1.Exploration and practice of constructing medical-rehabilitation integration working mode in rehabilita-tion hospital under new circumstances

Driven by multiple factors such as policy incentives,enhanced economic capacity of residents,aging popula-tion,and medical system reform,China's rehabilitation industry has been in a rapid development stage where opportunities and challenges coexist.Various rehabilitation hospitals urgently need new development thinking on how to seize the new opportunities for the development of rehabilitation medicine,give full play to the benefits of high-quality rehabilitation medical resources,and deal with the development of rehabilitation institutions under the new situation.Taking the tertiary rehabilitation hospital where the authors work as an example,this paper deeply analyzes the practical difficulties and challenges faced by the development of reha-bilitation hospital under the new circumstance,further explores the construction of a new working mode based on people-oriented and medical-rehabilitation integration.It also briefly summarizes the beneficial experiences and practices in the reform of rehabili-tation service models,and elaborates on the periodic achievement,so as to provide a meaningful reference for the majority of re-habilitation hospitals to plan and practice high-quality development and reform.
2.A qualitative study of facilitators and hindrances of smoking cessation in patients with chronic obstructive pulmonary disease

Objective:To investigate the facilitators and hindrances of smoking cessation in patients with chronic obstructive pulmonary disease (COPD), and to provide a basis for developing individualized smoking cessation intervention strategies for COPD patients.Methods:Based on the health ecology theory and using a phenomenological approach in qualitative research, purposive sampling was used to select 15 COPD patients with smoking history who were admitted to Tianjin Fourth Central Hospital from March to May 2023 and face-to-face semi-structured interviews were conducted. Colaizzi seven-step analysis was used to analyze the interview content.Results:A total of 15 COPD patients were interviewed, including 13 males and 2 females, aged 61-75 years old. The facilitators of smoking cessation in COPD patients included complications, sequelae of novel coronavirus infection, fear of death, smoking cessation counseling by medical staff, heavy family financial burden, and smoke-free environment. The hindrances of smoking cessation in COPD patients included milder disease symptoms of COPD, higher levels of nicotine dependence, false disease-related perceptions, family supervision and control, and occupational experience.Conclusions:Smoking cessation in COPD patients is influenced by five factors: personal characteristics, behavioral characteristics, interpersonal network, living and working conditions, and policy environment. Medical staff and relevant national institutions should formulate corresponding smoking cessation strategies according to address the facilitators and hindrances of smoking cessation in COPD patients, so as to further reduce the smoking prevalence of COPD patients, promote the health of patients and reduce the burden of disease.
3.Somatic CDKN2A copy number variations are associated with the prognosis of esophageal squamous cell dysplasia

Background::Somatic copy number variations (SCNVs) in the CDKN2A gene are among the most frequent events in the dysplasia-carcinoma sequence of esophageal squamous cell carcinoma. However, whether CDKN2A SCNVs are useful biomarkers for the risk stratification and management of patients with esophageal squamous cell dysplasia (ESCdys) is unknown. This study aimed to investigate the characteristics and prognostic value of CDKN2A SCNVs in patients with mild or moderate (m/M) ESCdys. Methods::This study conducted a prospective multicenter study of 205 patients with a baseline diagnosis of m/M ESCdys in five high-risk regions of China (Ci County, Hebei Province; Yanting, Sichuan Province; Linzhou, Henan Province; Yangzhong, Jiangsu Province; and Feicheng, Shandong Province) from 2005 to 2019. Genomic DNA was extracted from paraffin biopsy samples and paired peripheral white blood cells from patients, and a quantitative polymerase chain reaction assay, P16-Light, was used to detect CDKN2A copy number. The cumulative regression and progression rates of ESCdys were evaluated using competing risk models. Results::A total of 205 patients with baseline m/M ESCdys were enrolled. The proportion of ESCdys regression was significantly lower in the CDKN2A deletion cohort than in the diploid and amplification cohorts (18.8% [13/69] vs. 35.0% [28/80] vs. 51.8% [29/56], P <0.001). In the univariable competing risk analysis, the cumulative regression rate was statistically significantly lower ( P = 0.008), while the cumulative progression rate was higher ( P = 0.017) in ESCdys patients with CDKN2A deletion than in those without CDKN2A deletion. CDKN2A deletion was also an independent predictor of prognosis in ESCdys ( P = 0.004) in the multivariable analysis. Conclusion::The results indicated that CDKN2A SCNVs are associated with the prognosis of ESCdys and may serve as potential biomarkers for risk stratification.
4.The relationship and influencing factors between serum Cys-C,UA,CD4+and CD8+ and intracranial artery stenosis in cerebral infarction and analysis of influencing factors

