1.Effects of pramipexole on parkinson's disease and serum CRP, Cystatin C, BDNF and oxidative stress

Objective To study curative efficacy of Pramipexole in treatment of Parkinson's Disease and its effects on Serum C-reactionprotein (CRP), cystatin C, brain derived neurotrophic factor(BDNF) and oxidative stress.Methods 100 patients of Parkinson's Disease who received therapy from February 2014 to April 2016 in our hospital were selected as research objects were randomLy divided into control group and observation group, 50 patients in each group.The control group was treated with Dobasidate tablets;The observation group was given pramipexole tablets on the basis of the control group.Then the clinical effects and adverse reactions were compared.CRP, Cystatin C, BDNF and oxidative stress were measured. Results After treatment, the total effective rate of the observation group [(90.00% (45/50)] was significantly higher than the control group[74.00%(37/50)] (P<0.05);The symptoms of palpitation, depression, nausea, anorexia, diarrhea and vomiting were lower in observation group than in observation group, and the differences of depression, anorexia and vomiting were statistically significant(P<0.05);The BDNF index of the observation group (13.38 ±1.93)ng/L was higher than that of the control group (12.03 ±1.83) ng/L(P<0.05);CRP (3.53 ±0.97) mg/L and Cystatin C index (0.98 ±0.12) mg/L were lower than the control group(4.62 ±1.15)、(1.06 ±0.14) mg/L (P<0.05).After treatment, the glutathione peroxidate (GPX),superroxide dismutase(SOD),glutathione(GSH)and catalase(CAT) of oxidative stress in observation group were higher than those in control group,the difference was statistically significant (P<0.05).Conclusion Pramipexole in the treatment of Parkinson's disease is obvious, can effectively improve the serum CRP, Cystatin C and BDNF index, high security, it is worth promoting the application.

Cough is one of common respiratory symptoms in pediatric pulmonology.But there′s no practical pediatric guideline for diagnosis and management in particular an evidence-based one.This article introduces American College of Chest Physicians(ACCP) evidence-based clinical practice guidelines for evaluation chronic cough in pediatrics from which can be borrowed ideas.The introduction of ACCP guidelines for evaluation chronic cough in pediatric provided modified naming to previous diseases,discussed etiology of cough caused by different diseases and drew detailed flow charts for chronic cough diagnosis and management.
4.Effect of 5 -aminolevulinic acid mediated photodynamic therapy in combination with recombinant hu-man interferon alpha 2b ointment on basal cell carcinoma in elderly

Objective To evaluate the therapeutic effect of 5-aminolevulinic acid mediated photody-namic therapy(ALA-PDT)in combination with recombinant human interferon alpha 2b ointment on superficial basal cell carcinoma(BCC)in elderly.Methods A total of 143 elderly patients with superficial BCC was ran-domly divided into PDT group(n=72 cases)and the surgical group(n=71 cases).The patients in the two groups were given the ALA-PDT in combination with IFN-α2b and routine surgical treatment respectively .The effica-cy after treatment was observed according to surgical wound area , lesion area , recurrence and security during 1 year follow-up.Results The results displayed the complete remission rate of lesion in the PDT group was sig-nificantly higher than that in the surgical group (81.94%vs.57.75%,P<0.05)after treatment,and the total ef-fective rate of BCC was also higher than that in the surgical group ,but no significant difference was identified in the two groups(91.66%vs.81.73%,P>0.05).During 1 year follow-up after treatment,the results showed the rate of recurrence in the PDT group was significantly lower than that in the surgical group (9.72% vs.22. 54%,P<0.05).Moreover,the mean wound area,the lesion area in the PDT group after treatment were also sig-nificantly smaller than that in the surgical group (P<0.05).Conclusion ALA-PDT in combination with IFN is an effective method with lower recurrence and minor trauma in the treatment of superficial basal cell carcino -ma.
5.The effect of topiramate on the treatment of epilepsy

Objective To assess the clinical efficacy and safety of topiramate in adult and children patients with epilepsy. Methods Open-label experience with topiramate as adjunctive, added-on to monotherapy and monotherapy were analyzed.The efficacy was assessed by comparing the average frequency per month for 3 months after topiramate added to the maintenance period of the baseline therapy. Results Topiramate was proven to be a valuable new antiepileptic drug either in monotherapy (86%) or in added-on therapy (70%) on partial onset epilepsy, West syndrome and Lennox-Gastaut syndrome, and showed no tolerance.The topiramate dosage in patients with monotherapy [(85.87?29.19)mg/d in adult] is lower than in those using added-on therapy [(161.11?58.18)mg/d] significantly, and the most common adverse events were CNS-related, but few patients had discontinued the therapy.Conclusions The topiramate used for monotherapy or added-on therapy should be regarded as an effective and widespread antiepileptic drug, without tolerance and severe adverse actions.
6.Feasibility and safety of dexmedetomidine used in motor evoked potentials monitoring in patients under-going neurosurgery

