3.A Case of Anterior Lens Capsule Rupture from Blunt Ocular Trauma.

PURPOSE: To report a case of isolated anterior capsule rupture with cataract after blunt ocular trauma. CASE SUMMARY: A 38-year-old male complained of decreased visual acuity in the right eye after blunt ocular trauma 5 days earlier. The visual acuity was counting fingers at 30 cm in the right eye, and intraocular pressure, measured using an applanation tonometer, was 25 mm Hg. Slit lamp examination showed a white intumescent cataract with anterior lens capsule rupture and cortical lens material extruding into the anterior chamber. Under local anesthesia, removal of the cataract was approached via a clear corneal incision. After removal of the cataract using irrigation and aspiration, the intact posterior capsule was observed. IOL haptic was implanted in the sulcus, and IOL optic was implanted in the bag. Postoperatively, the BCVA improved in the right eye to 0.8 at 1 month, and the intraocular pressure, by the Goldmann applanation tonometer was 13 mm Hg at that time.

4.Alveolar Soft-Part Sarcoma of the Female Genital Tract.

Alveolar soft-part sarcoma of the female genital tract are extremely rare. Fewer than 30 cases have been described in the literature. We experienced a case of alveolar soft-part sarcoma of the female genital tract which was diagnosed by routine light microscopic study using ultrastructural and immunohistochemical stain. We report this case with a brief review of the literature.

5.Effects of Exercise on Glycemic Control in Patients with Type 1 Diabetes Mellitus.

PURPOSE: Exercise is frequently recommended in the management of diabetes mellitus(DM) and can improve blood control by increasing insulin sensitivity and psychological benefits. Exercise can result in good glycemic control in type 2 DM, when combined with diet and drug therapy. However, in type 1 DM, the expected improvement in glycemic control with exercise have not been clearly established. Effects of exercise on glycemic control in patients with type 1 DM were investigated. METHODS: 20 patients with of type 1 DM, who were no retinopathy, neuropathy, nephropathy and cardiac disorders, were enrolled and exercised for 30min. with 50% of individualized maximum VO2. Blood sugar concentration were measured before, immediate and 15min after exercise. The results were evaluated with HbA1c, C-peptide and DM duration. RESULTS: Blood sugar concentration were significantly decreased from pre-exercise 198+/-9.7mg/dL to immediate 145+/-7.1mg/dL and at 15min. Post-exercise 134+/-4.0mg/dL(P<0.05). In case with HbA1c<10%, there was significantly decreased in blood sugar level from pre-exercise 176+/-9.7mg/dL to immediate 123+/-63.2mg/dL and at 15min post-exercise 113+/-1.9mg/dL(P<0.05). In case with HbA1c>10% there was also significantly decreased in blood sugar levels 222+/-64.7mg/dL, 169+/-6.1mg/dL, 157+/-1.2mg/dL respectively(P<0.05). Group with moderate decreased blood sugar(40-99mg/dL) was 8 patients(42.1%), group with mild decreased blood sugar(<40mg/dL) was 7 patients(36.8%) and group with decreased more than 100mg/dL was 4 patients(21.5%). There was no correlation in degree of decreased blood sugar concentration among HbA1c, C-peptide, DM duration. CONCLUSION: Proper exercise in type 1 diabetic children can decrease blood sugar level significantly and make good glycemic control and can decrease DM microvascular complications in addition to motivating physically active lifestyle.

6.A Clinical Study on the Treatment of Open Fractures of Tibial Shaft: Using AO External Fixator with Additional Interfragmental Lag Screw Fixation

Fourty one cases of open fractures of tibial shaft complicated with varying degrees of soft tissue injuries were trested by one-plane unilateral external fixation with AO fixator. In seventeen csses among them, interfragmental lag screw fixation was applied in addition, for more rigidity and stability of the fixation. All of thern were followed up at least for one year and we obtained the following results. 1. Rigid stable external fixation provided by additional minimum internal fixation may not increase the rate of infection but rather enhance prompt primary bone healing. 2. One-plane unilatersl external fixation may facilitate good access to the wound for subsepuent reconstructive procedures, and enhance pain-free early full range of joint motion, thus promote early bone union. 3. Comparing with bilsteral fixation, there wss almost no difference in the union time of the fracture even with one-plsne unilatersl fixation.

7.Expression of p73 in Null-p53 SKOV3 Ovarian Cancer Cell Line.

p73, a first p53 relative, has been identified at chromosome 1p36, a region that is deleted in variety of human cancers. This protein shares strong homology with p53 protein, suggesting functional similarities with p53. Indeed, p73 can activate p53 downstream genes inducing apoptosis or growth arrest in tumor cells lacking p53. This phenomenon leads us to investigate the function of p73 in ovarian cancer because aberrant p53 was very frequently found in this cancer. We hypothesize that DNA damaging agents trigger p53 dependent apoptotic pathway through p73 instead of p53 in ovarian cancer having aberrant p53. We selected SKOV3 ovarian cancer cell line having no p53 gene and treated this cell line with cisplatin. After the treatment, we examined the transcriptional level of p73 and p21. Moreover, to identify whether the status of p53 influence to the function of p73, we performed same experiment after inserting adenovirus mediated p53(Avp53) into cell line. We detected significantly increased transcripts of p73 whcn treated with cisplatin. But treated with Avp53 or combined treatment with cisplatin, the transcriptional levels were not changed. These data suggest that overexpression of p73 may be important to trigger apoptotic pathway when the p53 gene is lost, but not so important in cells having normal p53.

