1.Freeze-dried bovine amniotic membrane as a cell delivery scaffold in a porcine model of radiation-induced chronic wounds

Background:
Locoregional stem cell delivery is very important for increasing the efficiency of cell therapy. Amnisite BA (Amnisite) is a freeze-dried amniotic membrane harvested from bovine placenta. The objective of this study was to investigate the retention of cells of the stromal vascular fraction (SVF) on Amnisite and to determine the effects of cell-loaded Amnisite in a porcine radiation-induced chronic wound model.
Methods:
Initially, experiments were conducted to find the most suitable hydration and incubation conditions for the attachment of SVF cells extracted from pig fat to Amnisite. Before seeding, SVFs were labeled with PKH67. The SVF cell-loaded Amnisite (group S), Amnisite only (group A), and polyurethane foam (group C) were applied to treat radiation-induced chronic wounds in a porcine model. Biopsy was performed at 10, 14, and 21 days post-operation for histological analysis.
Results:
Retaining the SVF on Amnisite required 30 minutes for hydration and 1 hour for incubation. A PKH67 fluorescence study showed that Amnisite successfully delivered the SVF to the wounds. In histological analysis, group S showed increased re-epithelialization and revascularization with decreased inflammation at 10 days post-operation.
Conclusions
SVFs had acceptable adherence on hydrated Amnisite, with successful cell delivery to a radiation-induced chronic wound model.

3.Anatomical Analysis of Clavicles in Korean Adults and Compatibility of Pre-Contoured Anatomical Plates.

PURPOSE: Due to the complex anatomy of clavicles, clavicular plates are not always compatible with clavicular fractures. The purpose of this study was to analyze basic data on the anatomy of the clavicle in order to determine compatibility between clavicles of Korean adults and pre-contoured anatomical plates. MATERIALS AND METHODS: We analyzed the anatomy of 600 clavicles of 300 patients who underwent three-dimensional (3D) computed tomography of clavicles in the emergency room of Konkuk University Chungju Hospital, between July 2010 and July 2011, using Andermahr's method; in addition, the compatibility between 3D axial images of clavicles and sectional images of pre-contoured anatomical plates was also examined using Adobe Photoshop. RESULTS: The mean length of the clavicle was 146.21+/-4.98 mm, the mean width was 9.63+/-1.67 mm, and the mean thickness was 9.54+/-1.67 mm. The location of the maximum superior bow was 36.17+/-0.60 mm from the lateral end of the clavicle and the mean magnitude was 5.88+/-0.62 mm. The mean depth of medial curvature was 15.89+/-1.33 mm, and the mean depth of the lateral curvature of the clavicle was 11.73+/-1.66 mm. The compatibility between clavicles and plates was 79% as above a fair compatibility in the 50% range of clavicles and 48% as above a fair compatibility in the 60% to 70% range of clavicles. On the contrary, in application of medial and lateral plates in the 60% to 70% range of clavicles, above a fair compatibility had increased to 67%. CONCLUSION: A more adequate pre-contoured anatomical plate is required for satisfactory improvement of the compatibility of clavicles of Korean adults.

4.The Influence of the Regional Weather in Geriatric Trauma.

PURPOSE: Many factors influence the occurrence and severity of geriatric trauma, and regional weather is regarded as one factor that influences geriatric trauma. In this study, to predict the type, severity, and incidence of geriatric trauma patient, we analyzed the influence of regional weather on geriatric trauma. METHODS: The subjects of our investigation were trauma patients over sixty-five years of age who visited the Emergency Department (ED) of Yeungnam University Hospital during a one-year period. We retrospectively reviewed the medical charts of 436 geriatric trauma patients, and the data were analyzed by using SPSS 12.0 for Window. The weather was based on data from the Korea Meteorological Administration. RESULTS: The average age was 72.8 years old, and the ratio of males to females was 1:1.1. The mean spell out ISS was 10.8, and no difference was found between males and females. Slips were the most common cause of trauma. The largest numbers of aged trauma patients, 46, visited the ED in May, and the smallest number of such patients, 24, visited the ED in December. In addition to, summer saw the largest number of aged trauma patients. The type of trauma, the Injury Severity Score, and the number of patients had no relationship with season. On sunny days, the ISS was larger in patients who had hypotension and who had tachycardia. On rainy day, the ISS was larger in male patients and cultivator accident patients. The number of patients was larger on partly cloudy days. CONCLUSION: In spring and summer and on partly cloudy days, we must be prepared to treat aged traumatized patients in the E.D. On rainy days, visual sensation, tactual sense, and acoustic sense must be closely examined. In addition,on rainy day, aged male traumatized patients or cultivator accident patients must to be closely observation.

6.Arthroscopic Olecranon Osteophyte Resection in the Baseball Players with Elbow Valgus Extension Overload Syndrome.

We evaluated the clinical outcome after arthroscopic olecranon osteophyte resection without ligament operation in the elite baseball players who had valgus extension overload syndrome without moderate or severe medial collateral ligament injury. From January 2007 to December 2011, twelve patients underwent arthroscopic osteophyte resection without ligament operation and they were followed for more than 12 months. The mean age was 19.2 years and mean follow-up period was 26 months. The clinical results were evaluated using range of motion, visual analogue scale (VAS) and Mayo elbow performance score (MEPS). After checking osteophyte size and location through 3-dimensional computed tomography, arthroscopic osteophyte resection was performed. Average preoperative extension, flexion, pronation and supination were 2.3degrees, 138.2degrees, 76.4degrees, and 69.1degrees. Average postoperative extension, flexion, pronation and supination had been changed into 0.7degrees, 137.3degrees, 79.1degrees, and 77.3degrees. Average preoperative pain VAS and MEPS were 5.5 and 67.5. Average postoperative pain VAS and MEPS had been changed into 0.4 (p<0.001) and 97.5 (p<0.001). Eleven patients returned to play. Ten cases returned to their own position. No patients were performed other operation for elbow pain. The arthroscopic osteophyte resection in valgus extension overload syndrome with low grade medial ulnar collateral ligament (MUCL) injury or without MUCL injury was a one of the ideal treatment option for early return to pre-injury levels and relief of pain.

7.The Measurement of the Sensory Recovery Period in Zygoma and Blow-Out Fractures with Neurometer Current Perception Threshold.

BACKGROUND: Facial hypoesthesia is one of the most troublesome complaints in the management of facial bone fractures. However, there is a lack of literature on facial sensory recovery after facial trauma. The purpose of this study was to evaluate the facial sensory recovery period for facial bone fractures using Neurometer. METHODS: Sixty-three patients who underwent open reduction of zygomatic and blowout fractures between December 2013 and July 2015 were included in the study. The facial sensory status of the patients was repeatedly examined preoperatively and postoperatively by Neurometer current perception threshold (CPT) until the results were normalized. RESULTS: Among the 63 subjects, 30 patients had normal Neurometer results preoperatively and postoperatively. According to fracture types, 17 patients with blowout fracture had a median recovery period of 0.25 months. Twelve patients with zygomatic fracture had a median recovery period of 1.00 month. Four patients with both fracture types had a median recovery period of 0.625 months. The median recovery period of all 33 patients was 0.25 months. There was no statistically significant difference in the sensory recovery period between types and subgroups of zygomatic and blowout fractures. In addition, there was no statistically significant difference in the sensory recovery period according to Neurometer results and the patients' own subjective reports. CONCLUSIONS: Neurometer CPT is effective for evaluating and comparing preoperative and postoperative facial sensory status and evaluating the sensory recovery period in facial bone fracture patients.
