1.Transperitoneal laparoscopic ureterovesical reimplantation (report of 17 cases)

Objective To introduce the new technique of transperitoneal laparoscopic ureterovesical reimplantation and to evaluate its feasibility and clinical significance. Methods Overall,17 cases (5 men and 12 women) of ureterovesical obstruction (left,7 and right,10) were included.Of them,10 cases were diagnosed with simple congenital ureter outlet stricture;1 had calculi and polyp formation at right ureter outlet;2 had recurrent ureter outlet stricture after open ureterovesical reimplantation (1 of them had 2 calculi at lower part of the ureter);1 had double nephroureteral upper unit ureter outlet stricture with 2 calculi;2 had recurrent ureter outlet stricture after holmium laser section;1 with urinary TB had right ureter outlet stricture after left nephrectomy.Among the 17 cases,severe hydronephrosis occurred in 12,and moderate,in 5.Transperitoneal laparoscopic ureterovesical reimplantation was performed in the 17 cases (ureterovesical anastomosis in 15 and bladder flap ureterovesical reimplantation in 2). Results The operations were all successful in 17 cases with operating time of 90~160 min (mean, 112 min) and blood loss of 60~100 ml.The drainage tubes were removed 1~3 days after surgery without urine leakage.The urinary catheters were removed 1 week after surgery and double J stents,1 month after surgery. Six months after surgery cystography showed Ⅲ?urine reflux in 6 cases, Ⅰ?-Ⅱ? urine reflux in 5 and no reflux in 6.One year after surgery cystography showed Ⅲ? urine reflux in 3 cases, Ⅰ?~Ⅱ? urine reflux in 4 and no reflux in 10.During 3~24 months' follow-up, ultrasound B and IVU showed moderate hydronephrosis in 4 cases,mild,in 5 and no obvious hydronephrosis in 8. Conclusions Laparoscopic ureterovesical reimplantation is a feasible,less painful and minimally invasive alternative method for treatment of ureterovesical diseases.By comparison,bladder flap ureterovesical reimplantation has better anti-urine reflux effect but more complicated than ureterovesical anastomosis does.Long term efficacies of the methods need further observation.

3.The blind assessment of human bitemarks by digital analysis

Objective Digital analysis of human bitemarks by blind method was assessed for its accura-cy. Method Digital analysis was used in double-blind method for identification of human bitemarks with control samples from 8 suspects. Photoshop 5.5 generates an overlay from bitemarks scanned and various coefficients were submitted for analysis by AutoCAD R14 engineering software to compare bitemarks with denture of suspects. Results Digital analysis shows that bitemarks tally with cast of denture from "suspects" in every coefficient. Conclusion Digital analysis is feasible for identification of experimental bitemarks and promises sound prospects in future forensic practice.
4.Training and Practice of Teachers for Occupation Therapy

5.An experimental study on human bitemarks digital analysis and its accuracy.

This experiment was designed to study the method of human bitemarks digital analysis and its accuracy. The human bitemarks were made on the dog skin by human dentition. The related parameters of human bitemarks and suspects criminal dentitions were digitally recorded and managed. The digital picture of human bitemark was obtained, and the dental study model, bite in wax and bitemark on pig skin of suspected criminal were scanned. The overlay was prepared with Adobe Photoshop 5. 5 and the parameters were measured with AutoCAD R14, then their matches were compared. The result shows that the human bitemarks digital analysis is a more accurate approach to human bitemarks identification. Three methods for collecting evidence dental study model, bite in wax and bitemark on pig skin all can be used as aids in forensic sciences. Dental study model is the most accurate one of all the three methods mentioned above.

6.Transdermal delivery of Gentiana macrophylla complex components system under micro-needle conditions.

The purpose of this study is to investigate the transdermal delivery characteristics of Gentiana macrophylla complex components system through different parts of the skin under micro-needles conditions. Two-chamber diffusion cells were used, different parts of isolated skin and micro-needle pretreated isolated mouse skin were applied separately, high performance liquid chromatography (HPLC) similarity evaluation methods were used to evaluate transdermal delivery characteristics of Gentiana macrophylla complex components system on receiving pool and the permeation rate and penetration amount of Gentiopicroside at different parts of mouse skin. In the 24 h, the similarity between receiving fluid which was on passive transdermal delivery and micro-needle transdermal delivery conditions and original fluid were ranged from 83.0% to 98.9%; By the micro-needle pretreatment with different parts of the mouse skin, the time that Gentiana macrophylla complex components system though abdominal skin to the receiving fluid which reached 90% similarity compared with that of original fluid was 4 h, which was 18 h at back skin and 12 h at neck skin separately. Micro-needles can be used as the ideal ingredients for traditional Chinese medicine complex transdermal delivery; transdermal absorption time delay could be greatly reduced and its bioavailability was improved. The permeation rate and similarity to original liquid of Chinese medicine complex components increased significantly in the abdominal skin relative to the neck and back skin under micro-needle conditions.
7.Influence of NGF-PEG-PLGA-NPs on neuronal differentiation of neural stem cells and PI3K/Akt signaling pathway

