3.A Case of Benign Symmetric Lipomatosis.

Benign symmetric lipomatosis is characterised by diffuse symmetric deposits of adipose tissue, predominantly on the neck, shoulder, back and upper extremities. We report a case of benign symmetric lipomatosis in a 61-year-old-man, which was first noted one year previously. It began growing rapidly about 3 months ago and was associated with alcoholic liver disease. Histopathological findings showed that the reticular dermis had been replaced by normal uncapsulated mature fat cells that were slightly increased in number in the fibrous connective tissue. He was treated with theophylline 300mg/day for two months.

4.Perifolliculitis Capitis Abscedens et Suffodiens Successfully Treated with Isotretinoin.

Perifolliculitis capitis abscedens et suffodiens is a rare chronic recalcitrant follicular disorder which clinically presents itself as pulstules, nodules, intercommunicating abscess and sinuses that leave atrophic, hypertrophic or keloidal scars. Although the etiology of this of condition is unknown, its association with acne conglobata and hidradenitis suppurativa, collectively termed the follicular occlusion triad, suggest a cammon basic pathogenic mechanism of follicular retention. The therapeutic problem is reflected in the variety of therapies that have been tried with varying degrees of success. We report a case of a 34-year-old man who showed a patch of alopecia on the vertex of his scalp. Clinically, the scalp showed multiple soft fluctuant elevated nodules with nonscarring alopecic patches. We performed a treatment with isotretinoin(13-cis-retinoic acid) during 6 months. The patients clinical appearance was much improved and local recurrence and hair loss have not been observed to date.

5.Changes of Arterial Oxygen Saturation after Pediatric Anesthesia.

In postanesthetic period, infants and children have a risk of hypoxemia due to decreased functional residual capacity(FRC) and increased alveolar-arterial O2 tension gradient(A-a DO2). We measured arterial oxygen saturation(SaO2) with a pulse oximeter in 60 ASA class 1 infants and children. Group 1 was breathing with supplemental oxygen(4L/min) by mask and Group 2 was breathing with room air in recovery room after general anesthesia. SaO2 was measured on arrival in the recovery room, 2,4,6, 8,10,15, and 20 minute after arrival The results were as follows: 1) Postanesthetic SaO2 measured on arrival in the recovery room had decreased significantly(p<0.05) to preanesthetic SaO2 2) SaO2 measured in room air had decreased significantly compared with SaO measured in supplemental oxygen. As a result of the study, it is thought to be safe that supplemental oxygen is administered to patient on transfer and in recovery room.

6.Availability of Laser Lithotripter in Ureteroscopic Removal of Stone.

We treated 550 ureteral calculi patients with stone basket or Candela MDL-l laser lithotripter using the rigid ureteroscope(URS) between April 1988 and November 1993. The laser lithotripsy was applied for the patients who had failed URS stone removal with stone basket. The success rates of URS stone removal with stone basket were 33.7%(120/356) in the lower ureter stones, 1.7%(2/112) in the mid, none(0/82) in the upper. The laser lithotripsy rates and success rates were 63.4%(226/356), 99.5%(225/226) in the lower ureter, 90.1%(101/112), 85.1%(86/101) in the mid, 79.2%(65/82), 80%(52/65) in the upper, respectively. The total URS stone removal success rates were 96.9%(345/356) in the lower ureter, 78.6%(88/112) in the mid, 63.4 %(52/82) in the upper. There were 65 failures due to upward migration(27 cases), approach failure to stone by URS (35 cases), no fragmentation(3 cases). Ureterolithotomy was required in 8 cases and shockwave lithotripsy in 11 cases and retry in 22 cases. The complication of URS were minimal, gross hematuria (11.2%), pain(9.5%) and infection (1.6%). Ureteral perforation occurred in only 10 patients. In conclusion, laser lithotripsy has been significantly contributed improving of success rates and expanding of applying area for URS stone removal in our series.

8.Cutaneous Horn Arising from Keratoacanthoma.

Cutaneous horn is a clinica) diagnosis based upon the presence of a large protuberant mass of keratin. We report a case of cutaneous horn arising from keratoacanthoma in a 76-year old woman. The patient showed a dome-shaped nodule 2n in diameter with a central crater on thc right temple area. A yellow brown colored cornical hyperkeratotic protruding lesion(0.7* 0.7cm size) from the central crater of the nodule was also observed. Histopathological findings of the lesions showed charaeteristic findings of keratoacanfhoma and cutaneous horn. The lesion was removed by surgical excision.

9.The Effect of Transcranial Direct Current Stimulation on Cortical Excitability and Motor Function in a Stroke Patient: A case report.

In this case study, we investigated the change of cortical excitability and motor function recovery after transcranial direct current stimulation (tDCS). A patient with chronic right middle cerebal artery territory infarction received tDCS. We performed anodal tDCS on her ipsilesional primary motor cortex which was found by motor evoked potential and conventional occupational therapy for 2 weeks. We evaluated upper extremity function with Fugl-Meyer Assessment (FMA) before and after, one and three months after tDCS. In addition, functional magnetic resonance imaging (FMR) was carried out to evaluate the change of cortical excitability. FMA score was improved after tDCS and the improvement was prominent at one and three months later compared to pre-treatment score. And cortical excitability of ipsilesional primary motor cortex was increased after tDCS. As these results show tDCS seems to be useful tool in promoting motor recovery through increasing cortical excitability in stroke patients.
