1.Acupuncture Therapy for Incomplete Rotator Cuff Tear

[Introduction] We evaluated the efficacy of treatment and clinical characteristics of a patient with incomplete rotator cuff tear with a clinical presentation similar to that of frozen shoulder. It seems difficult to differentiate incomplete rotator cuff tear from frozen shoulder on physical findings alone since the former lacks the specific characteristics of rotator cuff tear. However, many patients with incomplete rotator cuff tears show a poor response to treatment due to mechanical disorders; thus, differentiation of incomplete rotator cuff tear from frozen shoulder may be demonstrated during the patient's clinical course.
[Patient] In this patient, the joint range of motion improved in all directions, and the pain score also improved from 10 to 5, showing reduction of pain. However, the symptoms did not resolve smoothly; frequent remis-sions and exacerbations of symptoms were observed.
[Discussion] We frequently encounter disorders diagnosed as periarthritis scapulohumeralis in daily clinical practice, and patients with such disorders usually respond well to treatment. However, we should realize that a variety of clinical conditions are involved in the disorders, and the clinical courses of patients should be followed closely during treatment.

In concequence of the rapid extension of the average life expectancy of the Japanese after World War II, this country has realized a society of longevity. Although longevity is in itself something to be celebrated, many people have misgivings about health in their old age. Advances in medical technology, one of the factors which promoted the society of longevity, continued overcoming various diseases while bringing about many patients troubled with after effects. Also the number of old people suffered from chronic diseases has been increasing gradually.Today, people wish healthy long life from the heart, not simply longevity. Nagano Prefecture has come to be known as the area eminent for long healthy life expectancy in recent years. Behined this, there are various activities aimed at healthier life by citizens. In this paper, we report one of the activities carried out at “Yachiho village” (as a result of the affiliation with Saku machi in 2005 renamed Sakuho machi) located in the eastern part of Nagano Prefecture.The health care program for all villagers, which the government of Yachiho village started in 1959 with cooperation of Saku Central Hospital, produced fruit while breeding many voluntary activities by citizens. We explored the practices of the group worked on the improvement of nutrition, and studied about the essential conditions which have enabled the formation of health promotion activities by citizens.

3.18F-FDG PET/CT for Diagnosis of Osteosclerotic and Osteolytic Vertebral Metastatic Lesions: Comparison with Bone Scintigraphy.

STUDY DESIGN: A retrospective study. PURPOSE: The aims of this study were to investigate the diagnostic value of 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) in PET/computed tomography (CT) in the evaluation of spinal metastatic lesions. OVERVIEW OF LITERATURE: Recent studies described limitations regarding how many lesions with abnormal 18F-FDG PET findings in the bone show corresponding morphologic abnormalities. METHODS: The subjects for this retrospective study were 227 patients with primary malignant tumors, who were suspected of having spinal metastases. They underwent combined whole-body 18F-FDG PET/CT scanning for evaluation of known neoplasms in the whole spine. 99mTc-methylene diphosphonate bone scan was performed within 2 weeks following PET/CT examinations. The final diagnosis of spinal metastasis was established by histopathological examination regarding bone biopsy or magnetic resonance imaging (MRI) findings, and follow-up MRI, CT and 18F-FDG PET for extensively wide lesions with subsequent progression. RESULTS: From a total of 504 spinal lesions in 227 patients, 224 lesions showed discordant image findings. For 122 metastatic lesions with confirmed diagnosis, the sensitivity/specificity of bone scan and FDG PET were 84%/21% and 89%/76%, respectively. In 102 true-positive metastatic lesions, the bone scan depicted predominantly osteosclerotic changes in 36% and osteolytic changes in 19%. In 109 true-positive lesions of FDG PET, osteolytic changes were depicted predominantly in 38% while osteosclerotic changes were portrayed in 15%. CONCLUSIONS: 18F-FDG PET in PET/CT could be used as a substitute for bone scan in the evaluation of spinal metastasis, especially for patients with spinal osteolytic lesions on CT.