BACKGROUND:Our previous studies confirmed that NGF-PEG-PLGA-NPs has good sustained release effect and biological activity in vitro, and can induce the differentiation of PC12 cel s into neuron-like cel s.
OBJECTIVE:To investigate the feasibility of neuronal differentiation of neural stem cel s from septal area of fetal brain induced by NGF-PEG-PLGA-NPs and its influence on PI3K/Akt signaling pathway.
METHODS:According to optimization prescription, NGF-PEG-PLGA-NPs were prepared by multiple emulsion solvent diffusion method. Neural stem cel s were induced to neuronal differentiation in six groups, including control group, NGF group, NGF-PEG-PLGA-NPs group, LY294002 group, LY294002+NGF group and LY294002+NGF-PEG-PLGA-NPs group. Neurons were identified by immunofluorescence, while phosphorylation levels of Akt in PI3K/Akt signaling pathway were detected by western blotting.
RESULTS AND CONCLUSION:The proportions ofβ-Tubulin III-positive neurons in control group, NGF group, NGF-PEG-PLGA-NPs group, LY294002 group, LY294002+NGF group and LY294002+NGF-PEG-PLGA-NPs group were (22.80±2.58)%, (35.80±3.98)%, (35.40±5.77)%, (26.60±3.87)%, (21.20±2.59)%and (25.80±7.22)%, respectively. There were no statistical differences in neuronal differentiation between NGF group and NGF-PEG-PLGA-NPs group (P>0.05), but the ratios of neural differentiation in the two groups were both higher than that in the other four groups (P<0.05). Western blotting results revealed that there were no statistical differences in Akt phosphorylation levels between NGF group and NGF-PEG-PLGA-NPs group (P>0.05), but the phosphorylation levels of Akt were both higher than other four groups (P<0.05). There were also no significant differences between LY294002+NGF and LY294002+NGF-PEG-PLGA-NPs groups and control group (P>0.05), but the phosphorylation levels of Akt were higher than LY294002 group (P<0.05). Results suggest that NGF-PEG-PLGA-NPs promoted neural differentiation of neural stem cel s. The role might be related to increasing phosphorylation levels of Akt in PI3K/Akt signaling pathway.
8.A biomechanical study on the morphological changing process of human bitemarks.

This is a biomechanical research in the morphological changing process of human bitemarks on the skin of dogs. The human bitemarks were made on the live and dead dogs by different tooth and occlusal force. The changing process of the bitemarks were recorded for a long time and its related morphological parameters were measured. Then multiple stepwise regression analysis was made to disclose the relationship of the shape of the bitemarks to occlusal force, time, and tooth area, tooth width and thickness. The mathematic relationship of the morphological changing process of the bitemark with occlusal force, time and tooth shape was established.

9.A Randomized, Double Blind, Double Dummy Parallel Controlled Study in Schizophrenia Treated with Ziprasidone Tablets and Risperidone Orally Disintegrating Tablets

Objective To compare efficacy and safety of ziprasidone tablets and risperidone orally disintegrating tablets in the treatment of schizophrenia.Methods The randomized, double blind, double dummy parallel controlled was designed for this study. All cases were diagnosed as schizophrenia in accordance with the Chinese Classification and Diagnostic Criteria of Mental Disorder-3(CCMD-3) diagnosis standard and randomized into two groups which were treated with ziprasidone tablets or risperidone orally disintegrating tablets for 8 weeks. The initial doses of ziprasidone tablets risperidone and orally disintegrating tablets were 40 mg and 2 mg per day, respectively; and the effective doses range of ziprasidone tablets and risperidone orally disintegrating tablets were 40~160 mg and 2~8 mg per day, respectively. The Positive and Negative Syndrome Scale (PANSS), Clinical Global Impression (CGI) and Treatment Emergent Side-effect Scale (TESS) were used to evaluate efficacy and side effects before and at the ends of 2, 4, 6, 8 weeks treatment,respectively. Results 114 adult schizophrenia patients were recruited in this study, including 55 patients in ziprasidone group and 59 patients in risperidone group. After 8 weeks treatment, the significant efficacy rates of ziprasidone group and risperidone group were 65.5% and 67.8%, respectively. The efficacy rates of ziprasidone group and risperidone group were 92.7% and 93.2%, respectively. There was no significant difference between the two groups (P>0.05). All of the side effects observed in this study were slight. The incidence rates of side effect were 36.4% and 32.2% of ziprasidone group and risperidone group, respectively. Electrocardiogram (ECG) abnormality was only observed in ziprasidone group (7.3%). Accordingly, the extrapyramidal side syndrome (EPS) incidence rate of risperidone group (11.9%) was higher than ziprasidone group (3.6%). The difference between the two groups was not statistically significant (P>0.05).Conclusion Both of the two medicines were effective and safe in treatment of schizophrenia.
10.The Effect of Selective Posterior Rhizotomy and Rehabilitation Therapy on Motor Function of Patients with Spastic Cerebral Palsy

Objects:To observe the effort of rehabilitation therapy after SPR.Methods:Twenty CP patients with spasticity were operated from December 1996 to October 1997.Occupational therapy was used in conjunction with Bobath method which formed the base of PT.Results:All values of muscle tone before and after operation were significantly different.Strength of many muscular groups was lower than normal level before operation and decreased further postoperatively.Sitting,kneeling and standing posture improved after operation and rehabilitation.There were 9 independent walk cases improved stride length,heel contact pattern and they had no scissors gait after rehabilitation.Conclusion:The goals of rehabilitation training focused on building muscle strength,stretching contractures and building up right posture control,inhibiting abnormal movement pattern,and gradually forming nornmal movement pattern.
