Objective To summarize the experiences on the diagnosis of pancreatic trauma and to explore the strategy of its management. Methods The clinical data of 48 patients with pancreatic trauma, who were admitted and treated in the Center for General Surgery,General Hospital of Chengdu Command of PLA from June 1998 to June 2008,were retrospectively studied, and the diagnosis, surgical management, and the therapeutic effects were analyzed. Results Of the 48 patients,32 patients(66.7%) were diagnosed as pancreatic injury before operation, and in 16(33.3%) the final diagnosis was confirmed intraoperatively. For these 48 patients, the degree of pancreatic injury was graded according to AAST-OIS as follows:Grade Ⅰ 9 cases,Grade Ⅱ 28 cases,Grade Ⅲ 5 cases,Grade Ⅳ 4 cases and Grade Ⅴ 2 cases.Of the 48 patients,44(91.7%) recovered and 4(8.3%) died.Early complications occurred in 2 cases(4.2%),one with traumatic pancreatitis and another subphrenic Abscess.Late complication such as pancreatic secretion insufficiency occurred in 3 patients(6.3%) in whom resection of distal portion of pancreas was done.7 patients(14.6%) had postoperative complications, including 2 cases of pancreatic pseudocyst,4 cases of pancreatic fistula and 1 case of pancreatic fistula combined with intestinal fistula. Conclusions The early diagnosis of pancreatic trauma should be based on a comprehensive patient history and scrupulous physical examination, aided by auxiliary examinations including serum amylase level, ultrasound,CT scanning, endoscopic retrograde cholangio-pancreatography(ERCP) or magnetic resonance cholangio-pancreatography(MRCP),and laparotomy if necessary.Pancreatic fistula is the main complication following pancreatic trauma and often occurs in the patients with severe pancreatic injury.Aggressive operation on the pancreas is not recommended, while correct judgement of injury to the main duct,a good debridement, effective drainage and flawless pancreaticoenterostomy are the key factors to avoid postoperative fistula.

Microbiology 2008;0(07):-
Molecular methods and fluoroscopic techniques suggest that rich microbial diversity exist in the marine environment, but less than 1% of these microbes can be cultured in the laboratory conditions, and that the cultivable dominant species were even less. This limitation has long been a barrier to the development of environmental microbiology and the utilization of marine resources. In the past decade, novel methods for culture and detection of these uncultured marine microbes have successfully applied to obtain several conventionally-uncultured microbes including those from extreme environments. Those progresses have inspired researchers greatly. Developments in the research of marine microbial resources are an important basis for the study of the micro-world and deserve increasing scientific attention.

OBJECTIVETo observe the effect of Yiqi Huoxue Qingre Huashi Recipe (YHQHR, a recipe capable of supplementing qi, activating blood, clearing heat, and dissipating dampness) on ulcer healing and Helicobacter pylori (Hp) eradication rate in Hp positive peptic ulcer patients, and to explore coccoid Hp occurrence in the eradication.
METHODSTotally 80 Hp positive peptic ulcer patients were assigned to the treatment group and the control groups by random digit table, 40 in each group. All patients received standard triple therapy of Western medicine for 2 successive weeks. Those in the control group additionally took omeprazole enteric coated tablet, 20 mg each time, once per day for 4 successive weeks. Those in the treatment group additionally took YHQHR, twice per day for 6 successive weeks. The ulcer healing was observed and recorded by gastroscope after discontinued medication of 14 days. The effective rate of ulcer healing under endoscope was statistically calculated. Rapid urease test (RUT) was performed in one small piece of tissue from corpora ventriculi and sinuses ventriculi using 14C breathe test (UBT). Gastric juice was collected from the stomach. Hp urease gene amplification test (urea A-PCR) was performed in living tissue from gastric antrum. Results obtained from the above three test methods were recorded and assessed to decide the final eradiation rate. Gastric mucosa tissue was observed under electron microscope, attempting to find non-eradicated Hp, which was further observed.
RESULTSThe total curative effect under gastroscope was 97.5% (39/40 cases) in the treatment group, obviously higher than that in the control group (80.0%, 32/40 cases) (P < 0.05). The eradication rate of Hp was 75.0% (30/40 cases), obviously better than that of the control group (52.5%, 21/40 cases) (P < 0.05). The total positive Hp numbers after treatment was 14C UBT (12), RUT (8), and urea A-PCR (27), respectively. The Hp positive rate detected by 14C UBT and RUT was lower than the Hp positive rate detected by urea A-PCR (P < 0.05). Rod-like and coccoid Hp bacteria could be observed under electron microscope.
CONCLUSIONYHQHR combined standard triple therapy was more effective than standard triple therapy alone in promoting ulcer healing and elevating the eradication rate of Hp.

The selective side-chain cleavage of phytosterol to 4-androstene-3,17-dione(4-AD)and 1,4-androstadiene-3,17-dione(ADD)by Mycobacterium sp.was described.Because of the similarity in chemical structure between 4-AD and ADD,it is difficult to separate them from the fermentation broth. So far,it has been verified that the ADD can be produced by dehydrogenation of 4-AD.In this reaction,3-Ketosteriod-1-Dehydrogenase(ksdD)plays an important role. The gene knocking out method was used to solve the problem.Partial sequence of ksdD was obtained by PCR which was 631bp in length. Then,a targeting vector pUC19-MK was constructed, which was electroporate into the original strain Mycobacterium neoauru. The method of homologous recombination was used to knock out ksdD gene located in the chromosome of Mycobacterium neoauru. In this way, ksdD would lose its enzyme activity. In the result,5 transformants were screened. The experiments of steroid transformation by the transformants were carried out.The productivity of 4-AD reached 17.52% after 144h,which is 192% higher than the original strain.Meanwhile, the productivity of ADD reached 6.12%,which is 89.9% lower than the original strain.

