1.Hospital Services Utilization by Insured and Non-insured Patients for Cesarean Section in a University Hospital.

In order to discover difference that may exist in quantity of medical care services, length of stay and hospital changes between insured and non-insured patients, records for primary Cesarean section patients discharged between July 1978 and June 1980 from a university hospital were examined. In addition, Cesarean section rates among the total deliveries for a two-year period between the groups were studied. The results showed that volume of services was greater and length of stay was longer among the insured, however, charges were higher among the non-insured. Cesarean section rates were statistically significantly different between insured and non-insured patients for every age group except the group of 35 or more.

2.Two Cases of Angioma Serpiginosum.

Angioma serpiginosum is a rare vascular naevoid disorder due to progressive ectatic dilatation of capillaries in the papillary dermis. We report two cases of angioma serpiginosum. A 24-year-old woman presented with asymptomatic multiple deeply red macules arranged in a serpiginous pattern on the left posterior thigh. In the other case, an 8-year-old girl had asymptomatic red nonpalpable puncta that were grouped closely together in a linear pattern from the left lower abdomen to thigh. On the histopathological examination, both cases commonly showed grouped, dilated, ectatic vessels in the upper dermis. These clinical and histopathological findings were typical of angioma serpiginosum.

3.Subepidermal Calcified Nodule of the Buttock.

Subepidermal calcified nodule, a form of idiopathic calcinosis cutis, typically present as a hard, 3 to 11 mm, solitary lesion on the exposed area of the head and the extremities. Here we re-port an unusual case of subepidermal calcified nodule occurring on the buttock. A 15-year-old girl presented with a 6-month history of a 12 × 6 mm, oval, hard, erythematous nodule on the right buttock. Histopathologic examination of an excision biopsy specimen revealed extensive deposition of calcium in the whole dermis as large multilobulated masses.

4.A Case of Retroperitoneal Neurilemmoma.

Neurilemmoma is a relatively rare, which was first described by Stout in 1935. Neurilemmoma is a neoplasm arising from Schwann cells. It is usually solitary and may be benign or malignant, solid or cystic, encapsulated or diffuse. We report one case of retroperitoneal neurilemmoma with review of literature.

5.The Diagnostic Usefulness of Computed Tomography of Transitional Cell Carcinoma of the Renal Pelvis and Ureter.

We evaluated the usefulness of preoperative computed tomography (CT) in 26 patients with transitional cell carcinoma (TCC) of the renal pelvis or ureter. Also we had comparative analysis between CT and conventional urography in diagnosis. The conventional imaging methods such as excretory urography and retrograde pyelography are helpful in localizing the lesions but usually cannot detect extension into the peripelvic and/or periureteral fat or metastasis to the lymph nodes. Of 17 patients who were thought localized by CT, 16 were localized in the mucosa or muscle layer and 1 was invasive to periureteral or peripelvic tissue pathologically. Of 8 patients who were thought invasive to periureteral or peripelvic tissue by CT, 5 were identical and 3 were metastatic to lymph nodes pathologically In conclusion, preoperative CT is thought to be useful tool for the diagnosis and staging in TCC of the pelvis or ureter and the sensitivity and specificity of CT were higher in localized diseases than in advanced diseases.

6.Two Cases of Invasive Carcinomas Arising from Bowen's Disease.

Bowen's disease is a premalignant condition, which may evolve into invasive carcinoma. Most invasive carcinomas arising from Bowen's disease show squamous differentiation, but rarely basaloid, pilar, pilosebaceous or sebaceous differentiation. We herein describe two cases of invasive carcinoma arising from Bowen's disease. First case, a 78-year-old male, presented with an erythematous plaque with ulcerated and fungating changes on his back. Biopsy specimen revealed Bowen's disease transformed into invasive squamous cell carcinoma. Second one, a 67-year-old male who had been diagnosed histologically as Bowen's disease on the right inguinal area 5 months ago, complained of enlargement and infiltration of the previous lesion. Biopsy specimen showed invasive carcinomatous change of basosquamous differentiation. Our cases represent unusual invasive changes of Bowen's disease into squamous cell carcinoma and carcinoma of basosquamous differentiation.

7.Two Cases of Infantile Eosinophilic Pustular Folliculitis.

Eosinophilic pustular folliculitis(EPF) is a rare inflammatory follicular disorder of unknown etiology. EPF is characterized by recurrent clusters of pruritic, follicular, sterile papules and pustules on the seborrheic area of young men. Compared with classical adult type, infantile EPF reveals the distinctive clinical features, such as constant involvement of the scalp, self-healing recurrent crops, and the possibility of lasting until 3 years of age. We describe two patients with infantile EPF. Both patients commonly had recurrent crops of papules and pustules affecting the scalp only. Bacterial and fungal cultures were negative, and one patient showed an eosinophilia. Biopsy specimens revealed folliculitis with a predominant eosinophilic infiltrate. Clinical response to oral antihistamines and topical corticosteroids was noted, but the lesions have been recurrent during the long-term follow-up period of 1 year.

8.Ultrasonic differential diagnosis of stones and carcinomas in the extrahepatic biliary tree

Ultrasonography is a simple, noninvasive and relatively accurate method to differentiate carcinomas from stones in the extrahepatic biliary tree. We analysed ultrasonic findings in 18 stoen cases and 10 carcinoma cases of the extrahepatic biliary tree, which were confirmed by microscopic examination or percutaneous transhepatic cholangiography during the period of May 1980 through May 1982 in Kangnam St. Mary's Hospital. The results were as follows: 1. The number of incidences of multiple echogenecities was 11 out of the 18 stone cases (61.1%) but there were no incidences of multiple echogenecities among the carcinoma cases. Eight cases, four of stoens and carcinomas each , were not detected by sonography. 2. The average diameter of the stones was below 20mm in 13 of the 18 cases (72 .2%) and the number of the stoens over 20mm in diameter was only one of the 18(5.6%), while the number of carcinomas over 20mm was three of the 10 cases (30%). 3. In the 18 patients with stoens, the degree of echogenecity was uniformly high in seven (38.9%) and mixed-moderate in seven (38.9%) . All the carcinomas, however, presented mixed-slight echogenecity. The marginal sharpness was well defined in nine of the 18 stone cases (50%), but only one among the carclnomas. 4. The obstructive site in the biliary tree in the stone cases showed a typical meniscus in 10 (55 .6%), abrupt cutting in four (22_2%) and were indiscernible in four (22.2%). 9.Two Cases of Crossed Renal Ectopia with Fusion.

Scar endometriosis is one of the rare conditions among the extrapelvic endometriosis, but it often occurs after pelvic operation and episiotomy. Four cases of scar endometriosis following Cesarean section and episiotomy were exp- erienced and treated with complete surgical excision.
