2.A Case of Penile Lymphangioma Circumcriptum.

We report a case of penile lymphangioma circumscriptum developed in a 22- year-old male soldier. Rice-sized papule on glans penis was noted at birth and progressed to yellow straw-colored, peanut-sized, asymptomatic grouped papulovesicles resembling frog spawn. The predilection sites of lymphangioma circumscriptum are the abdomen, axilla and tongue. Glans penis is a very rare site. Histopathologic findings showed papillornatosis, hyperkeratosis and cystically dilated lymph vessels lined by a single layer of endothelial cells.

3.Spontaneous Spinal Epidural Hematoma.

The authors reviewed 16 cases of spontaneous spinal epidural hematoma(SSEH) described in the Journal of the Korean Neurosurgical Society and one case unpublished case of our own. Attention was focused on sex,age, medical history, position and vertebral level of the hematoma, preoperative neurological condition and operative result. This study comprised 17 patients, 14 males and 3 females, between five and 79 years old. All vertebral segments were affected, though the thoracic area predominated. In 13 patients, sensory and/or motor deficit was incomplete and in four, SSEH resulted in complete preoperative sensory and motor loss. In all patients, the hematomas were removed surgically through total laminectomy. In four, postoperative recovery was total, but in ten, was incomplete, and the neurological condition of three showed no improvement. We concluded that in cases of SSEH, this operation is the treatment of choice.

4.Supratentorial Epidural Hematoma as a Complication of Acoustic Neurinoma Surgery.

The authors report a case of postoperative supratentorial epidural hematoma as a complication of acoustic neurinoma surgery. The 43-year-old man presented with hearing difficulty on the left side and decreased facial sensation. MR imaging demonstrated a huge, cystic left acoustic neurinoma and the retromastoid suboccipital approach was used for gross-total resection of the tumor. Postoperatively, the patient was drowsy and showed right hemiparesis. Computed tomographic scanning revealed a huge epidural hematoma in the left parietooccipital convexity. The intraoperative course was uneventful. Immediate craniotomy was performed and the epidural hematoma was removed. Bleeding from the superior sagittal sinus occurred, but was completely controlled. Postoperatively, the patient was neurologically normal, except for mild left facial weakness. Remote postoperative intracranial hemorrhage is a rare complication of craniotomy with significant morbidity and mortality. Such hemo-rrhages are likely to develop at or soon after surgery and can be related to the craniotomy site, operative positioning, and nonspecific mechanical factors. In this case, mechanical displacement of the brain secondary to excessive dehydration and CSF drainage was thought to be the cause of postoperative epidural hematoma. Clinical awareness of this rare but potential complication is essential to its early diagnosis and treatment. Difficulty in awakening from anesthesia and the development of new neurological deficits not attributable to the operative site are the most important keys to early diagnosis. Computed tomography is the diagnostic method of choice.

5.Risk factors of work-related low back pain in manufacturing workers.

Work-related low back pain is one of the major factors that cause the loss of working power especially in actively working age, therefore controlling the work-related low back pain is one of the major issues in the field of industrial safety and health. This study was performed for detecting the risk factors and proposing the efficient control program of the work-related low back pain. The subjects were male workers employed at the manufacturing industry in Pusan with longer than 6 month's working duration. The data was collected by self-reported questionnaire and interview from May 1 to October 31, 1992. The contents of questionnaire were as follows : the experience of low back within the recent one month, general characteristics(age, marital status, education level, religion, regular exercise), physical characteristics(height, weight), employment status(working duration, daily working hours, rest during work, satisfaction for the job), type of work and working environments(posture, forward bending, lifting and movement, satisfaction for table and chair). The result was analyzed for 608 entire respondents by case-control comparative method. The number of cases was 152 with a history of work-related low back pain, so the relative frequency of self-reported work-related low back pain was 25.0%, and the number of controls was 344 without any history of low back pain. As a result, two characteristics of the employment status(working duration, satisfaction for the job) and all characteristics of type of work and working environments showed a statistical significance between the case and control groups(p<0.01). The Odds ratios of these variables for Work-related low back pain were calculated. They were 7.88 for the satisfaction for chair, 7.86 for lifting and movement, 3.31 for satisfaction for table, and 2.22 for bending forward(p<0.01). And for the multivariate logistic regression analysis confirmed that unsatisfaction for table was independent risk factor for Work-related low back pain. In summary, though this study was based upon the self-reported questionnaire and the subjective complaints, the satisfaction for chair, lifting and movement, satisfaction for table, and bending forward concerned with the type of work and working environments were considered as the main factors causing the work-related low back pain, and the work-related low back pain may be preventable by the ergonomic control of these factors.