Objective To observe the feasibility and safety of dexmedetomidine used in motor evoked potentials(MEP)monitoring in patients undergoing neurosurgery.Methods Thirty ASA Ⅰ orⅡ patients,male 1 5 cases,female 1 5 cases,aged 20-60 years,weighing 40-80 kg undergoing neuro-surgery receiving MEP monitoring were randomly divided into 2 groups (n =1 5 each):control group (group C)and dexmedetomidine group (group D).In group D,dexmedetomidine 0.5 μg/kg was in-fused over 10 minutes before anesthesia induction,and then was infused at a rate of 0.5 μg·kg-1 · h-1 toward the end of operation.Group C received the equal volume of normal saline.HR,MAP and BIS were recorded at admission to the operating room (T0 ),skin incision (T1 ),when the muscle re-laxants were stopped (T2 )and 50 minutes later (T3 ).The current intensity and the time when first MEP was induced after muscle relaxant was stopped,the amplitudes and latencies of MEP on thenar muscle at T3 ,the total consumption of anesthetics,and development of adverse effects were also re-corded.Results Compared with T0 ,HR in group C at T1 ,T3 and MAP in group C at T1-T3 was in-creased,HR in group D was decreased at T2-T3 (P <0.05).Compared with group C,HR and MAP were decreased at T1-T3 in group D(P <0.05).The amount of propofol consumed and the current in-tensity inducing MEP were lower in group D than in group C (P <0.05).The amplitude of MEP at T3 was higher in group D than in group C (P <0.05).Compared with group C,the incidences of hy-pertension and tachycardia were decreased in group D,and the incidence of bradycardia was increased (P <0.05).Conclusion Dexmedetomidine used in MEP monitoring in patients undergoing neurosur-gery can meet the operation requirements,maintain hemodynamic stability,reduce the incidences of adverse reactions,and improve monitoring quality of MEP.It is a safe and feasible anesthesia method.

To determine the content of astragaloside IV in Xiao' er Weijining Mixt ure, TCL conditions were optimized by comparing various refined methods. And the y were as follows: the underlayer of CHCl3-CH3OH-H2O(65∶35∶10) below 10 ℃was developed at 15~18℃ and sprayed with 10%H2SO4-C2H5OH,the heating tempe rature was 105℃ for 3~5 min,and λS=400 nm, λR =700 nm.

9.Analysis the relationship between SLC26A4 mutation and current diagnosis of inner ear malformation in children with sensorineural hearing loss.

OBJECTIVE:
Explore the relationship between the pathogenic mutations of SLC26A4 gene and inner ear malformation, and analyze the feasibility of genetic testing to help current diagnosis in part of children with sensorineural hearing loss.
METHOD:
2094 cases of children were detected by SLC26A4 with the method of DNA sequence. CT phenotypes of those children were classified according to the method proposed by Sennaroglu. We analyzed the relationship between the pathogenic mutations of gene and the CT phenotypes.
RESULT:
(1) 685 cases of inner ear malformations were found in 2094 cases of children with sensorineural hearing loss by CT examination (371 cases of cochlea malformation were consisted of the follow types of malformation. Michel deformity was 6 cases, cochlea aplasia was 8 cases, common cavity deformity was 12 cases, incomplete partition type I was 27 cases, cochlea hypoplasia was 30 cases and Mondini malformation was 288 cases); Vestibular aqueduct was 265 cases; Vestibular/semicircular canal/internal auditory canal were 49 cases, normal was 1409 cases. (2) The DNA sequence results revealed that 465 cases carried pathogenic mutations (Bi-allelic mutations) of SLC26A4 gene, among which 135 cases were homozygous, 330 cases were compound heterozygous. (3) Pathogenic mutations of SLC26A4 gene detected 100% (465/465) in the group related to vestibular aqueduct malformation.
CONCLUSION
The results suggest that pathogenic mutation of SLC26A4 gene is closely related to the CT phenotype of vestibular aqueduct malformation. Detecting of pathogenic mutations for hearing loss is binging the possibility to identify children with inner malformations at an early stage. As a consequence, it will improve the current diagnosis and therapeutical option.

10.Study on 2,747 cases of inner ear malformation for its classification in patient with sensorineural hearing loss.

OBJECTIVE:
Analyze the data of the patients with sensorineural hearing loss in China and study the classification and incidence of inner ear malformationsby the high-resolution computed tomography.
METHOD:
The investigation took a retrospective review of CT findings relating to the 2,747 cases of outpatients. The inner ear malformations diagnosed by CT were classified according to the methods proposed by Sennaroglu.
RESULT:
(1)843 cases of inner ear malformations were found in 2747 cases of patients with sensorineural hearing loss by CT examination. The incidence of inner ear malformation was 30.69%(843/2747). (2) The epidemiological information of 843 cases of inner ear malformation according to Sennaroglu's classification was as follows: cochlea was 52. 31%(441/843), simple vestibular aqueduct was 40.33%(340/843), simple vestibular/ semicircular canal/internal auditory canal were 7. 35%(62/843) of the group. (3) 441 cases of cochlea malformation were consisted of these types of malformation: Michel deformity was 1.13% (5/441), cochlear aplasia was 1. 81% (8/441), common cavity deformity was 3. 17% (14/441), incomplete partition type I was 8. 62% (38/441), cochlea hypoplasia was 9. 07% (40/441) and incomplete partition type II was 76. 19% (336/441) of the group.
CONCLUSION
The results suggested that 30. 69% cases of inner ear malformation can be found in patients with sensorineural hearing loss, which is more higher than reported by the high-resolution computed tomography. Sennaroglu's classification is instructively significant in investigating the status of inner ear malformations.