Objective To explore the relationship between serum cystatin C (Cys-C),uric acid (UA),CD4+T cells and CD8+T cells with intracranial artery stenosis and cerebral infarction and analyze their influencing factors.Methods The clinical data of 124 patients with cerebral infarction who were hospitalized in our hospital from June 2020 to September 2021 were collected.All patients were given Brain MRA or /and CTA and routine biochemical examinations.According to the degree of proximal stenosis of intracranial large vessels,patients were divided into no stenosis group (36cases),mild stenosis group (43 cases),and moderate to severe stenosis group (45 cases).The clinical data of the three groups of patients and the differences in serum Cys-C,UA,and T cell subsets were compared.The factors affecting intracranial artery stenosis in cerebral infarction were analyzed by multivariate Logistic regression.The ROC curve was used to evaluate the value of serum CysC,UA and T cell subsets in patients with the cerebral infarction intracranial artery stenosis narrow.Results The serum levels of Cys-C,UA,CD4+T cells were lower in no stenosis group than those in mild stenosis group and those in medium-severe stenosis group,and positively correlated with cerebral infarction intracranial artery stenosis.The serum levels of CD8+T cells were higher in no stenosis group than those in mild stenosis group and those in medium-severe stenosis group,and were negatively correlated with cerebral infarction intracranial artery stenosis.Conclusion The increase of serum Cys- C,UA,CD4+ T cells and the decrease of CD8+T cells are risk factors for intracranial artery stenosis in cerebral infarction.As the degree of intracranial artery stenosis worsens,Cys-C,UA,CD4+T cells increase,and CD8+T cells decrease.
5. Cordyceps sinensis extracts attenuates HBx-induced mesangial cell proliferation and extracellular matrix synthesis by suppression of the PI3K/Akt pathways

AlM: To investigate the effect of C. sinensis extracts on HBx (Hepatitis B virus X protein) induced cell proliferation and extracellular matrix (ECM) accumulation and the underlying mechanism in cultured human mesangial cells (HMCs). METHODS: Human mesangial cells were stable transfected with pCMV-HBx to establish an HBx over-expression model, and a control group was transfected with an empty vector. Cell proliferation was determined by MTT assay and DNA synthesis assay. Western blotting was used to measure the expression of PI3K/Akt signaling pathway-related proteins and extracellular matrix. RESULTS: HBx transfection induced cell proliferation, matrix accumulation. HBx-transfected mesangial cells had increased activity of the PI3K/Akt pathways, and treatment with C. sinensis suppressed this effect. CONCLUSlON: C.sinensis attenuates the HBx-induced human mesangial cell proliferation and matrix production in human mesangial cells via inhibition of the PI3K/Akt pathways.
6.Clinical characteristics and prognosis of 44 cases of infantile neuroblastoma

