1.Comparison of magnetic attachment and ball-cap attachment for retained mandibular over-denture

BACKGROUND:Precision attachment has been widely used in complete denture because of its strong retention, good appearance, comfort and durability. Among them, magnetic attachment and bal-cap attachment are commonly applied for mandibular complete denture.
OBJECTIVE:To compare the clinical effects of magnetic attachment, bal-cap attachment and traditional clasp retainer used in the mandibular removable partial denture.
METHODS:A total of 45 patients with dentition defects were included in this study, they were treated with magnetic attachment, bal-cap attachment and traditional clasp retention of mandibular removable partial denture. Fifteen patients received one means. The satisfaction of patients and the periodontal health of abutments were observed after 3 years.
RESULTS AND CONCLUSION:Comparison of the patient satisfaction showed that, the aesthetics, retention conditions, masticatory function and comfort in magnetic attachment group were significantly better than traditional clasp retention group (P<0.05). The aesthetics and comfort in bal-cap attachment group were also better than traditional clasp retention group (P<0.05). There were significant differences on the retention conditions and comfort between magnetic attachment group and bal-cap attachment group (P<0.05). Gingival index was the highest in magnetic attachment group, then in bal-cap attachment group, and the lowest in traditional clasp retention group (P<0.05). The abutment tooth mobility degree was the lowest in magnetic attachment group, then in bal-cap attachment group, and the highest in traditional clasp retention group (P<0.05). The periodontal pocket depth in magnetic attachment group and bal-cap attachment group was lower than that in traditional clasp retention group (P<0.05). The alveolar bone height was the highest in magnetic attachment group, then in bal-cap attachment group, and the lowest in traditional clasp retention group (P<0.05). Experimental findings indicate that, precision attachment over-denture retention superior to traditional clasp removable partial denture in mandibular denture repairing. Magnetic attachment can provide better retention power and better protect the health of periodontal tissue compared with bal-cap attachment.
2. The effect of pre-freezing method on crystallinity of alanylglutamine

OBJECTIVE: To research and regulate the crystal quality of alanylglutamine for improving their stability and pharmacodynamics as well as vacuum freeze-drying efficiency. METHODS: Scanning electron microscopy (SEM), X-ray powder diffractometer (XRD), differential scanning calorimetry (DSC), and thermogravimetric analyzer (TG) as well as lyophilized experiments were used to investigate the effect of pre-freeze method on crystallinity and uniformity of particle size of alanylglutamine. RESULTS: The result of SEM revealed that the lyophilized alanylglutamine with accelerated pre-freeze method is small-size particle with low crystallinity. Then the lyophilized alanylglutamine with multi-step annealing pre-freeze method become uniformly large particle with high crystallinity. The RESULTS of XRD and DSC indicted that, comparing with the lyophilized alanylglutamine with accelerated pre-freeze method, the lyophilized sample with multi-step annealing pre-freeze method is easier to form the crystal medicine with uniform particle size, less crystal defects, high crystallinity. And the former has lower stability than the later. CONCLUSION: The final lyophilized experiment give us a CONCLUSION that using the multi-step annealing pre-freeze method can improve the freeze-drying efficiency and crystal quality of alanylglutamine, which to achieve the ultimate goal of cost and energy saving.
3.Hyaluronic acid for the treatment of chronic periodontitis can effectively promote periodontal tissue regeneration

BACKGROUND:Hyaluronic acid has antioxidant, immune regulation, anti-inflammatory roles as wel as promotes wound healing. Hyaluronic acid is more and more applied in the treatment of chronic periodontitis and achieves a significant effect. OBJECTIVE: To observe the clinical effects of local application of sodium hyaluronate in the treatment of chronic periodontitis. METHODS:Sixty cases of chronic periodontitis (300 teeth) were selected. According to self-control method, the teeth in district A, 150 premolars or molars, were as experimental group, and other 150 teeth in district B served as control group. In the experimental group, periodontal scaling, subgingival scaling, and local application of Gengigel gel (0.2% hyaluronic acid) in the gingival sulcus were employed; in the control group, the treatment was same to that in the experimental group except local application of Gengigel gel. During 8-week folow-up, dental plaque index, sulcus bleeding index, probing depth, and clinical attachment loss were changed dynamicaly. RESULTS AND CONCLUSION:After treatment, al the measurement indexes were improved remarkably in the two groups. However, there was no difference in the dental plaque index between the two groups at 1, 4, 8 weeks after treatment. The sulcus bleeding index was lower in the experimental group than the control group at 1, 4, 8 weeks after treatment (P < 0.05). The periodontal probing depth of the experimental group was lower than that of the control group at 4 and 8 weeks (P < 0.05). The clinical attachment loss was lower in the experimental group than the control group at 8 weeks (P < 0.05). These findings indicate that periodontal treatment with local application of sodium hyaluronate in the gingival sulcus is significantly effective for the treatment of chronic periodontitis.
4.The construction of conceptual model for community-dwelling elderly people's fatigue experience and its use value