6.Chronic Recurrent Methicillin Resistant Staphylococcus Hemolyticus Endophthalmitis after Cataract Surgery

PURPOSE: To report a case of recurrent endophthalmitis due to methicillin resistant Staphylococcus hemolyticus after phacoemulsification and posterior chamber intraocular lens (IOL) implantation. CASE SUMMARY: A 76-year-old female visited our outpatient clinic with decreased vision 40 days after uncomplicated cataract surgery in her right eye. At the visit, anterior chamber inflammation and cloudy fluid between the posterior capsule and IOL were observed. Uveitis due to residual cortex of lens or capsular block syndrome was suspected, so YAG laser capsulotomy and subconjunctival injection of dexamethasone were performed. Two days later, hypopyon and vitreous opacity were seen. The patient underwent an emergency vitrectomy and intravitreal antibiotic injection with suspicion of bacterial endophthalmitis. The culture was negative. Twenty days after the vitrectomy, anterior chamber inflammation and vitreous opacity developed. The recurrence of endophthalmitis was suspected due to infection by bacteria in the surrounding tissue of the IOL, so the patient underwent an IOL and lens capsule removal with intravitreal antibiotic injection. At this time, the culture revealed methicillin resistant staphylococcus hemolyticus. Systemic and topical vancomycin was then administered, resulting in decreased inflammation. Twenty days after the IOL removal, decreased vision, anterior chamber inflammation, and vitreous opacity developed. Endophthalmitis was decreased by intravitreal antibiotic injection and topical antibiotic treatment. CONCLUSIONS: Methicillin resistant staphylococcus hemolyticus should be considered in the differential diagnosis of chronic recurrent endophthalmitis after cataract surgery.

7.A Case of Idiopathic Infratentorial Superficial Siderosis

Superficial siderosis is attributed to hemosiderin deposition in the subpial layers of the nervous system. The clinical features of infratentorial superficial siderosis (ISS) are hearing loss, cerebellar ataxia, and corticospinal tract signs and the most common cause of idiopathic ISS is a dural defect. As magnetic resonance imaging (MRI) has advanced, the diagnosis of infratentorial superficial siderosis can be confirmed by unique radiological findings in MRI. Here, we report on a female patient diagnosed with idiopathic ISS by means of clinical symptoms and radiological findings.
8.Expression of p63 and its Isoform, deltaNp63, in Non-Small Cell Lung Carcinoma.

BACKGROUND: Several studies have been conducted on the role of the p63 gene family in non-small cell lung carcinoma (NSCLC). Nevertheless, the role of these genes in the development and progression of NSCLC remains controversial. This study was designed to examine the expression and clinicopathologic significance of the p63 family in NSCLC. METHODS: Immunohistochemical staining was performed on 92 cases of NSCLC (47 squamous cell carcinomas [SqCCs] and 45 adenocarcinomas [ACs]) using tissue microarray blocks. The results were analyzed and correlated with clinicopathologic data. RESULTS: The expression of delta Np63 (Delta Np63) was elevated in SqCC (39/47), but not in AC (2/45; p<0.01). Both p63 and Delta Np63 had high expression in 39 SqCCs; p63 and Delta Np63 also had a similar geomorphologic distribution in most positive tumors. The expression of Delta Np63 was correlated with histologic type, gender, pT stage, p53 expression, and p63 expression. pT and pN stages were independent factors in survival (p<0.05, respectively). CONCLUSIONS: The major p63 isoform in NSCLC, Delta Np63, had a strong correlation with p53 and p63, and was exclusively expressed in SqCC. However, our findings suggest that Delta Np63 was not an independent prognostic factor for NSCLC.

9.A Case of Klinefelter Syndrome associated with Unilateral Multicystic Dysplastic Kidney in a Newborn Infant.

Klinefelter syndrome is the most common chromosomal abnormality, with a 47, XXY karyotype and typical clinical findings of infertility, hypogonadism, reduced body hair, gynecomastia, tall stature, and incresed gonadotropins and decreased testosterone levels. In addition to this classic description, several other diseases have been discribed in Klinefelter syndrome such as unilateral renal aplasia, autoimmune disease, diabetes mellitus, sexual precoxity, renal cell carcinoma, intravesical ureterocele, and osteoporosis. The incidence is 1 in 400-1,000 of the population and urological abnormalities are not common. However a case of Klinefelter syndrome associated with multicystic dysplastic kidney has not been not reported up to date. Therefore, we describe a 1- day-year old baby boy who presented with Klinefelter syndrome with unilateral multicystic kidney dysplastic disease, plus with a brief review of the literature.

10.Minimum Specimen Volume Analysis of ABO/RhD Typing and Unexpected Antibody Screening Using an Automated Immunohematology System DAYmate S.

BACKGROUND: Phlebotomy performed for laboratory testing has the potential to cause anemia in newborns and infants. This study investigated the minimum specimen volume required for an automated immunohematology analyzer DAYmate S. METHODS: Three combinations of tubes were evaluated: I. 6 mL EDTA tube, II. 0.5 mL microtainer (on top of 3 mL EDTA tube), and III. 1 mL sample cup (on top of 6 mL EDTA tube). ABO/RhD cell typing was done using centrifuged red cells; unexpected antibody screening was carried out using plasma, and Type & Screening was conducted using whole blood samples. The lowest specimen volume capable of performing 10 repetitive tests without errors was investigated. RESULTS: ABO/RhD cell typing could be performed from I. 30 μL, II. 25 μL, and III. 25 μL. Unexpected antibody screening could be performed from I. 170 μL, II. 150 μL, and III. 140 μL. According to the hematocrit levels, Type & Screening could be performed from 30%, I&III 650 μL, II. 800 μL; 40%, I&III 650 μL, II. 900 μL; and 50%, I&III 1,000 μL, II. Testing using specimen volumes below 1,000 μL was difficult. CONCLUSION: By separating red cells and plasma, pre-transfusion testing of ABO/RhD cell typing and unexpected antibody screening could be conducted with very small specimen volumes using DAYmate S compared to Type & Screening using whole blood. The application of small-sized sample tubes was more competitive and this is expected to be very useful for preventing iatrogenic anemia in neonates and infants less than 4 months old.