Objective:To investigate the clinical characteristics of infantile neuroblastoma (NB) and the factors that affect prognosis.Methods:We retrospectively analyzed the clinical data collected from 44 cases of NB who received treatment in The Fourth Affiliated Hospital of China Medical University from March 2008 to March 2017 to summarize the clinical characteristics of NB and the factors that affect prognosis.Results:Among the 44 cases, 21 were male and 23 were female, with a median age of 5.5 months (range, 0 days-12 months). Four cases had stage I NB, seven cases stage II NB, five cases stage III NB, 15 cases stage IV NB, and 13 cases stage IVs NB. The tumors were located in the adrenal glands and retroperitoneum ( n = 26, 59.1%), posterior mediastinum ( n = 15, 34.1%), pelvis ( n = 2, 4.5%), and neck ( n = 1, 2.3%). The median follow-up time was 90 months (range, 2-144 months). The 3-year and 5-year overall survival rates were 93.2% and 90.9%, respectively. Among 35 cases who survived more than 5 years, 30 cases survived healthily, 5 cases survived with tumor, and 4 cases died. Bone marrow metastasis, bone metastasis, and the extent of tumor resection greatly affect the prognosis of NB ( χ2 = 6.92, 12.19, 4.70, all P < 0.05). Conclusion:The overall prognosis of NB is good in infants. NB mainly occurs in the abdomen. The survival rate of infants with stage IVs NB is lower than that of infants with stages I, II, and III NB. The prognosis of NB occurring in the abdomen is poorer than that occurring in other regions. Bone marrow metastasis, bone metastasis, and the extent of tumor resection are adverse factors affecting the prognosis.
7.Prevalence of mild visual impairment in type 2 diabetes patients over 50 years old in Funing County: Jiangsu Diabetic Eye Disease Study

Objective:To investigate the prevalence and influencing factors of mild visual impairment in type 2 diabetic patients aged ≥50 years living in Funing County, Jiangsu Province.Methods:A population-based cross-sectional study was adopted.Cluster sampling was conducted among diabetic patients aged ≥50 years registered in the Chronic Disease Information Center of Funing County Center for Disease Prevention and Control from March 7th to April 20th, 2019.General information of selected patients was collected.Visual and ocular examinations on site were performed by ophthalmologists and technicians.The International Council of Ophthalmology standard for mild visual impairment, 0.3≤best corrected visual acuity<0.8 was adopted.The influencing factors of mild visual impairment were analyzed by logistic regression.This study protocol was approved by the Ethics Committee of Affiliated Hospital of Nantong University (No.2014-071).Results:Among the included 2 067 patients, 1 909 cases completed the investigation, with a response rate of 92.4%.There were 1 146 patients with mild visual impairment in at least one eye, and the prevalence of mild visual impairment was 60.00%.Among the eyes examined, 1 832 eyes were with mild visual impairment, accounting for 47.98%.Cataract (896/1 632), refractive error (515/1 632) and diabetic retinopathy (166/1 632) were the main causes of mild visual impairment in diabetic patients, accounting for 54.90%, 31.61% and 10.19%, respectively.Aging, female gender, low educational attainment, oral hypoglycemic drugs or subcutaneous insulin injection were risk factors for mild visual impairment in both eyes.Higher educational attainment, more sunshine exposure and exercise were protective factors for mild visual impairment in both eyes.Conclusions:The prevalence of mild visual impairment is relatively high in type 2 diabetic patients.Cataract surgery, correction of refractive error may help to improve mild visual impairment.Strengthening blood glucose control, more physical exercise and outdoor activities may slow the progression of mild visual impairment and prevent it from moderate and severe visual impairment.
8.Comparison of diagnostic value of ultrasound and multi-slice spiral CT in wrist joint lesions of rheumatoid arthritis