Objective To investigate the fatigue experience of the community-dwelling frail elderly to establish the conceptual model.Methods Grounded theory was adopted.With MAXQDA v10.4.15.1for 3 levels coding,the conceptual model of fatigue experience of community-dwelling frail elderly people was structured through constant comparison and in-depth analysis.Results Multi-response,selfmanagement,safe and belonging,hitting rock bottom,accepting and transferring formed the conceptual model.Conclusions Based on the conceptual model,nurses should pay more attention to concern about the fatigue experience of community-dwelling frail elderly people,and probe different assessment methods to enrich comprehensive geriatric assessment (CGA),then improve the community health service level and the quality of elderly people's life.
5.Clinical comparison of different materials as posterior dental implants

BACKGROUND:Studies have shown that selection of implant crowns that directly impact the service life of implants and patient’s periodontal health is very important.
OBJECTIVE:To compare the clinical effects of Lava zirconia crowns, goal-platinum al oy ceramic crowns and silver-pal adium al oy crowns supplemented by dental implants used in the single missing posterior tooth.
METHODS:Sixty patients with first molar missing (120 teeth) were divided into three groups and treated with Lava zirconia, goal-platinum al oy ceramic and silver-pal adium al oy crowns applied to the dental implants for posterior column defects. The clinical restorative effects of three prostheses were compared.
RESULTS AND CONCLUSION:During 6-48 months of fol ow-up, the gingival edge coloring, gingival margin microleakage, the color of prosthesis of Lava zirconia al oy ceramic crowns and gold-platinum al oy ceramic crowns were better than those of silver-pal adium al oy crowns, and the gingival edge coloring and the color of prosthesis of Lava zirconia al oy ceramic crowns were better than those of gold-platinum al oy ceramic crowns, while the gingival margin microleakage of gold-platinum al oy ceramic crowns was better than that of Lava zirconia al oy ceramic crowns. Silver-pal adium al oy crowns were lower than Lava zirconia ceramic crowns and gold-platinum al oy ceramic crowns in the fracture extent. But silver-pal adium al oy crowns had the highest gingival index, the worst gingival health and rapidest and most severe plaque formation. Therefore, Lava zirconia al-ceramic crowns and gold platinum al oy-porcelain crown are ideal prostheses for implant restorations. The former has better biocompatibility, and the latter shows better marginal adaptation.
6.Sesquiterpenoids from Solanum lyratum.

Ten compounds were isolated and purified by column chromatography over silica gel, preparative TLC, and Sephadex LH-20 from the whole plant of Solanum lyratum. The structures were elucidated on the basis of physico-chemical properties and spectral data as 1beta-hydroxy-1 ,2-dihydro-alpha-santonin (1) , boscialin (2) , blumenol C (3), 3beta-hydroxy-5alpha, 6alpha-epoxy-7-megastigmen-9-one(4), dehydrovomifoliol(5) , blumenol A(6), (1'S,2R,5S, 10R) -2-(1', 2'-dihydroxy-l1'-methylethyl) -6,10-dimethylspiro[4,5] dec-6-en-8-one(7) , (1'R,2R,5S,10R)-2-( 1',2'-dihydroxy-l '-methylethyl) -6,1 l0-dimethylspiro[4,5]dec-6-en-8-one( 8) , 2-(1',2'-dihydroxy-1 '-methylethyl) -6,1 0-dimethyl-9-hydroxyspiro [4,5] dec-6-en-8-one (9) , and grasshopper ketone (10). Compounds 1-10 were isolated from this plant for the first time.

7.Clinical analysis of atypical pulmonary embolism

Objective To analyze the common reasons for misdiagnosis of atypical pulmonary embolism (APE) ,and to im‐prove the identification of APE .Methods The risk factors ,clinical manifestations ,laboratory examinations and radiographic data of 120 cases of APE diagnosed from January 2006 to December 2013 in the department of cardiovascular medicine and respiratory medicine of Xinqiao Hospital and the Affiliated Hospital of Luzhou Medical College were studied retrospectively .Results Among those 120 cases of APE ,39 cases were misdiagnosed on admission (32 .5% ) .8 cases were misdiagnosed as acute coronary syn‐drome ,7 cases as stable angina pectoris ,7 cases as chronic cor pulmonale ,5 cases as pneumonia ,3 cases as pleural effusion ,3 cases as tuberculosis ,3 cases as asthma ,1 case as atrial septal defect ,1 case as acute heart failure ,and 1 case as cardiogenic syncope .Con‐clusion APE is easy to be misdiagnosed for its non‐specific clinical manifestation .Pulmonary enhanced CT or CTPA should be car‐ried out in time for those highly suspected patients ,in order to reduce the misdiagnosis of APE .
8.Follow-up study on pedicle fixation for treatment of thoracolumbar burst fractures in patients with osteoporosis

