus was found in 13. 7% (71 cases). 3) The development of genitourinary complication was closely related to the duration of the diabetes. While only 38 cases (10.3%) of 367 diabetics with duration of disease of less than 5 years showed genitourinary complication, 14 cases (33.3%) of 58 diabetics with history of more than 10 years manifested diabetic complication in genitourinary tract. 4) The lesions of genitourinary tract were found more frequently in women than in men. Seventy one patients of genitourinary complication were present in 43 of 352 men (12%) and 28 of 165 women (17.5%). 5) Concerning the frequency of genitourinary complications (71 cases) in each disease, urinary infection was found in 46 cases (65%), Kimmelstiel-Wilson's syndrome in 32 cases (45%), and only 2 cases of papillary necrosis, 3 cases of impotence and 2 cases of neurogenic bladder were observed. 6) In 71 cases of urogenital complication, the proteinuria was detected in 47 cases (66%), elevation of blood urea nitrogen level was 36 cases (50%) and positive result of urine culture was 12 cases with E. coli and aerobacter. 7) The frequency of urogenital complication in diabetes mellitus demands that a definite plan of urological investigation be included in its management. In many our cases, no specific questions relative to the genitourinary tract were asked in history taking. It is evident that the necessity of close cooperation between the physician and urologist in the management of diabetes mellitus. To attain the optimum results in the care of diabetes mellitus, consultation with the urologist is essential.

4.Clinical Observation on the Cryptorchidism.

A clinical observation was made on cryptorchism of the in patients in the Department of Urology, Severance Hospital during the period January, 1960 through December, 1969. 1) During the period, 823 male patients hospitalized, there were 53 cases of cryptorchism, giving a rate of 6.5% respectively. 2) Of the cases of cryptorchism, 15 cases (28. 4%) were observed at 5 year old or below and to one's misery, 26 cases (50%) were seen at over 11 year old. 3) Lateralization shows 18 cases or 34% in the bilateral and 35 cases 66% in the unilateral (19 cases in right side and 16 cases in left side). 4) Position of undescended testis is most popular in the inguinal type (73%) and intraabdominal type is 11 testes (15%). 5) Of presenting symptoms, pain sensation on the inguinal region was observed in 10 cases(19%).

5.Two Cases of Transpubic Urethroplasty in Posterior Urethral Strictures.

Two patient underwent transpubic repair of posterior urethral strictures. Wedge resection of pubic bone was provided excellent exposure for urethroplasty and no orthopedic disability. The result was satisfactory in the patient without affect continence or potency.

7.A Case of Pheochromocytoma.

A l3-year-old boy was admitted to our hospital with severe hypertension, visual blurring, excessive sweating, polyuria and polydipsia. On admission, the blood pressure was persistently elevated at 230/180 mmHg. and an ophthalmologist was consulted, the funduscopic examination showed bilateral grade IV retinopathy. Cardiomegaly with dyspneic on exertion and persistent tachycardia were present. In urine examinations, proteinuria was sustained and glycosuria was traced. B. U. N. was 9.4 mg/dl. Urinary catecholamine was 12480 mic.g./day ; urinary V. M. A. was 20.2 mg./day. Regitine test was positive but histamine test was not performed. Presacral air insufflation with nephrotomography showed egg sized soft tissue mass locating at just above the upper pole of left kidney. He was operated with transabdominal approach. There were bilateral pheochromocytomas on suprarenal areas. Two weeks after operation, he was discharged with good general condition and we report this case with review of literatures.

8.Two Cases of Congenital Unilateral Renal Agenesis with Genitourinary Tuberculosis.

The congenital unilateral agenesis is defined as the complete absence of one kidney due to the fact that the ureteral bud fails to develop or, if it does develop, does not reach the metanephros. It is usually found by accident on urography. We have experienced 2 cases of congenital unilateral renal agenesis accompanied by tuberculous epididymitis, proved by radiologically and surgically and report with brief review of the literatures.

9.A case of Primary Retroperitoneal Teratoma, Including the Upper Pole of the Left Kidney.

A case of retroperitoneal teratoma in a 37-year old woman is presented. For approximately 20 years, she suffered from a slowly growing palpable mass and discomfort and pain in the LUQ. Upper GI series revealed compression and displacement of the greater curvature of the body of the stomach, and the outline of a large LUQ. mass overlapping the left kidney. IVP demonstrated compression of the left renal pelvis and calyceal system by this large mass, and considerable amorphous calcification in the upper pole of the mass. Pre-operatively, the mass was suspected to be a dermoidcyst or teratoma or pancreatic pseudocyst. At the operation, it proved to be a large retroperitoneal tumor mass of infant-head size, and it extended to the pancrestic hilum, including the upper pole of the left kidney, with adhesion to the left mesocolon. This huge tumor mass and involved left kidney were widely removed.

10.False Negative of Doppler Ultrasonography During Testis Torsion in Animal Model.

PURPOSE: Our aim is to identify the false negative rate of doppler ultrasonography and to compare color doppler ultrasonography with pathologic findings during testis torsion in animal model. MATERIALS AND METHODS: Seventeen male house rabbits weighted 1.9-2.6kg were examined with doppler ultrasonography as control group and 1 rabbit was orchiectomized as pathologic control group. In 16 rabbits, right spermatic cord was torqued 720o count-clockwise and placed back into the scrotum, and 1, 3, 6, 9, 12, 18, 24, 48 hours after torsion, doppler ultrasonographic and pathologic findings were examined at twisted testicles. RESULTS: In 1 hour and 3 hours group, false negative rate of doppler ultrasonography were 100% and 6 hours after torsion false nagative rate was 63%. Nine hours after torsion false negative rate was 50%, 12 hours after torsion false negative rate was 25%. But after 18 hours group, all had decreased or disappeared blood flow. Before 9 hours after torsion, there was no pathologic changes in seminiferous tubules. In seminiferous tubules, number of spermatid decreased in 12 hours group and spermatid was absent and spermatocyte were markedly degenerated in 18 hours group. In 48 hours group, spermatid and spermatocyte were absent, number of Sertoli cell decreased markedly and diffuse infarction were seen. CONCLUSIONS: As time goes by, false negative rate of doppler ultrasonography was decreased. So if blood flow have been normal when doppler ultrasonography was done before 12 hours after torsion, doppler ultrasonography should be re-examined in suspected testis before 12 hours after torsion because of spermatocytes are degenerated after 12 hours.