Objective:To investigate the diagnostic value and imaging characteristics of ultrasound and multi-slice spiral CT in rheumatoid arthritis (RA) wrist joint lesions.Methods:The clinical data of 54 cases of RA wrist arthropathy in Jianhu Hospital Affiliated to Nantong University from December 2018 to April 2020 were analyzed. All the patients were examined by ultrasound and multi-slice spiral CT. The imaging characteristics of ultrasound and multi-slice spiral CT were analyzed, and the detection rates of synovitis, joint effusion, tenosynovitis, bone erosion and wrist joint lesions were calculated.Results:A total of 1 188 joints were examined in 54 patients with RA, including 108 wrist joints, 540 metacarpophalangeal joints and 540 proximal interphalangeal joints. There was no significant difference in the detection rate of wrist joint synovitis, joint effusion and tenosynovitis between ultrasound and CT ( P>0.05). The wrist joint lesions detection rates of ultrasound and CT were 97.2%(105/108) and 46.3% (50/108), the difference was statistically significant ( χ2 = 36.52, P<0.05). Ultrasound image features: synovitis ultrasound image showed hypoechoic joint cavity, not by joint displacement or compression image, color Doppler flow imaging (CDFI) examination could see blood flow signal;joint effusion in ultrasound image of joint effusion was strip, strip-shaped, non-uniform distribution, image was affected by joint displacement or compression, CDFI examination had no blood flow signal; bone erosion ultrasound image showed bone cortical continuous interruption or "crater" "insect erosion" like defects. CT imaging features: wrist joint space narrow, osteoporosis, some patients with scaphoid, distal radius visible bone erosion. Conclusions:The detection rate of wrist synovitis, joint effusion and tenosynovitis by ultrasound and multi-row spiral CT are similar. The multi-row spiral CT examination have a slight advantage in evaluating the joint space and bone changes. The sensitivity of wrist bone erosion and wrist joint lesions are higher than those of multi-row spiral CT, which can guide the early clinical diagnosis and treatment of RA.
9. Clinical Value of Serum Calcium Level Combined With Choline Esterase and MELD-Na Score in Evaluation of Prognosis of Patients With Decompensated Cirrhosis

Background: Decompensated liver cirrhosis is a common disease in department of gastroenterology, and the prognosis is poor. There are many liver cirrhosis-related scoring models, but all have certain limitations. Aims: To explore the clinical value of serum calcium level combined choline esterase, MELD-Na score for predicting prognosis in patients with decompensated cirrhosis. Methods: A total of 169 patients with decompensated cirrhosis from Jan. 2017 to Dec. 2020 at the Quzhou Affiliated Hospital of Wenzhou Medical University were enrolled. Serum calcium level, choline esterase, creatinine, international normalized ratio (INR), prothrombin time, total bilirubin (TBIL) and serum sodium level were detected, and MELD score and MELD-Na score were calculated. ROC curve was used to analyze the value of parameters in evaluating prognosis of patients with decompensated cirrhosis. Results: After 6 months' follow-up, 125 patients survived and 44 died. TBIL, creatinine, INR, prothrombin time, MELD score, MELD-Na score were significantly lower in survival group than in death group (P<0.05), while serum albumin, sodium, calcium, choline esterase levels were significantly higher (P<0.05). ROC curve showed that AUC of serum calcium level, choline esterase, MELD score and MELD-Na score for predicting prognosis of patients with decompensated cirrhosis were 0.824, 0.783, 0.781 and 0.839, respectively. AUC of serum calcium level combined with choline esterase and MELD-Na score was 0.897. Conclusions: The combination of serum calcium level, choline esterase and MELD-Na score has a higher predictive value for the 6-months prognosis of decompensated cirrhosis patients.
10.Biomechanical study of cystic lesions of the mandible based on a three-dimensional finite element model.

OBJECTIVE:
To analyze the biomechanics of cystic lesions in the mandibular body in a three-dimensional (3D) finite element model.
METHODS:
A 3D finite element model of cystic lesion of the mandibular body was constructed based on the CT images of the mandible of a healthy adult female volunteer with normal occlusion. The size of the cyst and the residual bone wall were analyzed when the lesion area approached the stress peak under certain constraints and loading conditions.
RESULTS:
When the size of the cyst reached 37.63 mm×11.32 mm×21.45 mm, the maximal von Mises stress in the lesion area reached 77.295 MPa, close to the yield strength of the mandible with a risk of pathological fracture. At this point, the remaining bone thickness of the buccal and lingual sides and the lower margin of the mandible in the lesion area was 1.52 mm, 0.76 mm and 1.04 mm, respectively.
CONCLUSIONS
Residual bone mass is an important factor to affect the risk of pathological fracture after curettage of cystic lesions. A thickness as low as 1 mm of the residual bone cortex in the cystic lesion area of the mandibular body can be used as the threshold for a clinical decision on one-stage windowing decompression combined with two- stage curettage.