Objective To explore the effect of pedicle fixation for treatment of thoracolumbar burst fractures in patients with osteoporosis,and to provide more evidence for the treatment.Methods Retrospectively analyzed the clinical data of 121 patients with osteoporotic vertebral burst fracture from June 2012 to October 2015.And these patients were divided into two groups according to different operation methods, namely the control group (n=56) who were given short segment fixation and the observation group (n=65) who were given single segment fixation.The visual analogue scale(VAS),Oswestry disability index(ODI),vertebral height,kyphotic angle and bone mineral density of the two groups were analyzed before surgery and 3 days,1 month,3 months and 12 months after surgery.Results The VAS score,ODI score,vertebral height,and Cobb angle of the injured vertebra were significantly improved in both of the two groups,and the difference was statistically significant (P<0.05).The VAS score of the observation group was better than that of the control group 3 days after surgery with statistically significant difference (P<0.05).But there was no significant difference 3 months,6 months and 12 months after surgery(P>0.05).The ODI score of the observation group was better than that of the control group 3 days and 3 months after surgery with statistically significant difference (P<0.05),and there was no significant difference between the two groups till the end of follow-up.Pedicle fixation at the injured vertebra significantly improved the vertebral height and Cobb angle with statistically significant difference (P<0.05).And the anti-osteoporosis treatment significantly increased the bone mineral density (P<0.05).Conclusion Pedicle fixation at the injured vertebra is useful in pain relief as well as function and anatomical structure restoring.And anti-osteoporosis treatment is necessary for the bone mineral density increase.
9.Effect of asymptomatic hyperuricemia on the postoperative functional recovery in patients with knee joint re-placement

Objective To evaluate the effect of asymptomatic hyperuricemia on the postoperative functional recovery in osteoarthritis (OA)patients with knee joint replacement.Methods Among the 294 patients who recieved knee joint replacement in our hospital from Feb-ruary 2013 to September 2015,a total of 187 patients were included in this study with 79 cases in the hyperuricemia group and the other 108 cases in the control group.The WOMAC index of the two groups were analyzed before the surgery,2 weeks,3 months,and 6 months after the knee joint replacement surgery.Multivariable linear regression was performed to test the effect of impact factors on the variation of WOMAC index.Results The WOMAC index of the two groups significantly increased 2 weeks after surgery,and the difference was statistically signifi-cant(P <0.05).And the WOMAC index of the control group was higher than that in the hyperuricemia group 2 weeks and 3 months after sur-gery with statistically significant difference(P <0.05).Conclusion The uric acid level before surgery was a influence factor of WOMAC index, and high uric acid level before knee joint replacement has negatively impact for the postoperative functional recovery in the osteoarthritis patients.
10.Effect of electro-acupuncture at Neiguan (PC6) and Lieque (LU7) on the expression of protein kinases in cardiomyocytes of myocardial ischemia rats.

OBJECTIVETo study the effect of electro-acupuncture (EA) at Neiguan (PC6) and Lieque (LU7) on the expression of protein kinases in cardiomyocytes of myocardial ischemia (MI) rats.
METHODSHealthy male SD rats were randomly divided into the control group, the model group, the Neiguan point group, the Lieque point group, and the non-meridian non-acupoint group, 10 in each group by random digit table. The MI rat model was established by injecting isoprenaline hydrochloride (85 mg/kg). EA at Neiguan (PC6), Lieque (LU7), and non-meridian non-acupoint were respectively performed. Changes of the expression of protein kinases [such as protein kinase A (PKA), protein kinase C (PKC), protein kinase G (PKG)] in rat cardiomyocytes were observed using Western blot.
RESULTSCompared with the control group, expression levels of PKA, PKC, and PKG increased obviously in the model group (P < 0.01). Compared with the model group, expression levels of PKA, PKC, and PKG decreased in the Neiguan point group and the Lieque point group (P < 0.01, P < 0.05). Expression levels of PKA decreased in the non-meridian non-acupoint group (P < 0.01). Compared with the Neiguan point group, expression levels of PKA, PKC, and PKG increased in the non-meridian non-acupoint group and the Lieque point group (P < 0.01, P < 0.05). Compared with the Lieque point group, expression levels of PKA, PKC, and PKG increased in the non-meridian non-acupoint group (P < 0.01, P < 0.05).
CONCLUSIONEA at Neiguan (PC6) and Lieque (LU7) could decrease protein expression levels of PKA, PKC and PKG in rat myocardial cells, and the effect of acupuncture at Neiguan (PC6) was better than that obtained by EA at Lieque (LU7).