8.Morphometric Analysis of the Skull by Moire Contourography.

The non-metric analysis of the skulls is very useful for estimating sex and determination of ancestry, the accuracy tends to depend on the amount of experiences of the observers, and so inter-observer errors might be happened. Many researchers are trying to find out more objective methods for determination of ancestry. The purpose of this presentation is to show the usefulness of moire contourography for analyzing the skull. The master screen that is similar to the gratings was made by steel rods, which were arranged as equally spaced parallel lines. Halogen light source was illuminated by lantern slide projector. The skeletal materials were documented crania, composed of 87 male and 47 female, from William M. Bass Donated Skeletal Collection housed at the Department of Anthropology, University of Tennessee. The skulls were placed just behind the master screen as anatomical position using cubic craniophore. The angle between the light source and camera was 65degrees, the distance between camera and the master screen was 1.2 m. Frontal view, left lateral and right lateral view were taken. From the frontal view, fringe patterns were analyzed for first five contour lines which were mainly located around the Glabella. The results were as followed; Type I for male was 53% and female was 4%; Type II for male was 29% and female was 2%; Type III for male was 2% and female was 15%; Type IV for male was 6% and female was 55%. From the lateral view, fringe patterns were analyzed for first four contour lines. However, first and second contour lines were critical to determine the shape and the results were as followed; Type I for male was 52% and female was 22%; Type II for male was 38% and female was 26%; Type III for male was 8% and female was 17%; Type IV for male was 2% and female was 35%. According to this study, different fringe patterns might be dependent on the degree of development of bone marker such as Glabella, Supercillary arch, Euryon and Mastoid process. For example, Supercillary arches were very well developed and slope of forehead above the Glabella was declined, fringe pattern showed reverse triangle shape. If Supercillary arches were poorly developed and slope of forehead above the Glabella was flat, fringe pattern showed home plate shape. The present research shows that moire contourography might be used as more objective methods for estimating sex. And it would be helpful to determine the ancestry when the lateral aspects were analyzed. In the future, continuing study need to be performed with same master screen for different ancestry.

9.Clinical syndrome of dystonia-parkinsonism.

We report 11 patients who developed dystonia and parkinsonian symptoms and signs. All of them developed the onset of parkinsonian symptoms 1 month to about 50 years(median: 2 yrs) after the onset of dystonia. Six of 11 patients developed dystonia affecting a hand; two affecting a foot; one affecting the neck; one affecting bilateral hands and a foot; one affecting a foot, trunk and neck Their parkinsonian symptoms had progressed very slowly and responded well to the levodopa treatment. Beside, none of them had clinical features suggestive of parkinsonian-plus syndrome. These findings suggest that they may have pathological changes confined to the nigrostriatal dopaminergic system as those with idiopathic Parkinson's disease. However, no change or worsening of dystonia after levodopa treatment suggest that they may have additional lesions out-side of the nigrostriatal dopaminergic system. More pathological studies and functional brain imaging studies are needed to define the exact pathogenesis of clinical syndrome of dystonia-parkinsonism.

10.Contribution of Color Doppler Scoring System to Diagnosis of Ovarian Malignancy.

The purpose of this prospective study was to evaluate the diagnostic value of color Doppler scoring system in characterization of ovarian masses. We performed transabdominal or transvaginal color Doppler sonopaphy on 82 women with ovarian masses and analyzed the sonographic findings. Ovarian lesions were assessed by means of morphological and color Doppler scoring system by Kurjak. Tumors were characterized by ultrasonographic findings as benign or suspected of being malignant. Then the results of each scoring systems were correlated with histopathological findings. The results were as follows; Of 82 ovarian masses, 64 were benign lesions(13 mucinous cystadenomas, 16 endometriomas, 20 teratomas, 8 serous cystadenomas, and 13 other abnormalities), and 18 were malignant(12 cancers and 6 borderline tumors). The color Doppler scoring system was useful in distinguishing malignant from benign masses, with a sensitivity of 88.2%, compared with the morphological scoring system of 86.7%. The specificity of color Doppler and morphological scoring system were 95.4% and 92.5%, respectively. The sensitivity of combination of both scoring systems was 88.9%, with a specificity of 96.9%. In conclusion, color Doppler scoring system is a useful tool in predicting the malignancy of ovarian lesions especially combined with morphological scoring system.