OBJECTIVETo study the relations between different feeding patterns and the body weight retention of the perinatal women living in rural areas of China.
METHODSA cluster sampling method was used to investigate 409 women, who are currently living in rural areas of Tianjin, at pregnant and perinatal status. While, their body weights and heights before pregnancy, antepartum and postpartum were measured, respectively. Body weight retention was the difference of the measured data after postpartum minus pre-pregnant weight. Variance analysis was used for statistic comparison.
RESULTSThe rate of exclusive breastfeeding was 70.9% (290/409) within four months. The net body weight retention of women (5.8 kg) using the exclusive breastfeeding was lower than that of the women (7.0 kg) using artificial feeding within 4 - 6 months, but there was no significantly statistic difference (F = 1.45, P = 0.236). However, there was the opposite result within 7 - 9 months, the data showed that the body weight retention in the women using the exclusive breastfeeding was 4.9 kg, which was significantly higher than that the women (2.9 kg) with artificial feeding (F = 3.17, P = 0.043). The food consumption of the women (901 g) using exclusive breastfeeding was the highest, followed by those (877 g) using mixed feeding and the women (750 g) using artificial feeding.
CONCLUSIONThe body weight retention after postpartum should be related to infant feeding patterns. After postpartum, the weight loss of women using the exclusive breastfeeding is relatively low. While, for the women using the exclusive breastfeeding, the net weight retention during pregnancy and after postpartum were lower than those with artificial feeding. Therefore, it is necessary to enhance health education and guidance on promoting exclusive breast-feeding as well as increasing awareness on pre-pregnant health.

OBJECTIVETo investigate the anti-hyperglycemic effect and its mechanism of ethanol extraction from Calamintha chinensis (EJCT).
METHODFasting serum glucose (FSG) in normal mice was determined after oral administration of EJCT. Effects of EJCT on hyperglycemia mice induced by adrenaline were investigated by observing the contents of FSG and liver glucogen. Effect of EJCT on the diabetic mice induced by alloxan was investigated by observing the contents of FSG and the injured degree of pancreatic islet. The antilipid-peroxidation of EJCT on liver homogenate was measured by determination of malondiadehyde (MDA) induced by Fe2+/Cys.
RESULTEJCT showed no obvious effect on FSG in normal mice. However, EJCT 300, 600 mg x kg(-1) could remarkably decrease the contents of FSG and increase liver glucogen in hyperglycemia mice induced by adrenaline. In diabetic mice induced by alloxan, EJCT 150, 300, 600 mg x kg(-1) could remarkably decrease the contents of FSG. The damage of pancreatic islet induced by alloxan was also significantly attenuated by EJCT. Furthermore, EJCT 30, 60, 90, 120 mg x L(-1) inhibited lipid peroxidation initiated by Fe2+/Cys in liver homogenate.
CONCLUSIONThese results suggest that EJCT can significantly attenuate hyperglycemia in diabetic mice, which is probably due to decreasing the decomposition of liver glucogen, increasing the synthesis of liver glucogen, antioxidation and amelioration of damaged pancreatic islet.

Chinese Journal of Experimental Ophthalmology 2017;35(11):1014-1018
Background Giant idiopathic macular hole (IMH) severely affects visual acuity and increases operative difficulty during the surgery,so modifying or optimizing the operation method is helpful for improving the prognosis. Objective This study aimed to evaluate the efficacy and safty of vitrectomy combined with free internal limiting membrane transplantation for large IMH.Methods A prospective serial cases-observational study was carried out under the informed consent of each patient.Forty-two eyes of consecutive 42 patients with IMH of mean diameter (814.31±112.95)μm were included in the Second Hospital of Hebei Medical University from January 2013 to November 2015.All the eyes received vitrectomy combined with free internal limiting membrane transplantation and 12% C3F8 filling. The best corrected visual acuity (BCVA) (LogMAR),inner segment/outer segment (IS/OS) defect range, external limiting membrane defect range, retinal thickness at macular fovea were measured with slit lamp microscope, indirect ophthalmoscope and spectral-domain optical coherence tomography (SD-OCT) before surgery and 1,3,6 and 12 months after surgery. Results IMH complete closure in 97.6% eyes (41/44) at 12 months after surgery.BCVA was improved after operation and showed a significant difference among various time points (F =28.032,P<0.001).The IS/OS defect range was (1 112.00±45.44),(859.00±84.55),(649.00±52.47),(486.00±46.88) and (320.00±45.13) μm before surgery and 1,3,6 and 12 months after surgery, showing a significant difference among different time points (F=38.761,P<0.001),and the IS/OS defect range was gradually shrinked after operation compared with that before operation (all at P<0.05).The mean defect range of external limiting membrane was (1 038.00 ±39.63),(748.00±64.12),(585.00±48.88),(438.00±42.84) and (265.00±28.97)μm before surgery and 1,3,6 and 12 months after surgery, with a significant difference among various time points (F=36.459,P<0.001),and the mean defect range of external limiting membrane was evidently reduced (all at P<0.05).The foveal retinal thickness value was increased at 3,6 and 12 months after surgery, which was significantly higher than that 1 month after surgery (all at P<0.05).The high reflect signal was faded away at 3 months after operation, indicating that implanted internal limiting membrane was decomposed and metabolized. Conclusions Vitrectomy combined with autologous internal limiting membrane transplantation seems to be safe and effective for large IMH.
